Abstract

Oncological outcomes are improving in gastrointestinal cancer with advancements in systemic therapies, and there is notable potential in combining immunotherapy and radiation therapy (RT) to allow for further improvements. Various preclinical and early phase II studies have shown promising synergy with immunotherapy and RT in gastrointestinal cancer. A few recent phase III studies have shown improved survival with the addition of immunotherapy to standard treatment for gastrointestinal cancer. The timing, duration, sequencing, and integration with other anti-cancer treatments are still areas of ongoing research. We have reviewed the published and ongoing studies of the combinations of immunotherapy and RT in gastrointestinal cancers.

Abstract

Abstract Background Several cytotoxic chemotherapies have demonstrated efficacy in improving recurrence‐free survival (RFS) following resection of Stage II–IV colorectal cancer (CRC). However, the temporal dynamics of response to such adjuvant therapy have not been systematically quantified. Methods The Cochrane Central Register of Trials, Medline (PubMed) and Web of Science were queried from database inception to February 23, 2023 for Phase III randomized controlled trials (RCTs) where there was a significant difference in RFS between adjuvant chemotherapy and surgery only arms. Summary data were extracted from published Kaplan–Meier curves using DigitizeIT. Absolute differences in RFS event rates were compared at matched intervals using multiple paired t‐tests. Results The initial search yielded 1469 manuscripts. After screening, 18 RCTs were eligible (14 Stage II/III; 4 Stage IV), inclusive of 16,682 patients. In the absence of adjuvant chemotherapy, the greatest rate of recurrence was observed in the first year (mean RFS event rate; 0–0.5 years: 0.22 ± 0.21; 0.5–1 years: 0.20 ± 0.09). Abstract

Name-based gender prediction has traditionally categorized individuals as either female or male based on their names, using a binary classification system. That binary approach can be problematic in the cases of gender-neutral names that do not align with any one gender, among other reasons. Relying solely on binary gender categories without recognizing gender-neutral names can reduce the inclusiveness of gender prediction tasks. We introduce an additional gender category, i.e., "neutral", to study and address potential gender biases in Large Language Models (LLMs). We evaluate the performance of several foundational and large language models in predicting gender based on first names only. Additionally, we investigate the impact of adding birth years to enhance the accuracy of gender prediction, accounting for shifting associations between names and genders over time. Our findings indicate that most LLMs identify male and female names with high accuracy (over 80%) but struggle with gender-neutral names (under 40%), and the accuracy of gender prediction is higher for English-based first names than non-English names. The experimental results show that incorporating the birth year does not improve the overall accuracy of gender prediction, especially for names with evolving gender associations. Abstract

Wireless capsule endoscopy (WCE) is a non-invasive method for visualizing the gastrointestinal (GI) tract, crucial for diagnosing GI tract diseases. However, interpreting WCE results can be time-consuming and tiring. Existing studies have employed deep neural networks (DNNs) for automatic GI tract lesion detection, but acquiring sufficient training examples, particularly due to privacy concerns, remains a challenge. Public WCE databases lack diversity and quantity. To address this, we propose a novel approach leveraging generative models, specifically the diffusion model (DM), for generating diverse WCE images. Our model incorporates semantic map resulted from visualization scale (VS) engine, enhancing the controllability and diversity of generated images. We evaluate our approach using visual inspection and visual Turing tests, demonstrating its effectiveness in generating realistic and diverse WCE images.

11. Using Machine Learning Explainability Methods to Personalize Interventions for Students

Abstract

Machine learning is a powerful method for predicting the outcomes of interactions with educational software, such as the grade a student is likely to receive. However, a predicted outcome alone provides little insight regarding how a student's experience should be personalized based on that outcome. In this paper, we explore a generalizable approach for resolving this issue by personalizing learning using explanations of predictions generated via machine learning explainability methods. We tested the approach in a self-guided, self-paced online learning system for college-level introductory statistics topics that provided personalized interventions for encouraging self-regulated learning behaviors. The system used explanations generated by SHAP (SHapley Additive exPlanations) to recommend specific actions for students to take based on features that most negatively influenced predicted learning outcomes; an "expert system" comparison condition provided recommendations based on predefined rules. A randomized controlled trial of 73 participants (37 expert-system condition, 36 explanation condition) revealed similar learning and topic-choosing behavior between conditions, suggesting that XAI-informed interventions facilitated student statistics learning to a similar degree as expert-system interventions. Abstract

Maternal asthma status, prenatal exposures, and infant gut microbiota perturbation are associated with heightened risk of atopy and asthma risk in childhood, observations hypothetically linked by intergenerational microbial transmission. Using maternal vaginal (n = 184) and paired infant stool (n = 172) samples, we identify four compositionally and functionally distinct Lactobacillus-dominated vaginal microbiota clusters (VCs) that relate to prenatal maternal health and exposures and infant serum immunoglobulin E (IgE) status at 1 year. Variance in bacteria shared between mother and infant pairs relate to VCs, maternal allergy/asthma status, and infant IgE levels. Heritable bacterial gene pathways associated with infant IgE include fatty acid synthesis and histamine and tryptophan degradation. In vitro, vertically transmitted Lactobacillus jensenii strains induce immunosuppressive phenotypes on human antigen-presenting cells. Murine supplementation with L. jensenii reduces lung eosinophils, neutrophilic expansion, and the proportion of interleukin-4 (IL-4)+ CD4+ T cells. Thus, bacterial and atopy heritability are intimately linked, suggesting a microbial component of intergenerational disease transmission.

Abstract

To address the performance gap of English ASR models on L2 English speakers, we evaluate fine-tuning of pretrained wav2vec 2.0 models (Baevski et al., 2020; Xu et al., 2021) on L2-ARCTIC, a non-native English speech corpus (Zhao et al., 2018) under different training settings. We compare \textbf{(a)} models trained with a combination of diverse accents to ones trained with only specific accents and \textbf{(b)} results from different single-accent models. Our experiments demonstrate the promise of developing ASR models for non-native English speakers, even with small amounts of L2 training data and even without a language model. Our models also excel in the zero-shot setting where we train on multiple L2 datasets and test on a blind L2 test set., Comment: All authors contributed equally.
