8. Predictors of Ascertainment of Autism Spectrum Disorders across Nine US Communities

Abstract

Autism spectrum disorders (ASD) prevalence estimates derived from a single data source under-identify children and provide a biased profile of case characteristics. We analyzed characteristics of 1,919 children with ASD identified by the Autism and Developmental Disabilities Monitoring Network. Cases ascertained only at education sources were compared to those identified at health sources. 38% were education-only. These were older at their earliest evaluation (54.5 vs. 42.0 months, p < 0.001) and earliest ASD diagnosis (62 vs. 53 months, p < 0.001). More lived in census blocks with lower adult education (p < 0.001). Lower educational attainment of adults in census blocks of residence of education-only cases suggests disparities in access to clinical services with the schools providing crucial services to many families.

19. Use of complementary and alternative medical therapies among children with special health care needs in Southern Arizona

Abstract

Objective. The objective of this study was to assess the frequency and type of complementary and alternative medicine (CAM) therapies used by families of children with special health care needs in southern Arizona, as well as the correlates of their use. Methods. Families of 376 children who were receiving services in a regional facility that serves children with special health care needs and were residing in southern Arizona were surveyed regarding CAM use. Results. Sixty-four percent of these families reported using CAM for their child. The most common CAM therapies were spiritual healing/prayer/blessings. Of the conditions that were evaluated as correctable, the use rate was 24% as compared with a 76% use rate for children with a nonrepairable condition. Use of CAM for the child was strongly related to the use of CAM in the past by the family member who responded to the survey. The reasons that parents most frequently chose for using CAM were advice from a medical practitioner and advice from a family member. Conclusions. Use of CAM for children with special health care needs is common. Its frequency and type are significantly associated with the child&apos;s condition and prognosis. Abstract

Prader-Willi syndrome is characterized by the loss of muscle tone during infancy, then later on, during early childhood, obesity, mental retardation, diminished pigmentation, small hands and feet, short height, impaired secretion from the ovaries and testes, and facial peculiarities. This disorder affects 1 in 16,000 infants and appears to result from a genetic defect. There is limited information about the growth of children with Prader-Willi syndrome. Certain anthropometric measurements were made in 71 children and adults under age 24 years with Prader-Willi syndrome in order to develop standards regarding growth. Weight, height, sitting height, three different head measurements, four hand and two foot measurements, and two skinfold measurements were made for each patient. The results were compared to data obtained from normal subjects. The anthropometric standards for Prader-Willi syndrome presented in this article should be useful in examining and comparing patients with this disorder and in the diagnosis of this syndrome in younger children. Abstract

To investigate factors related to lung cancer mortality in four Arizona copper-smelter towns, the authors identified 142 lung cancer cases and 2 matched controls per case from decedent residents during 1979-1990. The authors obtained detailed information on lifetime residential, occupational, and smoking histories via structured telephone interviews with knowledgeable informants. The authors linked estimated historical environmental exposures to smelter emissions (based on atmospheric diffusion modeling of measured sulfur dioxide concentrations) with residential histories to derive individual profiles of residential exposure. The results of this study provided little evidence of a positive association between lung cancer and residential exposure to smelter emissions.
