8. Clients' constructions of the keyworker relationship in adult mental health : a collaborative alliance?

The aim of the present study was to describe the keyworking relationship in adult mental health from the point of view of clients. The precise nature of this relationship is not well defined. Whether what people want from service providers and what is offered match is not known. More specifically this research set out to explore clients' perceptions of the keyworker relationship, with particular reference to: the role of the keyworker, the aims and functions of keyworking, and the aspects of the keyworker relationship that are perceived as more or less helpful. The aim of this was to explore the experiences of six clients. All six participants were interviewed using a semi-structured format and the interviews were analysed using a social constructionist revision of grounded theory. Based on the analysis, a core category was identified and termed developing understanding within a working alliance. A dynamic process model highlighted particular factors and characteristics that enabled engagement. Empathic understanding, positive regard, instillation of hope and trust were seen to be characteristics of a good keyworker. Participants highlighted therapeutic context rather than content. This main finding indicated that person-centred approaches informed the general style of preferred engagement for participants. However, although these factors were seen to be helpful, the lack of clarity around the role of the keyworker generated anxiety and confusion for some participants. The findings are discussed in the light of previous research on psychological helping and keyworking in other settings. Some preliminary recommendations for working more effectively with clients are made. Implications in terms of clinical psychology training and clinical practice are discussed.

Growing attention in aquatic ecology is focusing on biogeographic patterns in microorganisms and whether these potential patterns can be explained within the framework of general ecology. The long-standing microbiologist's credo 'Everything is everywhere, but, the environment selects' suggests that dispersal is not limiting for microbes, but that the environment is the primary determining factor in microbial community composition. Advances in molecular techniques have provided new evidence that biogeographic patterns exist in microbes and that dispersal limitation may actually have an important role, yet more recent study using extremely deep sequencing predicts that indeed everything is everywhere. Using a long-term field study of the 'invasive' marine haptophyte Prymnesium parvum, we characterize the environmental niche of P. parvum in a subtropical impoundment in the southern United States. Our analysis contributes to a growing body of evidence that indicates a primary role for environmental conditions, but not dispersal, in the lake-wide abundances and seasonal bloom patterns in this globally important microbe. Transition metal ion homeostasis is absolutely crucial for the survival of all organisms. Zinc (Zn(II)) is perhaps one of the most important, yet least studied transition metals. Previous studies indicate that intracellular Zn(II) levels in E. coli are in the low millimolar range, yet there is less than one “free” Zn(II) ion per cell. All of the intracellular Zn(II) must then be bound and Zn(II) must be delivered from transporters and inserted into Zn(II)-metalloproteins. The cytoplasmic transport of transition metals, such as copper, iron, nickel, manganese, and arsenic, is accomplished by a group of proteins called metallochaperones. No such metallochaperone has been identified for Zn(II). In an effort to identify the Zn(II) metallochaperones in E. coli, proteomic and genomic studies were conducted. Proteomic studies were used to probe for the time-dependent response of E. coli to stress by Zn(II) excess. Genomic studies were used to probe for the transcriptional response of E. coli to stress by Zn(II) excess and deficiency. Several Zn(II)-metallochaperone candidates were identified, and these proteins were cloned, over-expressed, purified, and characterized. Trigger factor was found to be down-regulated at the proteomic level in response to excess Zn(II). Over-expression and characterization of trigger factor show that it tightly binds 0.5 Zn(II)/monomer; however, spectroscopic studies showed that Zn(II) binding is most likely adventitious. GatY/GatZ Zn(II)-responsive proteins that are part of the galactitol catabolic pathway. GatY was over-expressed and shown to bind 2 Zn(II) equivalents per enzyme. GatZ, reported to be necessary for GatY function, was tested for Zn(II)-binding and shown to not bind Zn(II). A transcript found to be highly up-regulated was ykgM. We cloned and over-expressed YkgM to elucidate why it is highly responsive to Zn(II). We determined that YkgM does not bind Zn(II), and may substitute for Zn(II)-containing ribosomal protein L31 in Zn(II)-limiting conditions. ZnuA was cloned, over-expressed, purified, and characterized. We found that ZnuA tightly binds 2 equivalents of Zn(II) per monomer. Our proteomic and genomic data suggest that there are no soluble, cytoplasmic Zn(II) metallochaperones in E. coli. Based on this conclusion, a novel model is hypothesized that explains Zn(II) transport in E. coli cytoplasm.

- Abstract
Growing attention in aquatic ecology is focusing on biogeographic patterns in microorganisms and whether these potential patterns can be explained within the framework of general ecology. The long-standing microbiologist's credo 'Everything is everywhere, but, the environment selects' suggests that dispersal is not limiting for microbes, but that the environment is the primary determining factor in microbial community composition. Advances in molecular techniques have provided new evidence that biogeographic patterns exist in microbes and that dispersal limitation may actually have an important role, yet more recent study using extremely deep sequencing predicts that indeed everything is everywhere. Using a long-term field study of the 'invasive' marine haptophyte Prymnesium parvum, we characterize the environmental niche of P. parvum in a subtropical impoundment in the southern United States. Our analysis contributes to a growing body of evidence that indicates a primary role for environmental conditions, but not dispersal, in the lake-wide abundances and seasonal bloom patterns in this globally important microbe.
