A survey population of 104 drug-dependent patients admitted to the obstetric service of Cook County Hospital was found to be similar to other groups of drug-dependent parturients. However, certain differences are reported: Among the 64 patients who delivered live infants, a low birthweight rate of 52% and a 50% rate of premature rupture of the membranes were noted. Therapy depends on whether the patient requests withdrawal during pregnancy. If withdrawal is accomplished during the antepartum period, the patient can be given the prenatal care that otherwise is often lacking. Abrupt withdrawal during the antepartum period is not recommended. Intrapartum medication should not be withheld from the patient experiencing withdrawal on the grounds that the medication depresses the fetus. Delivery room personnel can treat fetal depression.
