Introduction: Serum protein electrophoresis (SPEP) is a laboratory test used in a wide variety of settings. Performing a SPEP can lead to the diagnosis of monoclonal gammopathy of uncertain significance (MGUS), a clinically asymptomatic premalignant clonal plasma cell or lymphoplasmacyte proliferative disorder that requires monitoring. If the growing incidence of MGUS can be explained by an aging population and more efficient laboratory techniques, the increasing number of SPEP prescriptions is also presumably a factor. We sought to understand the motives for the prescription of SPEP in everyday practice. Methods: A descriptive observational study of 295 SPEP collected from 17 laboratories in Gironde from May the 30th to June the 10th 2016. We gathered informations from patients’ medical records through a standardized questionnaire sent to the prescriber of each test. Results: 60.7% of SPEP were prescribed because patients presented with specific symptoms or abnormal laboratory tests. The main symptoms and signs were fatigue, weight loss, joint pain, repeated infections, raised inflammatory markers. In 24.1% of cases SPEP was prescribed as part of the follow-up of a known disorder (mainly haematological). Finally 14.6% of SPEP were requested as screening tests in asymptomatic patients. Overall, 9 monoclonal gammopathies and 24 hypogammaglobulinaemias were discovered. Conclusion: SPEP is largely prescribed as a screening tool in asymptomatic patients or in those presenting with non-specific symptoms such as fatigue and weight loss, in disagreement with recent recommendations by the HAS., Introduction : l’électrophorèse des protéines sériques (EPS) est un examen de biologie utilisé dans des circonstances cliniques et paracliniques variées. La réalisation d’une EPS peut conduire à la découverte fortuite d’une gammapathie monoclonale de signification indéterminée (GMSI), entité asymptomatique témoignant de la prolifération clonale de plasmocytes ou lymphoplasmocytes mais dont l’évolution potentiellement maligne nécessite une surveillance. Si l’augmentation de l’incidence de la GMSI s’explique par le vieillissement de la population et les améliorations techniques au laboratoire de biologie, l’accroissement du nombre de prescription des EPS est aussi probablement en cause. Nous avons cherché à comprendre les motifs de prescription de cet examen biologique en pratique quotidienne. Matériel et méthodes : étude observationnelle descriptive de 295 EPS collectées auprès de 17 laboratoires girondins entre le 30 mai et le 10 juin 2016. Nous avons recueilli les données spécifiques à chacun des examens auprès de son médecin prescripteur. Résultats : l’EPS est prescrite lorsque sont observées des anomalies cliniques ou biologiques, ce qui représente 60,7% des cas, les principaux signes étant une asthénie (citée dans 57% de ces observations), un amaigrissement, des douleurs articulaires, des infections à répétition ou un syndrome inflammatoire biologique… Dans 24,1%, la prescription rentrait dans le cadre d’un bilan de suivi ou de découverte d’une pathologie précise (principalement hématologique). Enfin, 14,6% résultent de prescriptions systématiques. Sur l’ensemble des 295 EPS, 9 pics d’immunoglobuline et 24 hypogammaglobulinémies ont été découverts. Conclusion : un grand nombre d’EPS sont prescrites lors de la réalisation d’un bilan systématique ou en présence de signe généraux non spécifiques (asthénie, amaigrissement…), en contradiction avec les recommandations récentes de la HAS.
