Abstract

Acetazolamide used alone produced a minor antihypertensive effect. Although the addition of reserpine enhanced the hypotensive response, the therapeutic combination of acetazolamide and reserpine appears to be less effective than combined thiazide-Rauwolfia regimens. The antihypertensive effectiveness of diuretic agents is roughly proportional to their natriuretic abilities. Their antihypertensive mechanism is briefly discussed. [ABSTRACT FROM AUTHOR]

Abstract

The present report describes five cases of transmural myocardial infarction occurring in patients without occlusive coronary artery disease or other discernible abnormalities. It is apparent from these cases and others described in the literature that such patients may present with or without angina and, in some, the clinical course will be complicated by recurrent infarction and/or significant residual myocardial dysfunction. At present the exact incidence and natural history of this syndrome is unclear. Undoubtedly the increasing application of coronary arteriography will identify many more such patients. Delineation of the genesis and the full clinical spectrum of myocardial infarction without coronary artery disease warrants further investigative attention.

Abstract

Guanethidine is a sympatholytic agent with a hypotensive potency similar to that of the ganglion-blocking compounds. Its effectiveness is further enhanced by the concomitant administration of thiazide derivatives.The decrease in blood pressure obtained with guanethidine is chiefly a postural response.A lower incidence of side effects has been observed with guanethidine than with the ganglionoplegic drugs.Results of a study by the authors have shown that guanethidine is particularly promising for patients with the more severe elevations of diastolic blood pressure.

Abstract

The patterns of renal function in renal parenchymal and renovascular hypertension are considered. In hypertension due to polycystic kidney disease the pattern indicates a proportional decrease of renal blood flow and of glomerular filtration rate. In hypertension associated with acute glomerulonephritis and in the early stages of chronic glomerulonephritis, the filtration fraction is characteristically low. In hypertension secondary to renal artery stenosis, split renal function studies reveal excessive tubular reabsorption of water in the affected kidney.

Abstract

Clinical observations with a new antihypertensive agent, alpha methyldopa, are reported. The drug possesses a rather unique pharmaco-dynamic mechanism, i.e., inhibition of decarboxylase activity with secondary depression of norepinephrine biosynthesis. It is as yet uncertain, however, whether this inhibitory effect accounts for the antihypertensive response obtained. Although further investigations are necessary, it is hoped that these new agents will ultimately enhance the antihypertensive armamentarium.
