Abstract

Cardiac surgery for Jehovah’s Witness (JW) patients is considered to be high risk because of patients’ refusal to receive blood transfusion. We report a successful mitral valve replacement for recurrent mitral stenosis after OMC with minimally invasive right thoracotomy, without any transfusion of allogeneic blood or blood products. This minimally invasive mitral valve replacement through right thoracotomy was an excellent approach for JW patients.

Abstract

A 60-year-old male was referred to our institution for investigation of intractable hypertension. Coarctation of the aorta was detected by computed tomography (CT). Bilateral internal thoracic arteries played an important role as a collateral source to the lower extremity. Coronary angiography showed stenosis at the ostium of left main trunk. Ascending aorta to bilateral external iliac artery bypass grafting and off-pump coronary artery bypass grafting (OPCAB) was done simultaneously, this is because we had to harvest left internal thoracic artery as a conduit for coronary artery bypass grafting(CABG).Postoperative course was uneventful. The patient was discharged 10 days after the operation. We could perform less-invasive surgery despite the simultaneous operations.

41. [Clinical impact of partial sternotomy for off-pump coronary artery bypass].

Abstract

History of cardiopulmonary bypass has been the pursuit for less invasiveness. Currently, about 60% of primary coronary artery bypass grafting is performed as off-pump coronary artery bypass (OPCAB). We tried to determine whether OPCAB by the lower partial sternotomy could be safely performed in the elderly patients with the age equal to or over 80 years. The number of grafts was 2.4 +/- 0.8 in patients with partial sternotomy (PS) and 2.3 +/- 0.9 in patients with full sternotomy (FS). Postoperative duration of mechanical ventilation in patients with PS was significantly shorter than that in patients with FS (p <0.05). Although the differences were not significant, the incidence of wound and respiratory complications were less in patients with PS than in those with FS. Theoretically, tracheostomy and percutaneous cricothyroidotomy can be safely performed in the early postoperative period after PS. Early ambulation can decrease the complications related to prolonged bed rest. Because dislocation of the sternoclavicular joint and/or brachial plexus injury do not occur, upper extremity movement remains unrestricted and shoulder pain does not occur. Our results suggested that OPCAB using PS could become the standard procedure in the elderly patients with multiple risk factors.

Abstract

We present here the indication, procedure and results of surgical treatment of gastroesophageal reflux (GER) for neurologically impaired children. We decide its indication based on clinical symptoms and findings of upper GI series, esophageal pH monitoring and GI fiberscopy, respecting the QOL of the patient and family. Laparoscopic fundoplication has become the first choice in surgical treatment of GER because of its good results. However, an anti-reflux procedure is not effective in patients with intractable aspiration. A laryngotracheal separation procedure should be applied for these cases.

Abstract

A 66-year-old female had been treated by hemodialysis since 1996. She was admitted to our hospital with acute pneumonia in January 2001. During admission, ischemic heart disease was identified. Her condition deteriorated and organic pneumonia of the right middle lobe progressed. She recovered after 6 months and coronary arteriography was performed. A 90% stenosis was detected at the ostium of the right coronary artery. An aberrant tortuous artery arose from the distal sinus node artery, and drained into the lung network, but also partially drained to the right segmental pulmonary artery branch. The diagnosis was significant stenosis of the right coronary artery, and pulmonary pseudosequestration or pulmonary sequestration receiving arterial supply from the sinus node artery. Surgical revascularization, ligation of the aberrant artery, and partial resection of the right middle lobe were performed. However, intraoperative findings did not identify the pulmonary sequestration. This rare case of pulmonary pseudosequestration received the arterial supply from the sinus node artery, originating from the right coronary artery with a significant stenotic lesion, and developed without recurrent pneumonia.
