BackgroundTwo-year longevity is a crucial consideration in revascularization strategies for patients with symptomatic lower extremity arterial disease (LEAD). However, factors associated with 2-year longevity and risk stratification in octogenarians or nonagenarians have been underreported. ObjectiveThis paper aims to investigate the associated variables and stratify the 2-year prognosis in older patients with LEAD. MethodsWe performed logistic regression and association rule mining based on the Apriori algorithm to discover independent variables and validate their associations with 2-year longevity. Malnutrition, inflammation, and stroke factors were identified. C statistics and Kaplan-Meier analysis were used to assess the impact of different numbers of malnutrition, inflammation, and stroke factors on 2-year longevity. ResultsWe recruited a total of 232 octogenarians or nonagenarians (mean age 85 years, SD 4.2 years) treated with endovascular therapy. During the study period, 81 patients died, and 27 of those (33%) died from a cardiac origin within 2 years. Association rules analysis showed the interrelationships between 2-year longevity and the neutrophil-lymphocyte ratio (NLR) and nutritional status as determined by the Controlling Nutritional Status (CONUT) score or Geriatric Nutritional Risk Index (GNRI). The cut-off values of NLR, GNRI, and CONUT were ≥3.89, ≤90.3, and >3, respectively. The C statistics for the predictive power for 2-year longevity were similar between the CONUT score and the GNRI-based models (0.773 vs 0.760; P=.57). This narrative review aimed to summarize evidence on responses to exercise in pre-clinical Alzheimer's disease (AD) and on how long-term exercise programs work to improve neuropsychiatric symptoms and cognitive performance. We conducted a narrative review of the body of research on the benefits of long-term exercise programs in improving cognitive performance and reducing neuropsychiatric scores in patients with AD. Long-term exercise therapy appears to improve blood flow, increase hippocampal volume, and promote neurogenesis in patients with AD. Higher levels of physical activity are associated with a lower chance of developing the disease, and most prospective studies have shown that physical inactivity is one of the most prevalent modifiable risk factors for the development of AD. Exercise appears to be beneficial in improving cognitive function, a neuropsychiatric symptom of AD. Exercise has been shown to have fewer side effects, such as non-pharmacological effects and better adherence than medication. In this review, experts provided a snapshot and authoritative summary of evidence for non-pharmacological exercise in patients with AD based on the best synthesis efforts, identified the main knowledge gaps and relevant barriers, and provided directions for future research. Furthermore, experts in randomized trial designs provided practical details and recommendations for researchers working in this area, underscoring the importance of relevant topics. ABSTRACT: Objectives: Diagnosing idiopathic short stature (ISS) in Traditional Chinese Medicine (TCM) remains challenging partly because the symptoms and syndrome patterns vary among ISS patients and studies. We aimed to use factor analysis of TCM symptoms to identify syndrome patterns associated with ISS in children on the basis of TCM theory. Materials and Methods: A cross-sectional study was conducted at Taipei Tzu Chi Hospital, New Taipei City, Taiwan, from October 1, 2014, to February 28, 2016. The study included 957 individuals who were newly diagnosed with ISS through simple random sampling. The outcome measures comprised 34 TCM symptoms related to children's growth, and these symptoms were assessed using a five-level self-report questionnaire, which was completed by children and their parents. A factor analysis was conducted for the extraction of underlying factors. Results: A total of 26 symptoms had factor loadings higher than the exact threshold value (0.4), and five factors were extracted. Factor 1, comprising seven symptoms, was interpreted as "yin deficiency and fire hyperactivity of kidney syndrome." Factor 2, which included eight symptoms, was interpreted as "phlegm dampness stagnation of spleen syndrome." Factor 4, which included five symptoms, was interpreted as "liver qi invading the spleen syndrome." Factor 5, which included four symptoms, was interpreted as "spleen-stomach weakness syndrome." Factor 3, which included four symptoms, was uninterpretable. Factors 1–5 accounted for 10%, 9%, 8%, 7%, and 6% of the total variance. Conclusion: Four major TCM syndrome patterns, namely, "yin deficiency and fire hyperactivity of kidney syndrome," "phlegm dampness stagnation of spleen syndrome," "Liver qi invading the spleen syndrome," and "spleen–stomach weakness syndrome" were identified and accounted for 40% of the total variance of the 34 TCM symptoms surveyed in children with ISS. Introduction Ankylosing spondylitis (AS) is a type of chronic arthritis characterized by inflammatory spondylitis, peripheral arthritis, and enthesitis. Typically, it occurs in young adult males and has a strong association [...], Ankylosing spondylitis (AS) is a type of axial inflammation. Over time, some patients develop spinal ankylosis and permanent disability; however, current treatment strategies cannot arrest syndesmophyte formation completely. Here, we used mesenchymal stem cells (MSCs) from AS patients (AS MSCs) within the enthesis involved in spinal ankylosis to delineate that the HLA-B27-mediated spliced X-box-binding protein 1 (sXBP1)/retinoic acid receptor-[beta] (RARB)/tissue-nonspecific alkaline phosphatase (TNAP) axis accelerated the mineralization of AS MSCs, which was independent of Runt-related transcription factor 2 (Runx2). An animal model mimicking AS pathological bony appositions was established by implantation of AS MSCs into the lumbar spine of NOD-SCID mice. We found that TNAP inhibitors, including levamisole and pamidronate, inhibited AS MSC mineralization in vitro and blocked bony appositions in vivo. Furthermore, we demonstrated that the serum bone-specific TNAP (BAP) level was a potential prognostic biomarker to predict AS patients with a high risk for radiographic progression. Our study highlights the importance of the HLA-B27-mediated activation of the sXBP1/RARB/TNAP axis in AS syndesmophyte pathogenesis and provides a new strategy for the diagnosis and prevention of radiographic progression of AS.
