Primary care providers are often the first to identify concerns for autism through routine screening in the first 2 years of life. However, most children do not receive a diagnosis until years later resulting in delays accessing appropriate intervention. Delays in diagnosis disproportionately impact those who are otherwise disadvantaged by society based on race or socioeconomic status. Embedding mental health clinicians into primary care clinics offers a promising opportunity to address barriers to accessing diagnostic and intervention services once primary care providers identify concerns. The goal of this study was to assess the Autism in Primary Care program through which embedded mental health professionals in an urban primary care setting, primarily serving Black and Latinx families with Medicaid, were trained to provide autism diagnostic evaluations. Two hundred and fifty children completed evaluations through Autism in Primary Care program. Wait times to access evaluations in primary care were found to be significantly shorter than through standard avenues of care (e.g. tertiary care clinics). Referring primary care providers and caregivers endorsed high levels of satisfaction with the program. Embedding autism evaluations into primary care settings offers a promising opportunity to improve earlier diagnosis and treatment access for families, reduce inequities in care, and increase caregiver and child well-being. Emotion regulation (ER) is a crucial construct in the study of child education, development, and psychopathology. The How I Feel (HIF) questionnaire, a self-report measure of ER for children, was validated in school-based, nonclinical samples. The current study examined the factor structure of the HIF in a clinical sample of children (N = 195; 73.33% male; 79.49% White; 61.03% diagnosed with ADHD). Confirmatory factor analysis did not support the original 3-factor model. An exploratory factor analysis revealed an altered five-factor model with identical Positive Emotion and Emotion Control factors, but the original Negative Emotion factor was divided into three factors: Mad Emotion, Sad and Scared Frequency, and Sad and Scared Intensity. Results suggest that the HIF may not accurately assess the intensity and frequency of negative emotions in clinical samples. [ABSTRACT FROM AUTHOR]

Objective: Children with ADHD frequently exhibit temper outbursts (TO). One related factor may be parental responses, such as harsh discipline and accommodation. This study tests the hypotheses that these responses will be associated with greater TO, and parental characteristics of higher anger and lower sense of competence, in children with ADHD. Method: Participants included 79 5- to 9-year-old children with ADHD (77.22% boys). Regressions were used to determine the association between parental psychological factors and responses to TO while covarying for TO frequency, severity, and duration. Results: Parental anger and sense of competence were not significantly related to any discipline responses after covarying for TO characteristics, although sense of competence predicted spanking at trend-level significance. Both parental anger and sense of competence significantly predicted accommodation over and above TO characteristics. Conclusion: Results underscore the importance of parental anger and sense of competency in predicting parental behavior, specifically parental accommodation. [ABSTRACT FROM AUTHOR]
