The proper use of inhaler devices can improve medication efficacy while decreasing dose and adverse effects. The main objective of this study was to investigate the effect of pharmacist-led asthma education interventions on inhaler techniques. This study was a controlled, randomized, prospective intervention, in Wad Medani Teaching Hospital outpatient clinic, between March and June 2021.Structured questionnaire with validated checklists of inhaler techniques were used to collect data, from patients at baseline and at three months of follow-up, for both intervention and control groups. Paired and independent t-tests were used to monitor the effectiveness of the interventions. A total of 130 asthmatic patients were enrolled in this study. The intervention group contains 70 patients (53.8%), while the control group contains 60 patients (46.2%). The mean score for the inhaler techniques of Pressurized metered-dose inhalers(PMDIs) was (3.50±1.43) for the intervention group at baseline and increased after the intervention to (6.78±0.515), this result was significant at the (P = 0.05) level, and the mean score of the dry powder inhaler technique at the initial of this study in the intervention group was (4.72 ±1.50) this means were improved after the intervention to (6.83± 0.44) this result was statistically significant. There was no significant difference between the control group before and after intervention (P=0.321).This study has shown that the pharmacist-led educational intervention improved the inhalation technique of asthmatic patients. It is important to consider these programs as routine health care to enhance inhaler techniques.

Pediatric Epstein-Barr virus-negative monomorphic post solid organ transplant lymphoproliferative disorder [EBV(-)M-PTLD] comprises approximately 10% of M-PTLD. No large multi-institutional pediatric-specific reports on treatment and outcome are available.A multi-institutional retrospective review of solid organ recipients diagnosed with EBV(-)M-PTLD aged ≤21 years between 2001 and 2020 in 12 centers in the United States and United Kingdom was performed, including demographics, staging, treatment, and outcomes data.Thirty-six patients were identified with EBV(-)M-PTLD. Twenty-three (63.9%) were male. Median age (range) at transplantation, diagnosis of EBV(-)M-PTLD, and interval from transplant to PTLD were 2.2 years (0.1-17), 14 years (3.0-20), and 8.5 years (0.6-18.3), respectively. Kidney (n = 17 [47.2%]) and heart (n = 13 [36.1%]) were the most commonly transplanted organs. Most were Murphy stage III (n = 25 [69.4%]). Lactate dehydrogenase was elevated in 22/34 (64.7%) and ≥2 times upper limit of normal in 11/34 (32.4%). Pathological diagnoses included diffuse large B-cell lymphoma (n = 31 [86.1%]) and B-non-Hodgkin lymphoma (B-NHL) not otherwise specified (NOS) (n = 5 [13.9%]). Of nine different regimens used, the most common were: pediatric mature B-NHL-specific regimen (n = 13 [36.1%]) and low-dose cyclophosphamide, prednisone, and rituximab (n = 9 [25%]). Median follow-up from diagnosis was 3.0 years (0.3-11.0 years). Three-year event-free survival (EFS) and overall survival (OS) were 64.8% and 79.9%, respectively. Of the seven deaths, six were from progressive disease.EFS and OS were comparable to pediatric EBV(+) PTLD, but inferior to mature B-NHL in immunocompetent pediatric patients. The wide range of therapeutic regimens used directs our work toward developing an active multi-institutional registry to design prospective studies.Pediatric Epstein-Barr virus-negative monomorphic post solid organ transplant lymphoproliferative disorders (EBV(-)M-PTLD) have comparable outcomes to EBV(+) PTLD, but are inferior to diffuse large B-cell lymphoma in immunocompetent pediatric patients. The variety of treatment regimens used highlights the need to develop a pediatric PTLD registry to prospectively evaluate outcomes. The impact of treatment regimen on relapse risk could not be assessed because of small numbers. In the intensive pediatric B-non-Hodgkin lymphoma chemoimmunotherapy group, 11 of 13 patients remain alive in complete remission after 0.6 to 11 years.

Asthma is a heterogeneous disorder associated with chronic airway inflammation, its prevalence is increasing globally and more prevalent in developed countries. This study aimed to determine the level of asthma control at Wad Medani Teaching Hospital Outpatient Clinic. A cross-sectional study was performed in 130 adult asthmatics in Wad Medani Teaching Hospital Outpatient Clinic between March to June 2021. Data were collected using a standardized questionnaire, and Asthma Control Test (ACT) was used to determine the level of the control. Multivariate analysis and Logistic Regression were used to determine the most factors that have a role in asthma control. The mean age of the study participants was 35 years (±14) and females were predominant (68.5 %). Among all asthmatics, the prevalence of uncontrolled asthma was 75.4% according to the ACT Score. Mangrove (Avicennia marina) is a “green lung” tree growing along the Arabian Gulf coastline in the United Arab Emirates. Here, we aimed to determine the impact of the application of a commercial seaweed extract (SWE) biostimulant and endophytic actinobacterial isolates on growth performance and endogenous hormonal levels of mangroves. Therefore, we isolated endophytic plant growth-promoting (PGP) actinobacteria (PGPA) from mangrove roots and evaluated their potential as biological inoculants on mangrove seedlings under greenhouse and open-field nursery conditions. Seven salt-tolerant isolates had the ability to produce different levels ofin vitroplant growth regulators (PGRs) and 1-aminocyclopropane-1-carboxylic acid (ACC) deaminase (ACCD) and to solubilize phosphorus. Accordingly, only one isolate,Streptomyces tubercidicusUAE1 (St), was selected based on its relative superiority in displaying multiple modes of action and in successfully colonizing mangrove tissues for 15 weeks. In the greenhouse experiments, plants treated withStand SWE significantly (p< 0.05) improved dry biomass by 40.2% and 55.1% in roots and 42.2% and 55.4% in shoots, respectively, compared to seawater-irrigated non-treated mangrove plants (control). However,St+SWE caused a greater significant (p< 0.05) increase in dry weight of roots (67.6%) and shoots (65.7%) than did control plants. Following the combined treatment ofSt+SWE,in plantaPGR levels were found to be greatly enhanced over the non-treated control plants grown in non-SWE supplemented sediments, or plants inoculated with onlySt without the supplementation with SWE,or with non-inoculated plants grown in sediments supplied with SWE only. This was evident from the significant (p< 0.05) increases in the photosynthetic pigments and production of PGRs, as well as the reduction in the endogenous ACC levels of plant tissues compared to those in other treatments. Tissue nutrient contents of seedlings also increased by at least two-fold inSt+SWE treatment as compared to control. Similar effects were observed on all growth parameters under natural open-field nursery conditions. CombiningStwith SWE not only stimulates plant growth but also potentially has additive effects on mangrove ecosystem productivity in nutrient-impoverished soils in the Arabian coastal areas. This report is the first in the field of marine agriculture that uses SWE as a nutrient base for actinobacteria capable of producing PGRs and ACCD.

Modern bearings used the lubricants having polymer components of larger molecular weight for higher speed and high load operations. These lubricants show rheological activity that is non-Newtonian. Power-law lubricants are commonly used for industrial and bearing lubrication operations. This article is modeled through the slip as interfacial condition to demonstrate the role of power-law lubrication on the stagnation point flow of Walters-B nanofluid. The thermophoretic force and Brownian movement factors are accounted in energy and mass species expressions. The expressions in partial differential forms are constructed for both the base and lubricant fluids. The system is re-structured into ordinary differential expressions by the implication of dimensional analysis. An approximate and numerical scheme namely the hybrid homotopy analysis method is reported to demonstrate the solutions of nonlinear expressions. The behavior of involved constraints on practical quantities is shown and analyzed. The nature of friction factor, thermal and solutal rates at the surface is reported through numeric data. Match of the present solution with previously published data shown better agreement. It can be seen that the slip phenomenon dominates over the free-stream phenomenon. The large values of viscoelastic parameter decay the thermal and solutal fields.

The present study focused on addressing the factors influencing the adoption of homestead vegetable farming by rural farmers in a selected region of Bangladesh. It also assessed the farmer’s perception of vegetable farming in their homestead areas. A total of 92 households were interviewed randomly through field survey method. Perception index, gross margin and net return analysis, Logit regression model were employed to analyze the collected data. Most of the household’s liquidity asset increased after involving in homestead vegetable farming which has been identified from perception index analysis. Vegetable cultivation in the homestead areas was profitable as the undiscounted benefit cost ratio for three selected vegetables was greater than one. The chances of farmers to practice homestead vegetable farming are increased with their level of education, farm size, household income, extension contact and availability of irrigation water. However, engagement in off-farm activities by farmers reduced the chance of involvement in homestead vegetable farming as the farmers had lesser time to devote in such activities. For encouraging farmers to practice homestead vegetable farming, both government and private institutions should play their role to provide regular extension service, training facilities and to ensure availability of irrigation water.

The addition of clinical pharmacists to the HF team positively impacts optimizing prognostic medications for a patient with HF with reduced ejection fraction (HFrEF).Objective: To assess the intervention of the clinical pharmacist as part of the multidisciplinary (MD) team in up-titration to achieve target doses of key therapeutic agents for HFrEF.Methods: This was a prospective one group pretest-posttest interventional study; a comparison of the target dose achievement of key therapeutic agents for HFrEF was performed before and after clinical pharmacist interventions.Results: Out of 110 HFrEF patients, 57.3% were males, and the mean age of patients was 55.8 years (SD 12.6). Cardiomyopathy was the leading cause of HF. At baseline, 86% were on angiotensin-converting enzyme inhibitors/angiotensin receptor blockers/angiotensin receptor-neprilysin inhibitors (ACEIs/ARBs/ARNi) and 93.6% on beta blockers (BBs). At the end of study, the proportion of patients achieved the target dose was significantly increased (0 vs 77.4%, 6.8 vs 85.4%, and 0 vs 55.6%) for ACEIs, ARBs and ARNi, respectively, and (8.6% vs 66.1%; P = 0.001) for BBs. Moreover, the up-titration process was associated with significant improvement in most clinical as ejection fraction and New York Heart Association (NYHA) scale and laboratory characteristics.Conclusion: As a part of the MD team in the outpatient HF clinic, the clinical pharmacists increased the percentage of HFrEF patients achieving the target or maximal doses of key therapeutic agents and improving clinical and laboratory parameters.Keywords: clinical pharmacist, intervention, heart failure, medications titration, Sudan

Aims: The aim of the study was to describe child health in relation to housing renovations in more than 800 rental units, consisting of repairs of dilapidated kitchens and bathrooms, in the disadvantaged neighbourhood of Herrgården in Rosengård, Malmö, Sweden. Methods: Data on housing conditions and self-reported health were collected during home visits to families living in Herrgården (building renovations area) and a comparison area (neighbouring Törnrosen, with generally better housing conditions). At baseline, 130 families with 359 children participated, while 51 families with 127 children participated at follow-up. All data were collected between 2010 and 2012. Additionally, regional register data on health-care usage/in- and outpatient contacts within the public health-care system between 2008 and 2013 were also collected for all 8715 children registered as living in the two areas. Results: Self-reported health seemed to somewhat improve in both areas, with 74% versus 86% and 78% versus 88% reporting good or very good health in Herrgården and in the comparison area at baseline and follow-up, respectively. In Herrgården, crowdedness increased, while it decreased in the comparison area. The number of health-care contacts remained stable over time in Herrgården, while it decreased in the comparison area. Conclusions: Partial housing renovations did not seem to result in clear health improvements as measured with the indicators used in the present study. This could possibly be due to persisting health effects due to increased crowdedness or persisting poor housing conditions, as only kitchens and bathrooms were renovated.

Objectives: The main objective of the present study is to obtain callus and cell suspension culture from Balanitesa egyptiaca sterile plantlets grown in vitro and to compare growth and the biosynthetic potential of saponins, flavonoids and glycosides by callus and cell suspension culture Balanitisa egyptiaca. Methods: Callus was induced from the mother plants on MS culture media supplemented with 2.0 mg/l BA + 2.0 mg/l 2,4-D with and without agar gelling. Total saponins, glycosides and flavonoids were estimated in both types of cultures over a period extending from 1 to 5 weeks to compare the productivity of such secondary metabolites in callus and cell suspension cultures. Results: The results obtained indicated that both calli and cell suspension cultures were able to synthesize the target active ingredients and that cell suspension culture was superior to the callus culture in the biosynthesis and accumulation processes. By the end of the incubation period, the amount of total saponins in cell suspension culture reached up 51.97±0.26 dry biomass compared to 35.02 ±0.06 mg/g in callus culture. The amount of total flavonoids in cell suspension culture reached up 10.88±0.24 dry biomass compared to 6.40±0.02 mg/g in callus culture and of total glycosides reached up 6.11±0.25 dry biomass compared to 5.06 ±0.05 mg/g in callus culture. Conclusions: The results obtained in this study may indicate the promising role that plant cell culture will play in the future in phytopharmaceutical industry. Massive extraction of groundwater for boro rice is the main cause of declining groundwater tables, especially in Rangpur Division, which is now a burning issue at the national level. It is now scientifically proven that rice is not a water-loving plant, but rather a water-tolerant plant. AWD can save water and energy, where both farmers and pump-owners can save around 30% of their irrigation water requirements. In Bangladesh around 4.8 million hectares of land are brought under irrigated boro rice where AWD technology can be utilized. With introduction of AWD technology, a savings of electricity costs equivalent to 5 billion Tk. or fuel cost equivalent to 7 billion Tk. will be incurred at the national level. Hence, the implementation of AWD technology will have major impacts at the farm and national levels, which can reduce irrigation cost significantly, thus saving foreign currency, as well as reducing excessive use of ground water.J. In the present paper, we investigated unsteady three-dimensional flow of an incompressible magneto Maxwell nanomaterial in the porous medium. The cause of flow is unsteady stretching of sheet in transverse lateral directions taking zero mass flux of nanoparticles. The developed system of partial differential equations is transformed into nonlinear ordinary differential equations by similarity variables, and the obtained system is solved for dimensionless temperature and nanoparticles concentration by homotopy analysis method. The behavior of physical parameters on the temperature and nanoparticles concentrations is examined through graphs and tabular data. The temperature distribution at the surface is also computed and analyzed and noticed that both the temperature and nanomaterial are reduced for higher values of unsteadiness parameter.

Objective: Prunus dulcis (Almond) belongs to the family Rosaceae and is rich in polyphenols, including hydroxybenzoic acids and aldehydes, flavonol and flavanone aglycones, and glycosides. The objective of current study was phytochemical screening of chemical constituents of Prunus dulcis extract. Methods: In this study methanolic and aqueous extracts of one plant namely Prunus dulcis were screened for the presence of phytochemical constituents and tested for their antimicrobial and antioxidant activity. The qualitative phytochemical analysis revealed the results showed presence of alkaloids, terpenoids, glycosides, resins, saponins, tannins, flavonoids, phenols, and amino acid were present in the methanol extract, with absence of glycosides, and amino acids in the aqueous extracts in leaves plant. Results: TLC tests conducted revealed Rf values in the leaves for alkaloids, flavonoids, tannins, phenols and saponins(0.92-0.96-0.96-0.95-0.96) respectively. The antimicrobial activity extracts against four bacterial isolates Staphylococcus aureus, Escherichia coli , Pseudomonas aeruginosa and Klebsiella sp. and a single fungal isolate Candida albicans with concentrations (0.5 mg/ml, and 1,0 mg/ml) of the extract were added to the disc and respective solvent was used as negative control. The antioxidative activity of leaf was evaluated by using 1,1- diphenyl-2 picrylhydrazyl (DPPH), the results showed are 85.5%, lowest from standard, ascorbic acid 87.5%. Conclusion: The present study showed that prunus dulcis are rich sources of useful secondary metabolites, suitable for use for medicinal purpose like treat wounds and burns diseases. Background: The growth rate of elderly is the fastest in most of the developing countries including Bangladesh. Nutrition plays fundamental role in maintaining good health especially in old age. This study was an attempt to assess the nutritional status of elderly and to compare it with their location.Methods: A total of 400 elderly in urban and rural area of Sunamgonj district have been interviewed through a structured questionnaire during July to September, 2019. A person aged 60 and above is included in the sample. Body mass index (BMI), mini nutritional assessment (MNA) short form, descriptive statistics, and Chi square test for association have been used for data analysis.Results: The analysis revealed that 17 percent elderly were under-weight, 66 percent were normal nutrition, 13 percent were over-weight and 4 percent were obese. The prevalence of malnutrition was found to be 40 percent. The rural elderly were at more risk of being malnourished compared to urban elderly. The mean BMI and MNA score of urban elderly were significantly different from the rural elderly.Conclusions: The findings show that more proportion of urban elderly are over-weight and obese compared to rural elderly. Therefore, measures should be taken to improve the nutrition status of elderly especially in rural area.

Background: Natural products with therapeutic properties such as plants, minerals and animal products, for many years, were the main sources of drugs for treatment of numerous diseases; hence selection of Lawsonia inermis L. (Henna) in order to study its hepatoprotective activity was considered. Objectives: This was an attempt to evaluate the hepatoprotective effect of Lawsonia inermis leaves methanolic extract on CCl4-induced hepatotoxicity in rats. Methods: The L. inermis leaves methanolic extract, which obtained by maceration, was orally administered in doses of 100mg/kg and 200mg/kg to the tested animals in order to assess its effects on serum levels of hepatotoxicity parameters, alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), bilirubin and total proteins along with histopathological liver sections examination, while silymarin (25mg/kg), a potent hepatoprotective drug, was used as standard control. Results: The two doses of the plant extract showed dose-dependent hepatoprotective effect, as evident by the significant reduction (P < 0.05) in serum levels of AST, ALT, ALP and bilirubin along with the improvement in histopathological liver sections compared to CCl4-only treated animals. Conclusion: As experimentally evident, it could be concluded that, this plant material could provide a hepatoprotective effect which could be attributed to its antioxidant properties. Abstract. Background: The long term efficacy of endovascular recanalization for chronic iliocaval occlusion secondary to inferior vena cava (IVC) filters is unknown. The purpose of this study was to evaluate the effectiveness of endovascular recanalization and stent placement across the filter in patients with filter-associated chronic iliocaval occlusion. Patients and Methods: Seven patients (mean age 56 ± 15 yrs; seven males) with symptomatic chronic iliocaval occlusion and occluded IVC filter were included. Immediate technical success rate, long term clinical effectiveness of endovascular recanalization and patency rate of the stents were assessed. Results: In all patients, the endovascular treatment consisted of percutaneous venous access, recanalization of the occluded iliac veins and the IVC, transluminal angioplasty and stenting of the infrarenal inferior vena cava and iliac veins with self-expanding stents. The IVC filter was not removed, but rather the stents were extended across the filter. In four of seven patients (57 %), adjunctive pharmaco-mechanical thrombolysis was performed. All patients received anticoagulation post procedure. The mean clinical follow-up was 51.1 ± 27 months. Technical success rate was 100 %. Clinical success rate with symptomatic improvement was 85.7 %; one patient developed post-thrombotic syndrome on long term follow-up despite initial symptomatic improvement. Poststenting, the primary patency rate was 85.7 % (six of seven patients) and the secondary patency rate was 100 % (seven of seven patients). Conclusions: Endovascular recanalization with balloon angioplasty and placement of a self-expanding stent across a chronically occluded IVC filer can be performed safely and effectively for patients with symptomatic iliocaval thrombosis. An adjunctive pharmacologic-mechanical thrombolysis may be considered for selected patients.

The relative efficiency of farming under tenancy systems in Bangladesh was measured in this study. Thirty cash tenants and 30 share tenants were randomly selected for data collection through field survey method from Narayanpur and Bhabokhali villages under Sadar upazila of Mymensingh district. The functional analysis of the study was based on Boro rice. The land tenancy situation in rural Bangladesh was explored in this study with a reasonable high incidence of owners who cultivate their own land in comparison to owner-cum-tenants and tenants. A number of tenurial arrangements including sharing agreements were also revealed in the study villages. Share tenant farmers earned significantly lower net return (Tk. 19,252.18) than the cash tenant farmers (Tk. 22,815.89) from Boro rice production. However, Boro rice production was profitable from the viewpoint of both tenant operators. Finally, the study tested and confirmed that all the explanatory variables (key production inputs) included in the Cobb- Douglas revenue type production function model were important for explaining the variations in gross returns under both tenancy arrangements. Background: Medicinal plants contain physiologically active principles that over the years have been exploited in traditional medicine for the treatment of various ailments. Objectives and Methods: This study aimed to investigate the effects of the methanolic extract of Pausinystalia yohimbe bark (5 mg/ml) on isolated rabbit aortic strip and rat uterus. Results: methanolic extract of P. yohimbe bark (5 mg/ml) produced relaxation of the phenylephrine precontracted-rabbit aortic strip. This relaxation may be resulted through nitric oxide (NO), since the pretreatment of the isolated rabbit aortic strip with methylene blue inhibited the NO-mediated relaxation. Moreover, the extract exhibited relaxation of rat uterine muscles, which appeared to be not mediated by activation of β2-adrenoceptors and/or H2 receptors, since the relaxant effect continued even after the pretreatment of the tissue with propranolol and ranitidine respectively. Conclusion: The obtained results revealed that methanolic extract of P. yohimbe bark caused relaxation of both isolated rat uterus and rabbit aortic strip through facilitating the role of endogenous compounds such as NO.

Haemostasis is the process of forming clots in the walls of damaged blood vessels to prevent abnormal bleeding and to maintain intravascular blood in a fluid state. Fenugreek is largely universal staple herb, popular throughout history and it has been consumed for treatment of different disorders. We aimed to study the possible anticoagulant effect of Fenugreek aqueous extract in vitro by using blood samples of normal individuals. In vitro anticoagulant effects of Fenugreek aqueous extract (5%) in different volumes (25, 50 and 75 μL) were examined in the blood samples of normal individuals by measuring prothrombin time (PT). The aqueous extract of Fenugreek was found to inhibit coagulation process in vitro and significantly prolonged prothrombin time in a dose-dependent manner. Fenugreek aqueous extract in different concentrations inhibits clot formation and increases prothrombin time. Subject to further studies on efficacy and safety, It can well be used, in the future, as a supplementary anticoagulant agent in cardiovascular diseases and to prevent hypercoagulable states.

This study was investigated with the time series data to know the trend of foreign visitors in Bangladesh. To estimate the trend of visitors, data was used for the year 1976–2008. The several growth models were used and found that semi-log trend model was more suitable for foreign visitors. It is also found that the number of foreign tourists visiting Bangladesh has registered a significantly upward trend with a growth rate of 5.79 percent per annum. The monthly arrivals data reveal pronounced seasonality. The best arrival month is January with seasonal index of 122.99 percent which implies that seasonally arrivals are increased by 22.99 percent in that month. September is the slower or worse month with seasonal index 81.81 percent which implies that seasonally arrivals are decreased by 18.19 percent in the month.

Microtubules in living cells are very important component for various cellular functions as well as to maintain the cell shape. Mechanical properties of microtubules play a vital role in their functions and structure. To understand the mechanical properties of microtubules in living cells, we developed an orthotropic-Pasternak model and investigated the vibrational behavior when microtubules are embedded in surrounding elastic medium. We considered microtubules as orthotropic elastic shell and its surrounding elastic matrix as Pasternak foundation. We found that due to mechanical coupling of microtubules with elastic medium, the flexural vibration is increased with the stiffening of elastic medium. We noticed that foundation modulus (H) and shear modulus (G) have more effect on radial vibrational mode as compared to longitudinal vibrational mode and torsional vibrational mode.

Motivated by the application of Winkler-like models for the buckling analysis of embedded carbon nanotubes, an orthotropic Winkler-like model is developed to study the buckling behavior of embedded cytoskeletal microtubules within the cytoplasm. Experimental observations of the buckling of embedded cytoskeletal microtubules reveal that embedded microtubules bear a large compressive force as compared with free microtubules. The present theoretical model predicts that embedded microtubules in an elastic medium bear large compressive forces than free microtubules. The estimated critical pressure is in good agreement with the experimental values of the pressure-induced buckling of microtubules. Moreover, due to the mechanical coupling of microtubules with the surrounding elastic medium, the critical buckling force is increased considerably, which well explains the theory that the mechanical coupling of microtubules with an elastic medium increases compressive forces that microtubules can sustain. The model presented in the paper is a good approximation for the buckling analysis of embedded microtubules.
