Abstract

This paper focuses on seismic fragility and damage scenario assessment of minor Italian historical centres through the development of urban fragility curves. With reference to the case study of Balvano, a small centre located in Basilicata Region of Italy, two hybrid models have been adopted. The first is a mechanic-based hybrid model developed by the authors to derive urban fragility curves specifically; the second is the macroseismic method, originally conceived to derive typological fragility curves for single building classes, expanded to derive urban fragility curve herein. Balvano was strongly struck by 1980 Irpinia-Basilicata earthquake (Ms = 6.90) and hence subjected to an intense reconstruction process during 1980s, where almost the 80% of the buildings were reconstructed with reinforced concrete structures in the place of unreinforced masonry ones. Seismic vulnerability and damage scenarios before and after 1980 have been assessed and compared with the purpose of validating the effectiveness of the urban scale fragility curves obtained through hybrid methodologies and quantifying the effect of the ‘new’ seismic hazard maps and first seismic codes and recommendations released by the Italian Government in the aftermath of 1980 for the construction of new buildings or for retrofitting the existing ones. A good matching between predicted and occurred damage scenario from the research outcomes emerged, confirming the effectiveness of the urban scale hybrid fragility curves to assess seismic vulnerability at urban scale. Moreover, the comparison of the damage scenarios pre and post-reconstruction highlighted the crucial role played by the code prescriptions adopted in that years for reducing the seismic vulnerability of the municipality and the importance of the ‘new’ seismic hazard maps introduced in 1980s. Finally, the differences between mechanical-based hybrid and macroseismic model have been discussed in the paper.

Abstract

The Erice 2018 Charter was unanimously approved at the conclusion of the 53rd Residential Course of the International School of Epidemiology and Preventive Medicine “Adapted Physical Activity in Sport, Wellness and Fitness; the role of the Departments of Prevention and of the National Health Service in doping prevention and health promotion”, held on 15-19 May 2018 in Erice, Italy, at the “Ettore Majorana” Foundation and Centre for Scientific Culture, and promoted by the Study Group on “Movement Sciences for Health” of the Italian Society of Hygiene, Preventive Medicine and Public Health. The event was part of a larger project supported by the Ministry of Health aimed at preventing doping in the general population involved in sport and physical activities. After an intense discussion the participants focused on ten statements involving the following critical issues: responsibility, priority, message, alphabetization, networks and alliances, school promoting health, player and opportunities, competences, know-how, programming and acting. These statements provide hints to approach doping within a public health frame and summarize the role of the Departments of Prevention and NHS in promoting and coordinating preventive actions with other institutions and stakeholders. Doping represents a complex phenomenon related to cultural, social, economic and legal issues. In addition to regulatory or repressive actions, education to health and legality is proposed as the fundamental strategy to contrast doping by promoting healthy lifestyles, based on scientific knowledge and respect for legality.

Abstract

Objective: To investigate the association between application of fundal pressure during the second stage of labor (Kristeller maneuver) and the risk of levator ani muscle (LAM) injury.Methods: This was a prospective case-control study of women recruited immediately after their first vaginal delivery in our university hospital between March 2014 and September 2016. Women who underwent the Kristeller maneuver were recruited as cases. For each case, a control (no Kristeller) was recruited matched for body mass index, use of epidural analgesia, duration of second stage of labor and birth weight. All women were invited to undergo four-dimensional (4D) transperineal ultrasound (TPU) 3-6 months postpartum. The main outcome measure was the presence of LAM avulsion on 4D-TPU. TPU results were compared between cases and controls. Multivariate logistic regression analysis was performed to identify independent risk factors for LAM avulsion.Results: During the study period, 134 women in the Kristeller maneuver group and 128 women in the control group underwent TPU assessment. Women who underwent the Kristeller maneuver had a higher prevalence of LAM avulsion than did controls (38/134 (28.4%) vs 18/128 (14.1%); P = 0.005). In addition, women in the Kristeller-maneuver group had a larger hiatal area on maximum Valsalva maneuver and a greater increase in hiatal area from rest to maximum Valsalva. On multivariate logistic regression analysis, use of the Kristeller maneuver was the only independent factor associated with LAM avulsion (odds ratio, 2.5 (95% CI, 1.29-4.51)).Conclusion: The Kristeller maneuver is associated with an increased risk of LAM avulsion when applied in women during their first vaginal delivery. This should be taken into account when deciding to use fundal pressure to accelerate the second stage of labor and when counseling women following childbirth. Copyright © 2018 ISUOG. Abstract

Objective: To compare the accuracy of transperineal (TP) ultrasound with transabdominal (TA) approach in the sonographic assessment of fetal occiput position during the second stage of labour.Methods: A series of low-risk women at term attending the labour ward of three university hospitals were prospectively recruited for the purpose of this study. During the second stage of labor patients were evaluated first by TP and than by TA ultrasound to determine the fetal position. The occiput position was labelled as DOA (direct occiput anterior), ROA (right occiput anterior), LOA (left occiput anterior), DOP (direct occiput posterior), ROP (right occiput posterior), LOP (left occiput posterior), ROT (right occuput transverse) and LOT (left occiput transverse). The agreement between the two techniques was assessed.Results: Overall 80 patients were recruited in the study group. Ultrasound examination was performed at 21(± 8) minutes from the beginning of the active pushing. The ultrasound findings of the fetal occiput position were recorded. In all cases TA ultrasound confirmed the fetal occiput position as determined at TP approach except in one case of ROA that had been recorded as ROT using TP ultrasound.Conclusions: Ultrasound TP examination is accurate in the diagnosis of fetal occiput position during the second stage of labor. 46. Caregiver's satisfaction with a video tutorial for shoulder dystocia management algorithm.

Abstract

In our questionnaire, a video tutorial illustrating the management of shoulder dystocia was considered by health personnel as a useful complementary training tool. We prepared a 5-min video tutorial on the management of shoulder dystocia, using a simulator that includes maternal pelvic and baby models. We performed a survey among obstetric personnel in order to assess their opinion on the tutorial by inviting them to watch the video tutorial and answer an online questionnaire. Five multiple-choice questions were set, focusing on the video's main objectives: clarity, simplicity and usefulness. Following the collection of answers, global and category-weighted analyses were conducted for each question. Out of 956 invitations sent, 482 (50.4%) answered the survey. More than 90% of all categories found the video tutorial to be clinically relevant and clear. For revising the management of shoulder dystocia most obstetric personnel would use the video tutorial together with traditional textbooks. Abstract

Seismic capacity assessment of existing buildings is of paramount importance for mitigating the seismic risk in earthquake-prone areas. In Europe, the next version of Eurocode 8 and the current version of the Italian standards suggest the adoption of both linear and nonlinear approaches to perform seismic analyses of existing masonry buildings. This paper discusses with the main issues concerning linear and nonlinear analyses methodologies provided by the Italian standards for existing unreinforced masonry buildings. An historical building located in Southern Italy, representative of the Mediterranean basin buildings, has been chosen as a case study and modelled through the equivalent frame model. According to the Italian standards, the influence of in-plane stiffness of floors and spandrels on the overall seismic capacity of the structure has been considered in the analyses. Beyond the code prescriptions, the effect of different ultimate drifts values assumed for the spandrels on nonlinear behavior of the structure has been analysed. The results of two types of linear analyses (lateral static force method and modal response spectrum analysis) have been compared among them and with those provided by the nonlinear static method. Conclusions show that either behavior and overstrength factors provided by the Italian standards are conservative than those estimated through the nonlinear static analyses. Moreover, it has been remarked that the value of ultimate drift adopted for the spandrel can affect significantly the nonlinear response of the structures, although this topic still represents an open issue.
