Abstract

ObjectivesThis study sought to determine the absolute and relative associations of diabetes mellitus (DM) and hemoglobin A1c (HbA1c) with sudden and/or arrhythmic death (SAD) versus other modes of death in patients with coronary artery disease (CAD) who do not qualify for implantable cardioverter-defibrillators.BackgroundPatients with CAD and DM are at elevated risk for SAD; however, it is unclear whether these patients would benefit from implantable cardioverter-defibrillators given competing causes of death and/or whether HbA1c might augment SAD risk stratification.MethodsIn the PRE-DETERMINE study of 5,764 patients with CAD with left ventricular ejection fraction (LVEF) of >30% to 35%, competing risk analyses were used to compare the absolute and relative risks of SAD versus non-SAD by DM status and HbA1c level and to identify risk factors for SAD among 1,782 patients with DM.ResultsOver a median follow-up of 6.8 years, DM and HbA1c were significantly associated with SAD and non-SAD (P 30% to 35%, patients with DM and/or elevated HbA1c are at much higher absolute risk of dying from non-SAD than SAD. Clinical risk markers, and not HbA1c, were associated with SAD risk in patients with DM. (PRE-DETERMINE: Biologic Markers and MRI SCD Cohort Study; NCT01114269).

Abstract

Cryogenic forming has significant potential as a novel method for fabricating aluminum alloys, because to its improved formability. Cryogenic deformation of alloys has interesting mechanical properties, but further study is needed to fully understand them. Here, the flow performance and underlying distortion mechanism of AA5052 at cryogenic conditions were extensively analyzed. To develop and modify a constitutive model, uniaxial tensile tests were conducted between 115-299 K at strain rates of 0.00030 to 0.02 s-1. The Erichsen cupping index (IE) values were then analyzed throughout a wide temperature range (299 K, 205 K, and 115 K). A smooth stress-strain curve was achieved below 205 K because dynamic strain aging was suppressed, in contrast to the serrated curve reported between 205 K and 299 K. The alloy's elongation and IE value were approximately 53% and 21% larger, respectively, at 115 K compared to those obtained at 299 K. This was because to the elevated critical dislocation density and persistent deformation that occur at extremely low temperatures. The 31% increase in tensile strength at 115 K is due to cryogenic deformation under high dislocation density. [ABSTRACT FROM AUTHOR]

Abstract

Background A proposed cause of dyspnea induced by ticagrelor is an increase in adenosine blood levels. Because caffeine is an adenosine antagonist, it can potentially improve drug tolerability with regard to dyspnea. Furthermore, association between caffeine and cardiovascular events is of clinical interest. Methods and Results This prespecified analysis used data from the PEGASUS TIMI 54 (Prevention of Cardiovascular Events in Patients With Prior Heart Attack Using Ticagrelor Compared to Placebo on a Background of Aspirin–Thrombolysis in Myocardial Infarction 54) trial, which randomized 21 162 patients with prior myocardial infarction to ticagrelor 60 mg or 90 mg or matching placebo (twice daily). Baseline caffeine intake in cups per week was prospectively collected for 9694 patients. Outcomes of interest included dyspnea, major adverse cardiovascular events (ie, the composite of cardiovascular death, myocardial infarction, or stroke), and arrhythmias. Dyspnea analyses considered the pooled ticagrelor group, whereas cardiovascular outcome analyses included patients from the 3 randomized arms. After adjustment, caffeine intake, compared with no intake, was not associated with lower rates of dyspnea in patients taking ticagrelor (adjusted hazard ratio (HR), 0.91; 95% CI, 0.76–1.10; P=0.34). There was no excess risk with caffeine for major adverse cardiovascular events (adjusted HR, 0.78; 95% CI, 0.63–0.98; P=0.031), sudden cardiac death (adjusted HR, 0.98; 95% CI, 0.57–1.70; P=0.95), or atrial fibrillation (adjusted odds ratio, 1.07; 95% CI, 0.56–2.04; P=0.84). Conclusions In patients taking ticagrelor for secondary prevention after myocardial infarction, caffeine intake at baseline was not associated with lower rates of dyspnea compared with no intake. Otherwise, caffeine appeared to be safe in this population, with no apparent increase in atherothrombotic events or clinically significant arrhythmias. Abstract

Spondylolisthesis is a spinal condition that affects the lumbosacral vertebrae. This disease causes one of the lower vertebrae to slip forward onto the bone directly beneath it. It's a painful condition but treatable in most cases. Although spondylolisthesis can be asymptomatic, patients with degenerative and isthmic spondylolisthesis typically present with low back pain, neurologic symptoms, and/or radicular symptoms. The surgical treatment of spondylolisthesis is indicated for cases of neurogenic claudication, intractable radicular pain, severe low-back pain, presence of neurological symptoms, failure of conservative management, radiological instability, progressive worsening of the listheses, Meyerding grade III and IV listhesis, and spondylosis. The ideal surgical treatment remains controversial. We have compared the functional outcome following instrumented posterior lumbar interbody fusion and posterolateral fusion for adult spondylolisthesis in our study. The prospective study was conducted in the Department of Orthopaedics. A total of 30 patients who satisfied both the inclusion and exclusion criteria and gave informed consent were recruited for the study from January to December 2019. The patients were randomized into two groups. Of 30 patients, Group 1(n=18) underwent Posterior lumbar interbody fusion and Group 2 (n=12) underwent Posterolateral fusion. Random number generators were used for allotting the patients to the specified group. Using Oswestry Disability Index, Japanese Orthopaedic Association Score and Visual Analog Score were used for pre-operative and post-operative functional scoring. The mean age among those who received PLIF was 53.67 years and among those who received PLF was 55.17 years. Spondylolisthesis at L4-L5 comprised 66.7% among those who received PLIF and 58.3 % among those who received PLF. Neurological deficit was present in 66.7% of the participants who had received PLIF and 100% of the participants who have received PLF. Concerning those who had received PLIF as treatment, 11 patients had excellent and 7 patients had better outcome in PLIF group, 7 patients had excellent and 5 patient had better outcome in PLF group.. The mean JOAS pre-intervention score was 6.66 and 6.40 for PLIF and PLF groups, respectively. In the 6th month mean JOAS score of the PLIF group was 11.16 and that of the PLF group was 10.16. The pre-intervention mean VAS score was 6.44 and 6.50 for PLIF and PLF groups, respectively. In both, the groups over the follow-up period mean VAS score had shown a decreasing trend. Our study did not show any significant difference in functional outcome between both groups, however there seems to better short term (3 months) and mid-term (6 months) pain relief in PLIF group when compared to PLF group. Long term follow-up studies along with radiological outcome may help in establishing superiority between both procedures.

47. Evaluation of the Knowledge regarding Infection Control in Prosthetic setup during Covid Pandemic among the Dental Students – A Cross Sectional Survey

Abstract

Background: Infection control is the important aspect of clinical dentistry because of the cross contamination of diseases via oral cavity is frequent between the dental team and the patients. So it is prerequisite to create awareness regarding infection control protocols among the dental students during their undergraduate/postgraduate training period. Aim: The aim of this cross sectional study is to describe the state of knowledge and create awareness among the dental students about infection control in prosthetic clinic during this COVID 19 pervasive circumstance. Materials and methods: An online questionnaire based survey was conducted among the dental students in July 2021.Totally 300 third, fourth, fifth year undergraduate students and postgraduates who are studying prosthodontics as their speciality were participated. The survey consisted of 7closed ended and 17open ended questions related to the knowledge towards infection control protocol in prosthodontic clinic and their self satisfaction about their knowledge. After proper validation from the experts, reliability was evaluated. The google form was prepared and distributed among the dental students across various dental colleges. Results: A total of 300 participants were involved in this study. Their knowledge towards infection control was varied between 24.33% and 78%. Some of the participants responded “good” or “fair” to the questions related to the evaluation of their knowledge towards infection control in prosthodontic clinic (P < 0.001). About 50% of students rated their knowledge as average and 7% of students rated their knowledge as poor towards infection control in prosthetic clinic. Conclusion: The results of this study show that the students have enough awareness, average knowledge and self satisfaction level about infection control protocol in prosthetic clinic during this COVID 19 pandemic.
