6. Transduodenal sphincterotomy with strict indications

Abstract

Of 2,962 biliary operations (including 1,075 choledochotomies) performed from 1950 to 1965, 113 transduodenal sphincterotomies were carried out, under strict indications for severe cholelithiasis associated with sclerosis of the sphincter of Oddi or chronic pancreatitis. In twenty-nine cases an impacted stone in the ampulla of Vater was the main cause of the symptoms. In seven cases the operation was performed for rupture of a hydatid cyst into the main bile ducts with or without sclerosis of the sphincter of Oddi. Besides the previous history and operative findings, the final indication for sphincterotomy was decided by intraoperative cholangiography and, in certain cases, by cholangioscopy. Total section of the sphincter was performed in 70 per cent of the cases and partial section in 30 per cent. The mortality was 7.07 per cent. Late results have been very satisfactory: 87.9 per cent excellent, 8.8 per cent good or moderate, and 3.3 per cent failures.

7. On the Synthesis of Unknown-Input Observers and Inverse Systems by Recursive Algorithms*

Abstract

In the present work the synthesis of unknown-input observers and inverse systems is approached in the state space by using some iterative computational procedures which have been previously developed by the authors in order to detect the most important structural properties of linear time-invariant dynamical systems. A substantial feature of this approach is the possibility of taking into account the constraint of being stable for the synthesized observer or inverse system. Moreover new very simple necessary and sufficient conditions for unknown-input state observability and invertibility, both without and with stability constraints, are obtained.

Abstract

Cable-guided winchdown, ready-launch and on-deck topping operations were evaluated in the UH-2A/B helicopter and found to be feasible. Flight test data for cable-guided winchdown and ready-launch operations compared favorably with analytical predictions for land-based operations. The established correlations were subsequently used to predict ship based winchdown characteristics. Aircraft cable guided ascents and retrievals were made in winds up to 50 knots without difficulty. Smooth and positive lift offs were made by holding cable tension between 600 and 1000 pounds. A cable tension of 1000-1500 pounds generally resulted in smoother retrievals due to the ability to maintain steady cable load and rate of descent. The rate of descent considered most comfortable from the pilot's standpoint was 150 ft./min. Static controllability was evaluated at cable angles up to 20 degrees longitudinally and laterally and at cable tensions from 500 to 2300 pounds. Control positions varied only 10 percent from trim for the various combinations of cable angles and cable tensions investigated, allowing ample control margins even at extreme conditions. The dynamic stability characteristics of the helicopter during winchdown operations are acceptable. (Author)
