223 sera of patients having a colonic cancer or other diseases were examined for the presence of antibodies agglutinating tanned sheep erythrocytes coated with saline extracts of human colonic cancers. Some patients' sera were also investigated for precipitating antibodies. Many positive results were found, mainly but not only, in colonic cancer patients' sera. By absorption studies the antibodies found in these sera have been shown to be directed against an antigen present not only in the tumours but also in normal and foetal tissues as well as in the microsomal fraction of the intestine. 1. According to the world's literature the incidence of error in the roentgenologic diagnosis of carcinoma of the colon (excluding the rectum) is approximately 10 per cent. 2. Various causes of error are reviewed, discussed, and suitable case material illustrated. 3. The authors recommend large field, high KV, spot compression roentgenography supplemented by overhead radiography with high KV technics in suitable projections as the most simple and most accurate method which can be used routinely for investigation of the colon. This method has proven highly effective for diagnosis of all diseases and abnormalities of the colon, including polyps and carcinoma. Investigation of the barium filled colon with compression technics during fluoroscopy is stressed as an extremely valuable adjunct for examination of the colon. 4. Palliative treatment is necessary in approximately 67 per cent of patients in whom a diagnosis of carcinoma of the colon or rectum is made. Extending the scope of surgery will not improve this figure. Decreasing the current 80 per cent delay in the diagnosis of the disease should increase the number of patients surviving five years. Before palliative measures are instituted it must be definitely determined that a curative procedure still could not be done. In case of doubt exploratory celiotomy is indicated. When the patient is beyond the curative stage there is still much that can be done if the physician takes an active interest and assumes a positive approach to the problem. The results are often rewarding with survivals beyond 5 years not uncommon. Four things can be accomplished by palliation: relief of pain, relief or prevention of obstruction, prolongation of a useful life, and support of the patient's morale. Before the patient is subjected to possible additional suffering from any palliative measure, the possibility of accomplishing these objectives should be weighed carefully. Palliative exenterations and similar radical palliative procedures have little to offer in prolonging the patient's life and relieving his pain. A "preventive" colostomy should not be established in the expectation that obstruction might develop at some time in the future. A "preventive" colotsomy is an unnecessary colostomy. When properly and sensibly employed, drugs, diet, radiation, fulguration, hypnosis, and surgery have much to offer as palliative measures in the management of carcinoma of the colon and rectum. The experience gained from the surgical treatment of 1,000 cases of carcinoma of the colon has led us to the following conclusions: 1. Preoperative sterilization of the bowel contents is essential and must be controlled by antibiotic sensitivity studies. 2. Operative procedures must be designed to prevent dissemination of tumor cells through the lymphatics and veins and via the intestinal lumen. 3. A more radical resection of the colon will reduce the incidence of second primary lesions. 4. Careful long-term follow-up studies and autopsy reports on all patients dying following resection for carcinoma will ultimately prove or disprove the validity of these observations.
