The two great modern naturalists, Linnaeus and Darwin, expressed their intuition about how best to visualize patterns of affinities, that is, morphological similarities and divergences between taxa. Linnaeus suggested that "all plants show affinities on all sides, like a territory on a geographical map," while Darwin thought that it was virtually impossible to understand the affinities between living and extinct species without a genealogical tree. Genealogical trees follow the diachronic, evolving logic of a timeline, whereas maps depict a synchronous pattern of extant taxa. Although the two seem unrelated, various naturalists made attempts to combine them. Surprisingly, these resulted in three-dimensional images that, in order to be observed, had to be projected on paper. The naturalists Max Fürbringer and Richard Bowdler Sharpe were aware of this fact, but even Darwin himself twice combined the basic intuitions underlying the two modes of representation to produce three-dimensional images. Post-chemotherapy retroperitoneal lymph node dissection (PC-RPLND) is an important element of the management of patients with residual tumour after chemotherapy for disseminated nonseminomatous germ cell tumour (NSGCT). This is a challenging procedure and the outcome varies widely between institutions. There is much debate concerning the anatomical extent of the dissection and the literature is conflicting regarding the outcome of this procedure. In this systematic review we aim to summarise the literature on the relapse rate of PC-RPLND. We performed a search of the literature of the PubMed/MEDLINE and Embase databases, in accordance with the PRISMA guidelines. Studies reporting on the relapse rate of PC-RPLND in NSGCT patients with residual tumour were eligible for inclusion. We calculated the weighted average relapse rates of included studies and assessed the risk of bias using the Newcastle-Ottawa scale. A total of 33 studies, reporting on 2,379 patients undergoing open PC-RPLND (O-RPLND) and 463 patients undergoing minimally invasive PC-RPLND (MI-RPLND) were included. The weighted average relapse rates were 11.4% for O-RPLND, and 3.0% for MI-RPLND. The rates of retroperitoneal relapse were 4.6% and 1.7% after O-RPLND and MI-RPLND, respectively. For O-RPLND specifically, the average retroperitoneal relapse rate was 3.1% after modified dissection and 6.1% after bilateral dissection. We conclude that modified template dissection is oncologically safe in carefully selected patients. Minimally invasive procedures are feasible but long-term data on the oncological outcome are still lacking. The sole diagram in On the Origin of Species is generally considered to be merely an illustration of Darwin's ideas, but such an interpretation ignores the fact that Darwin himself expressly stated that the diagram helped him to discover and express his ideas. This article demonstrates that developing the so-called "tree diagram" substantially aided Darwin's heuristics. This demonstration is based on an interpretation of the diagram and of 17 sketches found in Darwin's scientific papers. The key to this interpretation is the meaning that Darwin assigned to the graphic elements (points, lines, and spaces) he used to construct the preliminary sketches and the diagram. I argue that each of the sketches contributed to the shaping of Darwin's ideas and that, in their succession, each added new elements that ultimately resulted in the fully developed published diagram.

The main aim of the present study was to test Rogers' theory, stating that parental styles characterized by unconditional positive regard (UPR) promote healthier adults than parental styles characterized by conditional regard (CR). For both caregivers CR was found to be associated with significantly higher scores on psychological complaints than UPR (on nearly all SCL-90 scales and the SCL-total score), even when controlling for gender. Although lack of emotional warmth by the father and harsh discipline by the mother were significant predictors of SCL-90-Total (indicating state neuroticism) it should be noted that both variables only explained a small amount of the total variance. Empirical evidence was found for Rogers' theory. Others factors than merely emotional warmth and discipline play a role in the etiology of state neuroticism. For future research it is therefore recommended to include other factors, such as daily worries, temperament, and alexithymia.

Objective: • To establish the effect of distraction on the performance of cystoscopy and basic endourological tasks by using a virtual reality (VR) simulator., Subjects and Methods: • A total of 86 third-year medical students from Maastricht University, who had no previous experience in performing the tasks on a VR simulator, were randomly assigned to an intervention or control group. • All participants performed three endourological tasks on the VR simulator. Participants in the intervention group were distracted 1 min into the third task. The distraction consisted of being asked to answer questions about a medical case that had been presented to all the participants before the hands-on session. After two adequate verbal responses the conversation was terminated. • Number of traumata, number of missed lesions in the bladder and time to completion were measured by the VR simulator., Results: • Number of traumata and missed lesions, as well as time to completion were significantly higher in the intervention than in the control group with effect sizes (using Cohen's categorization) of 0.48, 0.41 and 0.50 respectively. • Nevertheless, only 9.5% of the participants in the intervention group reported feeling burdened by the distraction., Conclusions: • Distraction during the performance of endourological skills results in significantly poorer performance by medical students on all the variables measured in a controlled learning environment. • Most students do not realize they are affected by distraction. • Further research is needed to determine the impact of distraction on more experienced participants and on patient safety., (© 2010 THE AUTHORS.
