Infantile hemangioma is the most common pediatric vascular tumor, with the following risk factors: low birth weight, prematurity, white skin, female gender, multiparity and advanced maternal age. The use of oral and topical beta-blockers, although recent, has emerged as the first line of treatment, with superior safety and efficacy to previously used therapies, such as corticosteroids and surgeries. This report describes two cases of nasal tip infantile hemangioma, treated with oral propranolol. Both presented excellent therapeutic responses.

Introdução – Há ainda lacunas no tocante ao completo entendimento da imunopatogênese da psoríase. Os monócitos ditos “inflamatórios”, que mostraram seus níveis aumentados em condições patológicas diversas, foram pouco avaliados na psoríase e com estudos restritos à análise sanguínea. Da mesma forma, marcadores de ativação monocítica foram escassamente pesquisados nestes pacientes. Ademais, é rara a análise, em nível sérico e cutâneo simultaneamente, das principais citocinas e quimiocinas envolvidas na psoríase. Tampouco se correlacionou estes parâmetros inflamatórios com a gravidade da doença e se pesquisou sobre possíveis inter-relações dos mesmos entre si. A partir destas ideias, elaborou-se o presente projeto, sem precedentes ao realizar coletas sanguíneas e cutâneas simultâneas, para o estudo tanto de subpopulações de monócitos como de biomarcadores inflamatórios, com adicionais pesquisas da correlação destes achados com a gravidade da doença e da inter-relação entre os parâmetros inflamatórios. Metodologia – Estudo transversal do qual participaram 32 pacientes portadores de psoríase vulgar em atividade e 32 indivíduos saudáveis. Foram coletados 15 ml de sangue periférico e fragmentos cutâneos (das lesões de psoríase dos pacientes e da pele normal dos controles) obtidos por biópsia por punch. A gravidade da psoríase foi avaliada através de aplicação do Índice de Gravidade e Área da Psoríase (PASI). Todas as amostras foram processadas conforme protocolos laboratoriais recomendados. Nas análises estatísticas, para variáveis contínuas, utilizou-se o teste t de Student (distribuição gaussiana) e o de Mann- Whitney (distribuição não gaussiana). Para as variáveis qualitativas, elegeu-se o teste de quiquadrado. Para pesquisa de correlação entre variáveis quantitativas, elegeu-se o coeficiente de correlação de Spearman. As diferenças foram consideradas estatisticamente significantes quando o valor de “p” foi menor que 0,05. Resultados – Na análise das subpopulações de monócitos, não houve diferença estatística entre pacientes e controles, tanto em nível sanguíneo quanto cutâneo. A frequência dos monócitos não clássicos no sangue periférico se correlacionou positivamente com os pacientes de PASI 10-20 (r = 0,81467 e p = 0,0073) e a expressão de CCL2 das lesões cutâneas. Os marcadores de ativação monocítica CD14s e CD163s no soro apresentaram maior expressão nos pacientes (p = 0,0113 e p < 0,0001, respectivamente). A expressão de CD14s no soro se correlacionou positivamente com a de CD14s nas lesões de pele (r = 0,89925 e p < 0,0001). Quanto às expressões das citocinas avaliadas, a de IL-17 (soro) apresentou correlação positiva com a expressão de CX3CL1 (soro) (r = 0,43450 e p = 0,0492); a de IL-1beta (pele) foi maior em pacientes (p = 0,0089); a de TNF (soro) apresentou correlação positiva com a expressão de CX3CL1 (soro) (r = 0,45674 e p = 0,0496); a de TNF (pele) foi maior nos pacientes (p < 0,0001); a de IFN (pele) apresentou correlação positiva com os pacientes de PASI > 20 (r = 0,81743 e p = 0,0148). Quanto às expressões das quimiocinas avaliadas, a de CCL2 (soro) apresentou correlação positiva com pacientes de PASI 10-20 (r = 0,80168 e p = 0,0094) e a de CCL2 (pele) apresentou correlação positiva com pacientes de PASI < 10 (r = 0,64813 e p = 0,0161). A de CXCL9 (soro) foi maior em pacientes (p = 0,0453) e se correlacionou positivamente com a expressão de IL-17 (soro) (r = 0,49239 e p = 0,0192) e de CX3CL1 (soro) (r = 0,52571 e p = 0,0203). A de CXCL9 (pele) foi maior nos pacientes (p = 0,0002) e apresentou correlação positiva com: IL-17 (soro) (r = 0,88244 e p < 0,0001), TNF (soro) (r = 0,83157 e p < 0,0001), IFN (pele) (r = 0,41550 e p = 0,0486) e CX3CL1 (soro) (r = 0,57658 e p = 0,0044). A de CX3CL1 (soro) apresentou correlação positiva com pacientes de PASI > 20 (r = 0,96944 e p = 0,0247). Conclusão – As subpopulações de monócitos inflamatórios, a despeito de outras condições patológicas, não se mostraram aumentadas nas amostras sanguíneas e cutâneas de pacientes. Por outro lado, os marcadores de ativação monocítica (CD14 s e CD163s), ainda pouco estudados na psoríase, podem ser promissores biomarcadores da doença. Algumas citocinas e quimiocinas – INF, CCL2 e CX3CL1 – se correlacionaram positivamente com a gravidade da doença, podendo ser potenciais biomarcadores no acompanhamento dos pacientes. A expressão de CXCL9, comparativamente maior nos pacientes tanto em nível sérico quanto cutâneo, apresentou o maior número de inter-relações com outros parâmetros inflamatórios e pode vir a ser um biomarcador superior aos demais avaliados. Introduction - There are still gaps in the full understanding of psoriasis immunopathogenesis. Inflammatory monocytes, that showed increased levels in diverse pathological conditions, were poorly evaluated in psoriasis and the studies were restricted to blood analysis. Similarly, monocytic activation markers were scarcely assessed in these patients. In addition, the main cytokines and chemokines involved in psoriasis were rarely analyzed at serum and cutaneous levels simultaneously. Nor were these parameters correlated to disease severity and the interrelations between them were investigated. Based on that, the present project was developed for the study of both subpopulations of monocytes and inflammatory biomarkers, unprecedently collecting simultaneous blood and cutaneous samples, additionally evaluating the correlation with disease severity and the interrelation between these inflammatory parameters. Methodology - A cross-sectional study involving 32 patients with active psoriasis and 32 healthy individuals was performed. Fifteen milliliters of peripheral blood and cutaneous fragments (from patients' psoriatic lesions and from the normal skin of controls) obtained from punch biopsy were collected. The severity of psoriasis was assessed using the Psoriasis Area and Severity Index (PASI). All samples were processed according to recommended laboratorial protocols. Statistical analyzes were performed using Student's t-test (Gaussian distribution) and Mann-Whitney test (non-Gaussian distribution). For the qualitative variables, the chi-square test was chosen. To evaluate the correlation between quantitative variables, the Spearman correlation coefficient was opted. The differences were considered statistically significant when the "p" value was lower than 0.05. Results – About the analysis of monocyte subpopulations, there was no statistical difference between patients and controls, at both blood and cutaneous levels. The frequency of non-classical monocytes in peripheral blood was positively correlated with PASI 10-20 patients (r = 0.81467 and p = 0.0073) and with CCL2 expression of cutaneous lesions. Seric monocyte activation markers CD14s and CD163s showed higher expression in patients (p = 0.0113 and p 20 patients (r = 0.81743 and p = 0.0148). Regarding chemokine expression, seric CCL2 presented positive correlation with PASI 10-20 patients (r = 0.80168 and p = 0.0094) and cutaneous CCL2 presented positive correlation with PASI < 10 patients (r = 0.64813 and p = 0.0161). Seric CXCL9 was higher in patients (p = 0.0453) and positively correlated with seric IL-17 and CX3CL1 (r = 0.499239 and p = 0.0192, r = 0.52571 and p = 0.0203, respectively). CXCL9 from skin was higher in the patients (p = 0.0002) and presented positive correlation with: seric IL-17 (r = 0.88244 and p 20 patients (r = 0.96944 and p = 0.0247). Conclusions - Subpopulations of inflammatory monocytes, despite other pathological conditions, were not shown to be increased in the blood and cutaneous samples of psoriatic patients. On the other hand, monocyte activation markers (CD14 s and CD163s), still poorly studied in psoriasis, may be promising biomarkers of the disease. Some cytokines and chemokines - INF, CCL2 and CX3CL1 - correlated positively with disease severity and may be potential biomarkers for patient’s follow-up. The expression of CXCL9, which was higher in patients at both serum and cutaneous levels, presented the highest number of interrelations with other inflammatory parameters and might be a superior biomarker in relation to the others evaluated.

The human T-cell lymphotropic virus-1 (HTLV-1) affects worldwide population; the estimated number of currently infected individuals is 10-20 million. In this report, we describe the clinical findings of three family members with vertical transmission of HTLV-1. This case report highlights the importance of healthcare providers who have optimal knowledge about HTLV-1 including its transmission and pertinent attributes, and who are able to provide affected individuals with adequate information regarding their condition.

Dermatoscopy of melanocytic lesions has guided the decision of when or not to biopsy a lesion. The use of this tool has increased clinical examination's sensitivity and specificity in 89% and 96% respectively. However, dermatoscopic evaluation of amelanotic or hypomelanotic melanomas, as well as metastases, can be difficult. There is still no standardization for the analysis of these pathologies, which relies mostly on their vascular pattern. We describe the dermatoscopy of acral metastatic amelanotic melanoma. A dermatoscopia das lesões melanocíticas tem auxiliado na decisão de biopsiar ou não uma lesão. A utilização desta ferramenta aumentou a sensibilidade e a especificidade do diagnóstico para 89% e 96%, respectivamente. No entanto, a avaliação dermatoscópica de melanomas amelanóticos ou hipomelanóticos, bem como a de metástases cutâneas, pode ser difícil. Ainda falta uma padronização para a análise destas patologias, que se baseia, majoritariamente, no seu padrão vascular. Descreve-se a dermatoscopia de melanoma metastático amelanótico acral.

Melasma is a common disorder of hypermelanosis that affects mainly young and middle-aged women of Fitzpatrick's phototypes III-V. The disease significantly impacts their lives. In vivo reflectance confocal microscopy, a spreading technology for the noninvasive evaluation of the skin up to the papillary dermis, provides real-time en face images with cellular resolution. We present a case of melasma with in vivo reflectance confocal microscopy findings closely correlated to the histopathological features described in the literature. O melasma é um distúrbio pigmentar caracterizado por hipermelanose, que afeta principalmente mulheres jovens e de meia-idade com fototipos III-V de Fitzpatrick e acarreta em impacto significativo na qualidade de vida das mesmas. A microscopia confocal reflectante in vivo, uma tecnologia em expansão voltada para análise da pele até a derme superior, proporciona imagens en face em tempo real com resolução celular. Apresentamos um caso de melasma com achados na microscopia confocal reflectante in vivo fortemente correlacionados com as características histopatológicas descritas na literatura.

A acroceratoelastoidose é um tipo de ceratodermia palmoplantar, originalmente descrita pelo dermatologista mineiro Oswaldo Gonçalves Costa. É uma rara genodermatose autossômica dominante, embora possa ocorrer de forma esporádica. A doença não é congênita, com as primeiras manifestações na infância e adolescência. A acroceratoelastoidose é um tipo de ceratodermia palmoplantar, originalmente descrita pelo dermatologista mineiro Oswaldo Gonçalves Costa. É uma rara genodermatose autossômica dominante, embora possa ocorrer de forma esporádica. A doença não é congênita, com as primeiras manifestações na infância e adolescência. O quadro clínico é caracterizado por múltiplas pápulas amareladas, por vezes translúcidas e ceratóticas, medindo aproximadamente 2-4 mm de diâmetro, ocasionalmente umbilicadas, localizadas na margem lateral das mãos e pés, simetricamente, o que é sua expressão clínica mais típica. Os achados histopatológicos mais comuns são: hiperceratose, acantose discreta e elastorrexe, que é observada pela coloração com orceína. Acrokeratoelastoidosis is a type of palmoplantar keratoderma first described by Oswaldo Gonçalves Costa, a Brazilian Dermatologist from the state of Minas Gerais. It is a rare autosomal-dominant genodermatosis; however it may occur sporadically. The disease is not congenital; rather, its onset occurs in childhood or adolescence. Clinically, the condition is characterized by multiple yellowish papules, sometimes glossy and keratotic, measuring approximately 2-4 mm in diameter, sometimes umbilicated, and located symmetrically on the sides of the hands and feet, symmetry being the most typical sign. The most common histopathological findings are hyperkeratosis, mild acanthosis and elastorrhexis, as revealed by orcein staining.

Lymphogranuloma venereum is an infection caused by a variety of the bacterium Chlamydia trachomatis. Both genital and extragenital manifestations of the disease can cause serious differential diagnostic difficulties, indirectly leading to progression and dissemination of the infection. This work describes cases of patients with lymphogranuloma venereum showing atypical clinical and/or histopathological findings. It also focuses on alternative therapeutic approaches, such as surgical excision at stage 1, that may lead to a positive outcome. It is not completely clear whether histopathological findings of lymphogranuloma venereum can reveal progression or changes in the course of the disease over time, as is the case in other diseases. We conclude that both clinical and histopathological observations in a larger number of patients are needed in order to further evaluate the findings presented in this article. Linfogranuloma venéreo é uma infecção causada por um tipo específico da bactéria Chlamydia trachomatis. Manifestações genitais e extragenitais da doença podem causar uma série de dificuldades no diagnóstico diferencial, indiretamente levando a progressão ou disseminação da infecção. Este trabalho apresenta casos de pacientes com linfogranuloma venéreo cujos achados clínicos e/ou histopatológicos são atípicos, além de abordar métodos de terapia alternativos, tais como excisão cirúrgica na fase 1, os quais podem trazer um resultado positivo. Ainda não se sabe se achados histopatológicos do linfogranuloma venéreo podem revelar progressão ou mudanças no curso da doença ao longo do tempo, como ocorre com outras enfermidades. Concluímos que observações clínicas e histopatológicas devem envolver um número maior de pacientes a fim de avaliar os resultados clínicos deste trabalho.

- Abstract
This research aimed to create an instrument for assessing the effectiveness of the Programa Mais Médicos (PMM-Program More Doctors), verifying the perceptions of a national non-probabilistic stratified sample of 643 Brazilians. Factor analysis was performed on the scale and the PMM assessment data were analyzed using descriptive, bivariate and multivariate statistics. The Principal Components method, indicating best psychometric values and theoretical explanation, with varimax rotation, structured the 35 items of dichotomous scale into 2 factors, which explain 56.92% of the total variance: "Structural and organizational aspects of the Programa Mais Médicos" (α = 0.96) and "The role of doctors in the Programa Mais Médicos" (α = 0.74). It is also contemplated that the effectiveness of PMM was negatively assessed. These findings reveal that the instrument is useful for evaluation of PMM, still short of instruments in the literature for its evaluation and can be properly used in future studies. - Abstract
Dermoscopy has its usefulness well established in the diagnostic evaluation of melanocytic lesions. Recently, however, it has also shown to be an important tool in monitoring therapeutic response to various dermatoses. We report the case of an elderly patient diagnosed with lentigo maligna of difficult surgical management, which we have chosen to treat with topical imiquimod. The dermoscopic monitoring of this alternative therapy has shown to be of great usefulness.

- Abstract
Sexually transmitted diseases (STDs) have a significant prevalence in both the general population and pregnant women. Accordingly, we consider the physiological changes of the maternal organism that can alter the clinical course of these diseases. In addition, obstetric and neonatal complications may occur, resulting in increased maternal and infant morbidity and mortality. We explore features of the natural course and treatment during pregnancy of the major STDs: soft chancre, donovanosis, gonorrhea, chlamydia, viral hepatitis, genital herpes, human papillomavirus (HPV) infection, lymphogranuloma venereum, syphilis, and vulvovaginitis. We believe that health professionals should pay careful attention to STDs, particularly in relation to early diagnosis and precautions on the use of drugs during pregnancy. Prevention and partner treatment to achieve effective results are also extremely relevant.
