Introduction: The caregivers of cancer patients may experience changes in their routine that affect their quality of life. The aim of the study was to identify the domains that compromise the quality of life (QoL) of the caregivers of patients with gastrointestinal cancer (GIC). Methods: A descriptive, observational study with caregivers of patients diagnosed with GIC during chemotherapy therapy was done. The caregivers answered 2 questionnaires, SF-36 (short form health survey 36) and CBS (caregiver burden scale). Results: 100 caregivers of 100 patients with GIC were included. The mean age of the patients was 62.2±13 years. Most of them had colorectal cancer (66%), ECOG 0-1 (87%) and stage II/III (81%) disease. Among caregivers, 81% had less than 60 years old and 76% were female. According to the SF-36, lower levels were observed in the emotional, physical aspects and vitality and the best average in functional capacity. Regarding the CBS instrument, the general strain domain and disappointment was the more affected. Stage or ECOG was not associated to the caregivers QOL or burden. High schooling level of the patients was associated with a better quality of life and less burden of the caregivers. Ageing caregivers had a worst QoL and an increased burden. Conclusion: Most of caregivers were women and youngers, and had a worst quality of life in emotional role, physical aspects and vitality, and an increased burden in general strain and disappointment.

ABSTRACT BACKGROUND: Worldwide, colorectal cancer (CRC) and gastric cancer (GC) are the third and the fifth most prevalent, respectively. Diarrhea is a common symptom in patients on chemotherapy or radiotherapy treatment and can reduce treatment tolerance. Surgical resections and chemotherapy change the intestinal microbiota that can lead to lactose intolerance, small intestinal bacterial overgrowth (SIBO). OBJECTIVE: The aim of the study was to evaluate the frequency of diarrhea in patients with CRC and GC on chemotherapy with SIBO or intolerance of lactose. METHODS: This is a descriptive and observational study with patients of both sexes, over 18 years old, in treatment in the Gastro-Oncology outpatient clinic of the Federal University of São Paulo. Patients with a confirmed diagnosis of CRC or GC during chemotherapy treatment were included. To detect bacterial overgrowth and lactose intolerance, breath hydrogen test with lactulose and lactose was done. Number and aspects of the evacuations and toxicity degree were collected. For the nutritional assessment, weight and height were performed to calculate the BMI. and the Patient Generated Subjective Global Assessment (PG-SGA). RESULTS: A total of 33 patients were included, 29 with CRC and 3 with GC. Most of them were male (57.57%), mean age of 60.03±10.01 years and in chemotherapy with fluoropyrimidine and oxaliplatin (54.5%). Diarrhea was present in 57.6% and 30.3% had toxicity grade 2. According to the BMI, 78.9% were eutrophics, obese or overweight, but according to PG-SGA, 84.9% had moderate or severe nutritional risk grade. Between patients, 45% had lactose intolerance and 9% SIBO. Diarrhea grade 2-3 was observed in 66.6% of patients with SIBO and 66.7% of that with lactose intolerance. No statistical difference was observed between patients with SIBO or lactose intolerance and grade of diarrhea. CONCLUSION: Diarrhea was a frequent symptom in chemotherapy patients with gastric or colorectal cancer independent of the presence of SIBO or lactose intolerance. Surgery and chemotherapy treatment impacted in the intestinal habit of patients. Diagnosis of other causes of diarrhea may contribute to a better tolerance to treatment and quality of life.

ABSTRACT BACKGROUND: Colorectal cancer is the third most common neoplasm in the world. Methylation of tumor related genes in CpG islands can cause gene silencing and been involved in the development of cancer. The potential role of DKK2 as a biomarker for early diagnosis of colorectal cancer remains unclear. OBJECTIVE: The aim of the study was to evaluate the profile of methylation and RNAm expression of DKK2 as potential predictors of colorectal cancer diagnosis and prognosis. METHODS: Expression of mRNAs encoding DKK2 in 35 colorectal cancer tissues was quantified using real-time polymerase chain reaction analysis. The DNA methylation was studied by high resolution melting analysis. The general characteristics of the patients were collected. DKK2 methylation and expression were compared to clinical, pathological aspects and overall survival. RESULTS: Among the 35 patients studied, 18 were male, 10 were on right colon and 25 on left colon. Among the 20 patients with high hypermethylation, 15 of them had mRNA low expression of DKK2. There was no significant association between DKK2 promoter methylation and mRNA DKK2 expression and clinical or pathological features. DKK2 promoter methylation (P=0.154) and DKK2 RNA expression (P=0.345) did not show significant correlation with overall survival. CONCLUSION: DKK2 promoter methylation and DKK2 RNA status appear to be biomarkers of cancer diagnosis but not predictors of prognosis.

Objetivou-se identificar a QV de pacientes com câncer gastrointestinal e dor crônica e caracterizar os dados sociodemográficos, clínicos, terapêuticos e de gradação da dor, associando-os aos domínios de QV. Trata-se de um estudo transversal, quantitativo, do qual participaram pacientes com câncer gastrointestinal e dor crônica, atendidos em ambulatório de gastroenterologia oncológica de um hospital geral, na cidade de São Paulo, Brasil. Utilizou-se um questionário sociodemográfico e clínico, o instrumento EORTC -QLQC30 e o Brief Pain Inventory (BPI). Os dados foram submetidos a análise descritiva, frequências absolutas e porcentuais, e analítica, buscando-se correlacionar os domínios de QV, Dor, Performance Status (PS) dos pacientes, mediante o coeficiente de correlação de Spearman. Para comparar os domínios das escalas do QLQ-C30 e BPI com as variáveis de interesse, utilizou-se Teste T. Participaram do estudo 13 pacientes, maioria sexo masculino, casado, religioso, classificação econômica C2 (61,5%). Todos encontravam-se com dor no momento da coleta de dados, com média 2,54 (DP=2,88) pelo BPI e com pior escore referido das últimas 24 horas em 8,15 (DP=2,03). As principais alterações de QV evidenciadas foram, além da dor: fadiga, insônia e perda de apetite, sendo que quanto mais avançada a doença, piores os sintomas e o prejuízo funcional. Os dados evidenciaram que os pacientes convivem com dor de intensidade moderada e, se pior estado clínico, maior intensidade. A dor compromete a qualidade de vida desses pacientes em múltiplos aspectos, evidenciando que medidas precisam ser empreendidas para favorecer o controle álgico, do esquema terapêutico medicamentoso às atividades educativas e assistenciais.

Older individuals are more likely to develop solid cancers, but at the same time are more sensitive to the side effects of chemotherapy. In addition, older adults are more likely to present with chronic diseases (comorbidities) and immunosenescence that may decrease immunosurveillance against cancer. Clinical outcomes for the older patient with cancer are different from the younger patient and require different research and treatment approaches. Thus, alternative therapeutic approaches tailored specifically to the older patients are required. Colorectal cancer (CRC) has a high incidence in older individuals and is the third leading cause of cancer death globally. Anti-hypertensives are used by a large proportion of older patients and some studies have pointed to a positive impact of angiotensin-converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARB) on CRC outcomes. As we have previously shown in a mouse model, lung metastases express ACE and contain many infiltrating myeloid-derived suppressor cells (MDSC); particularly high levels of MDSC are also present in the blood of older patients with CRC and other cancers, and are associated with disease severity. In this Commentary, we hypothesize that one mechanism responsible for the positive impact of ACEi or ARB on the outcome of CRC is the modulation of myeloid cells contributing to their maturation to non-suppressive neutrophils/monocytes and diverting them away from retaining an immature MDSC phenotype.

ABSTRACT BACKGROUND: Colorectal cancer (CRC) is one of the leading causes of cancer worldwide. Early diagnostic methods using serum biomarkers are required. The study of omics, most recently lipidomics, has the purpose of analyzing lipids for a better understanding of human lipidoma. The evolution of mass spectrometry methods, such as MALDI-MS technology, has enabled the detection and identification of a wide variety of lipids with great potential to open new avenues for predictive and preventive medicine. OBJECTIVE: To determine the lipid profile of patients with colorectal cancer and polyps. METHODS: Patients with stage I-III CRC, adenomatous polyps and individuals with normal colonoscopy were selected. All patients underwent peripheral blood collection for lipid extraction. The samples were analyzed by MALDI-MS technique for lipid identification. STATISTICAL ANALYSIS: Univariate and multivariate (principal component analysis [PCA] and discriminant analysis by partial least squares [PLS-DA]) analyses workflows were applied to the dataset, using MetaboAnalyst 3.0 software. The ions were identified according to the class of lipids using the online database Lipid Maps (http://www.lipidmaps.org). RESULTS: We included 88 individuals, 40 with CRC, 12 with polyps and 32 controls. Boxplot analysis showed eight VIP ions in the three groups. Differences were observed between the cancer and control groups, as well as between cancer and polyp, but not between polyps and control. The polyketide (810.1) was the lipid represented in cancer and overrepresented in polyp and control. Among the patients with CRC we observed differences between lipids with lymph node invasion (N1-2) compared to those without lymph node invasion (N). CONCLUSION: Possible lipid biomarkers were identified among cancer patients compared to control and polyp groups. The polyketide lipid (810.1) was the best biomarker to differentiate the cancer group from control and polyp. We found no difference between the biomarkers in the polyp group in relation to the control.

Abstract Background The prognosis of colorectal cancer (CRC) patients can be influenced by genetic mutations and nutritional status. The relationship between these variables is unclear. The objective of the study was to verify the variables involved in the nutritional status and genetic mutations, which correlate with survival of CRC patients. Methods Patients with surgical intervention for tumor resection were evaluated using body mass index, nutritional screening, patient self-produced global subjective assessment, phase angle, and computed tomography to calculate the areas of visceral adipose tissue (VAT) and subcutaneous adipose tissue, and muscle mass for the determination of sarcopenia. Ten gene mutations involved in CRC carcinogenesis were studied (PIK3CA, KRAS, BRAF, EGFR, NRAS, TP53, APC, PTEN, SMAD4, and FBXW7). DNA was extracted from fresh tumor or paraffin tissues. Results Of the 46 patients, 29 (64.4%) were at nutritional risk and 21 (45.7%) were moderately malnourished. However, there was a high percentage of VAT in 24 (61.5%) and sarcopenia in 19 (48.7%) patients. These variables were associated with a higher risk of mortality. ABSTRACT Background : The II Brazilian Consensus on Gastric Cancer of the Brazilian Gastric Cancer Association BGCA (Part 1) was recently published. On this occasion, countless specialists working in the treatment of this disease expressed their opinion in the face of the statements presented. Aim : To present the BGCA Guidelines (Part 2) regarding indications for surgical treatment, operative techniques, extension of resection and multimodal treatment. Methods: To formulate these guidelines, the authors carried out an extensive and current review regarding each declaration present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases initially with the following descriptors: gastric cancer, gastrectomy, lymphadenectomy, multimodal treatment. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results : Of the 43 statements present in this study, 11 (25,6%) were classified with level of evidence A, 20 (46,5%) B and 12 (27,9%) C. Regarding the degree of recommendation, 18 (41,9%) statements obtained grade of recommendation 1, 14 (32,6%) 2a, 10 (23,3%) 2b e one (2,3%) 3. Conclusion : The guidelines complement of the guidelines presented here allows surgeons and oncologists who work to combat gastric cancer to offer the best possible treatment, according to the local conditions available.

ABSTRACT Background: The II Brazilian Consensus on Gastric Cancer by the Brazilian Gastric Cancer Association (ABCG) was recently published. On this occasion, several experts in gastric cancer expressed their opinion before the statements presented. Aim: To present the ABCG Guidelines (part 1) regarding the diagnosis, staging, endoscopic treatment and follow-up of gastric cancer patients. Methods: To forge these Guidelines, the authors carried out an extensive and current review regarding each statement present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases with the following descriptors: gastric cancer, staging, endoscopic treatment and follow-up. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results: Of the 24 statements, two (8.3%) were classified with level of evidence A, 11 (45.8%) with B and 11 (45.8%) with C. As for the degree of recommendation, six (25%) statements obtained grade of recommendation 1, nine (37.5%) recommendation 2a, six (25%) 2b and three (12.5%) grade 3. Conclusion: The guidelines presented here are intended to assist professionals working in the fight against gastric cancer with relevant and current information, granting them to be applied in the daily medical practice.

ABSTRACT Background: Since the publication of the first Brazilian Consensus on Gastric Cancer (GC) in 2012 carried out by the Brazilian Gastric Cancer Association, new concepts on diagnosis, staging, treatment and follow-up have been incorporated. Aim: This new consensus is to promote an update to professionals working in the fight against GC and to provide guidelines for the management of patients with this condition. Methods: Fifty-nine experts answered 67 statements regarding the diagnosis, staging, treatment and prognosis of GC with five possible alternatives: 1) fully agree; 2) partially agree; 3) undecided; 4) disagree and 5) strongly disagree A consensus was adopted when at least 80% of the sum of the answers “fully agree” and “partially agree” was reached. This article presents only the responses of the participating experts. Comments on each statement, as well as a literature review, will be presented in future publications. Results: Of the 67 statements, there was consensus in 50 (74%). In 10 declarations, there was 100% agreement. Conclusion: The gastric cancer treatment has evolved considerably in recent years. This consensus gathers consolidated principles in the last decades, new knowledge acquired recently, as well as promising perspectives on the management of this disease.

Purpose: It has become crucial to translate scientific findings and to find ways by which to mobilize local resources to improve the quality and accessibility of cancer care in developing countries. This study seeks to provide insight into challenge through examining differences in clinician documentation of patients with cancer treated at a Brazilian Public University Hospital. Methods: ASCO Quality Oncology Practice Initiative (QOPI) measures were used to examine the care provided in the departments of breast, colorectal, lymphoma, gynecology, and lung cancers. For this study, data from a representative sample of patients receiving chemotherapy in the previous month were extracted and quality of cancer care indicators examined. Results: Certain elements of medical care were consistently and appropriately documented, including cancer diagnosis and stage, chemotherapy planning, administration, and summary. In general, considering the specific cancer management measures, patients received recommended care in accordance with recognized guidelines. Despite this, a number of important gaps in care were identified, including the assessment and treatment of pain, documentation of chemotherapy intention, symptom and toxicity management, patients’ psychosocial status, and provision of a treatment summary at care completion. Conclusion: These findings are encouraging in terms of adherence to core treatment guidelines in cancer care in Brazil. However, results suggest important opportunities for improving care across a number of domains, many of which represent a challenge throughout both developing and developed countries. This study may also provide preliminary guidance for enhancing educational and training programs for professionals and students alike, to implement high-quality, comprehensive cancer care.

Background - Colorectal cancer is one of the main cause of cancer in the world. Colonoscopy is the best screen method, however the compliance is less than 50%. Quantification of human DNA (hDNA) in the feces may be a possible screen non-invasive method that is a consequence of the high proliferation and exfoliation of cancer cells. Objective - To quantify the human DNA in the stools of patients with colorectal cancer or polyps. Methods - Fifty patients with CRC, 26 polyps and 53 with normal colonoscopy were included. Total and human DNA were analyzed from the frozen stools. Results - An increased concentration of hDNA in the stools was observed in colorectal cancer patients compared to controls and polyps. Tumors localized in the left side of the colon had higher concentrations of hDNA. There were no difference between polyps and controls. A cut off of 0.87 ng/mL of human DNA was determined for colorectal cancer patients by the ROC curve, with a sensitivity of 66% and a specificity of 86.8%. For polyps the cut off was 0.41, the sensitivity was 41% and the specificity 77.4%. Conclusion - A higher concentration of hDNA had been found in colorectal cancer patients The quantification of hDNA from the stools can be a trial method for the diagnosis of colorectal cancer.

Colorectal cancer is the third most common cancer worldwide, accounting for more than 610,000 mortalities every year. Prognosis of patients is highly dependent on the disease stage at diagnosis. Therefore, it is crucial to investigate molecules involved in colorectal cancer tumorigenesis, with possible use as tumor markers. Heparan sulfate proteoglycans are complex molecules present in the cell membrane and extracellular matrix, which play vital roles in cell adhesion, migration, proliferation, and signaling pathways. In colorectal cancer, the cell surface proteoglycan syndecan-2 is upregulated and increases cell migration. Moreover, expression of syndecan-1 and syndecan-4, generally antitumor molecules, is reduced. Levels of glypicans and perlecan are also altered in colorectal cancer; however, their role in tumor progression is not fully understood. In addition, studies have reported increased heparan sulfate remodeling enzymes, as the endosulfatases. Therefore, heparan sulfate proteoglycans are candidate molecules to clarify colorectal cancer tumorigenesis, as well as important targets to therapy and diagnosis.

Context Capecitabine, an oral drug, is as effective as traditional chemotherapy drugs. Objectives To investigate the adhesion to treatment with oral capecitabine in breast and colorectal cancer, and to determine any correlation with changes in patient’s quality of life. Methods Patients with colorectal cancer or breast cancer using capecitabine were included. The patients were asked to bring any medication left at the time of scheduled visits. The QLQ-C30 questionnaire was applied at the first visit and 8-12 weeks after treatment. Results Thirty patients were evaluated. Adherence was 88.3% for metastatic colon cancer, 90.4% for non-metastatic colon cancer, 94.3% for rectal cancer and 96.2% for metastatic breast cancer. No strong correlation between adherence and European Organisation for Research and Treatment of Cancer QLQ-C30 functional or symptom scale rates had been found. There was no statistically significant correlation between compliance and the functional and symptom scales of the questionnaire before and after chemotherapy, with the exception of dyspnea. Conclusions Although no absolute adherence to oral capecitabine treatment had been observed, the level of adherence was good. Health professionals therefore need a greater focus in the monitoring the involvement of patients with oral treatment regimens. Patients with lesser degrees of dyspnea had greater compliance.

Background: The aim of this study was to evaluate the association between adiponectin and tumor necrosis factor-α (TNF-α) serum levels in colorectal cancer (CRC) patients and compare these levels to clinical stage and nutritional status. Methods: A total of 79 patients were enrolled in the study (39 with CRC and 40 in the control). Nutritional status was assessed by Patient-Generated Subjective Global Assessment (PG-SGA), body mass index (BMI), and phase angle (PhA). Adiponectin and TNF-α serum concentrations were determined using an enzyme-linked immunosorbent assay. Results: Serum adiponectin levels were higher among CRC patients (p = 0.001). TNF-α serum levels were not significantly different between the groups, but patients with stage III or IV CRC had higher levels of TNF-α than those with lower stage disease (p = 0.037). The three tools used for the assessment of nutritional status (BMI, PhA, and PG-SGA) demonstrated that patients with a more severe nutritional deficit had higher adipocytokine levels, although these differences were significant only to TNF-α, when distributed PhA in tertiles. Conclusions: Adiponectin levels were higher among CRC patients. Although TNF-α serum levels from CRC patients did not differ significantly to the control group, CRC patients with stage III or IV had higher levels compared to those with stage I and II tumors. Nutritional status, as determined by BMI, PhA, and PG-SGA, demonstrated that patients with a greatest nutritional deficit, had higher levels of adipocytokines; however, these differences were significant only for TNF-α, when distributed PhA in tertiles.

Context Genomic alterations play important roles in gastric cancer carcinogenesis. Cyclooxygenases (COX) are important enzymes in the maintenance of mucosal integrity and in pathological processes, mainly in inflammation and cancer. The -765G>C COX-2 polymorphism has been implicated in gastric cancer risk. Objectives To evaluate the COX-2 gene polymorphism as a predictor of gastric cancer risk. Methods One hundred gastric cancer patients and 150 controls were enrolled from a Brazilian centre. Personal data regarding related risk factors, including alcohol consumption and smoking behavior, were collected via questionnaire. DNA was extracted from peripheral blood and the genotypes were analyzed using PCR-restriction fragment length polymorphism. Results G/G, G/C and C/C genotypes frequencies was 42.7%, 50% and 7.3%, respectively in controls and 59.0%, 34.0% and 7.0% in gastric cancer. The frequency of the genotypes differed between the groups (P = 0.033). A higher risk of gastric cancer was associated with COX-2 -765G/G genotype (P = 0.048; OR:1.98, 95% CI = 1.01-3.90). Alcohol consumption and smoking in patients with -765G/G genotype also increased the risk of gastric cancer. Conclusions The -765G/G genotype and the -765G allele had been associated with an increased risk for gastric cancer. The presence of smoking and alcohol consumption increased the risk for gastric cancer in subjects with -765G/G genotype compared with the control group. Objective: To evaluate methods for the identification of nutrition risk and nutritional status in outpatients with colorectal (CRC) and gastric cancer (GC), and to compare the results to those obtained for patients already treated for these cancers. Methods: A cross-sectional study was conducted on 137 patients: group 1 (n = 75) consisting of patients with GC or CRC, and group 2 (n = 62) consisting of patients after treatment of GC or CRC under follow up, who were tumor free for a period longer than 3 months. Nutritional status was assessed in these patients using objective methods ; nutritional screening tools [Malnutrition Universal Screening Tool (MUST), Malnutrition Screening Tool (MST), Nutritional Risk Index (NRI)], and subjective assessment [Patient-Generated Subjective Global Assessment (PG-SGA)]. The sensitivity and specificity of each method was calculated in relation to the PG-SGA used as gold standard. Results: One hundred thirty seven patients participated in the study. Stage IV cancer patients were more common in group 1. There was no difference in BMI between groups (p = 0.67). Analysis of the association between methods of assessing nutritional status and PG-SGA showed that the nutritional screening tools provided more significant results (p < 0.05) than the objective methods in the two groups. PG-SGA detected the highest proportion of undernourished patients in group 1. The nutritional screening tools MUST, NRI and MST were more sensitive than the objective methods. Phase angle measurement was the most sensitive objective method in group 1. Conclusion: The nutritional screening tools showed the best association with PG-SGA and were also more sensitive than the objective methods. The results suggest the combination of MUST and PG-SGA for patients with cancer before and after treatment.

CONTEXT: It still remains an open debate whether Helicobacter pylori eradication is beneficial or not for the improvement of symptoms in functional dyspepsia. Differences in geographic distribution, the worldwide H. pylori genetic variability and the fact that the outcome of infection is strongly related to the virulence of the infecting strain are factors that might be driving ongoing controversies. OBJECTIVE: To study the correlation between gastric histology and H. pylori serology status in patients with dyspepsia. METHODS: This is a cross-sectional study where 40 consecutive dyspeptic patients (28 women and 12 men, mean age 48.5 years) with endoscopically normal stomachs were selected from the endoscopy unit at a university hospital in Recife, PE, Northeast of Brazil, between March 1998 and July 1999. Patients underwent gastric mucosal biopsy and serological tests (anti-Hp and anti-CagA antibodies). Gastric biopsies were examined using H-E and Giemsa stains and gastritis was classified and graded (mild, moderate or severe) according to "the updated Sydney System - Houston, 1994". RESULTS: Among 40 patients with dyspepsia the gastric histology revealed that about ¼ had moderate (25%) or severe (2.5%) gastritis. This subgroup of patients also had a greater positive frequency of anti-Hp (100% vs 41%; P = 0.0005) and anti-CagA (91% vs 58%; P = 0.09) antibodies when compared with those with normal histology (27.5%) or mild gastritis (45%). CONCLUSION: Since upper gastrointestinal endoscopy is part of the functional dyspepsia investigation and serology for anti-CagA antibody is not available in daily clinical practice, by biopsying gastric mucosa we would only be able to selectively apply H. pylori eradication therapy for those with histology that best correlate with virulent infecting strains (moderate or severe gastritis) - around ¼ of our study patients with dyspepsia.CONTEXTO: O benefício da terapia de erradicação do H. pylori como parte do tratamento da dispepsia funcional ainda é uma questão em aberto. Diferenças na distribuição geográfica, a ampla variabilidade genética e o fato de que a expressão clínica da infecção está fortemente relacionada com a virulência das cepas infectantes, são fatores que provavelmente guiam as controvérsias. OBJETIVO: Estudar a correlação entre histologia gástrica e sorologia para H. pylori em doentes com dispepsia. MÉTODOS: Estudo descritivo-transversal com 40 pacientes consecutivos com sintomas dispépticos (28 mulheres e 12 homens, média de idade de 48,5 anos) e achado endoscópico de estômago normal, selecionados a partir da sala de endoscopia (Hospital das Clínicas da Universidade Federal de Pernambuco, Recife, PE.) entre março de 1998 e julho de 1999. Todos foram submetidos a biopsias gástricas e testes sorológicos (anti-Hp e anti-CagA). As biopsias foram analisadas pelos métodos de H-E e Giemsa e os achados de gastrite classificados de acordo com o Sistema Sydney atualizado. RESULTADOS: A histologia dos 40 pacientes revelou que cerca de ¼ apresentava gastrite moderada (25%) ou severa (2,5%). Esse grupo também apresentava maior frequência de positividade anti-Hp (100% vs 41%; P = 0,0005) e anti-CagA (91% vs 58%; P = 0,09) quando comparado com os casos com histologia normal (27,5%) ou gastrite leve (45%). CONCLUSÃO: Considerando que a endoscopia digestiva alta é parte da rotina de investigação da dispepsia funcional e que a sorologia anti-CagA não está disponível na prática clínica diária, através da histologia pode-se selecionar e aplicar terapia de erradicação do H. pylori apenas para os casos que muito provavelmente estão associados a cepas patogênicas de H. pylori (doentes com gastrite moderada ou severa) - cerca de ¼ da presente amostra.

CONTEXT: Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor. Cellular proliferation and apoptosis is gaining importance for predicting prognosis in several cancers. OBJECTIVE: To investigate the Ki67 and p53 immunostaining in GISTs. METHODS: Specimens from 40 patients with GIST were assessed for immunohistochemical expression of Ki67 and p53. The tumors were divided according the risk of recurrence in two groups: I with high or intermediate risk and; II with low or very low risk. RESULTS: Among the 40 patients, 21 were men, the mean age was 56 years, 16 occurred in the small intestine and 13 in the stomach, 5 in the retroperitonium, 4 in the colon or rectum and 2 in the mesenterium. Thirty two tumors were from group I and 8 from group II. Half of the patients developed recurrence, being 90% of the group I (P = 0.114). The tumor Ki67 labelling index ranged from 0.02 to 0.35 (mean level 0.12). This index was marginally higher in the group I patients with recurrence (P = 0.09) compared to the patients of the same group without recurrence. p53 staining was expressed in 65% of the GISTs. A higher frequency of p53 and Ki67 had been found in the group I tumors when compared to the other group (P = 0.022; OR = 8.00 - IC 95%: 1.32-48.65). CONCLUSION: The most common site was the small intestine and 80% had a malignant potential justifying the high recurrence observed. No significant correlation was found between p53 and overall outcome of the patients. In group I patients, the evaluation Ki67LI may be a marker of prognosis. The positivity of both markers is higher among the patients with worst prognosis than in the others.CONTEXTO: Os tumores estromais gastrointestinais (GIST) são os tumores mesenquimais mais frequentes. A proliferação intestinal e a apoptose são cada vez mais importantes na avaliação do prognóstico de diversos cânceres. OBJETIVO: Avaliar a imunoexpressão de Ki67 e p53 em GIST. MÉTODOS: Foram estudados a expressão de Ki67 e p53 por imunoistoquimica em tumores de 40 pacientes com GIST. Os tumores foram divididos segundo o risco de recurrência em 2 grupos: I com risco alto ou intermediário e II com risco baixo ou muito baixo. RESULTADOS: Entre os 40 pacientes, 21 eram do sexo masculino, a idade média foi de 56 anos, 16 ocorreram no intestino delgado, 13 no estômago, 5 no retroperitônio, 4 no cólon e reto, e 2 no mesentério. Trinta e dois tumores foram classificados no grupo I e 8 no grupo II. Metade dos pacientes desenvolveu recurrência, sendo 90% de cólon (P = 0,114). O índice de proliferação tumoral Ki67 variou entre 0,02 e 0,35 (média = 0,12). Este índice foi marginalmente superior nos tumores do grupo I com recurrência (P = 0,09), quando comparado aos do mesmo grupo sem recurrência. A expressão do p53 foi observada em 65% dos GISTs. Nos tumores do grupo I foi observada com maior frequência, expressão de p53 e Ki67 (P = 0,022; OR = 8.00 - IC 95%: 1,32-48,65). CONCLUSÃO: A localização mais comum foi no intestino delgado, 80% tinham potencial maligno, justificando a alta recurrência encontrada. Não se observou correlação significante entre p53 e evolução dos pacientes. Nos pacientes do grupo I, a avaliação do KI67LI pode ser um marcador de prognóstico. A positividade dos dois marcadores é maior entre os pacientes de pior prognóstico.

RACIONAL: Cerca de 35% dos casos de câncer estão envolvidos com fatores advindos da dieta e de outros como o álcool, o fumo, a luz solar, agentes químicos e infecções por vírus. O câncer de estômago é o segundo tipo de câncer mais diagnosticado no mundo, sendo responsável por cerca de 9,9% de todos os diagnósticos e cerca de 12,1% das mortes. OBJETIVOS: Comparar o peso corpóreo atual e habitual, grau de escolaridade, saneamento básico, tabagismo, etilismo e freqüência pregressa de consumo de alimentos entre pacientes com câncer de estômago e população controle. MÉTODO: Setenta pacientes com câncer gástrico foram pareados a outros 70 indivíduos sem diagnóstico de neoplasia. Foram coletados peso, altura, informações sobre grau de escolaridade, saneamento básico, tabagismo e etilismo e preenchido um questionário de freqüência alimentar pregressa. RESULTADOS: Dos 75 pacientes elegíveis para o estudo, 42 eram homens, com média de idade de 59,5 anos. O grupo de pacientes com câncer apresentou menor peso e índice de massa corporal atual do que o grupo controle. Em relação ao grau de escolaridade, verificou-se que, no grupo de pacientes com câncer, 21 indivíduos nunca ingressaram na escola e dos que estudaram, cerca de 55% não conseguiram concluir o primeiro grau. Nesse grupo, 32,9% dos indivíduos tiveram acesso ao saneamento básico e 37,1% à eletricidade no passado; no grupo controle, esse percentual foi de 68,6% para cada uma das variáveis. No grupo caso, 58,6% dos pacientes moraram na zona rural, enquanto no grupo controle apenas 7,1%. No grupo caso, 65.7% eram fumantes versus 44.3%, no grupo controle. Observou-se também que os pacientes com câncer fumaram por período maior. O alcoolismo também foi mais freqüente entre os pacientes com câncer (45% versus 19%). Verificou-se que, entre os pacientes com câncer, houve maior consumo de alimentos ricos em sal, condimentos, nitratos, gordura animal saturada, carboidratos complexos, açúcar refinado e salgados fritos, comparado ao grupo controle. CONCLUSÃO: Os doentes com câncer gástrico, quando comparados ao grupo controle, apresentaram: perda de peso, queda da qualidade de vida verificada pelo baixo acesso a saneamento básico, eletricidade e escolaridade, residiram predominantemente em área rural, alta incidência de alcoolismo e alta ingestão de alimentos ricos em gorduras, alimentos industrializados e álcool.BACKGROUND: About 35% of the cancer patients are involved in factors coming from the diet and others like alcohol, smoking, sunlight, chemical agents and infections caused by virus. The stomach cancer is the second cause of cancer in the world with 9.9 % of all diagnosis and about 12.1 % of death cases. AIMS: Evaluate the body weight, educational achievement, basic sanitation, smoking, alcoholism and eating habit among patients with gastric cancer and a control group. METHODS: Seventy patients with gastric cancer were paired with 70 subjects without cancer. Data on the weight and height, educational attainment, basic sanitation, smoking, alcoholism and eating habits of the patients were collected from the clinical records and from interviews. RESULTS: Forty two patients were men, the mean age were 60 years old. The actual weight and body mass index of the patients were smaller when compared to the controls. Within the group of patients with gastric cancer, 21 never attended school, and for those who attended, 55% did not finish the elementary school. Among the patient group, 32.9% of them lived in housing with basic sanitation and 37.1% with electricity, against 68.6% of the controls, and 58.6% of the patients lived in rural area, against only 7.1% of the controls. Among the test group, 65.7% of the patients were smokers, whereas in the control group, 44.3% were smokers. In addition, there was also difference in the duration of smoking habit. Alcoholism was also more frequent in the cancer group (44% vs 19%). Food rich in salt, condiments, nitrates, saturated fat, complex carbohydrates, refined sugar and fried salted food had been more used by gastric cancer patients. CONCLUSION: The patients with gastric cancer presented with: less weight, low quality of life as indicated by lower or no access to basic sanitation, electricity and schooling, lived predominantly in rural area, high incidence of alcohol intake and higher intake of high fat foods and industrialized foods.

RACIONAL: A neoplasia de esôfago está entre as 10 mais incidentes no Brasil. O diagnóstico é geralmente tardio e a sobrevida média é de 4 a 6 meses, independente da terapêutica. O alívio da disfagia e a melhora da qualidade de vida são os objetivos principais da terapêutica paliativa. OBJETIVO: Avaliar a qualidade de vida e a paliação da disfagia obtida com diferentes tipos de tratamento oferecidos aos pacientes com neoplasia avançada de esôfago. PACIENTES E MÉTODO: Avaliação prospectiva de 38 pacientes com neoplasia avançada de esôfago, com disfagia, sem possibilidade de tratamento curativo, entre setembro de 2001 a junho de 2005. Os pacientes foram alocados aleatoriamente, de acordo com a disponibilidade da terapia ou preferência do paciente ou do médico responsável, sendo 14 tratados com colocação de prótese (9 metálicas auto-expansíveis, 4 plásticas e 1 plástica auto-expansível), 4 com cirurgia paliativa, 8 com gastrostomia (7 cirúrgicas e 1 endoscópica) e 12 com sonda nasoenteral. RESULTADOS: Houve melhora do índice médio de disfagia em 30 dias em todos os grupos, exceto no da gastrostomia. A colocação da prótese de esôfago melhorou a disfagia de forma estatisticamente significante em relação às outras terapias paliativas. A qualidade de vida avaliada pela mediana do índice de Karnofsky não apresentou melhora em nenhum grupo de pacientes. O número de internações necessárias não foi diferente entre os grupos. A duração média das internações foi maior no grupo de tratamento cirúrgico (42 dias), embora sem diferença significativa. Não houve diferença na sobrevida média dos pacientes, independente do tipo de tratamento. CONCLUSÃO: A paliação ideal para todos os casos não existe. O método deve ser individualizado para cada paciente. O tratamento cirúrgico paliativo é o mais oneroso, devido ao prolongado tempo médio de internação desses pacientes. Infelizmente, o diagnóstico de tumor de esôfago em nosso meio ainda é muito tardio, limitando o benefício que poderia advir dos métodos de ponta de paliação endoscópica.BACKGROUND: Esophageal cancer is one of the 10 most common cancers in Brazil. Diagnosis is usually late and mean survival ranges from 4 to 6 months, no matter the treatment. Relief of dysphagia and increase in life quality are the main targets of palliative therapy. AIM: To evaluate patients with advanced esophageal tumor submitted to various palliative treatment options. PATIENTS AND METHOD: We prospectively evaluated 38 patients with advanced esophageal cancer, with dysphagia and no chance of curative treatment, between September 2001 and June 2005. Patients were treated according to available resources, patient or referring physician's preference: 14 patients were treated with esophageal stent (9 self-expandable metallic, 4 plastic, 1 expandable plastic), 4 with palliative surgery, 8 gastrostomy (7 surgical and 1 endoscopic) and 12 nasogastric tube. RESULTS: The mean dysphagia score 30 days after the procedure was improved in all groups except in the gastrostomy. Karnofsky score, reflecting quality of life, showed no improvement. The number of hospital admissions was not different among groups. Although the length of hospitalization was longer in the surgical group (42 days), it did not reach statistical significance. There was not statistically significant difference in the mean survival time among all patients. CONCLUSION: An ideal palliative treatment does not exist. The method must be individualized for each patient. Surgical treatment is the most expensive, once it requires longer periods in hospital. Unfortunately, the diagnosis of esophageal tumors is still delayed, limiting the benefits of top endoscopic palliation therapy.

RACIONAL: O câncer colorretal é a quarta causa de câncer no Brasil e o 5-fluourouracil uma das principais drogas usadas no tratamento adjuvante e paliativo dessa doença. A toxicidade da quimioterapia e as alterações de qualidade de vida, causadas pela própria doença e pelo tratamento, são motivo de muitos estudos. OBJETIVO: Avaliar nos doentes com câncer colorretal em tratamento quimioterápico, a toxicidade e possíveis alterações da qualidade de vida. MÉTODOS: Durante o período de março de 2001 a maio de 2003 no Ambulatório de Oncologia da Disciplina de Gastroenterologia Clínica da Universidade Federal de São Paulo, foram acompanhados 45 pacientes com câncer colorretal em tratamento quimioterápico adjuvante ou paliativo com 5-fluourouracil e ácido folínico durante seis ciclos. A toxicidade gastrointestinal e hematológica foi analisada utilizando-se as Recomendações para a Graduação da Toxicidade Aguda e Subaguda. Após o término de cada ciclo quimioterápico, os resultados foram anotados de acordo com os respectivos graus que variaram entre 0 e 4. A qualidade de vida foi pesquisada pelo questionário WHOQOL bref (World Health Organization Quality of Life) que consta de 26 questões e é composto por 4 domínios: físico, psicológico, relações sociais e meio ambiente, no início, no 3° e no 6° ciclo de tratamento. RESULTADOS: Entre os 45 pacientes, 28 eram do sexo masculino, a média de idade foi de 58,4 anos (34 a 79 anos). Segundo a classificação da União Internacional Contra o Câncer, 34 (75,6%) eram estádio II ou III e 11 estádio IV (24,4%). Quanto à localização, 64,4% eram de cólon. Em 57,7% a quimioterapia foi adjuvante e nos demais paliativa. As toxicidades mais comumente encontradas foram náuseas (42%), diarréia (38%) e neutropenia (15,7%). Não houve diferença significante entre os graus de toxicidade nos diferentes ciclos, assim como entre os doentes em tratamento adjuvante ou paliativo. Quanto à qualidade de vida foram observadas alterações significantes nos domínios físico e psicológico quando comparadas a primeira com a segunda ou a primeira com a terceira aplicação do questionário. Não foi encontrada alteração da qualidade de vida entre os doentes em quimioterapia adjuvante quando comparada aos em tratamento paliativo. Independente da indicação terapêutica, a média dos escores de qualidade de vida diminuiu em relação aos domínios físico e social na terceira aplicação do teste. CONCLUSÃO: As toxicidades gastrointestinais foram mais freqüentes que as hematológicas com o esquema utilizado. A qualidade de vida diminuiu após o início da quimioterapia em relação à atividade física e psicológica. No estudo da média dos escores observou-se queda dos mesmos nos domínios físico e social. A análise do questionário não mostrou alteração de qualidade vida quando comparados os doentes em tratamento paliativo com os em adjuvância.BACKGROUND: The colorectal cancer is the fourth cause of cancer in Brazil and 5-fluorouracil is the drug most commonly used in the adjuvant or palliative treatment of this disease. AIM - Evaluating in patients with colorectal cancer and chemotherapy, the toxicity and the quality of life. PATIENTS AND METHODS: From March 2001 and May 2003, 45 patients treated with colorectal cancer treated with 5-fluourouracil and folinic acid were followed closely during six cycles. The gastrointestinal and hematologic toxicity was analysed making use of the chart "Recommendations for the Graduation of Acute and Subacute Toxicity". After the end of each cycle of chemotherapy, the results were registered according to the respectives degrees that vary from 0 to 4. The quality of life was researched through the WHOQOL bref (World Health Organization Quality of Life) questionary that consists of 26 questions and 4 domains: physical, psychological, social relations and environmental, in the beginning, on the 3rd and 6th cycles of treatment. RESULTS: Among the 45 patients, 28 were male, the average age was 58.4 years old (from 34 to 79 years old). According to the International Union Against Cancer classification, 34 patients (75.6%) had tumors stage II or III and 11 had tumors stage IV (24.4%), 64.4% were in the colon. In 57.7% the chemotherapy was adjuvant and in the others palliative. The toxicities more commonly found were nauseas (42%), diarrhea (38%), and neutropenia (15.7%). There was no significant difference among the degrees of toxicity in the different cycles as well as among the patients in adjuvant or palliative treatment. Significant alterations was found among the quality of life in the physical and psychological domains when the 1st and the 2nd or the 1st and the 3rd application of the test were done. Alterations of the quality of life were also found in the social domain when the first evaluation was compared with the last one. There was no difference between the quality of life and the treatment.

RACIONAL: Polimorfismos genéticos são variações genéticas que podem ocorrer em seqüências codificadoras e não-codificadoras, levando a alterações qualitativas e/ou quantitativas das proteínas em questão. O p53 é o gene mais comumente alterado no câncer humano. O polimorfismo desse gene no códon 72 ocorre por substituição de uma base e tem sido associado a maior risco de câncer. OBJETIVO: Determinar a possível associação entre o polimorfismo no códon 72 (72 arginina/prolina) do gene p53 e câncer colorretal. CASUÍSTICA E MÉTODOS: Foram avaliados em 100 pacientes com câncer colorretal e em 100 indivíduos sem câncer, pareados quanto ao sexo idade, o hábito de fumar, o etilismo e no grupo caso o estádio, o grau de diferenciação e a evolução da doença. O genótipo (72 arginina/prolina) foi determinado por PCR, utilizando-se primers (seqüências de nucleotídeos) específicos. RESULTADOS: O genótipo homozigoto arginina/arginina foi prevalente em 56% no grupo controle e em 58% no grupo caso. Não se observou diferença entre os dois grupos. No estádio IV este genótipo foi mais freqüente quando comparado ao estádio I (80% versus 14%). Não se observou diferença entre as variações do genótipo e fumo, álcool, evolução clínica ou grau de diferenciação. CONCLUSÃO: A prevalência do genótipo arginina/arginina foi a mais freqüente nos dois grupos. Não foi encontrada correlação entre maior risco de câncer e o polimorfismo no códon 72 prolina/arginina do gene p53. Apesar do pequeno número de doentes com câncer em estádio avançado (IV), estes tiveram maior prevalência do genótipo arginina/arginina.BACKGROUND: Polymorphisms are genetic variations that can occur in sequences of codons, leading to defective proteins. p53 is the most commonly gene affected in human cancer. The polymorphism of this gene occurs by a substitution of a base in codon 72 and may increase the risk of cancer. AIM: To investigate the possible association between p53 arginine/72 proline polymorphism and susceptibility to colorectal cancer. PATIENTS AND METHODS: This polymorphism was studied by polymerization chain reaction using specific primers in 100 patients with colorectal cancer paired by sex and age to 100 patients without cancer. Alcohol and tobacco used by all the patients and clinical aspects as stage, grade of differentiation and recurrence in the case group was compared with the genotype analyzed. RESULTS: The frequency of homozygosis for arginine was 56% in the cancer group and 58% in the control group. No significant difference was observed among both groups. This genotype was more frequent in colorectal cancer patients stage IV than in stage I (80% versus 14%). There was no significant difference between genotypes and alcohol, tobacco, grade of differentiation or recurrence. CONCLUSION: Homozygosity for arginine was the most prevalent genotype in both groups. The frequency of codon 72 proline/arginine p53 gene polymorphism was not correlated with a higher risk of colorectal cancer. Arginine/arginine genotype was more prevalent in advanced cancer patients (stage IV).

BACKGROUND: Higher proliferation is commonly observed in cancer cells. Apoptosis can be a useful measure of a tumor cell kinetic. Alteration of the balance between proliferation and apoptosis is associated with cancer. AIM: To study proliferation and apoptosis on gastric cancer and in intestinal metaplasia. METHODOLOGY: Twenty-two samples from gastric adenocarcinomas and 22 biopsies from intestinal metaplasia were studied. The apoptotic bodies in hematoxylin-eosin slides and the expression of p53, bcl-2 and Ki67 were determined by immunohistochemistry. RESULTS: The number of the apoptotic cells was higher in cancer. Ki 67LI increased from intestinal metaplasia to gastric cancer. p53 was positive in 68% of the patients with cancer, more frequently in advanced stage and negative in samples of intestinal metaplasia. Although there was no significant difference between the groups, bcl-2 was positive in 45% of gastric cancer tissue and in 68% of metaplasia. In gastric cancer patients bcl-2 was expressed in early gastric cancer more frequently than in advanced stage. CONCLUSION: The positivity of bcl-2 was higher in metaplasia and probably is involved in the progression of carcinogenesis. p53 was negative in metaplasia and positive in more than half of the gastric cancer, mostly in stage IV, suggesting a late event in gastric cancer.RACIONAL: O aumento da proliferação celular é comumente observado no câncer e a apoptose pode ser medida útil na avaliação da cinética celular do tumor. Alterações do equilíbrio entre proliferação celular e apoptose estão associadas ao câncer. OBJETIVO: Avaliar a proliferação celular e a apoptose no câncer gástrico e na metaplasia intestinal. CASUÍSTICA E MÉTODOS: Vinte e dois tecidos de adenocarcinoma gástrico e 22 biopsias de metaplasia intestinal foram estudados. Os corpos apoptóticos foram estudados em lâminas coradas pela hematoxilina-eosina e a expressão de p53, bcl-2 e Ki67 por imunoistoquímica. RESULTADOS: O número de corpos apoptóticos foi maior no câncer. O Ki67LI aumentou no câncer gástrico em relação ao tecido com metaplasia intestinal. O p53 foi positivo em 68% dos pacientes com câncer, principalmente nos tumores avançados e negativo nos indivíduos com metaplasia intestinal. O bcl-2 foi expresso em 45% dos tecidos com câncer gástrico e em 68% dos tecidos com metaplasia, no entanto, esta diferença não foi significante. Nos doentes com câncer gástrico precoce, o bcl-2 foi expresso com maior freqüência quando comparados aos com câncer em estádio avançado. CONCLUSÃO: A expressão do bcl-2 foi maior no tecido com metaplasia, sugerindo envolvimento desta proteína na evolução da carcinogênese. O p53 foi negativo nos tecidos com metaplasia intestinal e positivo em 50% dos tecidos tumorais, principalmente nos doentes com câncer estádio IV, sugerindo ser um evento tardio no câncer gástrico.

CONTEXT: The body mass index (BMI) is the most common marker used on diagnoses of the nutritional status. The great advantage of this index is the easy way to measure, the low cost, the good correlation with the fat mass and the association to morbidity and mortality. OBJECTIVE: To compare the BMI differences according to the WHO, OPAS and Lipschitz classification. METHODS: A prospective study on 352 patients with esophageal, gastric or colorectal cancer was done. The BMI was calculated and analyzed by the classification of WHO, Lipschitz and OPAS. RESULTS: The mean age was 62.1 ± 12.4 years and 59% of them had more than 59 years. The BMI had not difference between the genders in patients

Racional - O tratamento e o prognóstico dos pacientes com câncer gástrico dependem, principalmente, do estádio clínico. Os marcadores tumorais séricos e do lavado peritonial podem auxiliar a avaliar o risco de recurrência da doença. Casuística e Métodos - Quarenta pacientes com câncer gástrico (11 estádio I ou II e 29 estádio III ou IV) e 24 com doença benigna foram estudados prospectivamente. Todos os doentes foram submetidos a laparotomia. O sangue e o lavado peritonial foram colhidos durante o ato cirúrgico, antes da retirada do tumor, para determinação dos marcadores CEA e CA72-4. Resultados - Vinte e cinco por cento e 47,5% dos pacientes com câncer gástrico apresentam elevação dos níveis séricos de CEA e CA72-4. Através das curvas ROC definiram-se os valores de corte dos marcadores no lavado peritonial. Através destas curvas, observaram-se que 60% e 57,5% apresentavam CEA e CA72-4 elevado, respectivamente no grupo com câncer gástrico. Os valores de CEA e CA72-4 foram maiores nos pacientes estádios III e IV. No lavado peritonial, os níveis de CEA foram maiores nos doentes com tumores T3-4. Os valores de CA72-4 no lavado peritonial diferenciaram o grupo controle do grupo com câncer gástrico. Conclusão - O CA72-4 foi o marcador sérico mais sensível no diagnóstico de câncer gástrico. Entretanto, no lavado peritonial, o marcador mais sensível foi o CEA. Os valores de CEA foram superiores nos tumores que ultrapassam a serosa e inferiores nos tumores que se restringem a mucosa e muscular.Background - The treatment and the prognosis of gastric cancer patients depends mainly on clinical stage. Serum and peritoneal tumoral markers levels can be helpful to evaluate individual risk for recurrence. Aims - To evaluate the sensibility of the tumoral markers in the serum and in the peritoneal washing on diagnosis of gastric cancer. Patients and Methods - Forty patients with adenocarcinoma of the stomach (11 stage I or II and 29 III or IV) and 24 patients with benign diseases were studied prospectively. All of them were submitted to laparotomy. Blood and peritoneal washed was collected during surgery before tumoral resection, for determination of CEA and CA72-4. Results - CEA and CA 72-4 serum levels were elevated in 25% and 47,5% respectively. Through the curves ROC, we defined the cut-off values for the markers in washed peritoneal fluid. Through these values CEA and CA72-4 rose in 60% and 57.5% respectively. The values of CEA and of CA 72-4 in the serum and in washed peritoneal fluid were higher in cancer patients stage III and IV. CEA levels in the peritoneal washed fluid were higher in the patients with tumor T3-4. Washed peritoneal CA72-4 differed the control group from the cancer group. Conclusion - CA72-4 was the most sensitive marker in the serum of the patients with gastric cancer. Otherwise in the washing peritoneal fluid the most sensitive marker was CEA. These levels were higher in patients with surpass the serosa and lower in patients with mucosa or muscular tumors.

Racional - O câncer de canal anal é um tipo raro de neoplasia, sendo responsável por 4% dos tumores de intestino grosso. São descritos os aspectos clínicos e o tratamento dos pacientes com câncer escamoso desta região. Casuística - Analisaram-se, retrospectivamente, 11 pacientes com carcinoma espinocelular de canal anal. Nove pacientes eram do sexo feminino e dois do masculino. A idade média dos pacientes foi de 57,6 anos (variação 35-82 anos). Resultados - Os sintomas mais freqüentes foram sangramento retal, tumoração e dor local. Seis pacientes tinham lesão benigna prévia local e dois tinham doença metastática na ocasião do diagnóstico. Todos foram submetidos a quimioterapia sistêmica com 5-fluorouracil e mitomicina. Após o tratamento quimio e radioterápico, quatro apresentavam doença residual e foram submetidos a ressecção abdômino-perineal. Três pacientes apresentaram recidiva e quatro morreram pela evolução da própria doença. Conclusão - A maioria dos doentes era do sexo feminino. A químio e a radioterapia podem ser tratamentos curativos nos pacientes com doença localizada; no entanto, aqueles com doença residual devem ser submetidos a ressecção abdômino-perineal. Embora o câncer espinocelular de canal anal seja doença curável, quatro pacientes foram a óbito por terem sido diagnosticados em fase avançada.

Introdução - A presença de uma segunda neoplasia maligna, tendo sítio e histologia diferentes da primeira, define o conceito de neoplasias primárias múltiplas. Com o aumento da sobrevida dos pacientes com câncer, houve um aumento de neoplasias primárias múltiplas. Este estudo objetiva avaliar a freqüência de neoplasias primarias múltiplas em pacientes com diagnóstico de câncer colorretal do Setor de Oncologia da Disciplina de Gastroenterologia da Universidade Federal de São Paulo. Pacientes e Métodos - No período de 1993 a 1998, 145 pacientes com câncer colorretal foram acompanhados no Setor de Oncologia da Disciplina de Gastroenterologia da Universidade Federal de São Paulo. Destes, cinco pacientes (3,4%) tinham neoplasias primárias múltiplas. O segundo tumor foi confirmado por exame histológico, sendo excluídas as possibilidades de metástase. Resultados - A idade média dos doentes com neoplasias primárias múltiplas foi de 60,6 anos, quatro eram do sexo feminino e um do masculino, três apresentaram doença no reto, um no cólon direito e um no cólon esquerdo. Os outros sítios foram mama, útero, útero e vagina, pele e lábio. Um paciente foi a óbito e os outros estão em acompanhamento, sendo que dois, há mais de três anos. Dois pacientes tinham sido submetidos a radioterapia pélvica antes do aparecimento da neoplasia colorretal. Em um paciente o tumor colorretal antecedeu o outro câncer e em quatro apareceu posteriormente ao diagnóstico da outra neoplasia primária. Discussão - A prevalência de neoplasias primárias múltiplas foi de 3,4%, sendo maior no sexo feminino. Câncer de endométrio foi o outro tumor maligno mais freqüente. Quarenta por cento dos nossos pacientes foram submetidos a radioterapia. Acreditamos ser importante o acompanhamento dos pacientes para o diagnóstico precoce de uma nova neoplasia em qualquer paciente que já tenha apresentado um câncer.Introduction - Multiple primary neoplasms are defined as a second malignance having histology and site different from the first. The increase of the life expectation in cancer patients leads to an increase in multiple primary neoplasms incidence. This study analyzes the characteristics of patients with colorectal cancer and another primary neoplasm. Patients and Methods - In the period from 1993 to 1998, 145 patients with colorectal cancer were accompanied in the Oncology Division of Gastroenterology of Federal University of São Paulo, São Paulo, SP, Brazil. Five patients (3.4%) had multiple primary neoplasms. The possibility of metastasis were excluded and the second cancer was confirmed by hystological examination. Results - The medium age was of 60.6 years old, four were female and one male. Three had rectum cancer and two colon cancer, one in the right colon and one in the left colon. The other site of cancer was breast, uterus, uterus and vagina, skin and lip. One patient died and the others were in attendance, two for more than three years. Two patients received pelvic radiotherapy before the rectal cancer. In one patient the tumor colorectal cancer appeared before the other cancer, and in four it appeared later on to the diagnosis of the other primary neoplasia. Discussion - The prevalence of multiple primary neoplasms was of 3,4%, being major in female. Uterus' cancer was the more frequent association. Radiotherapy was performed in 40% of patients. We believe that attendance of cancer patients is very important to precocious diagnosis and treatment of multiple primary neoplasms.

Introdução - Estudos epidemiológicos e gênicos mostram que os pólipos colônicos do tipo adenoma são lesões pré-neoplásicas. O seguimento de um paciente com câncer colorretal visa principalmente o diagnóstico e retirada de pólipos adenomatosos. Objetivo - Estudar a freqüência do aparecimento de pólipos adenomatosos em doentes com câncer colorretal em acompanhamento clínico após ressecção tumoral. Casuística e Métodos - Foi estudada, retrospectivamente, a freqüência de pólipos do tipo adenoma em 68 pacientes com idade média de 59 anos, submetidos a cirurgia curativa para tratamento de câncer colorretal e a colonoscopia de controle. Resultados - A incidência de pólipos adenomatosos foi de 18%, sendo maior acima de 45 anos (20%) e no cólon esquerdo (38%). Em relação ao tipo histológico, 61% eram tubulares, 22% vilosos e 17% túbulo-vilosos. Discussão - Assim como em outros relatos da literatura, a incidência de pólipos foi superior em indivíduos com mais de 45 anos, sendo em mais da metade do tipo tubular. A freqüência de pólipos foi maior nos dois primeiros anos de acompanhamento.Introduction - Epidemiologic and molecular biologic studies have already demonstrated that adenomatous colonic polyps are precancerous diseases. The main indication of the colonoscopy in the surveillance of colorectal cancer treated patients is the diagnosis and resection of adenomatous polyps. Aim - To study the frequency of adenomatous polyps after surgically resection of colorectal cancer. Material and Methods - Sixty eight patients, mean age 59 years old, with total resection of colorectal cancer, submitted to various colonoscopies during the follow up were studied retrospectively. The histological type and the characteristics of the polyp were described. Results - The frequency of polyps was 18%, being higher in the patients with more than 45 years (20%). The site of the polyps was in the left colon in 38% of the patients with cancer. The histological type of adenomas was tubular in 61%, villous in 22% and mixed in 17%. Discussion - As described by other authors, the incidence of polyps were higher after 45 years old and more than a half of them were tubular. The frequency of polyps was higher in the first two years of follow up.

We report herein a rare instance in which a patient presented with a hemorrhagic cerebral metastasis as the initial manifestation of a hepatocellular carcinoma (HCC). A few cases of cerebral metastasis from HCC have been reported in the literature, mainly from eastern countries. This is the first report from South America of a cerebral metastasis from hepatocellular carcinoma.
