Abstract

BackgroundFamily screening of a 48-year-old male with recently diagnosed IgG4-related disease (IgG4-RD) revealed unanticipated elevations in plasma IgG4 in his two healthy teenaged sons.MethodsWe performed gene sequencing, immune cell studies, HLA typing, and analyses of circulating cytotoxic CD4+ T lymphocytes and plasmablasts to seek clues to pathogenesis. DNA from a separate cohort of 99 patients with known IgG4-RD was also sequenced for the presence of genetic variants in a specific gene, FGFBP2.ResultsThe three share a previously unreported heterozygous single base deletion in fibroblast growth factor binding protein type 2 (FGFBP2), which causes a frameshift in the coding sequence. The FGFBP2 protein is secreted by cytotoxic T-lymphocytes and binds fibroblast growth factor. The variant sequence in the FGFBP2 protein is predicted to form a disordered random coil rather than a helical-turn-helix structure, unable to adopt a stable conformation. The proband and the two sons had 5-10-fold higher numbers of circulating cytotoxic CD4 + T cells and plasmablasts compared to matched controls. The three members also share a homozygous missense common variant in FGFBP2 found in heterozygous form in ~40% of the population. This common variant was found in 73% of an independent, well characterized IgG4-RD cohort, showing enrichment in idiopathic IgG4-RD.ConclusionsThe presence of a shared deleterious variant and homozygous common variant in FGFBP2 in the proband and sons strongly implicates this cytotoxic T cell product in the pathophysiology of IgG4-RD. The high prevalence of a common FGFBP2 variant in sporadic IgG4-RD supports the likelihood of participation in disease.

Abstract

The burden of living with an undiagnosed condition is high and includes physical and emotional suffering, frustrations, and uncertainty. For patients and families experiencing these stressors, higher levels of empowerment may be associated with better outcomes. Thus, it is important to understand the experiences of patients with undiagnosed conditions and their families affected by undiagnosed conditions in order to identify strategies for fostering empowerment. In this study, we used the Genetic Counseling Outcome Scale (GCOS-24) to assess levels of empowerment and support group participation in 35 adult participants and 67 parents of child participants in the Undiagnosed Diseases Network (UDN) prior to their UDN in-person evaluation. Our results revealed significantly lower empowerment scores on the GCOS-24 in adult participants compared to parents of child participants [t(100) = - 3.01, p = 0.003, average difference = - 11.12, 95% CI (- 3.78, - 18.46)] and no significant association between support group participation and empowerment scores. The majority of participants (84.3%, 86/102) are not currently participating in any support groups, and participation rates were not significantly different for adult participants and parents of child participants (11.4 vs. 19.7%, respectively, FE p = 0.40). Open-ended responses provided additional insight into support group participation, the challenges of living with undiagnosed conditions, and positive coping strategies. Future research will evaluate the extent to which empowerment scores change as participation in the UDN unfolds.

Abstract

WNT10A is a signaling molecule involved in tooth development, and WNT10A defects are associated with tooth agenesis. We characterized Wnt10a null mice generated by the knockout mouse project (KOMP) and six families with WNT10A mutations, including a novel p.Arg104Cys defect, in the absence of EDA,EDAR, or EDARADD variations. Wnt10a null mice exhibited supernumerary mandibular fourth molars, and smaller molars with abnormal cusp patterning and root taurodontism. Wnt10a (-/-) incisors showed distinctive apical-lingual wedge-shaped defects. These findings spurred us to closely examine the dental phenotypes of our WNT10A families. WNT10A heterozygotes exhibited molar root taurodontism and mild tooth agenesis (with incomplete penetrance) in their permanent dentitions. Individuals with two defective WNT10A alleles showed severe tooth agenesis and had fewer cusps on their molars. The misshapened molar crowns and roots were consistent with the Wnt10a null phenotype and were not previously associated with WNT10A defects. The missing teeth contrasted with the presence of supplemental teeth in the Wnt10a null mice and demonstrated mammalian species differences in the roles of Wnt signaling in early tooth development. We conclude that molar crown and root dysmorphologies are caused by WNT10A defects and that the severity of the tooth agenesis correlates with the number of defective WNT10A alleles.

20. Incompatibility and Mental Fatigue

Abstract

A straightforward prediction from attention restoration theory is that the level of incompatibility in a person's life should be positively correlated with that person's level of mental (or directed attention) fatigue. The authors tested this prediction by developing a new self-report measure of incompatibility in which they attempted to isolate all the six categories of incompatibility described by S. Kaplan: distraction, deficit of information, duty, deception, difficulty, and danger. Factor analysis revealed six factors that corresponded reasonably well to those six categories. This article found that a composite incompatibility score was positively correlated with a separate self-report measure of mental fatigue and so were all six of the subscale scores. With the exception of the score for duty, these positive correlations remained after partialing out a separate measure of the level of stress in the person's life. The authors concluded that the proposed categories of incompatibility can be validly measured; that the constructs of incompatibility, mental fatigue, and stress are discriminable from each other; and that incompatibility is generally positively correlated with mental fatigue. Abstract

The administration of spike monoclonal antibody treatment to patients with mild to moderate COVID-19 is very challenging. This article summarizes essential components and processes in establishing an effective spike monoclonal antibody infusion program. Rapid identification of a dedicated physical infrastructure was essential to circumvent the logistical challenges of caring for infectious patients, while maintaining compliance with regulations and ensuring the safety of our personnel and other patients. Our partnerships and collaborations among multiple different specialties and disciplines enabled contributions from personnel with specific expertise in medicine, nursing, pharmacy, infection prevention and control, EHR informatics, compliance, legal, medical ethics, engineering, administration and other critical areas. Clear communication and a culture where all roles are welcomed at the planning and operational tables are critical to the rapid development and refinement needed to adapt and thrive in providing this time-sensitive beneficial therapy. Our partnerships with leaders and providers outside our institutions, including those who care for underserved populations, have promoted equity in the access of monoclonal antibodies in our regions. Strong support from institutional leadership facilitated expedited action when needed, from a physical, personnel, and system infrastructure standpoint. Our ongoing real-time assessment and monitoring of our clinical program allowed us to improve and optimize our processes to ensure that the needs of our COVID-19 patients in the outpatient setting are met.

Abstract

The authors investigated the impact of perceived danger on judged likelihood of restoration. Participants imagined that they were in a state of directed attention fatigue and then that they were taking a walk in a potentially restorative setting. The authors varied two properties of the setting in a factorial design. The setting was either a nature trail or a busy urban street, and it contained either no obvious source of danger or an ominous stalker. Measures of perceived danger and of judged likelihood of restoration were obtained. For both types of measures, in the low-danger condition the two setting categories differed, with the natural setting seen as less dangerous and more likely to be restorative. In the high-danger condition, the difference between the setting categories was eliminated. The authors conclude that the presence of a serious and potentially uncontrollable source of danger can damage the perceived restorative potential of a setting. Abstract

We administered a questionnaire measuring contact with nature, sense of humor, and psychological well-being. Factor analysis of the humor items yielded four factors: humor production, humor appreciation, coping humor, and humor tolerance. Factor analysis of 14 well-being measures yielded three factors: emotional well-being, personal development, and effective functioning. The best sense-of-humor predictor of the well-being measures and factors was humor appreciation. Regression models for each of the well-being factors as dependent variables with humor appreciation and contact with nature as independent variables showed that additive models with both predictors were appropriate for personal development and effective functioning and that a simple model with humor appreciation as the sole predictor was sufficient for emotional well-being. Secondary analyses suggested that contact with nature was the better predictor of effective functioning, whereas sense of humor was the better predictor of personal development. 32. Mystery and preference in within-forest settings

Abstract

Recent studies have found negative correlations between mystery and preference for forest settings. Our reanalysis of earlier data suggested that those findings may have stemmed from a failure to examine setting categories within the forest domain. In the current study of 70 within-forest settings, factor analysis of preference ratings revealed two setting categories corresponding to high- and low-visual-access settings. In the high-access category, preference was positively correlated with mystery but uncorrelated with visual access. In the low-access category, preference was uncorrelated with mystery but positively correlated with visual access. We also examined an expanded definition of mystery that made explicit the distinction between mystery and surprise. The expanded definition produced results identical to the standard definition in the high-access category, but in the low-access category, expanded mystery was positively correlated with preference. 35. Pathway curvature and border visibility as predictors of preference and danger in forest settings

The authors investigated pathway curvature and pathway border visibility as predictors of preference and perceived danger in forest settings. Participants rated 56 forest settings containing pathways for one of four variables--mystery, visual access, pathway length, and pathway width--or for one of four additional predictors. Pathway curvature was unrelated to preference, danger, or mystery. Border visibility and visual access were positively related to preference, negatively related to danger, and positively related to each other. Preference and danger were negatively related. Danger was more strongly related to the predictors than was preference. Mystery had a modest negative relation with preference and substantial relations with danger (positive) and visual access (negative). Factor analysis of the predictor intercorrelations revealed a visibility construct. Visibility scores were positively related to preference and negatively related to danger. The authors attempt to address some unresolved issues within the Kaplans' preference-matrix model of environmental preference. These issues involve the relation between legibility and preference and the relations among preference, danger, and mystery. Participants rated 70 within-forest settings containing no visible pathways for preference, danger, or one of seven predictor variables. Legibility and coherence were found to be independent, positive predictors of preference. Likewise, landmarks and visual access were independent positive predictors of legibility. Legibility fully mediated the positive relation between landmarks and preference. Visual access interacted with legibility in predicting preference: The relation between preference and either predictor was strongest and positive at low values of the other predictor. Preference and danger were negatively correlated, and there was some tendency for each of them to have a different pattern of relations with predictors. Abstract

The physical environment can promote the functional ability of persons with dementia. Many care facilities use environmental signage (e.g., names on doors) to facilitate adaptive behavior (e.g., room finding). However, the effects of such signage on residents' functioning are not well understood. In three experiments, we investigated if persons with moderate to severe dementia had the required skills necessary to benefit from signage. Compared to a control condition (recognition of fellow residents' photographs), a high percentage of participants could identify written names and photographs of themselves (Experiment 1). Moreover, name and photographic labels helped participants identify belongings (Experiment 2). Training improved some participants' recognition of their own photographs but not of their fellow residents' photographs (Experiment 3). These findings are consistent with research on self-reference and age-related changes in face recognition and reading, and they suggest that many persons with dementia may have the requisite abilities to benefit from prosthetic signage. 40. Tea and cancer prevention: studies in animals and humans

Abstract

The role of tea in protection against cancer has been supported by ample evidence from studies in cell culture and animal models. However, epidemiological studies have generated inconsistent results, some of which associated tea with reduced risk of cancer, whereas others found that tea lacks protective activity against certain human cancers. These results raise questions about the actual role of tea in human cancer that needs to be addressed. This article is intended to provide a better perspective on this controversy by summarizing the laboratory studies in animals and humans with emphasis on animal tumor bioassays on skin, lung, mammary glands and colon, and the molecular and cellular mechanisms affected by tea. 41. Searching for legibility

Abstract

Legibility has been ineffective as a predictor of environmental preference primarily because of its correlation with another predictor, coherence. The authors tried to separate the two predictors by careful selection of field/forest settings and by using nontraditional definitions. The alternate definitions emphasized landmarks (for legibility) and the two-dimensional picture plane (for coherence). These strategies proved unsuccessful for the entire sample of settings. However, when an empirically derived subset of forest settings was examined, the desired pattern of relations among the traditionally defined constructs was found: Legibility had a slightly stronger correlation with preference than coherence, and legibility was clearly the stronger predictor in regression models that included several predictors. Post hoc analyses involving openness suggested visual access is a major component of legibility in forest settings. The authors now believe the forest setting category is a good domain for establishing the salience of legibility as an independent predictor of preference. Abstract

The authors investigated preference, perceived danger, and fear for a sample of 70 field/forest settings. Predictor variables included perception-based variables (visual access, penetration, movement ease), information-based variables (mystery, concealment, refuge), and variables thought to intervene between concealment and danger (entrapment, rearview concern). All variables were rated by independent groups. Danger and fear were strongly positively correlated for these settings, but preference and danger had a more modest negative correlation. Factor analysis of the strongly intercorrelated predictor variables yielded two factors, interpreted as Visibility and Locomotor Access. Both factors were positive predictors of preference and negative predictors of danger. Further analyses suggested feelings of entrapment could mediate the positive relation between concealment and danger and that after controlling for other indicators of visibility, mystery has a positive relation to preference. In general, the role of visual and locomotor access in accounting for preference or danger reactions is highlighted by these findings.

Abstract

Herzog and Miller (1998) reported that people judged alleys with sharper curves as less dangerous than straighter alleys. The authors investigated the role of perceived alley length as a possible confounding influence. Raters judged a large sample of urban alleys for two target variables (preference and danger) and six predictor variables (setting care, mystery, shadow, curvature, length, and width). The partial correlation of danger and curvature, with length partialed out, was not significant. However, in regression analyses controlling for the entire set of predictors, curvature had a modest negative partial relation with preference and a tendency toward a modest positive partial relation with danger. Thus, when controlling for all predictors, the counterintuitive relation between danger and curvature disappears. In the regression analyses, the strongest predictors were setting care (positive for preference, negative for danger) and shadow (the opposite pattern). Mystery tended to be positively related to both target variables.

Abstract

The authors explore the role of complexity in the relation between building age and preference. Age was assessed as a categorical (via stimulus selection) and a continuous (via ratings of 64 color slides of urban buildings) variable. In either case, the authors replicated earlier research in showing that modern buildings were preferred over older buildings when building maintenance was not controlled, but when it was controlled, the relation reversed, and the older buildings were better liked. However, when a composite-rating measure of complexity was introduced, a somewhat different pattern emerged. Complexity interacted with rated age. The nature of the interaction was that throughout most of the range of complexity scores, age was negatively related to preference, but at the higher end of the complexity range, there was no relation between age and preference. Other findings: Buildings with visible entrances were preferred to those without, and distant views were preferred over near views.

Abstract

The authors compared several Australian subgroups and American college students on their preferences for Australian natural landscapes. Preference correlations across groups were generally high, with the correlations for Australian adults somewhat lower. Factor analysis yielded six perceptual categories: Vegetation, Open Smooth, Open Coarse, Rivers, Agrarian, and Structures. Both the Australian and American samples liked Rivers best and the Open categories least. Only the Australians included willow trees in the Agrarian category. The Australians liked the settings overall better than the Americans. Among the Australians, primary students liked the settings most, secondary students least; aboriginal college students liked the settings better than other college groups, but they disliked the Structures category; and DENR (Department of Environmental and Natural Resources) staff members liked the settings better than other Australian adults but, unlike other adults, did not like willows better than nonwillow settings. Cultural and evolutionary reasons for the complex pattern of results were explored.
