Abstract

The present results of our treatment of acute lymphoid leukaemia patients are summarized: 7 out of 20 randomized patients given active immunotherapy after chemoradiotherapy are still in complete remission after periods varying from seven to ten years (compared to none in the control group). The actuarial results on 100 patients show remission and survival curves presenting a plateau between three and five years for a certain percentage, suggesting a possible cure. Several parameters studied in 200 patients indicate that the factors affecting this percentage are age, cytological type, volume of the tumour, and the localization of leukaemic cells in certain areas. Experiments with L1210 leukaemia show that immunotherapy enhances the effect of chemotherapy when administered after chemotherapy but decreases it when administered before, which is in favour of the use of the sequence chemotherapy-immunotherapy clinically. more...

SUMMARYMany drugs have been recently tested in the treatment of leukaemias and hematosarcomas. Some of them have been found active: Dibromomannitol in chronic myeloid leukemia, Leurocristine in lymphoblastic diseases, Vincaleucoblastine and Methyl-Hydrazine in reticulohistiocytal diseases, and Methyl-Glyoxal-Bis (Guanylhydrazone) associated with 2-Hydroxystilbamidine in acute granulocytic leukaemias. Abstract

A combination of prednisone, vincristine, and 'Rubidomycin' was given to 11 children with untreated acute lymphoblastic leukaemia, and complete remissions were obtained in all cases. The quality of the remissions was striking, and detailed investigation to detect persisting leukaemic cells—consisting of leucoconcentration, examination of marrow from six different sites, examination of the cerebrospinal fluid, and estimation of ornithine-carbamyl-transferase levels—was negative in 9 out of 11 cases. The same combination given to 5 children with acute lymphoblastic leukaemia during their first or second relapse induced remissions in 4 cases. Detailed investigation was negative in 3 cases. 3 out of 5 adults treated passed into remission, and in 2 of these cases the detailed investigation was negative. This drug combination thus represent a striking advance in the treatment of primary acute lymphoblastic leukaemia because of both the high frequency of remissions and the quality of the remissions—that is, the reduction of the leukaemic-cell population. Owing to the rapidity with which remissions are induced, the neurological side-effects of vincristine and the cardiac effects of rubidomycin can be avoided. In acute leukaemia complicating lymphosarcoma only 2 remissions were obtained in 6 patients treated. Use of this drug combination is commonly complicated by myeloid aplasia; but infectious complications can be avoided if the patient is placed in a pathogen-free room. Abstract

Forty-three patients with chronic myeloid leukaemia have been treated with hydroxyurea in order to be subjected to leucopheresis for white cell transfusions. Hydroxyurea decreases leucocytosis when it is administered and the blood granulocyte number increases soon after the drug is stopped. The survival of the patients is not different from the survival of the patients treated with conventional chemotherapy (busulphan, mitobronitol) and it is superior to the survival of patients treated with external radiotherapy or with (32)P. Half of the patients were subjected to splenectomy during first remission for a phase II trial. They were not randomized, but the distribution according to age was similar in the two groups. A slight difference appears in favour of splenectomy so far as survival is concerned, but there were three post-operative deaths out of 18 patients. We conclude that a phase II trial on the value of splenectomy is indicated ethically, but that the patients should be operated on and nursed in a microbiologically controlled environment. more...

Abstract

Resume L'immunotherapie doit avoir une double visee: preventive et curative. L'immunotherapie preventive consiste aeviter que les traitements, notamment l'hormonotherapie et la chimiotherapie, ne reduisent les defenses immunitaires; les auteurs suivent ces defenses par des vaccinations repetees au BCG et montrent que certains composes, certains modes d'administration, peuvent les inhiber, tandis que d'autres les laissent intacts. L'immunotherapie a visee curative , peut etre active, passive, ou adoptive. L'immunotherapie active peut consister en stimulation non specifique des defenses immunitaires par le BCG, ou en vaccination par des cellules neoplasiques; les auteurs appliquent ces methodes aux leucemies aigues pendant les remissions. L'immunotherapie passive consiste en l'administration de serum et de gammaglobulines. L'immunotherapie adoptive peut consister notamment en l'utilisation de la reaction du greffon contre l'hote; celle-ci est employee dans le cas de greffe de moelle osseuse ou de transfusion de globules blancs. Les resultats obtenus dans le traitement des leucemies aigues sont rapportes. Abstract

A patient suffering from acute lymphoblastic leukemia in the third progressive phase, refractory to the majority of available treatments, was given total-body irradiation of 800 r. dosage, after administration of methyl-nitro-imidazolyl-mercaptopurine (300 mg. daily for 4 days). One week after irradiation, he received an infusion of a mixture of equal parts of bone marrow from six donors (mother, sister, father and three brothers), a total of 5.8 x 1010 nucleated cells. Myeloid restoration began 2 weeks after the infusion and continued to progress rapidly. The reality of the graft was demonstrated by study of the erythrocytic antigens. Nearly all the erythrocytes found in the blood after 6 months belonged to the phenotype of one of the male donors. Studies of chromosomes and leukocyte sex chromatin suggested that a small proportion of leukocytes were being produced by the cells of one of the female donors. The patient was found to produce beta 2A, beta 2M and gamma globulins of the Inv type of the donors, suggesting that lymphoid cells may also have been successfully grafted. A skin graft from the male donor whose marrow graft had apparently been accepted was still perfectly intact 7 months after grafting, whereas the grafts from other donors were rejected, indicating a specific tolerance to tissues from this donor. Previous studies of compatibility between the recipient and the various donors by a histocompatibility test described by us, or by means of similarities in leukocyte antigens, indicated that the accepted donor had been antigenically more nearly compatible than the others. What appeared to be a secondary syndrome became manifest 1 week after marrow transfusion; it consisted of weight loss, digestive disorders (anorexia, nausea, vomiting, diarrhea), hepatic disturbances (hepatomegaly, increased serum concentrations of various enzymes), transient polyadenopathy (histologically composed of histiocytic proliferation and lymphocytic aplasia), desquamative erythrodermia, eosinophilia exceeding 50 per cent, and superinfection (including a probable miliary tuberculosis). The syndrome was controlled by careful symptomatic treatment; its intensity gradually abated. Reactivation of the secondary syndrome was accomplished by reinjection of leukocytes from the donor whose graft had been tolerated. This was controlled by Δ-1-cortisone and symptomatic treatment. The patient was still alive and in apparently complete hematologic remission from the leukemia 12 months after the treatment described above. Abstract

Fifty-two patients suffering from severe neutropenia were treated with leukocyte transfusions. Except for two of these patients, who received cells from normal donors, the cells transfused were obtained from patients with chronic myeloid leukaemia. In 50 per cent of the patients the transfusions were followed by disappearance of fever and associated signs of infection, and a rise in the leukocyte count, which was dependent on the establishment of a graft by the transfused cells. The rise in leukocyte count occurred when chronic myeloid leukaemia cells were transfused into the recipients. The beneficial symptomatic effects of the transfusions were obtained only when a sufficiently large number of cells was given. In twenty-four patients with acute leukaemia given these transfusions nine remissions were induced by this treatment. No anticancer effect was noted in diseases other than acute leukaemia. The antileukaemic effect is related to a graft versus host reaction. The reaction produced by the transfusions of cells was identical with the secondary syndrome that complicates the grafting of allogeneic bone marrow in man. The frequency of this syndrome did not depend on the underlying disease but, as with the antileukaemic effect in acute leukaemia, on the number of cells transfused. Incubation of the cells to be transfused for two hours at 37 °C. reduced the frequency and severity of the secondary syndrome without affecting their ability to produce a symptomatic effect. Storage of the transfused cells at −70 °C. reduced the severity and frequency of the secondary syndrome but also the value of these transfusions to combat the agranulocytosis syndrome. more...

Abstract

Allogeneic bone marrow grafts carried out after previous administration of antilymphocytic serum alone were attempted in 16 patients. Of these, six had acute myeloblastic leukaemia, four acute lymphoblastic leukaemia, and one a blast cell crisis in polycythaemia vera. Ten of these patients were in an overt phase of the disease and resistant to chemotherapy, while nine had complete agranulocytosis. In five of these patients erythrocyte and leucocyte antigenic markers demonstrated the establishment of the graft. One patient had thalassaemia major, and four others had aplasia of the bone marrow, in one case due to chloramphenicol poisoning and in another to virus hepatitis. The grafts were successful in the last two patients and transformed their clinical condition.No signs of early acute secondary disease were noted in any of the patients, either when the donor had been given antilymphocytic serum or when he was untreated. The grafts had no adoptive immunotherapeutic effect on the acute leukaemia. These observations have clearly shown that antilymphocytic serum has an immunosuppressive effect in man when it is used alone. more...

Abstract

The recent information on tumour cell and haemopoietic stem cell kinetics has led us to replace, for phase or cycle dependent chemotherapeutic agents, continuous doses by large intermittent doses. An experimental study has shown that, for most products, the superiority of the antitumoral effect is more important with a single administration of the same total dose then when given over a period of 20 days. Some drugs show that a single dose has a less immunosuppressive effect than the prolonged low dose administration; this has been confirmed clinically. Clinical studies have enabled drug selection to be rationalized according to the tumour doubling time and histological type. The results of intermittent high dose therapy are encouraging and their tolerance is remarkably good. more...
