[Introduction] Since the first description of gain of function (GOF) mutations in signal transducer and activator of transcription (STAT) 1, more than 300 patients have been described with a broad clinical phenotype including infections and severe immune dysregulation. Whilst Jak inhibitors (JAKinibs) have demonstrated benefits in several reported cases, their indications, dosing, and monitoring remain to be established., [Methods] A retrospective, multicenter study recruiting pediatric patients with STAT1 GOF under JAKinib treatment was performed and, when applicable, compared with the available reports from the literature., [Results] Ten children (median age 8.5 years (3–18), receiving JAKinibs (ruxolitinib (n = 9) and baricitinib (n = 1)) with a median follow-up of 18 months (2–42) from 6 inborn errors of immunity (IEI) reference centers were included. Clinical profile and JAKinib indications in our series were similar to the previously published 14 pediatric patients. 9/10 (our cohort) and 14/14 patients (previous reports) showed partial or complete responses. The median immune deficiency and dysregulation activity scores were 15.99 (5.2–40) pre and 7.55 (3–14.1) under therapy (p = 0.0078). Infection, considered a likely adverse event of JAKinib therapy, was observed in 1/10 patients; JAKinibs were stopped in 3/10 children, due to hepatotoxicity, pre-HSCT, and absence of response., [Conclusions] Our study supports the potentially beneficial use of JAKinibs in patients with STAT1 GOF, in line with previously published data. However, consensus regarding their indications and timing, dosing, treatment duration, and monitoring, as well as defining biomarkers to monitor clinical and immunological responses, remains to be determined, in form of international prospective multicenter studies using established IEI registries., Open Access funding provided thanks to the CRUE-CSIC agreement with Springer Nature. Bien que l’arthrose de genou concerne une grande portion de la population âgée, les options de traitement sont limitées et il n’existe pas de moyen de guérison. Le développement de cette maladie est fortement lié aux contraintes mécaniques durant la marche. Cette observation est la base de nombreux types d’interventions ayant pour objectif de diminuer les contraintes afin ralentir la progression de la maladie. Les semelles orthopédiques ont souvent été utilisées à cette fin, mais les résultats des études cliniques ne sont pas concluants. Une raison possible pour ce manque de concordance dans la littérature provient du fait que les études précédentes n’ont pas considéré la globalité de la fonction ambulatoire du genou ; en se focalisant sur le pic du moment d’adduction externe (pEKAM) elles ont peut-être écarté des effets secondaires qui pouvaient expliquer les variations avec des personnes pleinement satisfaites et d’autres pas du tout. De plus ces études n’ont pour la plupart pas non plus évalué les répercussions que peuvent avoir ces interventions sur les articulations adjacentes, à savoir la cheville et la hanche. -L’objectif de ce travail était d’analyser la fonction du genou, de la cheville et de la hanche, chez de patients avec une arthrose débutante portant différents types de semelles orthopédiques et marchant à différentes vitesses, dans le but d’évaluer un possible système de réponse entre les différentes variables à l’intérieur du genou, mais aussi sur les articulations adjacentes. -Méthodologie : 1. Revue de littérature dans le but de : - Se familiariser et de sélectionner les variables décrivant la fonction du genou pendant la marche en lien avec la gonarthrose. - Formuler les hypothèses de recherche sur la base des variables sélectionnées 2. Collecte de données : Analyse de la marche en laboratoire pour un groupe de 7 patients avec arthrose débutante. 3. Analyse statistique: Des analyses statistiques ont ensuite été effectuées afin de modéliser les effets des propriétés des semelles et de la vitesse sur la fonction du genou et d’identifier une corrélation entre les différentes variables biomécaniques sélectionnés. -Les résultats escomptés étaient d’obtenir une compréhension plus globale de l’effet des semelles et de la vitesse sur la fonction du genou de la cheville et de la hanche, et de mettre en évidence un éventuel système de réponse entre les différentes variables sélectionnées. A terme, cela devrait aider au design de nouvelles interventions ou semelles orthopédique dans le traitement de l’arthrose du genou.

Fifty-six primary neuroendocrine lung tumors were examined morphologically and histologically and their apoptosis level was determined. Malignant carcinomas were characterized by increased apoptotic index and enhanced expression of Bcl-2, Bak, p53, and Ki-67 compared to typical carcinoid. However, apoptosis in these tumors was not completed. Proteins of the Bcl family play an important role in the regulation of spontaneous apoptosis in neuroendocrine lung tumors. Bcl-2 accumulating in the nucleus is a morphological analogue of phosphorylated inactive form of this protein, which does not inhibit apoptosis. Expression of Bcl-2 and Bax decreases in small-cell lung carcinoma (SCLC) with metastases indicating attenuation of apoptosis and development of metastatic clones resistant to apoptosis induces. The stability of the native quaternary structure of bovine α-crystallin was studied, by sedimentation analysis and electron microscopy, as a function of pH (7–11), ionic strength (0.01–0.5), temperature (6–60 °C) and calcium ion concentration (0 and 10 mM). Three successive transitions are distinguished at 20 °C. Firstly, a slow transconformation step, which is independent of pH, ionic strength or calcium ions. Secondly, an irreversible primary dissociation step, favoured by increasing pH above 8 and/or a lower ionic strength, with formation of ‘alkali-modified α-crystallin’, which is spherically shaped like the native protein but has a smaller average diameter, sedimentation coefficient and molecular weight. Thirdly, with further increase of pH above 9, a rapidly reversible dissociation of alkali-modified a-crystallin characterized by a single reaction boundary in sedimentation velocity analysis. In the presence of calcium ions the quaternary structure is stabilized to the extent that no dissociation is observed up to at least pH 10.3. Upon increase of temperature, at pH 7.3, a slow irreversible dissociation and swelling run parallel until a limit is reached around 37 °C with formation of ‘temperature-modified a-crystallin’. which is indistinguishable from the native protein by electron microscopy, but has a higher relative viscosity and lower sedimentation coefficient and molecular weight. Calcium ions have little or no effect on this transition. Above 37 °C a reversal of this transition or aggregation is indicated. These findings, together with previous structural data on microheterogeneity, reassociation from urea, and aging of α-crystallin in vivo, are incorporated into a hypothetical scheme of transitions, based on a three-layer model for the quaternary structure. The primary structure of the most basic (pI = 4.88) of the two major parvalbumins from frog skeletal muscle (Rana esculenta) has been determined by partial automatic sequencing of the protein which exhibits a free N terminus, and a study of overlapping peptides obtained by cyanogen bromide cleavage and digestion with trypsin, thermolysin and Armillaria mellea protease. This protein shows the typical structure of an α-parvalbumin. Comparison of the primary structure of ion binding loops of α and β-parvalbumins does not provide a clear-cut explanation of their differences in ion-binding properties. [ABSTRACT FROM AUTHOR]

48. Medial collateral ligament section during unicompartmental knee arthroplasty managed by direct repair and fascia lata augmentation autograft: a case report and surgical technique.

- Abstract
Background: The medial collateral ligament (MCL) is crucial for ensuring implant stability after unicompartmental knee arthroplasty (UKA). Intraoperative MCL lesions can cause valgus instability, affecting function and implant longevity, and thereby negatively impacting the patient's outcome. Every surgeon who performs UKA may encounter this complication in their daily practice. In this context, this case report presents a rescue technique. The existing literature does not specify a protocol for managing this complication. This article presents the first instance of accidental midsubstance section of the MCL during medial UKA, managed through primary suture and augmentation repair with a fascia lata (FL) autograft. The procedure was subsequently replicated step by step on an anatomical specimen., Case Description: A 54-year-old woman, previously successfully treated with right medial UKA, was referred to our clinic following an unsuccessful attempt at conservative treatment for osteoarthritis in the left knee. Scheduled for a left medial UKA, an inadvertent midsubstance transection of the deep part of the MCL was encountered during the procedure, resulting in valgus instability. The MCL was promptly repaired and reinforced using an ipsilateral FL augmentation autograft. Subsequent UKA surgery was successfully completed. Follow-up at one year revealed favorable post-operative outcomes, with symmetrical stability on stress radiographs and no indications of early loosening., Conclusions: To our knowledge, this article represents the first documentation of the direct management for this rare yet severe complication. This case report could therefore inspire any surgeon facing this complication. The technique, grounded in biomechanical principles, ensures direct medial stability whilst allowing uninterrupted continuation of the initial procedure. Characterized by simplicity and reproducibility, the approach demonstrates favorable short-term outcomes. 49. Walking with Different Insoles Changes Lower-Limb Biomechanics Globally in Patients with Medial Knee Osteoarthritis.

- Abstract
Using insoles to modify walking biomechanics is of keen interest for the treatment of medial-compartment knee osteoarthritis. So far, insole interventions have focused on reducing the peak of the knee adduction moment (pKAM) and have led to inconsistent clinical outcomes. This study aimed to evaluate the changes in other gait variables related to knee osteoarthritis when patients walk with different insoles to provide insights into the necessity to enlarge the biomechanical analyses to other variables. Walking trials were recorded for 10 patients in four insole conditions. Changes among conditions were computed for six gait variables, including the pKAM. The associations between the changes in pKAM and the changes in the other variables were also assessed individually. Walking with different insoles had noticeable effects on the six gait variables, with high heterogeneity among patients. For all variables, at least 36.67% of the changes were of medium-to-large effect size. The associations with the changes in pKAM varied among variables and patients. In conclusion, this study showed that varying the insole could globally influence ambulatory biomechanics and that limiting measurement to the pKAM could lead to an important loss of information. Beyond the consideration of additional gait variables, this study also encourages personalized interventions to address inter-patient variability.

- Abstract
Ulnar/cubital tunnel syndrome is the second most common compressive neuropathy of the upper limb. Permanent location of the ulnar nerve anterior to the medial epicondyle is extremely rare, with only five cases reported in the literature. Using ultrasound elastography and diffusion tensor imaging with fiber tractography, we diagnosed a case in which ulnar nerve entrapment was associated with anterior nerve location.
