Considering that people with Parkinson’s disease (PD) experience challenges in the control of both balance and eye movements, this study investigated the effects of saccadic eye movements on body sway in people with PD in two bases of support positions (side-by-side and tandem stances). Ten people with PD and 11 healthy individuals performed (a) fixation; (b) horizontal saccadic eye movements to the right and left; and (c) vertical saccadic eye movements up and down. The protocol for each postural task consisted of one block of six trials, making a total of 12 trials. Body sway and gaze parameters were measured during the trials. In both people with PD and healthy individuals, anterior–posterior body sway was significantly reduced in horizontal saccadic eye movements in contrast to fixation, regardless of the body position (side-by-side and tandem stances). Furthermore, vertical saccadic eye movements increased the area of sway in contrast to horizontal ones (and not to fixation) in people with PD. In addition, people with PD showed a higher number of fixations in all experimental conditions, without changes in the mean duration of fixations in both body positions. In conclusion, individuals with PD can improve body sway by coupling eye movements and postural sway when performing horizontal saccadic eye movements but not when performing vertical saccadic eye movements.

BackgroundDue to improvement in movement performance, post-activation performance enhancement (PAPE) may open new possibilities to improve gait performance. However, no study has attempted to translate this phenomenon into walking. Therefore, the study aimed to test whether acute stretching followed by a conditioning exercise can improve subsequent gait performance in healthy adults.Research questionCan an exercise protocol subsequently improve gait performance?MethodsSixteen individuals walked four 10-m trials (in each period) before and after 7 min of an exercise protocol composed of stretching (focusing on the lower limb) and a conditioning exercise (standing calf-raise wearing a vest of 20 kg). Gait impairments negatively affect the quality of life of people with Parkinson's disease (PwPD). Aerobic exercise (AE) is an alternative to alleviate these impairments and its combination with transcranial direct current stimulation (tDCS) has demonstrated synergistic effects. However, the effect of multitarget tDCS application (i.e., motor, and prefrontal cortices simultaneously) combined with physical exercise on gait impairments is still little known. Thus, the proposed randomized clinical trial will verify the acute effects of AE combined with tDCS applied on motor and prefrontal cortices separately and simultaneously on gait (spatial-temporal and cortical activity parameters) in PwPD. Twenty-four PwPD in Hoehn & Yahr stages I-III will be recruited for this crossover study. PwPD will practice AE on treadmill simultaneously with the application of anodal tDCS during four intervention sessions on different days (∼ one week of interval). Active tDCS will be applied to the primary motor cortex, prefrontal cortex, and both areas simultaneously (multitarget), with an intensity of 2 mA for 20 min. For sham, the stimulation will remain at 2 mA for 10 s. The AE will last a total of 30 min, consisting of warm-up, main part (20 min with application of tDCS), and recovery. Exercise intensity will be controlled by heart rate. Spatial-temporal and cortical activity parameters will be acquired before and after each session during overground walking, walking with obstacle avoidance, and walking with a cognitive dual task at self-preferred velocity. An accelerometer will be positioned on the fifth lumbar vertebra to obtain the spatial-temporal parameters (i.e., step length, duration, velocity, and swing phase duration). Prefrontal cortex activity will be recorded from a portable functional near-infrared spectroscopy system and oxygenated and deoxygenated hemoglobin concentrations will be analyzed. Two-way ANOVAs with repeated measures for stimulation and moment will be performed. The findings of the study may contribute to improving gait in PwPD. We aimed to evaluate the intra-session relative and absolute reliability of obstacle-crossing parameters during overground walking in young adults, and to determine the number of trials required to ensure reliable assessment. We analysed data from 43 young male adults who were instructed to walk at a self-selected velocity on a pathway and to step over an obstacle (height = 15 cm; width = 80 cm, thickness = 2 cm) three times. Spatial–temporal gait parameters of the approaching and crossing phases (i.e., before and after the obstacle) and obstacle clearance parameters (i.e., vertical and horizontal distance between the foot and the obstacle during crossing) were computed using a three-dimensional motion analysis system. Intraclass correlation coefficients were used to compute the relative reliability, while standard error of measurement and minimal detectable change were used to assess the absolute reliability for all possible combinations between trials. Results showed that most spatial–temporal gait parameters and obstacle clearance parameters are reliable using the average of three trials. However, the mean of the second and third trials ensures the best relative and absolute reliabilities of most obstacle-crossing parameters. Further works are needed to generalize these results in more realistic conditions and in other populations.

BackgroundGait asymmetry and deficits in gait initiation (GI) are among the most disabling symptoms in people with Parkinson’s disease (PwPD). Understanding if PwPD with reduced asymmetry during GI have higher asymmetry in cortical activity may provide support for an adaptive mechanism to improve GI, particularly in the presence of an obstacle.ObjectiveThis study quantified the asymmetry of anticipatory postural adjustments (APAs), stepping parameters and cortical activity during GI, and tested if the presence of an obstacle regulates asymmetry in PwPD.MethodsSixteen PwPD and 16 control group (CG) performed 20-trials in two conditions: unobstructed and obstructed GI with right and left limbs. Abstract This study verified whether mechanical variables influence the anaerobic capacity outcome on treadmill running and whether these likely influences were dependent of running experience. Seventeen physical active and 18 amateur runners, males, performed a graded exercise test and constant load exhaustive running efforts at 115% of intensity associated to maximal oxygen consumption. During the constant load were determined the metabolic responses (i.e., gas exchange and blood lactate) to estimate the energetic contribution and anaerobic capacity as well as kinematic responses. The runners showed higher anaerobic capacity (16.6%; p = 0.005), but lesser time to exercise failure (−18.8%; p = 0.03) than active subjects. In addition, the stride length (21.4%; p = 0.00001), contact phase duration (−11.3%; p = 0.005), and vertical work (−29.9%; p = 0.015). For actives, the anaerobic capacity did not correlate significantly with any physiologic, kinematic, and mechanical variables and no regression model was fitted using the stepwise multiple regression, while to runners the anaerobic capacity was significantly correlated with phosphagen energetic contribution (r = 0.47; p = 0.047), external power (r = −0.51; p = 0.031), total work (r = −0.54; p = 0.020), external work (r = −0.62; p = 0.006), vertical work (r = −0.63; p = 0.008), and horizontal work (r = −0.61; p = 0.008), and the vertical work and phosphagen energetic contribution presented a coefficient of determination of 62% (p = 0.001). Based on findings, it is possible to assume that for active subjects, the mechanical variables have no influence over the anaerobic capacity, however, for experienced runners, the vertical work and phosphagen energetic contribution have relevant effect over anaerobic capacity output.

The imbalance in the concentration of metallic nanoparticles has been demonstrated to play an important role in multiple sclerosis (MS), which may impact cognition. Biomarkers are needed to provide insights into the pathogenesis and diagnosis of MS. They can be used to gain a better understanding of cognitive decline in people with MS (pwMS). In this study, we investigated the relationship between the blood concentration of metallic nanoparticles (blood nanoparticles) and cognitive performance in pwMS. First, four mL blood samples, clinical characteristics, and cognitive performance were obtained from 21 pwMS. All participants had relapse–remitting MS, with a score of ≤4.5 points in the expanded disability status scale. They were relapse-free in the three previous months from the day of collection and had no orthopedic, muscular, cardiac, and cerebellar diseases. We quantified the following metallic nanoparticles: aluminum, chromium, copper, iron, magnesium, nickel, zinc, and total concentration. Cognitive performance was measured by mini-mental state examination (MMSE) and the symbol digit modalities test (SDMT). Pearson’s and Spearman’s correlation coefficients and stepwise linear regression were calculated to assess the relationship between cognitive performance and blood nanoparticles. We found that better performance in SDMT and MMSE was related to higher total blood nanoparticles (r = 0.40; p < 0.05). Also, better performance in cognitive processing speed and attention (SDMT) and mental state (MMSE) were related to higher blood iron (r = 0.44; p < 0.03) and zinc concentrations (r = 0.41; p < 0.05), respectively. The other metallic nanoparticles (aluminum, chromium, copper, magnesium, and nickel) did not show a significant relationship with the cognitive parameters (p > 0.05). Linear regression estimated a significant association between blood iron concentration and SDMT performance. In conclusion, blood nanoparticles are related to cognitive performance in pwMS. Our findings suggest that the blood concentration of metallic nanoparticles, particularly the iron concentration, is a promising biomarker for monitoring cognitive impairment in pwMS.

Abstract Background Automated peripheral mechanical stimulation (AMPS) has been proposed as a new complementary therapy with potential for improving motor and cardiovascular abnormalities in Parkinson’s disease (PD). However, AMPS long-term effects and its combination with physical exercise are unknown. Thus, this study aims to compare the effects of a program of 12 weeks of physical exercise with a 12-week intervention program combining physical exercise and AMPS on the aerobic capacity, cardiac autonomic control, and gait parameters in patients with PD. Methods A randomized, controlled clinical trial will be conducted. Older volunteers with PD will be randomly assigned to one of the two groups studied: (1) exercise or (2) AMPS + exercise. Both groups will undergo an exercise program of 24 sessions, for 12 weeks, performed twice a week. Before exercise sessions, the group AMPS + exercise will receive a session of active AMPS, while the group exercise will receive an AMPS sham intervention. Shapiro-Wilk’s and Levene’s tests will be used to check for data normality and homogeneity, respectively. In case parametric assumptions are fulfilled, per-protocol and intention-to-treat analyses will be performed using a mixed model analysis of variance to check for group*time interaction. Significance level will be set at 5%. Discussion Several non-pharmacological treatment modalities have been proposed for PD, focusing primarily on the reduction of motor and musculoskeletal disorders. Regular exercise and motor training have been shown to be effective in improving quality of life. However, treatment options in general remain limited given the high prevalence and adverse impact of these disorders. So, developing new strategies that can potentiate the improvement of motor disabilities and also improve non-motor symptoms in PD is relevant. It is expected that the participants from both groups will improve their quality of life, gait parameters, and their cardiac autonomic control, with greater improvements being observed in the group combining active AMPS and physical exercise. BackgroundOverweight and obesity are significant global health concerns that involve deficits in gait and balance that affect daily activities. Although much is reported about the effect of overweight and obesity on gait during unobstructed walking, not much is known about how overweight and obesity could impact gait under more challenging conditions, such as environments with obstacles. ObjectiveThe aim of this study is to systematically review and synthesize the available data regarding the effects of overweight and obesity on obstacle crossing during walking. MethodsThis review will follow the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines. PubMed, Web of Science, Scopus, and SPORTDiscus will be systematically searched with no limitations on publication date. Only full-text English-language articles published in a peer-reviewed journal will be included. Included articles must have compared obstacle crossing during walking in individuals with overweight or obesity to individuals of normal body weight. A total of 2 independent reviewers will select the articles and extract the following 4 sets of data: (1) study characteristics, (2) sample description, (3) obstacle crossing task protocol, and (4) main results obtained. If a considerable number of homogeneous papers are included, a meta-analysis will be conducted. A preliminary search was conducted in November 2021. ResultsThe results will include the article selection flowchart as well as tables and figures synthesizing the extracted data on the effects of overweight and obesity on obstacle crossing during walking. The preliminary search identified 73 original records, of which 5 articles met the inclusion criteria. ConclusionsThis review will present researchers and clinicians with an overview of published studies that have compared the performance of obstacle crossing for individuals with overweight and obesity to those of normal body weight. Gaining insight into the control strategies adopted by individuals with overweight and obesity is critical for safe and successful obstacle crossing in this population. We therefore believe that our findings could be useful for identifying people at risk of falls and developing and implementing fall prevention programs for individuals with overweight and obesity. Abstract Aim: This review aimed to provide evidence and highlight the importance of including physical activity (PA) and/or exercise training (ET) as part of the comprehensive multiple sclerosis (MS) care. Methods: Using the current literature on the subject, we provide a brief overview of MS incorporating its definition, common symptoms, prevalence, and potential disease consequences. We further succinctly describe MS as the first line of treatment, as well as the role of PA and ET in the disease. We end the commentary highlighting important recommendations from an international initiative to improve MS-related physical activity research that we believe will help not only improve the area of study but also best practices within this population. Results: There is compelling evidence for the beneficial effects of PA and/or ET on MS-related symptoms and consequently health and quality of life. There is preliminary evidence suggesting the potential for a disease-modifying effect. Conclusion: Fomenting this discussion is timely due to the increased prevalence of MS in different regions of the globe, and people with MS report low levels of PA participation and high amounts of sedentary time. The consequences of inactivity in this population can be drastic. The current body of evidence supports the notion that PA and ET are safe health behaviors that should be adopted as an adjuvant treatment option within the comprehensive and complex MS care due to its benefits on a variety of disease-related symptoms and its potential for improving health and quality of life in this population.

COVID-19 in Brazil is threatening, and it has forced the government to adopt partial lockdown as a strategy to stop the spread of the virus in the first wave of pandemic (March 2020). These preventive measures during the COVID-19 pandemic may affect the motor and non-motor symptoms in people with Parkinson’s disease (PD) and Multiple sclerosis (MS). Thus, the purpose of this study was to investigate the perception during the first wave of COVID-19 pandemic lockdown on motor and non-motor symptoms, and also measure physical activity level, quality of life, and sleep quality in Brazilian people with PD and MS. One hundred and fifty-three participants (PD-97 and MS-56) answered an online survey to identify the perception of motor and non-motor symptoms, and characterize the physical activity level, and quality of life and sleep in these neurological Brazillian population. During the beginning of pandemic lockdown in Brazil, our results indicated that 69% of people with PD and 55% of people with MS reported worse on motor aspects and lower amount of physical activity performed. Also, 75.2% (PD) and 92.9% (MS) of our cohort were considered inactive or sedentary. Based on the perception and behavior of the population studied, people with PD and MS should be encouraged to perform more physical activity in order to reduce the effects of isolation in motor and non-motor aspects of the diseases. Teleinterventions, such as home-based exercise, should be included in the new routine of people with PD and MS to reduce the impacts of lockdown and to maintain quality of life at a good level.

This study evaluated the influence of acute fatigue on shooting skill performance in goalball penalty shots. Eleven sub-elite male players were evaluated in an official court using a two-dimensional procedure (60 Hz) to compute the mean radial error, bivariate variable error and accuracy, and three-dimensional kinematics (240 Hz) to measure ball velocity. Rating of perceived exertion, blood lactate concentration and knee extension isometric maximal voluntary contractions were assessed at rest and immediately after the completion of an incremental test specific to goalball. Although significant differences were observed for rating of perceived exertion (pre: M = .36 AU, SD = .67; post: M = 9.27 AU, SD = .79), blood lactate concentration (M = 1.33 mM, SD = .51; M = 9.04 mM, SD = 2.25) and peak force (M = 597.85 N, SD = 132.35; M = 513.37 N, SD =107.00) at the end of the incremental test, no differences were identified in the pre and post moments for mean radial error (M = .10 m, SD = 1.01; M =.37 m, SD = 2.19), bivariate variable error (M = 3.56 m, SD = .70; M = 3.25 m, SD = .70), accuracy (M = 3.68 m, SD = .74; M = 3.78 m, SD = 1.13) and ball velocity (M = 18.53 m.s-1, SD = 1.53; M =18.46 m.s-1, SD = 1.65). We concluded that even though significant levels of fatigability can be attributed to the incremental test specific to goalball, shooting skill performance in goalball penalty shots does not appear to be affected. However, further investigations are needed so that goalball coaches can develop strategies that contribute to improve performance in this technical action. Such improvements are decisive for the outcome of a match.

This study had the purpose of analyzing dominant and non-dominant limb performances (frequency of use and accuracy) during match-play technical actions with ball possession (receiving, passing, and shooting a ball) in professional futsal and also check for the possible influence of playing position and the quality of opponent. We have analyzed data pertaining to eight matches of the FIFA Futsal World Cup Thailand 2012™ in which 76 male professional senior futsal players participated (44 right-footed and 32 left-footed). In total, we coded 5,856 actions (2,550 ball receptions, 3,076 passes, and 230 shoots). Our main findings were that (a) players used the dominant limb more frequently than the non-dominant limb for all actions considered [p < 0.001; effect size (ES) medium-to-large]; (b) accuracy was generally greater when using the dominant limb, regardless of the quality of opponent (p < 0.01; ES large); and (c) in shooting actions, pivots showed similar accuracy between dominant and non-dominant limbs (p = 0.51; ES small). The study suggested that when completing technical actions with the ball in futsal, high-level players depended to a greater extent on the use of their dominant lower limb during official matches. Excepting a similarity detected between limbs on shooting performance of pivots, players from all positional roles generally showed a higher accuracy rate in receiving, passing, and shooting a ball when using their dominant limb as compared to their non-dominant one during match-play and the limb usage and accuracy seemed to be independent of the quality of opponents.

Background: Repetitive sit-to-stand (rSTS) is a fatigue perturbation model to examine the age-effects on adaptability in posture and gait, yet the age-effects on muscle activation during rSTS per se are unclear. We examined the effects of age and exhaustive rSTS on muscle activation magnitude, onset, and duration during ascent and descent phases of the STS task.Methods: Healthy older (n = 12) and younger (n = 11) adults performed rSTS, at a controlled frequency dictated by a metronome (2 s for cycle), to failure or for 30 min. We assessed muscle activation magnitude, onset, and duration of plantar flexors, dorsiflexors, knee flexors, knee extensors, and hip stabilizers during the initial and late stages of rSTS. Before and after rSTS, we measured maximal voluntary isometric knee extension force, and rate of perceived exertion, which was also recorded during rSTS task.Results: Older vs. younger adults generated 35% lower maximum voluntary isometric knee extension force. During the initial stage of rSTS, older vs. younger adults activated the dorsiflexor 60% higher, all 5 muscle groups 37% longer, and the hip stabilizers 80% earlier. Older vs. younger adults completed 467 fewer STS trials and, at failure, their rate of perceived exertion was ~17 of 20 on the Borg scale. At the end of the rSTS, maximum voluntary isometric knee extension force decreased 16% similarly in older and younger, as well as the similar age groups decline in activation of the dorsiflexor and knee extensor muscles (all p < 0.05).Conclusion: By performing 467 fewer STS trials, older adults minimized the potential effects of fatigability on muscle activation, voluntary force, and motor function. Such a sparing effect may explain the minimal changes in gait after rSTS reported in previous studies, suggesting a limited scope of this perturbation model to probe age-effects on muscle adaptation in functional tasks.

Resumo Objetivo Analisar o impacto da complexidade da tarefa sobre mobilidade e equilíbrio de idosos saudáveis. Métodos Noventa idosos foram incluídos neste estudo. Os participantes foram submetidos a tarefas que simularam problemas comuns ao envelhecimento, como baixa acuidade visual, alterações na base de apoio e dificuldades em realizar atividades simultâneas. Para avaliação da mobilidade utilizou-se o teste Timed Get Up and Go associado a atividades de dupla-tarefa cognitiva e motora. Para o equilíbrio utilizou-se uma plataforma de força avaliada em diferentes bases de apoio e informações visuais. Testes cognitivos foram aplicados com objetivo de caracterizar a amostra e analisar a associação entre as variáveis motoras e cognitivas. Na análise estatística utilizou-se o teste de Friedman para verificar o efeito da complexidade da tarefa nos idosos e o coeficiente de correlação de Spearman para verificar a associação entre as variáveis motoras e cognitivas. O nível de significância foi estipulado em 5%. Resultados A complexidade da atividade interferiu na mobilidade dos participantes, exigindo um maior número de passos e tempo para realizar a tarefa (p=0,001). De maneira semelhante, uma base de apoio diminuída e informações visuais restritas causaram uma maior insegurança nos participantes frente às reações de equilíbrio (p=0,001). The aim of this study was to investigate the effects of ankle and hip muscle fatigue on motor adjustments (experiment 1) and symmetry (experiment 2) of postural control during a quiet standing task. Twenty-three young adults performed a bipedal postural task on separate force platforms, before and after a bilateral ankle and hip muscle fatigue protocol (randomized). Ankle and hip muscles were fatigued separately using a standing calf raise protocol (ankle fatigue) on a step and flexion and extension of the hip (hip fatigue) sitting on a chair, at a controlled movement frequency (0.5Hz), respectively. In both experiments, force, center of pressure, and electromyography parameters were measured. The symmetry index was used in experiment 2 to analyze the postural asymmetry in the parameters. Our main findings showed that muscle fatigue impaired postural stability, regardless of the fatigued muscle region (i.e., ankle or hip). In addition, young adults used an ankle motor strategy (experiment 1) before and after both the ankle and hip muscle fatigue protocols. Moreover, we found increased asymmetry between the lower limbs (experiment 2) during the quiet standing task after muscle fatigue. Thus, we can conclude that the postural motor strategy is not muscle fatigue joint-dependent and a fatigue task increases postural asymmetry, regardless of the fatigued region (hip or ankle). These findings could be applied in sports training and rehabilitation programs with the objective of reducing the fatigue effects on asymmetry and improving balance.

Ankle muscle fatigue has been shown to increase body sway. In addition, body sway in quiet upright standing is reduced when saccadic eye movements are performed. Abstract Aim: This study aimed to compare the effects of linear periodization (LP) and undulating periodization (UP) of strength training on acceleration in skater children. Methods: Twenty-nine girls (9.67±1.29 years-old, 34.47±8.06kg, 1.39±0.13m) were distributed into two groups: linear periodization group (LPG, n=14) and undulating periodization group (UPG, n=15). Six levels of progressive training were designed based on activities such as multi-jumps, plyometrics, sled towing, and facilitated exercises. The training lasted 16 weeks, with a 1-h session on three non-consecutive days per week. The acceleration was recorded by digital videography following the major trochanter of the femur, at four moments and under two conditions: static (SS) and dynamic start (DS) in a rectilinear path. INTRODUCTION:While fatigue is ubiquitous in old age and visibly interferes with mobility, studies have not yet examined the effects of self-reported fatigue on healthy older adults' gait. As a model that simulates this daily phenomenon, we systematically reviewed eleven studies that compared the effects of experimentally induced muscle and mental performance fatigability on gait kinematics, variability, kinetics, and muscle activity in healthy older adults. METHODS:We searched for studies in databases (PubMed and Web of Science) using Fatigue, Gait, and Clinical conditions as the main terms and extracted the data only from studies that experimentally induced fatigue by sustained muscle or mental activities in healthy older adults. RESULTS:Eleven studies were included. After muscle performance fatigability, six of nine studies observed increases in stride length, width, gait velocity (Effect Size [ES] range: 0.30 to 1.22), inter-stride trunk acceleration variability (ES: 2.06), and ankle muscle coactivation during gait (ES: 0.59, n = 1 study). After sustained mental activity, the coefficient of variation of stride outcomes increased (ES: 0.59 to 0.67, n = 1 study) during dual-task but not single-task walking. CONCLUSION:Muscle performance fatigability affects spatial and temporal features of gait and, mainly, inter-stride trunk acceleration variability. In contrast, sustained mental activity tends only to affect step variability during dual tasking. A critical and immediate step for future studies is to determine the effects of self-reported fatigue on gait biomechanics and variability in healthy older adults to verify the viability of experimentally induced fatigue as a model for the study of gait adaptability in old age.

Abstract The aim of this study was to verify whether patients with Parkinson's disease (PD) are able to adjust their motor behavior according to restrictions imposed by the task instruction during walking with obstacle crossing. Eighteen elderly people (moderate motor compromise) with a diagnosis of PD walked on a pathway and cross an obstacle according to the following conditions: walking at preferred velocity; walking at maximum vertical elevation of the feet to cross the obstacle; walking at maximum step length to cross the obstacle; walking at maximum velocity. The modulations were directly related to the instructions provided to patients with PD before performing each task, which seems to indicate that attentional cues can influence and benefit strategies during obstacle crossing. In conclusion, patients with PD are able to adjust walking during obstacle crossing according to instructions given to them, which increases their safety.

Abstract AIM This study aimed to identify the effects of aging and Alzheimer’s disease (AD) on gait parameters after a four-month period and to investigate the effects of a four-month program of physical activity, with emphasis on the cognitive components of gait during single and dual task, in people with AD. METHODS Twenty-three people with AD, divided into the Training Group (TG; n=12; aged 79.17±7.62 years) and the Control Group (CG; n=11; aged 77.00±5.57 years), and eleven healthy older adults (Healthy Group - HG; aged 75.82±4.83 years) were included in this study. TG participated in a physical activity program for four months. The CG and HG were instructed not to participate in any kind of regular physical activity in this period. The physical activity program includes motor activities and cognitive tasks simultaneously. The participants attended a 1-h session three times a week. The kinematic parameters of gait were analyzed under two conditions, before and after a physical activity program: single and dual task. Deltas for all dependent variables between pre and post training were calculated. The deltas were compared using two-way ANOVAs with group (TG x CG and CG x HG) and task (single x dual task) as factors, with repeated measures for task. RESULTS After the training period, the TG improved stride length, duration, velocity and cadence compared to the CG. CONCLUSION Physical activity with emphasis on cognitive components promotes better reallocation of attention while walking in people with AD, improving attentional focus on the gait and thus resulting in a safer locomotive pattern.

AbstractPlantar cutaneous stimulation has been shown to improve gait in Parkinson's disease (PD), but the effects of different types of insoles have not been tested. We evaluated the immediate effect of different types of insoles on gait in PD patients and healthy older adults. Nineteen PD patients and nineteen healthy older adults performed and performed a walking task at their self-selected speed in three conditions: conventional insole, insole with a raised ridge around the foot perimeter, and insole with half-spheres. Plantar sensation was evaluated before and after the walking protocol. There were no differences between groups for plantar sensation before and after the walking task. PD patients demonstrated reduced stride length and stride velocity. There were no immediate benefits offered by the insoles on gait of either group. The increased plantar cutaneous stimulation does not promote immediate benefits on gait in PD patients and healthy older adults.

Postural sway variability was evaluated in Parkinson’s disease (PD) patients at different stages of disease. Twenty PD patients were grouped into two groups (unilateral, 14; bilateral, 6) according to disease severity. The results showed no significant differences in postural sway variability between the groups (p ≥ 0.05). Postural sway variability was higher in the antero-posterior direction and with the eyes closed. Significant differences between the unilateral and bilateral groups were observed in clinical tests (UPDRS, Berg Balance Scale, and retropulsion test; p ≤ 0.05, all). Postural sway variability was unaffected by disease severity, indicating that neurological mechanisms for postural control still function at advanced stages of disease. Postural sway instability appears to occur in the antero-posterior direction to compensate for the stooped posture. The eyes-closed condition during upright stance appears to be challenging for PD patients because of the associated sensory integration deficit. Finally, objective measures such as postural sway variability may be more reliable than clinical tests to evaluate changes in balance control in PD patients.

This study investigated the effect of a multimodal exercise program on executive functions and memory in people with Parkinson's disease, taking into account disease severity and gender. Twenty-three patients with Parkinson's disease (PD) were evaluated before and after a 6-month exercise program to improve executive functions and memory. We observed the effects of the intervention on executive functions (ability to abstract: p = .01), immediate memory (p= .04) and declarative episodic memory (p < .001). Women showed higher scores on declarative episodic memory (p = .03) than men, however there was no interaction between gender and the intervention. Regardless of sex and disease severity, these preliminary results indicate that the multimodal exercise seems to be effective in improving cognitive functions in patients with PD, suggesting that this program can be indicated as a preventive strategy to mitigate progressive cognitive deficits in the later stages of the disease.

Gait variability may serve as a sensitive and clinically relevant parameter to quantify adjustments in walking and the changes with aging and neurological disease. Variability of steps preceding obstacle avoidance (approach phase) are important for efficiency in the task, especially in people with Parkinson's disease (PD). However, variability of gait during the approach phase to obstacle avoidance in people with PD has been rarely reported, particularly when ambulating obstacles of different heights. The aim of the present study was to investigate the effects of obstacle height on step-to-step variability (step-to-step variability provides information on the variation between the "equivalent steps" for all trials, and walking variability (indicates the within-step variability of each, providing information about the modulations between the steps performed. of spatial-temporal parameters during the approach phase to obstacle avoidance in people with PD and neurologically healthy older people. Twenty-eight older people; 15 with PD and 13 neurologically healthy individuals (control group), participated in the study. Participants were instructed to walk at their preferred speed until the end of the pathway and to avoid the obstacle when it was present. Each subject performed 10 trials of the following tasks: unobstructed walking, low obstacle avoidance (3cm length, height equal ankle's height, 60 cm wide), intermediate obstacle (3cm length, low plus high obstacle height divided by 2, 60 cm wide) avoidance and high obstacle avoidance (3cm length, knee's height, 60 cm wide). The obstacle was positioned 4m from to the start position. The step-to-step and walking variability of the spatial-temporal parameters (acquiring with GAITRite®) of the four steps before obstacle avoidance were analyzed. MANOVAs were used to compare the data. PD group showed the characteristic gait deficits associated with PD. The obstacle increased the spatial-temporal variability (step-to-step and walking variability) during the approach phase to the obstacle. Specifically, both groups increased i) the step-to- step variability of the step length during low obstacle avoidance when compared to the other conditions; ii) the variability during low obstacle avoidance in the last step before obstacle (n-1) compared to higher obstacle avoidance; iii) variability during higher obstacle avoidance in further steps (n-3 and n-4). In conclusion, the presence of the obstacle during walking increased the variability of spatial-temporal parameters in older people with PD and the control group during the steps preceding obstacle avoidance. In addition motor planning (and motor adaptations) was initiated much earlier in the approach phase for the higher obstacle conditions compared to the low obstacle condition.

The aim of this study was to investigate the effect of eight months of a multimodal program of physical exercise on articular range of motion of the lower limb of patients with Parkinson disease (PD), considering gender and disease stage. Seventeen individuals with PD participated in this study. Participants were assessed before of multimodal program of the physical exercise and after four and eight months of physical exercise. In these periods were evaluated the clinical aspects and articular range of motion of the lower limb. For statistical analysis, patients were distributed according to gender and disease stage. A MANOVA considering exercise, gender and stage of disease, with repeated measures for the first factor, was performed. The clinical results showed regression of disease progression, indicating cognitive decline for women. The articular range of motion improved after four and eight months of physical exercise, especially for the hip and ankle, independent of gender and stage of PD. It was concluded that the multimodal exercise program of the eight months was effective in improving articular range of motion in patients with PD. The benefits of exercise for this physical capacity were independent of gender and severity of PD.

The purpose of this study was to investigate the effects of a multimodal exercise program (MEP) on the functional capacity of patients with Parkinson's disease (PD) according to disease severity and gender. Fourteen patients with PD participated in the study and were distributed into groups according to 1) stage of disease and 2) gender. Functional capacity was evaluated before and after 6 months of intervention. The overall PD patient group improved their coordination and strength. Men and women improved in strength performance after exercise. Men also improved on coordination. For severity of disease, the unilateral group improved in strength, while the bilateral group improved in strength, balance, coordination and the UPDRS-functional score. In conclusion, a MEP is efficient in improving components of functional capacity in patients with PD, especially in strength. Gender may be considered in the exercise program. Individuals in the bilateral disease group appeared to benefit more from exercise.
