In 2021, a patient died from Marburg virus (MARV) disease in Guinea, which was the first confirmed case in West Africa. The source of the outbreak has not been identified. It was revealed only that the patient had not traveled anywhere before the illness. Prior to this outbreak, MARV had been found in bats in the neighboring Sierra Leone. In Guinea, this virus has never been found before. Therefore, the question of the source of infection arose: was it an autochthonous case with spillover from a local population of bats or an imported case with spillover from fruit bats foraging/migrating from Sierra Leone?In this paper, we aimed to conduct a study ofRousettus aegyptiacusin Guinea and determine the source of the most likely infection of a patient who died from Marburg virus disease in 2021 in Guinea.We caught bats at 32 sites in Guéckédou prefecture, including seven caves and 25 locations on the flight path. A total of 501 fruit bats (Pteropodidae) were captured, including 66R. aegyptiacus. The subsequent screening showed three PCR-positive MARV bats. We have found and described two caves in Guéckédou prefecture where MARV-positiveR. aegyptiacusroost. Sequencing has shown that at least two different MARV genetic variants circulate inR. aegyptiacusin Guinea: an Angola-like strain and MARV strains of major marburgvirus lineages.

To study the structure of human immunodeficiency virus (HIV)-1 drug resistance (DR) in patients with newly diagnosed infection. Residents of the Republic of Guinea (N = 2168) were tested for HIV using enzyme-linked immunosorbent assay (ELISA). Individuals with a positive result were further examined for the presence of viral load in blood plasma. HIV was analyzed using Sanger sequencing. The obtained sequences were genotyped using REGA (version 3.0) and analyzed in MEGA 7. Analysis for the presence of DR mutations was performed using the Stanford University HIV DR Database. Serological markers of HIV were detected in 239 people, which represents 11.02% of the entire sample. HIV RNA was detected in 58 people. The following subtypes were seen: HIV CRF02_AG (41.9%); A1 (29.1%); A3 (12.9%); URF A1_G (12.9%); and G (3.2%). In 25% of patients, at least one significant mutation was encountered leading directly to HIV DR. The mutations encountered cause resistance to NRTI and NNRTI; one case of multiple resistance was identified. Major resistance to protease inhibitor was not seen. The detection of HIV-1 mutations associated with DR, in individuals who have never received antiretroviral therapy, is a cause for concern. It suggests that: new infections are occurring with strains that already have resistance; and the expansion of resistance is not always directly associated with selective drug pressure. Among the likely reasons for the high prevalence of primary HIV DR in the Republic of Guinea, drug availability is probably the key. The consequence of this is the lack of adherence of patients to treatment, the formation and transmission of resistant variants of the virus in the population. These findings suggest the need to test patients for resistant virus variants before initiating treatment.

Rhipicephalus microplus is an ixodid tick with a pantropical distribution that represents a serious threat to livestock. West Africa was free of this tick until 2007, when its introduction into Benin was reported. Shortly thereafter, further invasion of this tick species into other West African countries was identified. In this paper, we describe the first detection of R. microplus in Guinea and list the vector-borne haemoparasites that were detected in the invading and indigenous Boophilus species. In 2018, we conducted a small-scale survey of ticks infesting cattle in three administrative regions of Guinea: N`Zerekore, Faranah, and Kankan. The tick species were identified by examining their morphological characteristics and by sequencing their COI gene and ITS-2 gene fragments. R. microplus was found in each studied region. In the ticks, we found the DNA of Babesia bigemina, Anaplasma marginale, Anaplasma platys, and Ehrlichia sp. The results of this study indicate that R. microplus was introduced into Guinea in association with cows from Mali and/or the Ivory Coast.

We caught bats in 32 sites of Guéckédou prefecture, including seven caves and 25 locations of the flight path. A total of 501 fruit bats (Pteropodidae) were captured, including 66 R. aegyptiacus. The PCR screening showed three positive MARV R. aegyptiacus, roosting in two caves discovered in Guéckédou prefecture. After Sanger sequencing and phylogenetic analyses it was shown that found MARV belongs to the Angola-like lineage but it is not identical to the isolate obtained during the outbreak of 2021.

Introduction. Acute febrile diseases kill more than 250,000 people annually in West Africa. Malaria and typhoid fever traditionally occupy most of the total structure of registered fevers. However, these data do not fully reflect the true overall disease patterns in the West African region. This is due to the fact that diagnosis is mainly based on the clinical signs of the infectious process, suggesting that a certain number of diseases may be caused by arboviruses. The detection of specific antibodies (ABs) to infectious pathogens in the blood sera of residents of a particular area is a reliable indicator of the circulation of these pathogens in a particular territory. The aim of this study was to determine the prevalence of antibodies to a number of arboviruses: Dengue (DENV), West Nile (WNV) (family Flaviviridae), Crimean-Congo hemorrhagic fever (orthonairo)virus (CCHFV), Batai (Batai virus), Bhanja (BHAV) (order Bunyavirales), Chikungunya (CHIKV), and Sindbis (SINV) (family Togaviridae) in the population of the Republic of Guinea. Material and methods. In total, a panel of 2,620 blood serum samples from people living in all landscape and geographical areas of Guinea was collected for the study. Detection of IgG antibodies was performed using an enzyme-linked immunoassay (ELISA). Results. In total, ABs to Batai virus were detected in 144 samples (5.5%), BHAV in 58 (2.2%), WNV in 892 (34.0 %), DENV in 659 (25.2 %), CCHFV in 58 (2.2 %), CHIKV in 339 (12.9 %), and SINV in 52 samples (2.0 %). Discussion. The obtained results indicate serological evidence of the spectrum of arboviruses in the population of all landscape and geographical zones of the Republic of Guinea, confirming their active circulation in this territory. Conclusion. Given the high epidemiological significance of arbovirus infectious diseases, it is an urgent task to continue studying its share in the structure of febrile diseases in the territory of the Republic of Guinea.

Lassa virus is the causative agent of a dangerous zoonotic disease distributed in West Africa. A primary reservoir host of Lassa virus is Mastomys natalensis. These mice associate closely with humans and are commonly found in villages. Consequently, previous studies of Lassa virus have focused on rural areas. The prevalence of the virus in large cities has not been studied.We conducted a study in N’Zerekore city, which has a population of approximately 300,000 residents. Small mammals were captured during a pilot study in May, and the main study was performed in August 2018. Based on the pilot study, we designed and implemented a stratified random sample to investigate the prevalence of Lassa virus among M. natalensis in N’Zerekore. The total sampling efforts consisted of 45 and 985 trapping nights in May and August, respectively. Samples of rodent tissues were screened for Lassa virus by RT-PCR.In May, we trapped 20 rodents, including 19 M. natalensis. Viral RNA was detected in 18 M. natalensis. In August, 149 small mammals were captured, including 43 M. natalensis. The prevalence of Lassa virus among M. natalensis in N’Zerekore was 23.3% (CI 95%: 11.8–38.6%). Sequencing showed that the isolates belonged to lineage IV. We detected four Lassa virus hotspots located in different parts of the city. The largest Lassa virus hotspot was found in the neighborhood of the central market, which suggests that the virus was originally introduced into the city through the market.

Intraerythrocytic protozoan parasites from the genera Babesia and Theileria may infect a wide range of animals and humans. The purpose of this study was to detect the 18S ribosomal RNA gene of Babesia spp. and Theileria spp. in ticks collected from household cows in the Republic of Guinea from 2017 to 2018 by PCR and then genotype the gene fragments by sequencing. A total of 907 ticks from 319 cows were collected in seven prefectures of Guinea (Boke, Faranah, Kankan, Kindia, Labe, Mamou and N'Zerekore). The following tick species on cattle were identified: Amblyomma variegatum (44.2%), Rhipicephalus decoloratus (34.7%), Rh. annulatus (10.3%), Rh. geigyi (7.3%) Hyalomma truncatum (2.4%), Rh. senegalensis (0.8%) and Haemaphysalis leachi (0.6%). Genetic markers for piroplasms were found in Am. variegatum, Rh. decoloratus, Rh. annulatus, and Rh. geigyi ticks, and the total infection rate for these ticks was 4.2%. The highest infection rate was found in Rh. annulatus ticks (10.9%). The piroplasms were genotyped as Babesia caballi, Theileria mutans and Theileria velifera by phylogenetic analysis of the 1150 bp 18S ribosomal RNA gene fragments. These pathogens were discovered in practically all studied prefectures in Guinea except for Mamou Prefecture. We propose that these ixodid ticks might play a major role in the transmission of piroplasm infections in domestic animals in Guinea.

Ngari virus is a mosquito-borne virus belonging to the genus Orthobunyavirus (Peribunyaviridae family). This virus is pathogenic to humans and causes severe illness. Ngari virus is present in several African countries, including Madagascar. Here, we report the detection of Ngari virus in ixodid ticks collected from cows in Guinea. A tick survey was conducted in March-November of 2018 in six regions of Guinea. The sample comprised 710 pools, with a total of 2067 ticks belonging to five species collected from 197 cows. At the initial stage, we screened a subsample of tick pools of vector-borne viruses with a multiplex genus-specific primer panel. In the second stage of the study, we narrowed the search and screened all the samples by qPCR for the detection of Ngari virus. All positive samples were sequenced with primers flanking Ngari virus-specific fragments on the S and M segments. We found Ngari virus in 12 pools that were formed from engorged ticks collected from livestock in three villages of the Kindia and Kankan regions. Sequencing of the S and M segments confirmed that the detected viruses belong to Ngari virus, and the viruses were most similar to the strain Adrar, which was isolated in Mauritania. We detected viral RNA in ticks of the following species: Amblyomma variegatum, Rhipicephalus geigyi, and Rh. (Boophilus) spp. There is no evidence that ixodid ticks are competent vectors of the Ngari virus. Most likely, the ticks obtained the virus through blood from an infected host. The study of engorged ticks can be recommended as a simpler approach for the wide screening of the Ngari virus and subsequent testing of cattle and mosquitos in those locations where the PCR-positive ticks were collected.

In 2018, a previously unknown Ebola virus, Bombali virus, was discovered in Sierra Leone. We describe detection of Bombali virus in Guinea. We found viral RNA in internal organs of 3 Angolan free-tailed bats (Mops condylurus) trapped in the city of N’Zerekore and in a nearby village.
