Treatment in hospital intensive care units (ICUs) is costly: Inpatient admissions with an ICU stay have an average cost of $61,800--roughly 2.5 times higher than an admission with no ICU [...], Objective. To estimate the effect of implementing a tele-ICU and a critical care residency training program for advanced practice providers on service utilization and total Medicare episode spending. Data Sources/Study Settings. Medicare claims data for fee-for-service beneficiaries at 12 large, inpatient hospitals in the Atlanta Hospital Referral Region. Study Design. Difference-indifferences design where changes in spending and utilization for Medicare beneficiaries eligible for treatment in participating ICUs was compared to changes in a comparison group of clinically similar beneficiaries treated at similar hospitals' ICUs in the same hospital referral region. Extraction Methods. Using Medicare claims data from January 2010 through June 2015, we defined measures of Medicare episode spending during the ICU stay and subsequent 60 days after discharge, and utilization measures within 30 and 60 days after discharge. Principal Findings. Implementation of the advanced practice provider residency program and tele-ICU was associated with a significant reduction in average Medicare spending per episode, primarily driven by reduced readmissions within 60 days and substitution of home health care for institutional postacute care. Conclusions. Innovations in workforce training and technology specific to the ICU may be useful in addressing the shortage of intensivist physicians, yielding benefits to patients and payers. To date, optimization models of endogenous technological change commonly deal with uncertainty in technological learning with risk- factor methods, i.e., by adding expected risk costs resulting from overestimating technological learning rates into an objective function with a subjective risk factor. This paper argues that another way of coping with the uncertainty, risk-constrained methods that have been ignored in existing literatures, could be more practicable (at least as a supplement) for decision support. With a simplified model, this paper explores technology development paths generated by two risk-constrained methods, and compares the two risk- constrained methods with a risk-factor method. Our study shows that comparing with the risk-factor method, the two risk-constrained methods also generate an S-shaped technology diffusion pattern, which accords with historical observations, and they can result in earlier as well as later adoption of an advanced but currently expensive technology, depending on different combinations of uncertainly levels of the technology learning rate and the upper limit on the expected risk cost. Another finding of our research is that two totally different technology development paths can both be optimal solutions, which implies that with early policy interventions there is the possibility that an economy could be led to a low-carbon economy with little additional cost. The civil engineering profession has been undergoing an identity search. With the advent of information technology and the global market, competition from engineering offices elsewhere and from other local professions is unprecedented. Technical engineering knowledge is no longer a guarantee for career success: rather a combination of numerous professional skills is required. The growing unease of civil engineers about their undefined role in the knowledge economy has led many to question civil engineering education. Although there is a push to enhance the humanistic and business aspects of the curriculum, there is a shove in the opposite direction to strengthen the technical content and keep abreast of technical change. Discussion of this socioeconomic problem within the ASCE forum has often used linear deterministic thinking that is characteristic of technical problems. Social and economic systems are usually more complex and harder to understand than technological systems. If we start making new policies to address the problems of the profession based on fuzzy, incomplete, and imprecise mental models, we may end up with counterintuitive results. Traditional methods to measure power system harmonics employ the power harmonic analyzer or a software package, such as Matlab or others, but they do have certain limitations in the graphical programming environment, applications for remote monitoring and control performance. This paper presents a virtual instrument (VI) that can carry out power system harmonic measurement based on fast Fourier transform (FFT) in remote surveillance capability via the Internet. The strategies and guidelines are also developed to enhance teaching and/or learning the basics of operating Internet-based systems. Critical issues pertaining to engineering education, such as design of course, expected learning outcomes, assessment of student performance, and course evaluation are discussed in detail. 49. Obstacle avoidance in novel visual environments improved by variable practice training

Abstract

Motor performance on simple tasks improves after training in variable practice. We asked if locomotor skill during an obstacle-avoidance task in a novel sensorimotor environment improved through training in variable practice on other complex tasks. 40 normal adults practiced gross motor skills while wearing either sham lenses, one of several visual distortion lens (constant practice), or three different visual distortion lenses (variable practice). Posttests on obstacle avoidance with novel lenses showed significantly better scores with variable practice and one of the constant groups vs sham lenses. Constant and variable practice groups did not differ. Thus, performance in a novel environment improves after training on similar type novelty, even when practice and test conditions differ. Constant practice was effective only if the subjects used the lens efficacious in training. Variable practice increases the likelihood of efficacious training when adaptive performance is required in a novel environment.
