Abstract

The relation between left ventricular end-diastolic pressure and stroke work index was inconsistent during increases in afterload with intravenous angiotensin in six conscious dogs and on several determinations in individual dogs. 6. Electrocardiogram of the Athlete: Alterations Simulating Those of Organic Heart Disease

Abstract

Routine medical evaluations of well-trained endurance athletes frequently disclose electrocardiographic abnormalities suggestive of organic heart disease. First- and second-degree atrioventricular (AV) block, altered ventricular conduction, criteria for atrial enlargement or ventricular hypertrophy, and repolarization abnormalities are commonly found. On the basis of such abnormal tracings, the athlete may be advised to refrain from his customary strenuous exertion, even though the results of physical examination are normal and there is no history suggesting cardiovascular disease. In fact, such athletes are usually quite fit for the activity for which they train so vigorously. The electrocardiographic findings probably reflect the anatomic and electrophysiologic effects of training on the cardiovascular system. It is important for physicians to recognize these changes as normal variants when seen in well-trained athletes.

7. Rapid Active External Rewarming in Accidental Hypothermia

Abstract

Three patients with marked hypothermia resulting from exposure to cold after heavy alcohol consumption were rewarmed in less than eight hours by means of a hyperthermic mattress. Electrocardiography and physical abnormalities reverted to normal during the rewarming period. There were no sequelae. We conclude from the literature and from our experience with these three patients that immediate, active, rapid external rewarming is a simple and effective method of therapy in accidentally cooled patients.

8. Methotrexate-Induced Hepatic Injury in an Adult: A Case Report

Abstract

In 1948 methotrexate, a folic acid antagonist, was introduced by Farber and his associates as a chemotherapeutic agent for the treatment of leukemia.1 Since that time many clinical and experimental studies have shown the folic acid antagonist to be of variable benefit in the treatment of a number of neoplasms,2-7 as well as the epidermal hyperplastic disorder, psoriasis.8The folic acid antagonists have produced undesirable side effects in many cases because of a narrow margin of safety between therapeutic and toxic dosages. Anemia, leukopenia, thrombocytopenia, stomatitis, and gastrointestinal ulcerations have been reported following this mode of therapy.1,2,4,9,10,12Skin rashes, alopecia, and erythrocytic macrocytosis with a megaloblastic bone marrow have also occurred.1,2,11,12A rare complication following treatment with folic acid antagonists is periportal fibrosis of the liver. In 1955 Colsky reported the occurrence of hepatic fibrosis in children with acute leukemia, following treatment with methotrexate.

9. The extrahepatic uptake of198Au as an index of portal hypertension

Abstract

A study was undertaken to determine if extrahepatic recticuloendothelial uptake of198Au during hepatic scanning could be used as a measure of the portal venous pressure in patients with chronic diffuse liver disease. Twenty patients were studied by history, physical examination, liver function studies, liver biopsy, esophagoscopy,198Au hepatic scan and wedged hepatic vein pressure measurements. Patients were separated into 2 groups: Group A—patients with neither spleen or lumbar vertebral bone marrow uptake of198Au; Group B—patients with significant spleen and/or vertebral bone marrow uptake of isotope. Each of the 6 patients in Group A had a hepatic wedge pressure under 18 mmHg, and none had varices on esophagoscopy. All 14 patients in Group B had hepatic wedge pressures of 18 mmHg or more. Only 8 Group B patients had varices on esophagoscopy. These data suggest that the198Au hepatic scan is a safe and valuable procedure for estimating the degree of portal venous hypertension in patients with diffuse chronic liver disease. In these 20 patients, if spleen or vertebral bone marrow uptake of isotope was present, the portal venous pressure was 18 mmHg or greater. If spleen and/or bone marrow uptake was lacking, then the portal venous pressure was below 18 mmHg.

11. Appearance of atrial sound after reversion of atrial fibrillation.

Abstract

To determine the incidence and time of onset of the atrial sound after reversion of atrial fibrillation in patients with myocardial disease, 12 patients with arteriosclerotic heart disease and 4 patients with primary myocardial disease were followed with serial auscultation as well as daily simultaneous apical phonocardiogram, apex cardiogram, and lead II electrocardiogram. Their average age was 52·3 years. Each patient had been in atrial fibrillation for 6 months to 2 years' duration; 13 were reverted to sinus rhythm with DC capacitor discharge and 3 with quinidine sulphate. (). Abstract

The article focuses on the result of a survey conducted concerning the five wishes of the youngsters of today. The survey considered 160 pupils from the seventh and eighth grade in the Wichita, Kansas school. Based on the result of the survey, fifty percent of the pupils wish for world peace, 20 percent of them wish for good career in life, and the remaining 30 percent wish for good education and a better standard of living.
