For up to one year, 22 patients with occular hypertension or glaucoma wore a seven-day sustained release device (Ocusert) containing pilocarpine with acceptable retention and no important side-effects. Comparison of prestudy medical regimens using eye drops with those defined as minimum controlling concentrations indicated a high rate of over-medication. The hypotensive effect of two Ocusert strengths was compared to four pilocarpine drop concentrations. Concurrent administration of epinephrine drops did not interfere with Ocusert action and had the anticipated additive effect. Further study indicated the need for continuous wearing and delimited the effective duration of action for single Ocuserts to seven days.

Two siblings, a 56-year-old white man and a 44-year-old white woman, with systemic mucopolysaccharidosis I-S (Scheie's syndrome) developed acute glaucoma successively in each eye. Their management was complicated by an inability to visualize the anterior chamber angle due to corneal clouding by acid mucopolysaccharide (AMP) deposition. The mechanism of their glaucoma resembled angle closure, which may have been related to thickening of anterior ocular structures due to abnormal AMP storage.

Comparative intraocular pressure measurements made with the pneumatonograph and the Goldmann tonometer showed close agreement with a statistically higher mean pressure when using the pneumatonograph. Significant changes in mean pressure differences between the two instruments were seen upon changing the pneumatonograph calibration and with change in the order of testing (which instrument was used initially). However, transient biologic changes in an individual eye producing minute-to-minute fluctuations in intraocular pressure may be equal to or larger than differences in measurement between instruments.
