This paper describes the occurrence in seven patients of localized ulceration of the lower lip due to the oral administration of various drugs. It is suggested that such ulcers achieve importance by affecting the same area of the lip in the same age group as squamous-cell carcinoma from which they must therefore be distinguished. It has been found that smoking is associated with extraversion. The present paper extends this finding by demonstrating that the association holds internationally: in nations where the level of extraversion is high, the consumption of tobacco is also high. It is not certain whether smoking should be regarded as a comparatively superficial manifestation of the extraverted personality, or whether there is some unknown constitutional factor responsible for extra- version, the tendency to smoke, and the predisposition to cancer. In order to determine the relationship between all forms of pemphigus and malignancy or thymoma, a search of the world literature was carried out. Fifty-nine case reports or isolated citations for pemphigus with malignancies or thymomas were noted, including twenty-one reports published before the histological separation by Lever of pemphigus and bullous pemphigoid in 1953. Of these cases, twenty-six were acceptable for analysis. In approximately 54% of patients surveyed with these associations, the malignancy involved the lymphoid or reticuloendothelial systems. Approximately 48% of the patients with pemphigus and malignancy or thymoma had associated autoimmune diseases. Pemphigus with malignancies or thymomas may be related through some intervening variables such as immuno-suppressive therapy or the antigenic and cross-reacting nature of the primary tumour, or through an immunological consequence of associated autoimmune disease. Although reports of pemphigus and concurrent malignancy or thymoma have appeared sporadically in the literature (Skog, 1964; Beutner et al., 1968; Krain et al., 1973), added impetus to the study of these associations has been provided by the demonstration of the production of pemphigus antibodies by a lymphoid neoplasm (Saikia, 1972). It is generally acknowledged that autoimmune mechanisms play an important though as yet ill-defined role in the pathogenesis of pemphigus (Beutner et al., 1968; Beutner, Chorzelski & Jordon, 1970), of thymomas (Stillman& Baer, 1972; Vetters et al., 1973) and of some malignant tumours (Moragas, Winkelmann & Jordon, 1970; Alexander, 1972; Saikia, 1972). In addition, immunosuppressive therapy employed in the management of patients with pemphigus and other autoimmune diseases has been incriminated as a possible inductive mechanism in the subsequent development of malignancy (Harris, 1971; Lowney, 1972; Krain & Bierman, 1973). Saikia (1973) has suggested that every patient with pemphigus should be investigated as far as is practicable to exclude the possibility of a co-existent malignancy, especially in those cases in which systemic corticosteroid therapy is unsatisfactory in controlling the pemphigus. The purpose of this paper is to review the current literature on the association of pemphigus with malignancy or thymoma and to examine critically the factors involved in these inter-relationships. An intra-epidermal epithelioma of 30 years duration invaded as squamous cell carcinoma. The tumour exhibited an unusual propensity for the production of parakeratotic debris. The potential of intra-epidermal epithelioma for invasion is reviewed. It no longer seems useful to attribute any diagnostic or prognostic specificity to the designation ‘Borst­Jadassohn epithelioma’ (Mehregan & Pinkus, 1964; Holubar & Wolff, 1969; Holubar, 1969). Rather, Pinkus & Mehregan (1964) have proposed that three different phenomena with intra-epidermal nests be designated by the names Borst, Jadassohn and Paget respectively. The Borst phenomenon is nest formation by intra-epidermal invasion from an adjacent squamous cell carcinoma or Bowen's disease. The Jadassohn phenomenon is the intra-epidermal origin of distinct nests of keratinocytes which long remain confined to the epidermis. The Paget phenomenon is the invasion of the epidermis, with nest formation, by non-keratinocytic tumour cells; instances include mammary and extramammary Paget's disease, malignant melanoma and epidermotropic metastatic adenocarcinoma of the bladder. Although this Paget phenomenon is of great biological interest, and often a concern in differential diagnosis, it does not require further consideration in this paper. This case report documents the development of invasive squamous cell carcinoma in a recurrent ‘intraepidermal epithelioma’ which initially exhibited only the Jadassohn phenomenon. The article discusses the paper "A Survey of Chromoblastomycosis in Queensland," by R.E. Powell. In the paper 35 proven cases in adult males were investigated. This paper provides pictures of lesions, and a full and clear discussion of the mycology. The author says it has been the practice at the Queensland Badium Institute to use a radon mould delivering approximately 4000 r over 7 days, and that the lesions respond well.

The past decade has opened the doors for the treatment of hepatic tumors; a disease that years before was considered hopeless and always fatal. In this paper, we have presented the newer concepts of anatomy which have led to resections of the left and right lobes of the liver, and the physiological aspects of liver regrowth. Two clinical cases are also presented; one of a right hepatic lobectomy, who lived 18 months and then succumbed to metastasis of the original carcinoma, and one of a left hepatic lobectomy, who is alive and well 18 months after surgery. The article presents a research study related to the effects of smoking on the grades of high schools and colleges students in the U.S. A physician who examined the class records of Harvard University students had found out that not one tobacco user had excelled in the class; tests proved that mental ability was impaired and that heart and general cardiac muscle strength was seriously damaged; it impaired memory and energy; slowed up the reflexes of the athlete and lowered the morale. According to a physician at Lansing, Michigan, a baby born out of a cigarette-smoking mother is a sick child, and caused of the death. According to the author, young people who smoke may lead to premature death through heart disease or cancer or both and may endangered his or her mental ability.

The article focuses on a paper regarding the disease melanotic whitlow. Melanotic whitlow probably represents some 3% or 4% of all melanomas. Approximately 80% of the patients are about equally distributed among the fifth, sixth and seventh decades of life. Patients with this form of melanoma are older than patients with other forms of melanoma. There is no significant difference in incidence between the sexes. The lesion most frequently involves the upper extremity, especially the left thumb, two-thirds of the lesions involve either a thumb or a great toe.

This article presents information on the paper "Skin Diseases in Africa," by S. Rothman that was published in a 1962 issue of the periodical "Archives of Dermatology." This is a most readable account of the author's trip to Africa and is full of fascinating observations, for example, basal-cell carcinomata are practically non-existent in the Bantu population except in Bantu albinos who almost all suffer from highly destructive lesions. Kaposi's sarcoma is very common in the Bantu and the lesion as seen in this race is identical with those reported elsewhere except that it commonly occurs in Bantu children, and here the process is characterized by massive involvement of lymph nodes preceding the appearance of skin lesions.

The article presents information on the paper "Seborrhoeic Keratoses: Pseudoepitheliomatous Hyperplasia," by Lyon Rowe that appeared in the 1957 issue of the "Journal of Investigative Dermatology." The paper describes some difficulties in the clinical and histological diagnosis of seborrhoeic keratoses, in particular that of avoiding the diagnosis of squamous carcinoma arising out of one.

The article presents information on the paper "Malignant Melanoma in Japan," by Yoshio Yoshida. The paper states that malignant melanoma is less frequent among Japanese than in the white races, but a higher proportion are found on the lower extremities, especially on the soles.

The article focuses on the research paper "Malignant Tumours Arising From the Axillary Sweat Glands," by N. Puente Duany. The heavily pigmented but otherwise normal appearing skin of a patient with diffuse melanosis secondary to metastatic malignant melanoma was examined by electron microscopy and electron microscopic cytochemistry. The findings indicate that the slate-blue colour of this melanosis is due, for the most part, to melanosomes derived from melanoma cells that have spread to and throughout the skin of the patient. Six patients with persistent nodular scabies are described. Initially the diagnosis was in doubt, and thus a lesion was biopsied in each case. The histology bore a striking resemblance to that of malignant lymphoid neoplasm in five cases. In the absence of a clinical diagnosis it was reported as such by experienced skin histopathologists. In the remaining case an acarus was seen histologically, but the type and extent of the dermal infiltrate suggested a concomitant adverse drug reaction. We feel that these histological appearances may be misleading and that even with accurate clinical details the true diagnosis may be made only on the subsequent progress of the patient. The treatment of gallbladder malignancy has been rather unrewarding because of the difficulty in establishing diagnosis in early stage. A review of 34 cases of malignant gallbladder neoplasm encountered during the past 17 years has been made in order to aide diagnosis by careful evaluation of the case history, presenting signs, laboratory findings and radiological studies. Tenderness and a mass in right upper quadrant and nonvisualization of the gallbladder by cholecystography seem to be the most frequent presenting signs for suspecting gallbladder malignancy. Recent advance of celiac angiography seems to hold promise in establishing preoperative diagnosis thus allowing initiation of early effective treatment. [ABSTRACT FROM AUTHOR]

The morphological variants of dermal fibrous histiocytomas have engendered a confusing terminology. One hundred and eighty-nine cases of fibrous histiocytomas were studied in order to define the morphological spectrum exhibited by these lesions. Based on the prevailing morphological components, fibrocollagenous, vascular, or histiocytic, an arbitrary division into three main categories was made. A fourth group was defined to include those cases in which a fully developed storiform pattern was the dominant morphological feature. The majority (15 cases) of the lesions were of the fibrocollagenous variety. Lesions with an angiomatous component (vascular variety) were the next most common (41 cases). Thirteen of the tumors were of the histiocytic variety. Twenty tumors were of the storiform variety and fulfilled many of the microscopic criteria of dermatofibrosareoma protuberans. This latter group often presents a diagnostic problem and may stimulate fibrosarcoma, aggressive fibromatosis or other connective tissue neoplasias. The combination of several clinical and pathological features are helpful in better separating this variety from the more benign forms.
