Oral lichen planus is a condition affecting the mouth and involving multiple areas of lacelike lesions or plaques that may be erosive. Oral lesions are more resistant to therapy and less likely to undergo remission than cutaneous forms of the disorder. Treatment of oral lichen planus is aimed primarily at relieving discomfort. A total of 16 patients with oral lichen planus and accompanying discomfort were studied; eight patients received cyclosporine, while eight received only the vehicle. Cyclosporine or the placebo was administered as a swish-and-spit medication three times a day for eight weeks. All eight patients who received the active drug experienced marked improvement in erosion, inflammation, pain, and the number of lesions. One patient who received the vehicle had moderate improvement and the remainder either were unaffected or experienced flare-ups of their ulcerations. Patients receiving the vehicle were later treated with cyclosporine and had clinical responses equivalent to those of the patients who received cyclosporine. Treatment of oral lichen planus is difficult and discontinuation of therapy frequently results in a relapse. Corticosteroids have been applied topically or injected, and retinoids have also been used to treat oral lichen planus; however, long-term use of these agents is not recommended because of their potential toxicity. The cause of oral lichen planus is unknown; however, treatment response from the agents used successfully indicates a possible role of the immune system. Malignant melanoma is increasing in incidence. The rate of survival from cutaneous (involving skin) melanoma has increased because of earlier diagnoses. The expected outcome of patients with oral melanoma is still poor, possibly because of delayed detection. There is also very limited information available on oral melanoma, and not enough emphasis is placed on the importance of oral cavity examination. Early diagnosis of oral melanoma is dependent on the ability of clinicians to recognize all pigmented lesions of the oral cavity. The oral melanoma may be mistaken for a birthmark; oral melanotic macules (discolored patches in the mouth); a tattoo; physiologic pigmentation; melanoacanthomas (pigmented, benign skin tumors); or Kaposi's sarcoma. The clinical characteristics of these oral lesions are discussed. Oral melanoma is a cancer that has a detectable phase of growth, which can be easily seen. Examination of the mouth is painless, rapid, noninvasive, and can be performed by dermatologists without dental training. Examination of the oral cavity should be included in the routine skin examination. All discolored lesions of unknown origin that are located in the mouth should be carefully examined and possibly biopsied. The medical literature concerning the effectiveness of topical cyclosporine for treating disorders of the mucous membranes of the mouth is reviewed. In general, topical cyclosporine has not been effective against conditions known to respond to systemic (for instance, orally administered) cyclosporine, but direct injection into skin lesions can produce results. This implies that increased skin absorption of the drug could improve its effectiveness. Absorption by the oral mucosa is known to be considerably better than by skin, thus presumably increasing drug action. The results are presented from cyclosporine treatment of oral lichen planus (red, itchy bumps and scales), bullous disorders (in which blisters form), and aphthous stomatitis (severe canker sores). Most patients with lichen planus of the mouth were helped by the drug. Many patients with oral bullous disorders are elderly, and the potential toxicity of cyclosporine is a concern. However, in studies involving small numbers of patients, several experienced reduction in lesion size as well as pain relief. Treatment with cyclosporine of aphthous stomatitis was effective in some cases but not others, for reasons that may be linked to the various causes of this disorder. Bullous and cicatricial pemphigoid and pemphigus vulgaris are disorders characterized by the appearance of painful skin blisters; pemphigus vulgaris involves other symptoms, as well, and can be fatal if untreated. Patients with any of these diseases can develop painful blisters in the mouth, and treatment is difficult because the sufferers are usually elderly and poorly tolerant of the toxic drugs used for therapy. Six patients with oral blisters were treated by swishing cyclosporine (an immunosuppressant drug) in their mouths three times daily for eight weeks. Results were positive for two patients with cicatricial pemphigoid and one with bullous pemphigoid, although two relapsed a few weeks after cyclosporine was stopped. Two of the three patients with pemphigus vulgaris also experienced reduction in pain and partial healing of the blisters. The remaining patient left the study before its completion because his disease needed additional treatment. Monitoring of liver, kidney, and blood showed no adverse effects from this dosage method of cyclosporine: efforts were made to verify that none had been swallowed. It is possible that the drug exerts a local effect on the tissue of the mouth, rather than a systemic effect. • BACKGROUND AND DESIGN.— To avoid systemic side effects, topical and intralesional administration of cyclosporine has been used; however, only intralesional administration has been successful. To understand more about the dosing requirements and resultant tissue levels of intralesional cyclosporine, we injected psoriasis plaques in a double-blind fashion with three different concentrations of cyclosporine (17 mg/mL in seven patients, 10 mg/mL in 13 patients, and 2.5 mg/mL in 11 patients) or matching vehicle three times weekly for 4 weeks. RESULTS.— Statistically significant improvement was observed in plaques treated with 17 mg/mL (P =.003) compared with vehicle-treated plaques; the improvements in plaques treated with 10 mg/mL (P=.078) and 2.5 mg/mL (P=.054) achieved marginal statistical significance compared with vehicle treatment. - Abstract
The oral antiviral valacyclovir, which is 3 to 5 times more bioavailable than its parent compound acyclovir, is a good candidate for effective therapy to suppress recurrent herpes labialis lesions. The efficacy of oral valacyclovir in the suppression of herpes labialis has not previously been reported. Two identical, randomized, double-blind, parallel-group studies were conducted to evaluate the efficacy of oral valacyclovir 500 mg (n=49) versus placebo (n=49) once daily for 16 weeks in the suppression of herpes labialis among patients with a history of 4 or more recurrent lesions in the previous year. Data from the studies were pooled for analysis. Twenty-eight patients (60%) in the valacyclovir group compared with only 18 patients (38%) in the placebo group were recurrence-free throughout the 4-month treatment period (P=.041). The mean time to first recurrence was significantly longer with valacyclovir (13.1 weeks) compared with placebo (9.6 weeks) (P=.016). The total number of recurrences in patients using valacyclovir was 24 compared with 41 in patients using placebo. The incidence of adverse events during the 4-month treatment period was slightly lower in the valacyclovir group (22 events, 33% of patients) compared with the placebo group (29 events, 39% of patients). The results of these small double-blind, placebo-controlled studies suggest that oral valacyclovir 500 mg once daily for 4 months is effective and well tolerated for the prevention of recurrent herpes labialis. More research with larger patient numbers is warranted to corroborate and extend these findings.
