The T. B. mass survey of 13,072 students of Seoul National University(12 colleges) was conducted from September to October 1957. 1) 91. 32 percent of the total number of students were X-rayed. 2) Among the x-rayed group, 4.84% showed positive chest findings and 2.40% showed suspected pulmonary tuberculosis.3) Of 163 T. B. cases, 85.89% showed the minimal stage and 10. 43% moderate advanced lesions, according to the clinical classification method of N. T.A.4) Of 163 cases of pulmonary TB., 6 (3.68%) cases were of cavitation and 5 (3.10%)cases of tuberculoma.5) Pulmonary TB. was highly Prevalent in the 19-22 age- group with the 21 age-group of students being the highest.6) On the analysis or 163 cases of pulmonary T. B ., 93 cases (60.12%) needed medical supervision, and 65 cases (3 9.83%) needed medical management.

Ultracentrifugal separation and analysis of chylomicron, very low, low and high density lipoproteins in the sera from patients with liver metastasis of cancer were carried out. Studies were made on the relationship between lipoprotein and alkaline phosphatase (Al-P) on cellogel electrophoresis. 1) On cellogel electrophoresis, the serum Al-P isozyme of a patient with liver metastasis was divided into Bands I and II. Band I which corresponds to macromolecular Al-P is characteristic of liver metastasis. 2) Band I was found only in high density lipoprotein (HDL) fraction. This result indicates that the density of Band I is very close to that of HDL. 3) The electrophoretic mobility of macromolecular Al-P was the same with that of HDL. 4) No relationship was found between Band II which corresponds to Al-P with a small molecular weight and lipoprotein. 5) By treatment with receptor destroying enzyme of the serum, the electrophoretic mobility of the lipoproteins remained unchanged. Band I, however, was detached from HDL and moved toward the origin. These results suggest that Al-P bands contain a large quantity of sialic acid.

The classic dictum that pulmonary tuberculosis is a disease located predominantly in the pieces of the lunge often suggests etiologic agents other than the tubercle bacillus as the cause of isolated lesions in the lower half of the lung fields. Only when tubercle bacilli are demonstrated in the sputum is the correct diagnosis arrived at. The comparatively low incidence of this type of tuberculosis on routine admissions and out-patient clinics have often made the physician unaware of its existence. Eighteen cases were forum, representing 1.16 percent of the total cases. A higher incidence of the disease was found in the second decade of life, with predominance in the right side and a larger number of females were affected than males(2.26 percent versus 0.98 percent). The respiratory symptoms, such as cough, sputum, hemoptysis and chest pain was more prevalent in the selected group than in the general group of tuberculosis. A tendency toward massive expectoration of tubercle bacilli was one of four observation. The incidence of cavity formation was higher in the lower lung field tuberculosis than in the general group of tuberculosis (61.1 percent versus 25.2 percent). The cavities in many cases were smaller than 4cm single and they had rarely fluid level (1case). The association of the parenchymal lesions with pleural effusion was also rare(1 case). No atelectasis was found. Endobronchial disease was present in 6 patients(33.3 percent).

6. α-Fetoprotein in infantile diseases

- Abstract
Infants with neonatal hepatitis, congenital biliary atresia, Wilms' tumor or embryonal carcinoma were immunologically analysed for α-fetoprotein in the sera. This antigen was confirmed in these ill infants, except in Wilms' tumor. By micro-Ouchterlony reaction and radial diffusion technique, the following results were obtained. The fetal protein was not detected in newborns after about 2 weeks of age. A remarkable rise up to 14.8–22.6 mg/100 ml of α-fetoprotein was observed in some instances of neonatal hepatitis, whereas in 8 infants aged 2 to 3 months with congenital biliary atresia, its values were within the range of 1.5 to 5.5 mg/100 ml. Two cases of gastric cancer as well as 24 cases of adult hepatomas revealed positive α-fetoprotein in their sera. The probable causes for the occurrence of α-fetoprotein in infantile diseases and in a few cases of gastric cancer are also described.

7. [Studies on paragonimiasis in Ko-Hoong County, Korea (Report 1.): An epidemiologic survey for human paragonimiasis by the use of intradermal screening test]

Authors carried out the survey for human paragonimiasis in Ko-Hoong County, Chun-Nam Province, Korea from July to September 1964 by the use of intradermal screening test with Veronal buffered saline antigen. Eight myuns (village) out of 13 myuns of this county were selected for the survey, regarding balanced selection. The number of 2,916 primary school children of high class and 764 residents of all age groups in these areas were examined (total 3,680; male 1,961; female 1,719). The following results were obtained in this survey. 1) As a whole, 20.1 per cent of the examiners showed positive reaction, with sexual difference of 20.9 per cent in male and 19.1 per cent in female. 2) In regional differences of positive rate, the highest rate(50.1 per cent) was showed at Po-Doo myun and lowest rate(3.6 per cent) was showed at Do-Yang myun. 3) In age group, the highest positive rate was showed in 16 to 20 age group (46.7 per cent). 4) In regional comparison of paragonimiasis infectivity of whole nation (province and county), Ko-Hoong County as nearly same as the Hea-Nam County, Chun-Nam Province follows next of Cheju Province, the highest prevalance area of paragonimiasis in Korea, in concentration of prevalence of paragonimiasis.

The alkaline phosphatase isozyme pattern of blood plasma and liver was studied in rats which were subjected to various treatments such as tumor inoculation, liver injuries and the ligation of bile duct, by means of agar-gel electrophoresis, gel-filtration on Sephadex G-200 and anion-exchange column chromatography on DEAE-Sephadex A-25. Acid phosphatase activity was also determined in some of these animals. The following results were obtained. (1) The elevated level of plasma alkaline phosphatase was shown in rats whose livers were experimentally damaged. This elevation was most remarkable when hepatobiliary system was occluded. (2) The alkaline phosphatase activities of plasma and liver, and plasma icterus index were found to increase in parallel in rats with liver metastases of tumor or with their bile ducts ligated, but not in those with the liver injured by CCl4 treatment. (3) By agar-gel electrophoresis, four kinds of isozyme pattern of this enzyme were observed in this experiment. (4) After gel-filtration on Sephadex G-200, rat serum proteins were separated into three peaks and alkaline phosphatase activity was found only in the second peak. The enzyme was further separable into two peaks when the second peak eluent from the rat with damaged liver was chromatographed on DEAE-Sephadex A-25. (5) In rats having ligated bile duct, the acid phosphatase activity of plasma showed no noteworthy changes throughout the entire period.

The question of infectivity and prevalence of Clonorchis sinensis in Cheju province (Quelpart Island) was arisen to authors since the several surveys on the endemic diseases were performed in this island. Therefore, authors decided to solute this question. Then the survey on the possible second intermediate host of Clonorchis sinensis was performed and reported already with negative finding. At present time, authors carried out the collection of all kinds of fresh-water snails through all areas of this island, to confirm the presence or not of Parafossarulus manchouricus, the only first intermediate host of Clonorchis sinensis in Korea. And the following results were obtained. 1)The fresh-water snails collected in the survey were Semisulcospira libertina Gould and Lymnaea ollula Gould. 2)Parafossarulus manchouricus Bourguigant was not collected in this island. 3)It is confirmed that the Clonorchis sinensis can not be prevalent in this island.

Authors carried out experimental study on chemotherapy with oral administration of Dithiazanine iodide (D.I.) and Bithionol sulfoxide(B.S.) in rabbit clonorchiasis. And the following result was obtained. 1)In change of the E.P.G (eggs per gram feces) by D.I. administration, it was rather increased in early stage of the administration than prior to administration, and thereafter decreased gradually. 2)In the change of the C.S. worm body by D.I. administration, there was not only prominent change of supporting tissue but also the change of reproductive organ was found. 3)In considering the wormicidal effect of D.I. from detecting rate of survival worms, the effect was slight in group of 100 mg(80 mg/kg) per day dosage, but the effects were very excellent and almost complete by proper times of administration in groups of 200 mg (130 mg/kg) per day or higher dosage. 4)But the side effect and intoxication sign of D.I. were appearent in groups of 200 mg or higher dosage. 5)By B.S. administration, E.P.G. was decreased gradually. 6)In changes of the C.S. worm body by B.S. administration, prominent inhibitory chnnge was seen in egg formation ability. 7)Slight wormicidal effect of B.S. was observed in groups of 140 mg (100 mg/kg) per day or higher dosage. 8)Side effect and intoxication sign of B.S. were found little in groups of 140 mg or lesser dosage.

12. PULMONARY TUBERCULOSIS, INFECTION RATE AND lNCHDENCE RATE AMONG THE TUBERCULIN NEGATIVE GROUP OF KOREAN UNIVERSITY WOMAN STUDENTS

Tuberculosis control is one of the major function of the University Health Service in Ewha Women's University. The purpose of this study was to investigate the incidence of pulmonary tuberculosis among the students whose tuberculin reaction was converted to positive during their school life. During period of 1956~1965, total 20,388 students were observed. At their entrance to The University the tuberculin-negative students were followed up by tuberculin test every 2 months until they were converted to positive for 4 school years. The tuberculin-positive converters were given chest X-ray examination every 3 months for one year after their conversion. The following results were obtained. 1) 94.4% of the students were tested for tuberculin reaction. 2) The infection rate at entrance of The University was 92.0%. 3) The conversion rate of tuberculin reaction among the negative students was 35.6%; the lowest was 2.5% in the freshman and the highest was 13.3% in the junior. 4) Out of 445 students who had been converted to positive,7 showed pulmonary tuberculosis; the prevalence was 1.5%.
