10. Evaluation of microdialysis as a tool for studying percutaneous drug absorption and cutaneous metabolism

Abstract

Cutaneous microdialysis is a method used to sample the extra-cellular fluid in dermal tissue. It uses a semi-permeable fibre, which is implanted in the dermis parallel to the skins surface. The fibre is perfused with a sterile physiological solution, which is collected at set periods. Low molecular weight compounds are able to diffuse freely into or out of the fibre in the direction of the concentration gradient. Once fibres have been implanted and are perfused, drug solutions can be applied to the skin surface directly above. Methyl salicylate (MeS) was the initial model drug. Occlusion of the drug site had no effect on the tissue Cmax but gave an increased AUC. Blood flow restriction using noradrenaline resulted in an increased Cmax and an increased AUC. Analysis for salicylic acid (the major metabolite) found higher concentrations than could be explained by the concentration present in the applied formulation, indicating that MeS was metabolised as it penetrated the skin. Comparisons of two vehicle formulations, one containing ethanol the other propylene glycol, showed that ethanol enhanced the transdermal absorption of methyl salicylate relative to the propylene glycol formulation. The transdermal absorption of salicylic acid and methyl salicylate was compared, salicylic acid gave the highest dermal concentrations despite being less lipophilic than methyl salicylate. MeS was introduced directly into the dermis via a microdialysis fibre in order to compare the metabolic activity of the epidermis to the dermis. Technical difficulties were encountered but the data indicated that MeS is metabolised in the dermis though to a lesser degree than in the epidermis. Ketoprofen was used to study the dialysis of highly protein bound drug, it has a literature protein binding value of 95%. Two fibres were used, a 2kDa fibre, which only dialysed free drug, and a 3MDa fibre, which dialysed both free and protein bound drug. The 3MDa fibres dialysed a higher concentration of ketoprofen however the increase was due to the increased efficiency of 3MDa fibres to dialyse free drug.

12. The Impact of an Electronic Health Record Intervention on Spirometry Completion in Patients with Chronic Obstructive Pulmonary Disease

Abstract

Spirometry is necessary to diagnose chronic obstructive pulmonary disease (COPD), yet a large proportion of patients are diagnosed and treated without having received testing. This study explored whether the effects of interventions using the electronic health record (EHR) to target patients diagnosed with COPD without confirmatory spirometry impacted the incidence rates of spirometry referrals and completions. This retrospective before and after study assessed the impact of provider-facing clinical decision support that identified patients who had a diagnosis of COPD but had not received spirometry. Spirometry referrals, completions, and results were ascertained 1.5 years prior to and 1.5 years after the interventions were initiated. Inhaler prescriptions by class were also tallied. There were 10,949 unique patients with a diagnosis of COPD who were eligible for inclusion. 4,895 patients (44.7%) were excluded because they had completed spirometry prior to the cohort start dates. The pre-intervention cohort consisted of 2,622 patients, while the post-intervention cohort had 3,392. Spirometry referral rates pre-intervention were 20.2% compared to 31.6% post-intervention (p < 0.001). Spirometry completion rates rose from 13.2% pre-intervention to 19.3% afterwards (p < 0.001). 61.7% (585 of 948) had no evidence of airflow obstruction. After excluding patients with a diagnosis of asthma, 25.8% (126 of 488) patients who had no evidence of airflow obstruction had prescriptions for long-acting bronchodilators or inhaled steroids. A concerted EHR intervention modestly increased spirometry referral and completion rates in patients with a diagnosis of COPD without prior spirometry and decreased misclassification of disease.

13. Use of Big Data to Aid Patient Recruitment for Clinical Trials Involving Biosimilars and Rare Diseases

Abstract

Patient recruitment and retention are arguably the greatest challenges to the timely execution of clinical trials. This is particularly true in the case of trials involving biosimilars and those focused on rare diseases. For biosimilars, recruitment success typically hinges on difficulty of access to reimbursement for the originator product and may be hindered by competition from studies with other biosimilars and those with new chemical entities. For rare diseases, recruitment success depends not only on finding enough patients but also on retaining them for the duration of the trial. Historical success with patient recruitment—in addition to site qualifications—needs to be considered in parallel with current market competition and results in an ever-changing patient recruitment environment. Multiple companies supporting the biopharmaceutical industry, such as Contract Research Organizations, have begun to leverage sophisticated data modeling tools and techniques to find additional patients for biosimilar and rare disease trials and/or to find patients more quickly. In addition, these companies seek to better understand the population of interest and refine their statistical assumptions when conducting clinical trials or real-world analyses. The largest and most well-established companies that support the biopharmaceutical industry now have unparalleled access to big data from clinical trials, electronic health records, medical claims, laboratory tests, and prescriptions. This paper will discuss how big data can be harnessed to aid patient recruitment with a focus on clinical trials for biosimilars and orphan rare diseases.

Abstract

External comparators, also referred to as historical or synthetic controls, present transformational opportunities for broad context and insights alongside clinical research results. The recent confluence of access to quality real-world data (RWD), advanced epidemiologic methods, and legislative directives to regulators for expanded use of RWD is increasing interest in real-world external comparators, opening the door to achieve broader generalizability and learn more, faster. In this less standardized area of research, tailored scientific methodology must be applied for external comparators to accomplish clinical development objectives. Here, we describe methodological considerations for design and illustrate how RWD comparators have been used for regulatory and reimbursement decisions.

Abstract

The oxygen isotopic compositions and inferred precipitation temperatures of quartz cements in a suite of deeply buried sandstones from the North Sea, Gulf Coast, and western Canada basins indicate that, in almost every case, the quartz precipitated from 18O-rich basinal waters. 18O-rich waters are found in strictly limited volumes in sedimentary basins. Silica transport and quartz cementation could not therefore have occurred as a result of large-scale advection of basinal or meteoric waters, because this would require very large fluid fluxes. Quartz cementation was driven by processes operating on a local scale. The isotopic data do not uniquely indicate the mechanisms of silica transport but are consistent with both diffusion and local recycling of fluid.

Abstract

PURPOSE: To evaluate the role of parasympatholytics in the resolution of acute attacks of asthma. METHODS: This study employed a prospective sequential design in which the influence of 0.5 and 1.0 mg of ipratropium bromide on peak expiratory flow rates (PEFR), hospital admissions, and length of stay (LOS) in the emergency department (ED) was evaluated. The parasympatholytic was added to a well-investigated standard therapeutic regimen that was anchored by the use of repetitive doses of albuterol, and employed pretested decision algorithms. RESULTS: One hundred and thirty-one patients received ipratropium (I) and 123 who did not (NI) served as controls. There were no significant pretreatment between group differences in gender, racial composition, clinical signs and symptoms, or PEFR. The presence of ipratropium in the regimen did not influence discharge/admission patterns, LOS, the rate of improvement of the patients, or the level of PEFR achieved. CONCLUSION: Anticholinergic agents such as ipratropium are not first-line treatments for acute asthma. They do not add any therapeutic benefit to the effects of albuterol given in divided doses over 1 hour, nor do they facilitate recovery in patients whose immediate response to sympathomimetics is impaired.

Abstract

This report characterizes the land-use around the Mussel Watch Program (MWP) monitoring sites in the Great Lakes. Land-use is an important parameter in the assessment of the nearshore environment, as contaminant monitoring sites are influenced by point and non-point source pollution, which in turn is a factor of the land-use practices and human activities in the watershed. Mussel Watch sites are categorized based on the most predominant land-use and classified as Agriculture, Undeveloped, Urban and Low-developed. Additional information on the land-use change from 1992-2006, the presence of major outfalls and the population trend since 1970 for each site is presented through reader friendly maps and graphics. While this report was written to enhance Mussel Watch Program monitoring objectives in the Great Lakes, the information contained herein may be useful to a variety of sectors including government, academic and private for interpretation of environmental or socioeconomic information.
