BACKGROUND: In India, the prevalence of Latent TB infection (LTBI) is estimated to be around 40%. Various formulations of PPD(Purified protein derivative) are available, for diagnosis of LTBI, which may give variable responses. The commercially available PPD in India is by Arkray Healthcare (TST-Arkray). It is unclear if this product may have a similar sensitivity compared to other internationally accepted tuberculins (TST-Tubersol). OBJECTIVES: To assess the performance of the two TSTs compared to Quantiferon-Gold Plus (QFT-Plus). METHODOLOGY: A blood sample was collected for the QFT-Plus test. Both the TSTs were placed in the right and the left volar aspect of the forearms and 48 hrs later, the subjects came back to the study site for reading. RESULTS: Among the 512 participants who were recruited, 326 subjects were healthcare professionals and 186 subjects were household contacts of patients with tuberculosis. They were tested with both TST-Tubersol and TST-Arkray, 139(27 %) participants tested positive for TST-Tubersol (≥10 mm), whereas 203 participants (40.1 %)tested positive for TST-Arkray. There was moderate agreement between the two tests with k = 0.58. Also, there was only poor agreement between both the TSTs with QFT Plus(kappa = 0.19 for Tubersol and 0.17 for Arkray). With QFT-Plus as gold standard, the sensitivity, specificity, PPV and NPV of TST-Tubersol, ast an induration cut-off of 10 mm was 46.8 %,76.3 %,31.8 % and 85.8 %. respectively and TST- Arkray; 60.6 %, 64 %, 28.5 % and 87.2 % respectively. CONCLUSION: The Indian TST (Arkray Diagnostics) has shown moderate agreement with the internationally accepted Tubersol. Additionally, there was poor agreement between the TSTs and QFT plus test.

Abstract The survival of animals is impacted by landscapes of spatially varying mortality factors including habitat type, predation risk or harvest risk, among others. Characterization of these spatial mortality processes is important for managing animal populations and their habitats, yet this information has proved challenging to capture. Advances in telemetry now make it possible to monitor tagged individuals' locations through time for a wide range of taxa, providing opportunity to assess movement and mortality simultaneously through spatial time‐to‐event data. Existing time‐to‐event modelling frameworks have largely ignored spatially varying mortality processes or have treated an animal's location as fixed at a regional level. Here we present a general spatial survival model for analysing time‐to‐event data arising from animal telemetry. Our model has a flexible Cox regression form and can estimate the effects of conventional non‐spatial risk factors (size, seasonality, etc.), spatial covariates (e.g. habitat type) and spatial variation in risk not explained by covariates on mortality. We show how to fit our model using Bayesian methods and demonstrate applications of our model with several simulated examples and two animal telemetry case studies. Our model produced consistent and unbiased parameter estimates throughout simulations with a variety of spatial and non‐spatial hazards. In the first case study, data from 147 tagged caribou in British Columbia, Canada, revealed a spatially heterogeneous mortality landscape with caribou survival varying by elevation, likely in response to space use by predators. Our second case study involved a dataset of 390 acoustically tagged Atlantic cod in a southern Norwegian fjord where a marine protected area (MPA) was established during the study. We found that the MPA led to a shift from mostly fishing mortality to mostly natural mortality within the fjord and that these risks had markedly different spatial hazard patterns. Our spatial time‐to‐event model can make use of data from a variety of widely used telemetry technologies to characterize landscapes of mortality risks for different taxa. Our work provides new opportunities to inform the spatial ecology and management of fish and wildlife populations.

Over the past 30 years, the nation has experienced a decline in the number of public school library programs. New York City has been trying to combat this trend by implementing several innovative programs. One of them is the VITAL Libraries program. VITAL stands for Vital, Instructional, Transformative, Accessible, Learning Libraries. It is a program New York City school librarians can apply to for a grant of up to $50,000 to enhance and revitalize their school libraries. The goal was to take existing school libraries to the next level by making them more sustainable. The program also sought to create model school libraries across the city. One way to make school libraries more viable and equitable was to require the school's administrators to be more heavily embedded in their school's library programs. How to persuade them to be more involved? The program would require the principals to commit to participating on the Library Advisory Committee (LAC), in return for this substantial and coveted grant. A LAC is a group consisting of the library's stakeholders, who have a vested interest in the operational success of the school library program. The purpose of the LAC is also to increase buy-in for the school library program from stakeholders and others in the school community, develop and design the school library program, and build community and create community ownership.

This historical research focuses on the role of women in physical education and sport during the 1930s. The purpose is to explore the trends that led to a vast difference between men's and women's roles and expectations in physical education and sport during this time period. To engage in effective research of this societal difference, a Feminist Theoretical Framework was employed to analyze primary and secondary sources related to physical education and sports during the 1930s. The research reveals that while sports and physical education became an increasing part of life in the United States during the 1930s, women's roles and expectations were drastically diminished from those of men. From the intensity levels of physical activity to the rules of the games played, women were seen as an inferior gender within the sports and physical education scope. This article found three main themes leading to the diminished role of women in sports and physical education. They are, cultural views of the 1930s, biologics leading to rules changes, and the sexualization of women athletes. This study contributes to the search for equity in the development of physical education programs and sports programs for women. By revealing past gender roles, a closer examination of current roles becomes clear.

Bottom-towed fishing gears produce significant amounts of seafood globally but can result in seafloor habitat damage. Spatial closures provide an important option for mitigating benthic impacts, but their performance as a fisheries management policy depends on numerous factors, including how fish respond to habitat quality changes. Spatial fisheries management has largely focused on marine protected areas with static locations, overlooking dynamic spatial closures that change through time. To investigate the performance of dynamic closures, we develop a spatial fishery model with fishing-induced habitat damage, where habitat quality can affect both fish productivity and movement. We find that dynamic spatial closures often achieve greater harvest and habitat protection than fixed marine protected areas or conventional nonspatial maximum sustainable yield management, especially under strong habitat-stock interactions. Determining optimal dynamic spatial closures may require considerable information, but we find that simple policies of fixed-schedule rotating closures also perform well. Dynamic spatial closures have received less attention as fisheries management tools, and our results demonstrate their potential value for addressing both harvest and habitat impacts from fishing. Abstract Sensitivity analysis of ecological and epidemiological models is often used to identify the best targets of opportunity for control or management, for example, which subpopulations should be prioritised for vaccination or protection. However, effectiveness of a management action depends not just on which system component is targeted but also on when an action is taken. Traditional methods of model sensitivity analysis apply to time‐invariant parameter perturbations, which limits their ability to address questions about the timing of management interventions. A semianalytic method for performing time‐dependent sensitivity analysis (TDSA) has been recently introduced to address this need (Ng et al., in press). However, some of the steps typically require substantial time and effort. We have developed an R package, tdsa, that automates all steps required for TDSA by using numerical rather than analytic differentiation of model terms to evaluate the sensitivity equations. By avoiding analytic differentiation, the package substantially reduces user effort and risk of human error and increases the generality of TDSA by making it possible to analyse nonparametric or equation‐free models. The results of TDSA may be sensitive to assumptions of the system model. By reducing the effort needed to perform TDSA, the tdsa package facilitates comparison of results under different model assumptions, hence allowing for more robust conclusions. We illustrate this using an example involving the transmission of Morogoro virus in the Natal multimammate mouse, under different assumptions about how the contact rate depends on mouse population density. By automating most of the mathematical and programming steps of the semianalytical approach, the tdsa package makes TDSA accessible to a wide range of users with varying quantitative expertise.

This paper critically examines Just War Theory and its philosophical foundations, which are conventionally positioned in opposition to pacifism and nonviolent conflict. This paper, however, takes the view that both, Just War Theory as well as pacifism and nonviolent conflict, are equally necessary and complementary approaches to living with the possibilities and tragedies of the human condition. Its approach is grounded in feminist theory and methodology and their connections with Galtung’s models of violence and peace. The paper argues that the weaknesses of Just War Theory are intrinsic to the concept and its intent. The inherent contradiction of Just War Theory being that it intended to translate universal moral principles into reality, which makes them context dependent. Fundamentally, Just War Theory is derived from an ethic of justice ultimately centred on the right to use violence and kill. The right is conditional but means that a path to peace inevitably starts from death. An ethic of care is a philosophy where feminist thought meets pacifism and nonviolent conflict. It starts from the creation of life and charts paths to positive peace through the nurture of the conditions for lives in dignity. If we seek to contain the destructive and give space to the creative aspects of the human condition and understand its two extremes, life and death, in their relation to human agency, better we need to complement the traditional ethic of justice (of war and violence) with an ethic of care.

BackgroundTens of millions of children are exposed to Mycobacterium tuberculosis globally every year; however, there are no contemporary estimates of the risk of developing tuberculosis in exposed children. The effectiveness of contact investigations and preventive therapy remains poorly understood.MethodsIn this systematic review and meta-analysis, we investigated the development of tuberculosis in children closely exposed to a tuberculosis case and followed for incident disease. We restricted our search to cohort studies published between Jan 1, 1998, and April 6, 2018, in MEDLINE, Web of Science, BIOSIS, and Embase electronic databases. Individual-participant data and a pre-specified list of variables were requested from authors of all eligible studies. These included characteristics of the exposed child, the index case, and environmental characteristics. To be eligible for inclusion in the final analysis, a dataset needed to include: (1) individuals below 19 years of age; (2) follow-up for tuberculosis for a minimum of 6 months; (3) individuals with household or close exposure to an individual with tuberculosis; (4) information on the age and sex of the child; and (5) start and end follow-up dates. Studies assessing incident tuberculosis but without dates or time of follow-up were excluded. Our analysis had two primary aims: (1) estimating the risk of developing tuberculosis by time-period of follow-up, demographics (age, region), and clinical attributes (HIV, tuberculosis infection status, previous tuberculosis); and (2) estimating the effectiveness of preventive therapy and BCG vaccination on the risk of developing tuberculosis. We estimated the odds of prevalent tuberculosis with mixed-effects logistic models and estimated adjusted hazard ratios (HRs) for incident tuberculosis with mixed-effects Poisson regression models. The effectiveness of preventive therapy against incident tuberculosis was estimated through propensity score matching. The study protocol is registered with PROSPERO (CRD42018087022).FindingsIn total, study groups from 46 cohort studies in 34 countries-29 (63%) prospective studies and 17 (37%) retrospective-agreed to share their data and were included in the final analysis. 137 647 tuberculosis-exposed children were evaluated at baseline and 130 512 children were followed for 429 538 person-years, during which 1299 prevalent and 999 incident tuberculosis cases were diagnosed. Children not receiving preventive therapy with a positive result for tuberculosis infection had significantly higher 2-year cumulative tuberculosis incidence than children with a negative result for tuberculosis infection, and this incidence was greatest among children below 5 years of age (19·0% [95% CI 8·4-37·4]). The effectiveness of preventive therapy was 63% (adjusted HR 0·37 [95% CI 0·30-0·47]) among all exposed children, and 91% (adjusted HR 0·09 [0·05-0·15]) among those with a positive result for tuberculosis infection. Abstract Matrix population models are frequently built and used by ecologists to analyse demography and elucidate the processes driving population growth or decline. Life Table Response Experiments (LTREs) are comparative analyses that decompose the realized difference or variance in population growth rate (λ) into contributions from the differences or variances in the vital rates (i.e. the matrix elements). Since their introduction, LTREs have been based on approximations and have not included biologically relevant interaction terms. We used the functional analysis of variance framework to derive an exact LTRE method, which calculates the exact response of λ to the difference or variance in a given vital rate, for all interactions among vital rates—including higher‐order interactions neglected by the classical methods. We used the publicly available COMADRE and COMPADRE databases to perform a meta‐analysis comparing the results of exact and classical LTRE methods. We analysed 186 and 1487 LTREs for animal and plant matrix population models, respectively. We found that the classical methods often had small errors, but that very high errors were possible. Overall error was related to the difference or variance in the matrices being analysed, consistent with the Taylor series basis of the classical method. Neglected interaction terms accounted for most of the errors in fixed design LTRE, highlighting the importance of two‐way interaction terms. For random design LTRE, errors in the contribution terms present in both classical and exact methods were comparable to errors due to neglected interaction terms. In most examples we analysed, evaluating exact contributions up to three‐way interaction terms was sufficient for interpreting 90% or more of the difference or variance in λ. Relative error, previously used to evaluate the accuracy of classical LTREs, is not a reliable metric of how closely the classical and exact methods agree. Error compensation between estimated contribution terms and neglected contribution terms can lead to low relative error despite faulty biological interpretation. Trade‐offs or negative covariances among matrix elements can lead to high relative error despite accurate biological interpretation. Exact LTRE provides reliable and accurate biological interpretation, and the R package exactLTRE makes the exact method accessible to ecologists.

Abstract Tuberculosis (TB) is a leading cause of morbidity and mortality from a single infectious agent, despite being preventable and curable. Early and accurate diagnosis of active TB is critical to both enhance patient care, improve patient outcomes, and break Mycobacterium tuberculosis (Mtb) transmission cycles. In 2020 an estimated 9.9 million people fell ill from Mtb, but only a little over half (5.8 million) received an active TB diagnosis and treatment. The World Health Organization has proposed target product profiles for biomarker‐ or biosignature‐based diagnostics using point‐of‐care tests from easily accessible specimens such as urine or blood. Here we review and summarize progress made in the development of pathogen‐ and host‐based biomarkers for active TB diagnosis. We describe several unique patient populations that have posed challenges to development of a universal diagnostic TB biomarker, such as people living with HIV, extrapulmonary TB, and children. We also review additional limitations to widespread validation and utilization of published biomarkers. We conclude with proposed solutions to enhance TB diagnostic biomarker validation and uptake.
