Background: The identification and treatment of psychiatric disorders in individuals with autism spectrum disorders (ASD) and ID presents many challenges. We describe the development of a professional network, together with a standardized protocol for clinical assessment, designed to promote clinical competence and professional development in eight clinical centers responsible for providing mental health services to autistic individuals with ID across all four health regions of Norway. Specific aims to describe: (1) patterns of psychiatric and behavior problems in patients treated by the network, (2) patterns of change over time, and (3) the relationship between psychiatric disorders and behavior problems. Method: A standardized protocol was used to assess individual progress in 132 patients (inpatients and outpatients) with autism and ID over 2 years (at referral (T1), after 1 year (T2), and after 2 years (T3)). Changes in psychiatric symptoms and behavior problems were assessed with the Psychopathology in Autism Checklist (PAC) and Aberrant Behavior Checklist (ABC). Results: Patients showed significant (p <.001) improvements from T1 to T2 on the psychosis, depression and anxiety subscales of the PAC, but no significant improvement on the obsessive compulsive disorder (OCD) subscale. Improvements were maintained from T2 to T3. Patients showed significant (p <.01) improvements on the ABC total score and on all ABC subscales except inappropriate speech from T1 to T2; these improvements were maintained from T2 to T3. Discussion The combination of a professional network and a standardized protocol for clinical assessment has promise as a strategy for improving professional competence and facilitating specialized mental health services for autistic individuals with ID and psychiatric disorders across an extensive geographical area. KONGRESSENS VEDTAK OM SANKSJONER MOT KONGSBERG VÅPEN-FABRIKK OG TOSHIBA MACHINE: - EN CASE-ANALYSE AV DEN AMERIKANSKE UTENRIKSPOLITISKE BESLUTNINGSPROSESS. Dette er en case-analyse av den amerikanske utenrikspolitiske beslutningsprosessen, med utgangspunkt i hvordan den amerikanske Kongressen kom fram til vedtaket om sanksjoner mot Kongsberg Våpenfabrikk og Toshiba Machine. Ved hjelp av tre forskjellige tilnærminger som gir ulike framstillinger av hvordan amerikansk utenrikspolitikk utformes, forsøker jeg å forklare Kongressens sanksjonsvedtak våren 1988. Den første tilnærmingen tar utgangspunkt i antagelsen om at de individuelle politiske aktørene først og fremst søker gjenvalg, slik det formuleres i David Mayhews "The Electoral Connection" fra 1974. Utfra dette perspektivet gikk Kongressen blant annet inn for sanksjoner for å presse to utenlandske forsvarsprodusenter utfra det amerikanske markedet. I analysen tar jeg for meg elleve av de mest aktive kongressrepresentantene i Kongsberg/Toshiba-saken, og forsøker å forklare deres atferd i den politiske prosessen utfra at de enten ønsket å markere seg selv eller ville fremme økonomiske interesser i sitt valgdistrikt. Jeg ser nærmere på deres politiske bakgrunn, deres avhengighet av pengestøtte fra næringslivet og deres politiske ambisjoner. Den andre tilnærmingen, som baserer seg på den pluralistiske teoritradisjonen i amerikansk politikk, fremstiller amerikansk utenrikspolitikk som et resultat av konkurranse mellom ulike politiske og ideologiske elitegrupper bestående av aktører fra ulike institusjoner i det amerikanske utenrikspolitiske systemet. I dette perspektivet skyldes sanksjonene mot Kongsberg/Toshiba at en eller flere grupper av politiske aktører klarte å påvirke den politiske prosessen som førte til sanksjonsvedtaket i Kongressen. Analysen viser hvordan en gruppe av aktører i Kongressen, forsvarsdepartementet og CIA samarbeidet i den politiske prosessen. Den siste tilnærmingen ser på amerikansk utenrikspolitikk som et resultat av organisasjonsprosesser og byråkratisk konflikt mellom ulike institusjoner. Tilnærmingen baserer seg i stor grad på Allisons artikkel om Cuba-krisen fra 1969, og den teori som ble fremmet i kjølevannet av denne. Utfra dette perspektivet skyldes sanksjonene mot Kongsberg/Toshiba byråkratisk konflikt mellom CIA og de andre etterretningsorganisasjonene, forsvarsdepartementet og handelsdepartementet og mellom Kongressen og presidenten. Analysen viser også hvordan organisasjonsprosesser i Kongressen og Det Hvite Hus påvirket det politiske prosessen som førte fram til sanksjonsvedtaket. Oppgaven konkluderer med at det er lite hold i antagelsen om at Kongressen vedtok sanksjoner mot Kongsberg/Toshiba fordi dette tjente amerikansk forsvarsindustri. Analysen viser også hvordan amerikansk utenrikspolitikk i stadig større preges av fragmentering og uforutsigbarhet, som følge av en hardere konflikt mellom politiske og ideologiske eliter og motstridende byråkratiske interesser som påvirker den amerikansk utenrikspolitiske beslutningsprosessen.
