3. Role of early fluid resuscitation in pediatric septic shock

Abstract

Septic shock occurs when an infection enters the blood stream and toxic substances (endotoxins) are released by dying bacteria. Part of the recommended treatment of sepsis includes aggressive fluid resuscitation, which means the infusion of large amounts of fluid over rather short periods. Although this approach is effective in adults, its effects in children have not been studied extensively. To learn more about this issue, the outcomes of 34 pediatric sepsis cases (median age, 13.5 months) were compared according to the amount of fluid administered. The criteria for diagnosing sepsis are presented. Patients who received the greatest volume of fluid after arrival in the emergency department were more likely to survive than those who received less. There was only one death among the nine patients who received the maximum dose, compared with 15 deaths among the 25 patients in the other two groups. Survivors received more fluid than nonsurvivors at one hour but similar quantities at six hours. Rapid fluid resuscitation with volumes in excess of 40 milliliters per kilogram in the first hour after the patient arrives in the emergency department led to the best outcome. Until recently, treatment of pediatric shock called for fluid administration of 10 to 20 milliliters per kilogram, but improvements in treating sepsis make it possible to infuse more fluid. 5. Carotid blood-flow velocity changes detected by Doppler ultrasound in determination of brain death in children: a preliminary report

Abstract

Doppler ultrasonography of the carotid arteries may be a useful technique for helping to determine brain death in children. Doppler ultrasonography is a kind of ultrasound imaging that analyzes the shift in ultrasonic waves. Seventeen children aged one month to five years with severe brain damage underwent Doppler ultrasonography of the carotid arteries. Seven children were diagnosed as being brain dead by multiple clinical criteria including an EEG. Five of these seven had an abnormal Doppler pattern indicative of brain death. No child who was not diagnosed as brain dead displayed this pattern. The two additional children not identified as brain dead with Doppler ultrasonography had the determining EEG several hours later, suggesting that there may have been further deterioration after the Doppler study. Given the consequences of the diagnosis of brain death, Doppler scans should only be performed and interpreted by skilled vascular ultrasound specialists.

Abstract

ANF is an exciting, newly discovered hormone that has significant potential for furthering our understanding of the complex interactions involved in fluid and electrolyte balance. In addition to effects on water and salt balance, it is a potent vasodilator, as well as inhibitor of renin, angiotensin II, aldosterone, and vasopressin. ANF is primarily produced in the atria, but production in the brain is suggestive of action as a neuropeptide and as a potential regulator of CSF production. Receptors are found throughout the heart, vascular tree, kidney, adrenal gland, and brain. The stimulus for release appears to be atrial stretch, which may be secondary to intravascular fluid changes. It causes hemoconcentration and may be an important regulator of interstitial fluid distribution as well as capillary permeability. Patients with CHF and renal failure have been found to have elevated levels that decrease in response to treatment. Potentially, it may be useful as a therapeutic agent in acute renal failure, CHF and other fluid disturbances.

Abstract

This paper presents a formalism for describing the behavior of computational networks at the algorithmic level. It establishes a direct correspondence between the mathematical expressions defining a function and the computational networks which compute that function. By formally manipulating the symbolic expressions that define a function, it is possible to obtain different networks that compute the function. From this mathematical description of a network, one can directly determine certain important characteristics of computational networks, such as computational rate, performance and communication requirements. The use of this formalism for design and verification is demonstrated on computational networks for Finite Impulse Response (FIR) filters, matrix operations, and the Discrete Fourier Transform (DFT). The progression of computations can often be modeled by wave fronts in an illuminating way. The formalism supports this model. A computational network can be viewed in an abstract form that can be represented as a graph. The duality between the graph representation and the mathematical expressions is briefly introduced.
