Abstract

International audience; Introduction: Assessing response to treatment with bypassing agents presents a substantial challenge in the treatment of patients with haemophilia and inhibitors. Rapid and accurate identification of bleeding episodes that are nonresponsive to bypassing therapy with either Factor Eight Inhibitor Bypassing Activity (FEIBA; Baxter AG, Vienna, Austria) or recombinant activated factor VII (rFVIIa; NovoSeven®, Novo Nordisk A/S, Bagsvaerd, Denmark) is essential to guide treatment decisions and optimise patient outcomes through early intervention. Although both bypassing agents are effective, differential responses to therapy necessitate multiple therapeutic options. Aim: This paper provides a consensus definition for non-life-threatening joint and muscle bleeds that are nonresponsive to bypassing agents. Methods: An international panel of 7 physicians met in December 2008 to develop the consensus definition using a modified National Institutes of Health Consensus Development Conference method. Results: The consequent definition of non-life-threatening bleeding episodes that are nonresponsive to bypassing treatment provides a global picture of the condition of the patient during such an event. Identification of nonresponsiveness is based on various criteria: pain, swelling/tension, mobility, patient perception, and laboratory parameters. Criteria can be assessed subjectively by the patient/parent, and/or objectively by the clinician. Although the precise timing of each determination should be at the discretion of the physician, bleeds should be considered nonresponsive if the clinical situation meets the specified criteria 24 hours from the start of treatment. Conclusion: Although it is not intended to replace clinical judgment, this definition can guide the optimal course of treatment for patients with haemophilia and inhibitors.

19. Hematologic Management of Hemophilia A for Surgery

Abstract

From mid-1967 to mid-1983, three hundred fifty surgical operations were performed on 163 patients with hemophilia A, without factor VIII inhibitor. One death occurred, in a patient with a serious head injury. Postoperative hemorrhages occurred after 23% of operations, but the incidence after surgery on the knee, 40%, was decidedly higher than the 15% incidence after operations at other sites. Concurrent plasma factor VIII levels were over 0.40 units/mL in 72% of instances and under 0.30 units/mL in only 15% of instances. The incidence of postoperative hemorrhage did not change over the study period despite a threefold increase in typical dosage of factor VIII (from 600 to 2,000 units/kg per operation) and doubling of typical trough factor VIII levels (from 0.37 units/mL to 0.70 units/mL). Circulating factor VIII levels apparently are not the sole determinants of postoperative bleeding in hemophilia A.(JAMA 1985;253:1279-1283)

Abstract

Cord blood samples from 29,611 infants born in Los Angeles over a 7 1/2 year period were examined for hemoglobinopathies. Among 4,134 Black newborns, the frequency of sickle cell anemia (SS) was 0.3 per cent of sickle cell trait (AS) 8 per cent, and of Hb-C trait (AC) 2 per cent, Of 17,781 Spanish surnamed (Mexican) newborn, 0.5 per cent had sickle cell trait, and 0.1 per cent had Hb-C trait. Of the 5,541 Caucasian infants, 0.7 per cent had sickle cell trait, and 0.15 per cent had Hb-C trait. Twenty-three Black children with major hemoglobinopathies included 16 with sickle cell anemia, two with hemoglobin C disease (CC), and five with SC disease. [ABSTRACT FROM AUTHOR]
