UNSTRUCTURED Electronic health records (EHRs) offer opportunities for research and improvements in patient care. However, challenges exist in using data from EHRs due to the volume of information existing within clinical notes, which can be labor intensive and costly to transform into usable data with existing strategies. This case report details the collaborative development and implementation of the postencounter form (PEF) system into the EHR at the Road Home Program at Rush University Medical Center in Chicago, IL to address these concerns with limited burden to clinical workflows. The PEF system proved to be an effective tool with over 98% of all clinical encounters including a completed PEF within 5 months of implementation. In addition, the system has generated over 325,188 unique, readily-accessible data points in under 4 years of use. The PEF system has since been deployed to other settings demonstrating that the system may have broader clinical utility.

In response to a range of stimuli, neutrophils produce web-like structures known as neutrophil extracellular traps (NETs). The benefits of NETs in pathogen control are commonly offset by excessive release as part of a pro-inflammatory response, as shown in several disorders. The discovery of potential drugs that regulate NET release has helped to enhance our understanding of the role of NETs in immunological protection, inflammatory diseases, and autoimmune disorders. Emerging evidence has indicated that antimicrobials play an immunomodulatory role by influencing the levels of circulating NETs. Herein, we address NETosis in several disorders and detail the mechanisms of NET-mediated damage in infections. We also aim to evaluate recent evidence on the effects of antimicrobials on NET levels. Relevant keywords were searched in PubMed. Studies were evaluated for their relevance, and a narrative review was written accordingly. Several antibiotics, including beta-lactams and cephalosporins, alter NET formation and degradation in a protective manner, resulting in minimal host organ damage. Additionally, some studies have highlighted the immunomodulatory effects of antivirals and antifungals on NET. Further studies are needed to fully understand the clinical implications of NET-antimicrobial interactions and their underlying mechanisms. [ABSTRACT FROM AUTHOR]

Coronary artery ectasia (CAE), or aneurysm (ballooning of blood vessel), occurs in one to five percent of adults, and is usually due to atherosclerosis and possibly inflammation. In particular, partial blockage of the right coronary artery by fatty deposits known as plaques can lead to compensatory ballooning of the artery. Most of the adults with CAE have been reported to have heart attacks or long-term angina. A case is reported of a 65-year-old man with a long history of high blood pressure who developed suffocating chest pain. A heart attack, which is due to coronary ischemia (loss of blood flow to heart muscle), was diagnosed, and cardiac catheterization showed CAE with thrombus (clot) in the right coronary artery. The patient was treated with streptokinase (which can break up clots), and then with heparin and aspirin, which are anti-clotting agents. The thrombus dissolved within seven days, and heart function was good. One year later, the patient had no further symptoms. The report indicates that thrombolytic (clot-destroying) therapy can be effective in preserving heart function and can resolve thrombosis involved in coronary artery ectasia.
