Abstract

OBJECTIVE: To examine early pregnancy levels of serum heme oxygenase-1, soluble vascular endothelial growth factor receptor-1, B-cell lymphoma/leukemia-2 in relation to unfavorable pregnancy outcomes, including preeclampsia, fetal growth restriction, spontaneous preterm birth, gestational diabetes mellitus and fetal macrosomia. STUDY DESIGN: A total of randomly selected 140 pregnancies were included in this prospective study. Peripheral blood samples were obtained between 110/7 and 136/7 gestational weeks. All pregnancies were followed up until the outcomes were obtained and classified as preeclampsia, fetal growth restriction, spontaneous preterm birth, gestational diabetes mellitus, fetal macrosomia, and uncomplicated ones. RESULTS: Significantly high levels of early serum heme oxygenase-1 were found in the cases who subsequently developed preeclampsia, spontaneous preterm birth, and fetal macrosomia (p<0.05), in concordance with high soluble vascular endothelial growth factor receptor-1 levels of the preeclampsia group, however, B-cell lymphoma/leukemia-2 s were similar in all groups. As soluble vascular endothelial growth factor receptor-1 predicted preeclampsia at a value of 11.905 ng/mL, the cutoff values for the heme oxygenase-1 to predict preeclampsia, spontaneous preterm birth, and fetal macrosomia were 0.372 ng/mL, 0.354 ng/mL and 0.494 ng/ml, respectively. CONCLUSION: Elevated first trimester heme oxygenase-1 levels are remarkable in the pregnancies associated with preeclampsia, spontaneous preterm birth, and fetal macrosomia in comparison with uncomplicated ones. A total of 93 participants; 30 patients with GDM, 33 healthy pregnant women with normal glucose tolerance (NGT), and 30 healthy non-diabetic women (control group) at the Endocrinology and Metabolic Diseases outpatient clinic of a tertiary care university hospital were enrolled in the study. Patients with GDM and NGT were examined in terms of GDM between the 24th and 28th week of pregnancy (2nd trimester). Patient data were collected during routine examinations, and asprosin levels were measured using the ELISA method. All participants underwent testing for measurements of serum hemoglobin, insulin, C-peptide, fasting plasma glucose, and glycated hemoglobin (HbA1c) levels following a fasting period of at least eight hours.Results: Asprosin levels were higher in pregnant women with NGT and with GDM versus controls (Control–NGT asprosin, p = 0.001; Control–GDM asprosin, p = 0.001). Pregnant women with GDM had higher asprosin levels than those with NGT (p = 0.001). In detecting GDM in pregnant women, an asprosin cutoff value of > 31.709 ng/mL yielded a sensitivity of 93.3%, specificity of 90.9%, positive predictive value of 90.3%, and negative predictive value of 93.75% (p < 0.001).Conclusion: Serum asprosin levels can potentially be used as a marker in the diagnosis of GDM.Keywords: asprosin, diabetes mellitus, insulin resistance, pregnancy

Abstract

It is recognized that bacterial arthritis, which causes significant losses in the poultry industry, occurs frequently after septicemia or localized infection to the joints and is associated with many bacterial agents. This study was aimed to identify aerobic bacterial agents and to investigate the antibiotic resistance of the isolates which cause arthritis in commercial broilers in mid-west Turkey. A total of 152 joint fluid samples were collected from the one commercial broiler farm from broilers with arthritis. Joint fluid samples were inoculated in mannitol salt agar, enterococcosel agar, eosine methylene blue agar and 7% ovine blood agar after pre-enrichment and then incubated for 48 hours at 37°C. Identification of bacteria and detection of antibiotic resistance was performed with VITEK II automated system (bioMérieux, France). After the culture period, no growth was observed in 53 (34.9%) joint fluid samples; a total of 112 aerobic bacteria (52 Gram negative and 60 Gram positive) were isolated from 99 (65.1%) broilers. While pure agent was isolated from eighty-six broilers, two microorganisms were isolated from 13 broilers. The data revealed that Staphylococcus spp. (56/112, 50.0%), Escherichia spp. (44/112, 39.3%) Salmonella spp. (7/112, 6.2%), Enterococcus spp. (4/112, 3.6%), Enterobacter spp. (1/112, 0.9%) were the significant pathogens isolated from joint fluid samples from broilers with arthritis. Fifty-two Gram negative bacterial isolates were the most resistant to cephalexin (90.0%) followed by cephalotine (78.8%), doxycycline (71.2%), tetracycline and chloramphenicol (69.2%). Sixty Gram positive bacterial isolates were the most resistant to marbofloxacin (78.3%) followed by amoxicillin/clavulanic acid (75.0%), benzylpenicillin (68.3%), clindamycin, tetracycline and doxycycline (66.6%). To the best knowledge of the authors this is the first study that evaluated simultaneously the prevalence of broad spectrum beta lactamase (ESBL) producing Escherichia coli and methicillin-resistant coagulase negative staphylococci (CNS) isolated from broiler with arthritis in Turkey. Twelve (12/46, 26.1%) CNS isolates were resistant to cefoxitin and eight (8/44, 18.2%) E. coli isolates had ESBL activity, indicating that these isolates could serve as a problem for public health. Abstract

Purpose: Our aim was to evaluate the alterations of subfoveal choroidal thickness (SFCT), photoreceptor layer thickness (PRT), and retinal vessel diameter in the dark and light adaptation.Methods: Twenty-four eyes of 24 healthy volunteers (12 males, 12 females) were included in this cross-sectional and observational study. The SFCT, PRT, retinal arteriole, and venule caliber measurements were performed with spectral domain optical coherence tomography in the dark (0.0 cd/m(2)) and under light (80 cd/m(2)) adapted conditions.Results: The mean age of the participants was 30.4 4.4 years (range: 22-42). The SFCT increased statistically significantly in dark adaptation (p < 0.001), then returned to baseline values following light adaptation. The PRT, retinal arteriole, and venule caliber measurements were similar in the dark and light (p > 0.05).Conclusions: While SFCT increased, PRT, and retinal vessel diameter did not change following transition from light to dark.

Abstract

Diabetes mellitus (DM) is a important health problem that induces ernestful complications and it causes significant morbidity owing to specific microvascular complications such as, retinopathy, nephropathy and neuropathy, and macrovascular complications such as, ischaemic heart disease, and peripheral vasculopathy. It can affect children, young people and adults and is becoming more common. Ocular complications associated with DM are progressive and rapidly becoming the world's most significant cause of morbidity and are preventable with early detection and timely treatment. This review provides an overview of five main ocular complications associated with DM, diabetic retinopathy and papillopathy, cataract, glaucoma, and ocular surface diseases.

Abstract

Purpose: To measure the choroidal thickness (CT) after dynamic exercise by using enhanced depth imaging optical coherence tomography (EDI-OCT). Materials and Methods: A total of 19 healthy participants performed 10 min of low-impact, moderate-intensity exercise (i.e., riding a bicycle ergometer) and were examined with EDI-OCT. Each participant was scanned before exercise and afterward at 5 min and 15 min. CT measurement was taken at the fovea and 1000 mu away from the fovea in the nasal, temporal, superior, and inferior regions. Retinal thickness, intraocular pressure, ocular perfusion pressure (OPP), heart rate, and mean blood pressure (mBP) were also measured. Results: A significant increase occurred in OPP and mBP at 5 min and 15 min following exercise (P < 0.05). The mean subfoveal CT at baseline was 344.00 +/- 64.71 mu m compared to 370.63 +/- 66.87 mu m at 5 min and 345.31 +/- 63.58 mu m at 15 min after exercise. CT measurements at all locations significantly increased at 5 min following exercise compared to the baseline (P < 0.001), while measurements at 15 min following exercise did not significant differ compared to the baseline (P > 0.05). There was no significant difference in retinal thickness at any location before and at 5 min and 15 min following exercise (P > 0.05). Conclusion: Findings revealed that dynamic exercise causes a significant increase in CT for at least 5 min following exercise.

Introduction: To identify the prevalence and types of fetal chromosomal polymorphisms in pregnant women and to examine possible associations with screening test parameters.Materials and Methods: Fetal chromosomal polymorphism rate was investigated in pregnant women who had been implemented for invasive prenatal test in a tertiary reference center in Thrace Region of Turkey. Fetal chromosomal polymorphisms were determined and their effects on screening tests' parameters were investigated. Possible differences in the first and second-trimester screening test parameters between women; with fetal chromosomal polymorphism who had screening test results (Group 1) and those with a normal karyotype (Group 2) were evaluated.Results: Fetal chromosomal polymorphism prevalence was 5.3% (n = 101). The most common polymorphisms were identified on chromosome 9, 1, and 16 [54.5% (n = 55); 8.9% (n = 9), and 6.9% (n = 7), respectively]. The most common polymorphic variant was 9qh+ (n = 23; 22.8%). Among the screening test parameters, significantly lower pregnancy-associated plasma protein-A (PAPP-A) (p = .028) and higher unconjugated estriol (uE3) (p = .019) values were found in Group 1. Abstract

Introduction: To identify the prevalence and types of fetal chromosomal polymorphisms in pregnant women and to examine possible associations with screening test parameters.Materials and Methods: Fetal chromosomal polymorphism rate was investigated in pregnant women who had been implemented for invasive prenatal test in a tertiary reference center in Thrace Region of Turkey. Fetal chromosomal polymorphisms were determined and their effects on screening tests' parameters were investigated. Possible differences in the first and second-trimester screening test parameters between women; with fetal chromosomal polymorphism who had screening test results (Group 1) and those with a normal karyotype (Group 2) were evaluated.Results: Fetal chromosomal polymorphism prevalence was 5.3% (n = 101). The most common polymorphisms were identified on chromosome 9, 1, and 16 [54.5% (n = 55); 8.9% (n = 9), and 6.9% (n = 7), respectively]. The most common polymorphic variant was 9qh+ (n = 23; 22.8%). Among the screening test parameters, significantly lower pregnancy-associated plasma protein-A (PAPP-A) (p = .028) and higher unconjugated estriol (uE3) (p = .019) values were found in Group 1. Abstract

Objective: To investigate the possible effects of isolated fetal choroid plexus cysts (CPCs) on middle cerebral artery (MCA) Doppler parameters. Material and Methods: A total of 113 pregnant women at 18 to 22 gestational weeks in whom level 2 ultrasonography (USG) was performed between August 2017 and January 2018 were included in the study. Fifty-five women (CPC group) had isolated fetal CPC, whereas 58 women (control group) had no fetal structural anomaly. We measured the following MCA Doppler parameters of fetuses: peak systolic velocity (PSV), end-diastolic velocity (EDV), the systolic/diastolic ratio (S/D), pulsatility index (PI), and resistance index (RI). We examined whether a significant difference exist between the CPC and control groups, and between the CPC and contralateral sides with respect to the Doppler parameters. Results: We found that cysts were present in the right lateral ventricle in 49.1% (n=27) individuals in the CPC group. There was no statistically significant difference between the groups in terms of PSV, EDV, PI and RI values; however, the S/D ratio exhibited statistical significance. The MCA S/D ratio of the control group was lower than that of the values reported for both cystic and contralateral sides in the CPC group. Moreover, we did not observe any significant difference in S/D ratios between the cystic and contralateral sides in the CPC group. Conclusion: Isolated fetal CPCs tend to occur more on the right side unilaterally. At 18 to 22 weeks of gestation, MCA S/D ratios of the cystic and contralateral sides in the CPC group were higher than that in the control group. [ABSTRACT FROM AUTHOR]

Abstract

To measure the choroidal thickness (CT) after dynamic exercise by using enhanced depth imaging optical coherence tomography (EDI-OCT).A total of 19 healthy participants performed 10 min of low-impact, moderate-intensity exercise (i.e., riding a bicycle ergometer) and were examined with EDI-OCT. Each participant was scanned before exercise and afterward at 5 min and 15 min. CT measurement was taken at the fovea and 1000 μ away from the fovea in the nasal, temporal, superior, and inferior regions. Retinal thickness, intraocular pressure, ocular perfusion pressure (OPP), heart rate, and mean blood pressure (mBP) were also measured.A significant increase occurred in OPP and mBP at 5 min and 15 min following exercise (P ˂ 0.05). The mean subfoveal CT at baseline was 344.00 ± 64.71 μm compared to 370.63 ± 66.87 μm at 5 min and 345.31 ± 63.58 μm at 15 min after exercise. CT measurements at all locations significantly increased at 5 min following exercise compared to the baseline (P ˂ 0.001), while measurements at 15 min following exercise did not significant differ compared to the baseline (P ˃ 0.05). There was no significant difference in retinal thickness at any location before and at 5 min and 15 min following exercise (P ˃ 0.05).Findings revealed that dynamic exercise causes a significant increase in CT for at least 5 min following exercise.

Abstract

A 28-year-old pregnant woman presented with positive rubella IgM and IgG antibodies at initial admission and at the 11th, 13th, 30th and 36th weeks of gestation. Rubella avidity test revealed high titers and the patient was followed-up to ensure birth of a healthy infant. Rubella infection may be asymptomatic and re-infection may demonstrate a subclinical course. It is easy to make the diagnosis of rubella by following the antibody titers during pregnancy. The critical point in such patients is to differentiate the condition of disease at admission and decide accordingly. Avidity test may be a useful method for the management of such patients. Background and Objective: Posterior capsular opacification (PCO) is the most frequent complication after cataract surgery. The most used method in order to disrupt the opacified posterior capsule is Nd-YAG laser posterior capsulotomy. Our aim was to evaluate the impact of Nd-YAG laser posterior capsulotomy on ocular pulse amplitude (OPA), central corneal thickness (CCT), corneal volume (CV), iridocorneal angle (ICA), and anterior chamber depth (ACD). Materials and Methods: Thirty-three eyes of 30 pseudophakic patients who underwent Nd-YAG capsulotomy due to posterior capsule opacification, were included in this prospective cross-sectional consecutive case series. The OPA was measured with the Pascal dynamic contour tonometer. The CCT, CV, ICA, and ACD measurements were taken with the Scheimpflug imaging system (Pentacam HR). The examinations were performed preoperatively, and repeated at the postoperative first hour, week, and month. Results: The mean OPA, ICA, and ACD values did not change statistically significantly after the posterior capsulotomy (P>0.05). The mean CCT increased markedly and the mean CV increased slightly after one hour from the capsulotomy (P = 0.001 and P = 0.02, respectively), and then returned to the baseline values at the first week and first month visits (P>0.05). Conclusions: Nd:YAG capsulotomy does not significantly alter OPA, ICA, and ACD; but it causes temporary increase of CCT and CV in the short term. (C) 2014 Wiley Periodicals, Inc.

Abstract

Objective: To evaluate the effect of chronic cigarette smoking on dry eye parameters, endothelial cells, and corneal thickness. Design: Prospective cross-sectional case series. Methods: In this cross-sectional study, 49 eyes of 49 chronic smokers (smoker group) and 53 eyes of 53 age-matched, healthy non-smokers (non-smoker group) were enrolled. All participants underwent measurements of tear breakup time (TBUT), central corneal thickness (CCT) measurements with contact pachymeter and the Schirmer test with anesthesia. Corneal endothelial cells were evaluated by non-contact specular microscopy and photographed for analysis of cell density and hexagonality and the coefficient of variation in cell size. Results: The mean Schirmer score and TBUT value were significantly lower in the smoker group compared to the non-smoker group (p = 0.015) and p < 0.001, respectively). No statistically significant difference was found in the mean CCT, mean endothelial cell density, endothelial cell size, SD of size, and CV of size between smokers and non-smokers (p > 0.05). However, a lower percentage of endothelial hexagonal cells were observed in smokers than non-smokers (p < 0.001). Discussion and conclusion: Our results suggest that cigarette smoking seems to affect the Schirmer score, TBUT value, and hexagonal cells of the corneal endothelium.
