Abstract

BACKGROUND: It is unknown whether dietary intake of polyunsaturated fatty acids (PUFA) modifies the cardiovascular disease (CVD) risk associated with a family history of CVD. We assessed interactions between biomarkers of low PUFA intake and a family history in relation to long-term CVD risk in a large consortium. METHODS: Blood and tissue PUFA data from 40 885 CVD-free adults were assessed. PUFA levels ≤25th percentile were considered to reflect low intake of linoleic, alpha-linolenic, and eicosapentaenoic/docosahexaenoic acids (EPA/DHA). Family history was defined as having ≥1 first-degree relative who experienced a CVD event. Relative risks with 95% CI of CVD were estimated using Cox regression and meta-analyzed. Interactions were assessed by analyzing product terms and calculating relative excess risk due to interaction. RESULTS: After multivariable adjustments, a significant interaction between low EPA/DHA and family history was observed (product term pooled RR, 1.09 [95% CI, 1.02-1.16]; P=0.01). The pooled relative risk of CVD associated with the combined exposure to low EPA/DHA, and family history was 1.41 (95% CI, 1.30-1.54), whereas it was 1.25 (95% CI, 1.16-1.33) for family history alone and 1.06 (95% CI, 0.98-1.14) for EPA/DHA alone, compared with those with neither exposure. The relative excess risk due to interaction results indicated no interactions. CONCLUSIONS: A significant interaction between biomarkers of low EPA/DHA intake, but not the other PUFA, and a family history was observed. This novel finding might suggest a need to emphasize the benefit of consuming oily fish for individuals with a family history of CVD.

Abstract

Chenopodium album seedling emergence studies were conducted at nine European and two North American locations comparing local populations with a common population from Denmark. It is hypothesized that C. album seedling recruitment timing and magnitude have adapted to environmental and cropping system practices of a locality. Limitations in the habitat (filter 1) were reflected in local C. album population recruitment season length. Generally, the duration of seedling recruitment of both populations (local; DEN-COM) increased with decreasing latitude, north-to-south. In general, compared to the local population, DEN-COM recruitment at locations north of Denmark was longer and south of Denmark was shorter, and ended sooner. Generally, the local cropping system disturbances (CSD) period increased with decreasing latitude. The total duration of the CSD period was over twice as long in the south as that in the north. Recruitment at each locality possessed seasonal structure (time, number) consisting of 2-4 discrete seasonal cohorts. This may be an adaptive means by which C. album searches for, and exploits, recruitment opportunity just prior to, and after, predictable disturbances. The control of C. album seedling emergence is contained in the heteroblastic traits of its locally adapted seeds, and is stimulated by a complex interaction of light, heat, water, nitrate and oxygen signals inherent in the local environment. Our observations of complex recruitment patterns occurring at critical cropping times is strong evidence that C. album possesses a flexible and sensitive germination regulation system adaptable to opportunity in many different Eurasian and North American agricultural habitats., Comment: 82 pages; 12 tables; 20 seedling recruitment calendars; 9 data summaries; this manuscript is the product of the germination and seedling emergence working group of the European Weed Research Society

16. Re-hospitalisation predicts poor prognosis after acute exacerbation of interstitial lung disease

Abstract

Background: Several markers have been identified to increase the risk for acute exacerbation of interstitial lung disease (AE-ILD) or mortality related to AE-ILD. However, less is known about the risk predictors of ILD patients who have survived AE. The aim of the study was to characterise AE-ILD survivors and investigate prognostic factors in this subpopulation. Methods: All AE-ILD patients (n = 95) who had been discharged alive from two hospitals located in Northern Finland were selected from a population of 128 AE-ILD patients. Clinical data related to the hospital treatment and six-month follow-up visit were collected retrospectively from medical records. Results: Fifty-three patients with idiopathic pulmonary fibrosis (IPF) and 42 patients with other ILD were identified. Two thirds of the patients had been treated without invasive or non-invasive ventilation support. The clinical features of six-month survivors (n = 65) and non-survivors (n = 30) did not differ in terms of medical treatment or oxygen requirements. Of the patients, 82.5% used corticosteroids at the six-month follow-up visit. Fifty-two patients experienced at least one non-elective respiratory re-hospitalisation before the six-month follow-up visit. In a univariate model, IPF diagnosis, high age and a non-elective respiratory re-hospitalisation increased the risk of death, although re-hospitalisation was the only independent risk factor in a multivariate model. In six-month survivors, there was no statistically significant decrease in pulmonary function test results (PFT) examined at the follow-up visit compared with earlier PFT examined near the time of AE-ILD. Conclusions: The AE-ILD survivors were a heterogeneous group of patients both clinically and in terms of their outcome. A non-elective respiratory re-hospitalisation was identified as a marker of poor prognosis among AE-ILD survivors.

Abstract

Multidisciplinary meeting (MDM) is a core element in the diagnosis of interstitial lung diseases (ILD). The aim of the study was to investigate the implementation and key elements related to ILD MDMs in Finnish specialized care, which is characterized by long travel distances and a large number of small centers treating patients suffering from ILDs. An electronic questionnaire was sent to ILD experts working at five academic centers of Finland regarding the implementation of ILD MDMs with the focus on utilization of virtual communication. Responses were received from all academic centers of Finland (n = 5) whose catchment areas cover all of Finland. ILD MDMs were organized in each center approximately every two weeks and the core participants included a radiologist, respiratory physicians, junior staff, pathologist and a rheumatologist. All non-academic centers could refer their patients to be evaluated in ILD MDM of an academic center. Virtual communication was utilized by all academic centers in the implementation of ILD MDMs, being most common among small centers located in Eastern and Northern Finland. Virtual access to ILD MDM of an academic center was available in most parts of Finland, enabling small centers to benefit from the ILD expertise of academic centers.

Abstract

The extent of vegetation openness in past European landscapes is widely debated. In particular, the temperate forest biome has traditionally been defined as dense, closed-canopy forest; however, some argue that large herbivores maintained greater openness or even wood-pasture conditions. Here, we address this question for the Last Interglacial period (129,000–116,000 years ago), before Homo sapiens–linked megafauna declines and anthropogenic landscape transformation. We applied the vegetation reconstruction method REVEALS to 96 Last Interglacial pollen records. We found that light woodland and open vegetation represented, on average, more than 50% cover during this period. The degree of openness was highly variable and only partially linked to climatic factors, indicating the importance of natural disturbance regimes. Our results show that the temperate forest biome was historically heterogeneous rather than uniformly dense, which is consistent with the dependency of much of contemporary European biodiversity on open vegetation and light woodland.

The Eurasian (née European) Modern Pollen Database (EMPD) was established in 2013 to provide a public database of high-quality modern pollen surface samples to help support studies of past climate, land cover, and land use using fossil pollen. The EMPD is part of, and complementary to, the European Pollen Database (EPD) which contains data on fossil pollen found in Late Quaternary sedimentary archives throughout the Eurasian region. The EPD is in turn part of the rapidly growing Neotoma database, which is now the primary home for global palaeoecological data. This paper describes version 2 of the EMPD in which the number of samples held in the database has been increased by 60 % from 4826 to 8134. Much of the improvement in data coverage has come from northern Asia, and the database has consequently been renamed the Eurasian Modern Pollen Database to reflect this geographical enlargement. Diagnoosi edellyttää keuhkojen ohutleiketietokonetomografiatutkimusta ja huolellista erotusdiagnostista arviota erikoissairaanhoidossa. Tehokasta lääkehoitoa ei tunneta, mutta kliininen käytäntö on hoito ­antibiooteilla ja glukokortikoideilla. Keuhkofibroosilääkkeet pirfenidoni ja nintedanibi vaikuttavat ehkäisevän pahenemisvaiheita ja parantavan niiden ennustetta.

27. National data on prevalence of idiopathic pulmonary fibrosis and antifibrotic drug use in Finnish specialised care

Abstract

Introduction: The previous data concerning the prevalence of idiopathic pulmonary fibrosis (IPF) and the frequency of antifibrotic drug use in Finland were based on research registries and medical records whereas nationwide data on the number of patients with IPF in specialised care and those on antifibrotic treatment have not been published. Methods: We made an information request to the Finnish National Hospital Discharge Register (Hilmo) covering the whole population of Finland to find out the annual numbers of patients with IPF treated in specialised care in 2016–2021. The numbers of the patients initiating and using pirfenidone and nintedanib were requested from the Social Insurance Institution of Finland (Kela) for the same time period. Results: The estimated prevalence of IPF in specialised care was 36.0 per 100 000 in 2021, having increased since 2016. The number of antifibrotic drug users and their proportion of outpatients with IPF had also risen during the follow-up period. In 2021, 35% of the patients with IPF used pirfenidone or nintedanib. The number of inpatients treated in specialised care because of IPF had declined during 2016−2021. Conclusions: The prevalence of IPF was higher than expected in Finnish specialised care and had increased during the 6-year follow-up time. The increase in the number of patients with IPF using antifibrotic drugs might have diminished the need for IPF-related hospitalisations.

Abstract

Objectives: Early hearing aid (HA) fitting and cochlear implants (CIs) aim to reduce the effects of hearing loss (HL) on spoken language development. The goals of this study were (1) to examine spoken language skills of children with bilateral HAs and children with bilateral CIs; (2) to compare their language skills to the age-norms of peers with normal hearing (NH); and (3) to investigate factors associated with spoken language outcomes. Design: Spoken language results of 56 Finnish children with HL were obtained from a nationwide prospective multicenter study. Children with HL comprised two groups: children with mild-to-severe HL who used bilateral HAs (BiHA group, n = 28) and children with profound HL who used bilateral CIs (BiCI group, n = 28). Children’s spoken language comprehension, expressive and receptive vocabulary, and phonological skills were compared with normative values of children with NH at the age of three years. Odds ratio (OR) was calculated to compare proportions of children below age-norms in BiHA and BiCI groups. Factors associated with spoken language outcomes were modeled with analysis of covariance. Results: At the age of 3 years, 50%–96% of children with HL performed 1 SD or more below the mean of the normative sample of age-peers with NH in spoken language skills, depending on the language domain. Receptive vocabulary and phonological skills were the most vulnerable language domains. In receptive vocabulary, 82% of the children in the BiHA group and 50% of the children in the BiCI group scored 1 SD or more below the normative mean. The BiHA group was 4.4 times more likely to have poorer receptive vocabulary than the BiCI group. In phonological skills, 96% of children in the BiHA group and 60% of the children in the BiCI group scored 1 SD or more below the normative mean. Abstract

Using unique administrative register data, we investigate old-age retirement under the statutory pension scheme in Finland. The analysis is based on multi-outcome modelling of pensions and working lives together with a range of explanatory variables. An adaptive multi-outcome LAD-lasso regression method is applied to obtain estimates of earnings and socioeconomic factors affecting old-age retirement and to decide which of these variables should be included in our model. The proposed statistical technique produces robust and less biased regression coefficient estimates in the context of skewed outcome distributions and an excess number of zeros in some of the explanatory variables. The results underline the importance of late life course earnings and employment to the final amount of pension and reveal differences in pension outcomes across socioeconomic groups. We conclude that adaptive LAD-lasso regression is a promising statistical technique that could be usefully employed in studying various topics in the pension industry.

Abstract

Sedimentary charcoal records are widely used to reconstruct regional changes in fire regimes through time in the geological past. Existing global compilations are not geographically comprehensive and do not provide consistent metadata for all sites. Furthermore, the age models provided for these records are not harmonised and many are based on older calibrations of the radiocarbon ages. These issues limit the use of existing compilations for research into past fire regimes. Here, we present an expanded database of charcoal records, accompanied by new age models based on recalibration of radiocarbon ages using IntCal20 and Bayesian age-modelling software. We document the structure and contents of the database, the construction of the age models, and the quality control measures applied. We also record the expansion of geographical coverage relative to previous charcoal compilations and the expansion of metadata that can be used to inform analyses. This first version of the Reading Palaeofire Database contains 1676 records (entities) from 1480 sites worldwide. Abstract

Scope: Nutrients stimulate the secretion of glucagon-like peptide-1 (GLP-1), an incretin hormone, secreted from enteroendocrine L-cells which decreases food intake. Thus, GLP-1 analogs are approved for the treatment of obesity, yet cost and side effects limit their use. L-cells are mainly localized in the distal ileum and colon, which hinders the utilization of nutrients targeting GLP-1 secretion. This study proposes a controlled delivery system for nutrients, inducing a prolonged endogenous GLP-1 release which results in a decrease food intake. Methods and Results: α-Linolenic acid (αLA) was loaded into thermally hydrocarbonized porous silicon (THCPSi) particles. In vitro characterization and in vivo effects of αLA loaded particles on GLP-1 secretion and food intake were studied in mice. A total of 40.4 ± 3.2% of loaded αLA is released from particles into biorelevant buffer over 24 h, and αLA loaded THCPSi significantly increased in vitro GLP-1 secretion. Single-dose orally given αLA loaded mesoporous particles increased plasma active GLP-1 levels at 3 and 4 h and significantly reduced the area under the curve of 24 h food intake in mice. Conclusions: αLA loaded THCPSi particles could be used to endogenously stimulate sustain gastrointestinal hormone release and reduce food intake.

Abstract

Fibrosing interstitial lung diseases (ILD) are a group of diseases in which healthy peripheral lung tissue is replaced by scar tissue and accumulation of extracellular matrix. The most common fibrosing ILD is idiopathic pulmonary fibrosis (IPF), a disease with a poor prognosis, with median survival of 3−5 years. In acute exacerbation of interstitial lung disease (AE-ILD), concurrently with the chronic fibrotic pattern of the lung tissue, a new change, namely diffuse alveolar damage develops. AE-ILD causes a rapidly progressive respiratory failure and often death. We searched hospital treatment periods caused by acute deterioration of respiratory symptoms in patients with fibrosing ILDs treated in Oulu University Hospital or Oulaskangas Hospital between 2008 and 2017. We also examined surgical lung tissue samples obtained from 47 IPF patients treated in our hospital. A total of 128 patients experienced an AE-ILD, of whom 62% had IPF. The median survival after AE-ILD was poorer in IPF compared with other ILDs, namely 2.6 months versus 21 months. ILD was the most common underlying cause of death of the study subjects. AE-ILD was the cause of the first respiratory-related hospital treatment period in 41% of the total of 237 patients. Other common causes were symptoms related to ILD in the absence of AE-ILD (19%), lower respiratory tract infections (22%), and cardiovascular diseases (7%). AE-ILD was associated with worse posthospitalization survival than lower respiratory tract infection. The non-IPF ILD patients whose treatment period was due to cardiovascular cause had a shorter survival than non-IPF patients with AE-ILD. We collected bronchoalveolar lavage (BAL) differential cell count data from all patients who had undergone bronchoscopy and BAL (n = 155). Occurrence of BAL basophils was associated with an increased risk for AE-ILDs and mortality. In patients who had undergone BAL during AE-ILD (n = 43), BAL neutrophils were associated with poor survival time and BAL lymphocytes with a more favorable prognosis. The mast cell density of fibrotic lung tissue areas was associated with gender, smoking, and lung diffusion capacity. The lung tissue mast cell density was significantly reduced in the patients suffering from AE-ILD. Our study provided important information on AE-ILDs in our area, which can be helpful for clinicians treating these seriously ill patients. Tiivistelmä Fibroottiset interstitiaaliset keuhkosairaudet, joita kutsutaan myös keuhkofibrooseiksi, ovat ryhmä sairauksia, joissa keuhkokudokseen muodostuu poikkeavalla tavalla sidekudosta. Yleisin näistä on idiopaattinen keuhkofibroosi (IPF), johon sairastuneenkeskimääräinen elossaoloaika on 3−5 vuotta. Keuhkofibroosin äkillisissä pahenemisvaiheissa fibroottisten muutosten rinnalle kehittyy äkillinen alveolivaurio. Tilaan liittyy nopeasti etenevä hengitysvajaus ja korkea kuolleisuus. Tutkimuksessamme etsittiin keuhkofibroosipotilaiden äkillisestä hengitystieoireiden vaikeutumisesta johtuvat ei-elektiiviset hoitojaksot Oulun yliopistollisessa sairaalassa ja Oulaskankaan aluesairaalassa vuosilta 2008–2017. Tutkimme lisäksi 47 IPF-potilaan keuhkokudosnäytteitä. 128 potilasta sai äkillisen pahenemisvaiheen. Heistä 79:llä oli IPF. IPF:n pahenemisvaiheeseen liittyi huonompi ennuste kuin muiden keuhkofibroosien pahenemisvaiheisiin, sillä mediaanielossaoloaika pahenemisvaiheen jälkeen oli 2,6 kuukautta IPF:ssä, kun se muissa keuhkofibrooseissa oli 21 kuukautta. Keuhkofibroosi oli aineistomme yleisin peruskuolinsyy. Keuhkofibroosin äkillinen pahenemisvaihe oli hoitojakson syy 41 prosentilla aineistomme 237 potilaasta. Muita syitä sairaalahoidolle olivat keuhkofibroosin vaikeutuneet oireet ilman äkillistä pahenemisvaihetta (19 %), alahengitystieinfektiot (22 %) ja sydän- ja verisuonisairaudet (7 %). Äkillisiin pahenemisvaiheisiin liittyi huonompi ennuste kuin alahengitystieinfektioihin. Sydän- ja verisuonisairauksiin liittyi suurempi kuolemanriski kuin äkillisiin pahenemisvaiheisiin niillä keuhkofibroosipotilailla, joilla oli muu kuin IPF. Tutkimuksessamme kerättiin tiedot 155 potilaan keuhkohuuhtelunestenäytteiden (BAL) solujakaumasta. BAL-huuhteen basofiilit olivat yhteydessä lisääntyneeseen äkillisten pahenemisvaiheiden riskiin ja kuolleisuuteen. Äkillisen pahenemisvaiheen aikana määritetyt BAL-huuhteen neutrofiilit olivat yhteydessä lisääntyneeseen kuolleisuuteen ja lymfosyytit taas parempaan ennusteeseen. Fibroottisten keuhkokudosalueiden syöttösolujen määrä oli yhteydessä sukupuoleen, tupakointiin ja keuhkojen diffuusiokapasiteettiin. Syöttösolujen määrä oli alentunut keuhkofibroosin äkillisessä pahenemisvaiheessa. Tutkimuksemme toi alueemme keuhkofibroosien äkillisistä pahenemisvaiheista tärkeää tietoa, jota voidaan jatkossa hyödyntää näitä potilaita hoidettaessa.
