Background: Driving capacity is affected by vestibular disorders and the medications used to treat them. Driving is not considered during medical consultations, with 92 per cent of patients attending a centre for dizziness not discussing it with the doctor. Objective: To investigate if medical record prompts facilitate dizziness and driving conversations in ENT balance clinics. Methods: A questionnaire was designed to reflect the current standards of practice and advice given regarding driving and dizziness during balance clinic consultations. Results: Medical record prompts facilitated the improved frequency and recording of shared decision-making conversations about driving and dizziness in 98 per cent of consultations. Conclusion: This study highlights the benefits of medical record prompts for documented and accurate shared decision-making conversations surrounding dizziness, vertigo, vestibular conditions and driving. This potentially improves safety for all road users, and protects the patient and clinician in the event of road traffic accidents and medico-legal investigations. [ABSTRACT FROM AUTHOR]

Background: Little is known about when youth may be at greatest risk for attempting suicide, which is critically important information for the parents, caregivers, and professionals who care for youth at risk. This study used adolescent and parent reports, and a case-crossover, within-subject design to identify 24-hour warning signs (WS) for suicide attempts. Methods: Adolescents (N = 1094, ages 13 to 18) with one or more suicide risk factors were enrolled and invited to complete bi-weekly, 8–10 item text message surveys for 18 months. Adolescents who reported a suicide attempt (survey item) were invited to participate in an interview regarding their thoughts, feelings/emotions, and behaviors/events during the 24-hours prior to their attempt (case period) and a prior 24-hour period (control period). Their parents participated in an interview regarding the adolescents' behaviors/events during these same periods. Adolescent or adolescent and parent interviews were completed for 105 adolescents (81.9% female; 66.7% White, 19.0% Black, 14.3% other). Results: Both parent and adolescent reports of suicidal communications and withdrawal from social and other activities differentiated case and control periods. Adolescent reports also identified feelings (self-hate, emotional pain, rush of feelings, lower levels of rage toward others), cognitions (suicidal rumination, perceived burdensomeness, anger/hostility), and serious conflict with parents as WS in multi-variable models. Conclusions: This study identified 24-hour WS in the domains of cognitions, feelings, and behaviors/events, providing an evidence base for the dissemination of information about signs of proximal risk for adolescent suicide attempts. The model for capacity assessment in the United States and much of the Western world relies upon the demonstration of four skills including the ability to communicate a clear, consistent choice. Yet such assessments often occur at only one moment in time, which may result in the patient expressing a choice to the evaluator that is highly inconsistent with the patient's underlying values and goals, especially if a short-term factor (such as frustration with the hospital staff) distorts the patient's preferences momentarily. These challenges are particularly concerning in cases, which arise frequently in hospital settings, in which patients demand immediate self-discharge, often during off-hours, while faced with life-threatening risks. This paper examines the distinctive elements that shape such cases and explores their ethical implications, ultimately offering a model for such situations that can be operationalized. Older people have increasingly been using the new information and communication technologies (ICTs), namely the internet. Seeking to contribute with substantive information on their digital inclusion, this paper focuses on the adoption and use of ICTs by older adults, highlighting their experiences and considering their plurality. Taking a qualitative approach, the core goal of the empirical research underlying this paper was to understand what mechanisms and modalities configure the relationship of older internet users with the new ICTs. With that objective in mind, interviews were conducted with 20 internet users over the age of 60. Those interviews, which included a biographical element, were subjected to multi-categorical analysis. Results suggest a typology of modes of relating to the new ICTs among older internet users that reflects different trajectories, practices, skills, significances and impacts. The analysis shows how life trajectories and differentiated uses are reflected in equally differentiated impacts for older adults, contributing to their quality of life in different ways and to different degrees. Digital skills play a fundamental role in enhancing or limiting those effects. The results of this research help break down the stereotypes associated with the older generations and may have relevant implications for the design of digital inclusion policies and initiatives. Paperwork has always been a central part of bureaucratic work. Over the last few years, bureaucratic procedures have become increasingly standardised and digitalised. Based on interviews and ethnographic fieldwork within welfare offices in Switzerland and Belgium, we reflect on the way evidence is constructed within social policy and cases built for or against noncitizen welfare recipients in order to show how paper truths are established and challenged. The focus on digital practices within public policy implementation highlights how it contributes to enhanced control mechanisms on the implementation level and how migration law continues to guide welfare governance for noncitizens. This allows targeting of the most marginalised groups, whose rights to access state support are institutionally impeded. Through database information flows, official forms, paper reports and face-to-face meetings, we further show how a hybrid form of bureaucratic work emerges, where direct contact with the client is still key, yet highly influenced by standardisation processes. As shown by Bélanger and Nadeau (2009), Québec's traditional identity-based axis appears less and less predominant on the political scene. This situation created a need for a different strategy in the various sovereigntist parties. In this context, how has the Bloc Québécois tried to reposition itself? To explore this question, we collected over 7,500 press releases issued by the party between 2002 and 2021. Using automated content analysis methods and a Latent Semantic Scaling approach developed by Watanabe (2020), we illustrate and discuss the prevalence of the sovereigntist issue throughout this period, as well as the party's tone on this issue. The results indicate that elections and election results play a crucial role in the Bloc Québécois' communications on the topic of sovereignty. More generally, we observe significant fluctuations in the attention paid to this issue, and significant variations in the framing of this issue around elections. Judges communicate outside of the courtroom on a regular basis. They give speeches at universities and to societies; appear before select committees; write for a range of publications; and engage in both media and outreach activities. Existing literature has charted the value and perils of such extrajudicial communication. This paper contributes an explanation of what motivates judges towards such communication, and what shapes their discourse. The work draws on 13 semi-structured interviews with senior serving and recently retired judges, along with an extensive range of examples of judicial speech beyond the bench. It argues that extrajudicial communication is shaped by a shared conception amongst the judicial community of what is appropriate. This conception of propriety is principally motivated by a communal pursuit of sustaining public confidence in the judicial office. Histories of conservation suggest that from the nineteenth century onwards, the custodianship and conservation of colonial antiquities enabled European powers to legitimize imperial claims. This article complicates this view by focusing on a series of visits made by British royals to the Caves of Elephanta, near Bombay, as part of their tours of India. Of particular interest are the visits in 1870 and 1875, which were essentially picnics, including fireworks and feasting, with little showcasing of ongoing conservation efforts. The article argues that these early visits also sought to advance a narrative of imperial legitimization through the British heirs' presence at an Indian monument. Rather than acts of rational governance, such as conservation measures, these picnics were transactions within the ceremonial economy that privileged consumption as a means of legitimizing empire. They present a register of imperial engagement with an Indian monument that is neither 'plunder' nor 'preservation'. Instead, they are posited as predecessors of the durbars (courts/assemblies) produced by the British administration from 1877 onwards. As acts of imperial political communication, the Elephanta visits drew upon the popularity of the picnic as a form of leisure, and consumption, and the long-standing aesthetic resonances of the site, such as the island's picturesque framing and the Caves' Romanticist associations. These enduring aesthetic frameworks made the acts of consumption legible as imperial political communication. The picnics at Elephanta demonstrate that colonial antiquities featured in imperial narratives of legitimization based on political pageantry, exceeding conservation and rational governance. Non-take-up, i.e. individuals not applying for a benefit they are eligible for, is a widespread problem limiting the reach of welfare and protection systems. This paper seeks to understand it by means of a theoretical framework comprising two levels of analysis: the claimants' individual characteristics in relation to the information barriers they face, and the administrative logic and functioning regarding the communications strategy used by public institutions. To test the hypotheses of these two levels of analysis, the paper analyses the B-MINCOME pilot scheme, a cash transfer programme implemented in the city of Barcelona between 2017 and 2019. Findings indicate that, although claimants' characteristics may play a significant role, the administrative functioning and the communications strategy are fundamental in determining take-up rates. Topics include Ibn Manzụ̄r's definition of lafz as a material practice involving the expulsion of matter, al-Jāhịz's discussion of lafz as the instrument of utterance and its relation to language and sociality, and the understanding of language as a collective form of linguistic life in relation to intention and poetic practic.
