Introduction: Degenerative changes of the cervical spine often cause disability and flight duty limitations among Finnish Air Force (FINAF) fighter pilots. We aimed to study the effect of +Gz exposure on degenerative changes in the cervical spine by comparing cervical MRIs of FINAF fighter pilots and controls. Methods: At baseline, the volunteer study population consisted of 56 20-year-old FINAF male fighter pilots (exposure group) and 56 21-year-old Army and Navy cadets (control group). Both groups underwent MRI of the cervical spine at the baseline and after 5 years. Degenerative changes evaluated using MRI included intervertebral disc (IVD) degeneration (Pfirrmann classification), disc herniations, uncovertebral arthrosis, Schmorl’s nodes, Modic changes, spinal canal stenosis, kyphosis and scoliosis. Results: The degree of IVD degeneration in the whole cervical spine increased significantly in both populations with no between-group differences. The prevalence of disc herniations also tended to increase in both populations with no difference in the incidence over the follow-up. However, pilots proved to have more disc herniations at the baseline and at the follow-up. There were virtually no between-group differences in other assessed degenerative changes. Discussion: We found that IVD degeneration and the prevalence of disc herniations increased at a similar rate for fighter pilots and non-flying military students when all cervical levels were summed up. The lack of difference may be explained by the relatively low cumulative +Gz exposure during the first 5 years of a pilots’ career.

Prolonged stress is known to be a risk factor for various kinds of diseases, such as cardiovascular diseases. If stress could be easily measured, it would enable monitoring of stress and help people make better choices to achieve a healthier lifestyle. In this study, a polysomnography system as well as a wearable smart ring were used to measure the responses of central and autonomic nervous systems from ten healthy test subjects (five male and five female), aged 23–26. The responses were measured in two conditions: cognitive stress induced by a mental calculation task and relaxation induced by a focused attention meditation exercise. Power spectral densities of two electroencephalography frequency bands, alpha and beta, were calculated to represent the central nervous system response. The autonomic nervous system response was measured using heart rate, heart rate variability and peripheral (finger) temperature. In cognitive stress, alpha and beta bands both showed higher activity, increasing by 53.26% and 94.70%, respectively. Heart rate also increased by 19.33%, while heart rate variability decreased by 25.65% and peripheral temperature change was 0.77℃ lower. Results show that the changes in autonomic nervous system responses acquired by the smart ring correlate with the changes in central nervous system responses acquired by the polysomnography system. This suggests that a smart ring could be used for an indirect measurement of human stress level. Follow-up studies with larger sample sizes are needed to confirm the findings of this study and to determine the most suitable features for representation of human stress level.Kognitiivisen stressin ja rentoutumisen vaikutusten mittaaminen älysormuksella. Tiivistelmä. Pitkittynyt stressi toimii riskitekijänä lukuisille sairauksille, kuten sydän- ja verisuonitaudeille. Stressin vaivaton mittaaminen mahdollistaisi stressitason seuraamisen, mikä vuorostaan auttaisi ihmisiä tekemään parempia valintoja terveellisemmän elämäntyylin puolesta. Tässä tutkimuksessa käytettiin polysomnografialaitteistoa sekä puettavaa älysormusta keskushermoston ja autonomisen hermoston vasteiden mittaamiseen kymmeneltä terveeltä koehenkilöltä (viisi miestä ja viisi naista), iältään 23–26. Vasteet mitattiin kahdessa tilassa: päässälaskutehtävän aikaansaamassa kognitiivisessa stressissä sekä hengitykseen keskittyvän meditaatioharjoituksen aikaansaamassa rentoutumisessa. Kahdelle elektroenkefalografian taajuuskaistalle, alfalle ja beetalle, laskettiin tehon spektritiheydet kuvastamaan keskushermoston vastetta. Lisäksi laskettiin syke, sykevälivaihtelu sekä ääreislämpötila (sormen lämpötila) kuvastamaan autonomisen hermoston vastetta. Kognitiivisessa stressissä sekä alfa- että beetakaistan aktiivisuus kasvoi, alfalla 53,26 % ja beetalla 94,70 %. Myös syke nousi 19,33 %, kun taas sykevälivaihtelu pieneni 25,65 % ja ääreislämpötilan muutos oli 0,77 ℃ pienempi. Tulokset osoittavat, että älysormuksella mitatut autonomisen hermoston vasteen muutokset korreloivat polysomnografialaitteistolla mitattujen keskushermoston vasteen muutosten kanssa. Tämä antaa ymmärtää, että älysormusta voitaisiin käyttää ihmisen stressitason epäsuoraan mittaamiseen. Suuremman kokoluokan jatkotutkimuksia tarvitaan varmistamaan tämän tutkimuksen löydökset sekä määrittämään sopivimmat fysiologiset piirteet kuvastamaan ihmisen stressitasoa.

Cancer treatments have remarkably improved over the past years since targeted therapies and immunotherapy have been introduced to the field of oncology. The benefit of these new therapies is often limited, however, by de novo or acquired therapy resistances, which should be noticed when making clinical decisions. In this current work, we studied the prognostic and predictive values of several immunological markers in metastatic HER2-positive breast cancer treated with trastuzumab, because trastuzumab is still given to patients according to the HER2 status only, without certainty of tumor response. We also determined the role of HER2 and HER3 for cancer stem cells (CSC) in ALK translocated non-small cell lung cancer (NSCLC) cell lines since the CSCs are causing therapy resistance and cancer recurrence. The results demonstrated that a high number of cytotoxic T cells, together with a high number of M1-like macrophages in the center of the tumor (CT), are promising and independent prognostic factors in HER2-positive breast cancer. These markers together also can predict the progression of the disease and the length of trastuzumab discontinuation in tumor response. Expression of HER2 and HER3 increased the stem-like properties of ALK translocated NSCLC cells, which were decreased when the expressions were downregulated. HER2-HER3-dependent CSCs also mediated the ALK therapy resistance. In conclusion, this study suggests that patients with a favorable immunological tumor profile (high number of cytotoxic T cells and M1-like macrophages in the CT) could be treated in a less-intensive manner, that trastuzumab discontinuation could be feasible for these patients, and that targeting of HER2 and HER3 receptors can lead to more effective killing of cancer stem-like cells and should be further studied. Tiivistelmä Syöpähoidot ovat kehittyneet huomattavasti, kun kohdennetut hoidot ja immunologiset hoidot ovat tulleet perinteisten hoitojen rinnalle. Usein näiden hoitojen hyötyä kuitenkin rajoittaa jo olemassa oleva lääkeresistenssi tai sen kehittyminen, mikä tulisi ottaa huomioon hoitoja suunniteltaessa. Tässä työssä tutkittiin immunologisia merkkiaineita, joilla voitaisiin ennustaa trastutsumabi-hoidon vastetta sekä potilaiden ennustetta levinneessä HER2-positiivisessa rintasyövässä. Tällä hetkellä trastutsumabi-hoitopäätös tehdään pelkän HER2-geenimonistuman mukaan ilman varmuutta siitä, hyötyykö potilas oikeasti hoidosta. Lisäksi tutkimme HER2- ja HER3-reseptorien merkitystä syövän kantasoluille ALK-translokoituneessa ei-pienisoluisessa keuhkosyövässä (NSCLC), sillä syövän kantasolut ovat yksi merkittävimmistä tekijöistä lääkeresistenssin kehittymisessä ja syövän uusiutumisessa. Työssä havaittiin, että kasvaimen keskellä oleva suuri määrä sytotoksisia T-soluja sekä M1-tyypin makrofageja on yhteydessä potilaiden parempaan ennusteeseen ja että kyseiset merkkiaineet ovat toisistaan riippumattomia. Merkkiaineet pystyivät ennustamaan myös taudin etenemistä sekä trastutsumabi-hoitokeskeytyksen pituutta. HER2- ja HER3-proteiinien tuotto lisäsi ALK-translokoituneiden NSCLC-solujen kantasolumaisia ominaisuuksia, jotka puolestaan vähenivät, kun proteiinien tuotto estettiin. Lisäksi HER2-HER3 -riippuvaiset syövän kantasolut säätelivät lääkeresistenssiä kyseisessä taudissa. Työn tulokset viittaavat siihen, että potilaita, joilla on suotuisa kasvaimen immunoprofiili (suuri määrä sytotoksisia T-soluja ja M1-tyypin makrofageja kasvaimen keskellä) pystyttäisiin hoitamaan keveimmillä hoidoilla ja HER2-hoitokeskeytys voisi olla mahdollinen näillä potilailla. Oulun yliopistollisen sairaalan sädehoidon yksikkö käyttää hoitojen suunnitteluun Varian Medical Systemsin Eclipse-hoitosuunnittelujärjestelmää. Työn tarkoituksena oli tutustua Eclipsen ohjelmointiympäristöön (Eclipse Scripting API, ESAPI) ja selvittää, mitä se mahdollistaa. Tämän jälkeen asetettiin uusi tavoite: tehdään ESAPI:a käyttäen uusi ohjelma, jolla voidaan valita useilta potilailta halutut sädehoitosuunnitelmat ja tuoda niistä struktuurien DVH-datat Matlab-yhteensopivaan tekstitiedostoon. Etsittiin internetistä tietoa ESAPI:in liittyen ja ajettiin esimerkkiskriptejä. Ohjelmaa kirjoittaessa hyödynnettiin kahta valmista skriptiä, joita muokattiin tarpeisiin sopiviksi ja yhdistettiin omaan koodiin. Ohjelmassa on selkeä hierarkia: ensimmäisellä tasolla käydään läpi käyttäjän syöttämät potilaat, toisella potilaan sisältämät suunnitelmat, kolmannella suunnitelman sisältämät struktuurit ja neljännellä struktuurin sisältämä DVH-data. Ohjelmassa valitaan kansio, jonne data halutaan tuoda. Jokaiselle potilaalle luodaan oma kansio, joka sisältää suunnitelmien tai ns. plan sumien mukaan nimetyt .csv-tiedostot. Yhdessä tiedostossa on suunnitelman tai plan sumin jokaisen struktuurin sisältämä DVH-data. Tätä dataa voidaan käsitellä mm. Matlab- ja Excel-ohjelmissa ja piirtää sen pohjalta DVH-kuvaajia. ESAPI:n avulla voidaan hakea tietoa ARIA-tietokannasta laajalta alueelta ja automatisoida tiedonhakuprosesseja, mistä on hyötyä sädehoidon henkilökunnalle. The radiation therapy unit of Oulu University Hospital uses Eclipse treatment planning system by Varian Medical Systems for treatment planning. Objective was to become acquainted with Eclipse’s programming interface (Eclipse Scripting API, ESAPI) and find out what it enables. After this a new objective was set: a new program with which one could choose treatment plans from several patients and export DVH data from their structures to a text file compatible with Matlab. Information about ESAPI was searched and example scripts were run. In the final program two written scripts were exploited. These scripts were remodeled to fit the needs and joined with self-written code. There is a clear hierarchy in the program. At the first level program goes through the patients that user has chosen. Then at the second level it goes through the plans that patient includes. At the third level program iterates through the structures in a plan and at the fourth through the DVH data in a structure. In the program user chooses a folder where to export the data. For every patient, a folder is created and inside one are the .csv-files named by plans or plan summaries. A file contains DVH-data of each structure in a plan / plan summary. One can process this data in Matlab or Excel and based on it, plot DVH graphs. With ESAPI one can gather information of ARIA database from a wide area and automatize information retrieval processes, which is useful for the staff of radiation therapy unit. The program met given demands and can be used in potential further studies.

BACKGROUND: Bortezomib and rituximab have shown additive activity in preclinical models of lymphoma, and have been shown to be active and generally well tolerated in a randomised phase 2 study in patients with follicular and marginal zone lymphoma. We compared the efficacy and safety of rituximab alone or combined with bortezomib in patients with relapsed or refractory follicular lymphoma in a phase 3 setting. METHODS: In this multicentre phase 3 trial, rituximab-naive or rituximab-sensitive patients aged 18 years or older with relapsed grade 1 or 2 follicular lymphoma were randomly assigned (1:1) to receive five 35-day cycles consisting of intravenous infusions of rituximab 375 mg/m(2) on days 1, 8, 15, and 22 of cycle 1, and on day 1 of cycles 2-5, either alone or with bortezomib 1·6 mg/m(2), administered by intravenous injection on days 1, 8, 15, and 22 of all cycles. Randomisation was stratified by FLIPI score, previous use of rituximab, time since last therapy, and region. Treatment assignment was based on a computer-generated randomisation schedule prepared by the sponsor. Patients and treating physicians were not masked to treatment allocation. The primary endpoint was progression-free survival analysed by intention to treat. This trial has been completed and is registered with ClinicalTrials.gov, number NCT00312845. FINDINGS: Between April 10, 2006, and Aug 12, 2008, 676 patients were randomised to receive rituximab (n=340) or bortezomib plus rituximab (n=336). After a median follow-up of 33·9 months (IQR 26·4-39·7), median progression-free survival was 11·0 months (95% CI 9·1-12·0) in the rituximab group and 12·8 months (11·5-15·0) in the bortezomib plus rituximab group (hazard ratio 0·82, 95% CI 0·68-0·99; p=0·039). The magnitude of clinical benefit was not as large as the anticipated prespecified improvement of 33% in progression-free survival. Patients in both groups received a median of five treatment cycles (range 1-5); 245 of 339 (72%) and 237 of 334 (71%) patients in the rituximab and bortezomib plus rituximab groups, respectively, completed five cycles. Of patients who did not complete five cycles, most discontinued early because of disease progression (77 [23%] patients in the rituximab group, and 56 [17%] patients in the bortezomib plus rituximab group). Rates of adverse events of grade 3 or higher (70 [21%] of 339 rituximab-treated patients vs 152 [46%] of 334 bortezomib plus rituximab treated patients), and serious adverse events (37 [11%] patients vs 59 [18%] patients) were lower in the rituximab group than in the combination group. Defoliations of Prunus padus by Yponomeuta evonymellus were monitored in two areas in southern Finland. During a long-term study (1980-1995) in the area with high peak defoliations, P. padus trees recorded two defoliation peaks, leading to complete defoliation of many trees. The negative relationship between pupal mass and current year tree defoliation during the peak years suggested shortage of food affected reproductive potential of Y. evonymellus. Furthermore, no delayed induced resistance was observed in these populations. Interannual correlations in degrees of defoliation experienced by individual trees were low, i.e. high defoliation in one year did not predict the degree of defoliation the following year. We also detected a sharp increase in parasitism rates with defoliation between years. In the other study area located about 150 km NW, only 4% of more than 200 study trees experienced defoliations higher than 50%. In this population, high defoliation of tree individuals in one year predicted low defoliation in the following year. In addition, rates of parasitism were less variable and intermediate compared to those in the high peak area. Altogether, the relatively rapidly operating negative feedback in the low peak - Compared with the high peak area - was consistent with the difference between both areas in the levels of peak densities.
