Objectives: This study aimed to systematically evaluate the effects of constraint-induced aphasia therapy (CIAT) for aphasic patients reported by randomized controlled trials. Methods: Relevant randomized controlled trials were retrieved from 11 electronic databases. A methodological quality assessment was conducted in accordance with the Cochrane Handbook, and meta-analyses were performed by using RevMan 5.2. A descriptive analysis was conducted when the included trials were not suitable for a meta-analysis. Results: A total of 12 trials were included. A statistically significant group difference was shown from the meta-analysis in the results measured by the Western Aphasia Battery (random-effects model, MD = 1.23, 95% CI = 0.31 to 2.14, P 0.05) and Aachen Aphasia Test (fixed-effects model, MD = −1.11, 95% CI = −4.49 to 2.27, P > 0.05). The descriptive analysis showed positive results in language performances of naming, repetition, and comprehension. Conclusion: This systematic review indicated that CIAT was efficient for improving language performance with regard to naming, comprehension, repetition, written language, and oral language based on the current evidence. And this review provides some meaningful guides for clinical practice: expand the therapy duration to 2 or 3 h per day, focus on naming, and choose the best assessment tool. It also indicates a need for more rigorous, large-scale, and high-quality trials in the future.

Background: Mitigated echolalia (ME) is a symptom of aphasia which refers to a seemingly deliberate repetition of just-heard words and phrase fragments. ME has historically been viewed as a compensatory strategy aimed to strengthen auditory comprehension. Nevertheless, this hypothesis and other possible functional deficits underlying ME have not been evaluated so far. Aims: This study aimed to (a) reappraise ME in the frame of modern neuroscience; (b) report the effects of Constraint-Induced Aphasia Therapy (CIAT) and a cognition-enhancing drug (memantine) on detrimental ME in a patient (CCR) with fluent aphasia; and (c) analyse the functional and structural brain correlates of ME in CCR with multimodal neuroimaging. Methods & Procedure: Tasks tapping verbal expression and auditory comprehension were administered to CCR to evaluate ME. After baseline testing, evaluations were performed under placebo alone (weeks 0-16), combined placebo with CIAT (weeks 16-18), placebo treatment alone (weeks 18-20), washout (weeks 20-24) and memantine (weeks 24-48). Instructions to reduce ME during CIAT were provided to CCR. Language evaluation and multimodal neuroimaging were also performed 10 years after ending treatment. Outcomes & Results: At baseline, ME occurred in spontaneous speech and in difficult-to-understand single words, indicating impaired meaning access. However, more instances of ME were heard in sentence comprehension, reflecting additional impairment in short-term memory. ME also occurred in words that were correctly defined and understood to the extent that even after accessing word meaning successfully, CCR repeated the same word several times, suggesting impaired inhibitory response control. In comparison with baseline, analysis of auditory sentence comprehension under treatment revealed significant decrements of ME just after ending CIAT and 2 weeks later. These gains were maintained under memantine 6 months later. No changes in ME were found during both placebo and washout phases. Instructions to constrain ME reduced the time to complete a sentence comprehension task 2 weeks after CIAT. ME returned to baseline levels 10 years later. Multimodal imaging suggested that ME in CCR resulted from residual activity of remnants of the left dorsal stream and the intact right white matter tracts after extensive damage to the left ventral stream. Conclusions: ME in CCR interfered with functional communication, and it may be attributed to deficits in sound-meaning mapping, auditory short-term memory, attentional control, and inhibition of repetition mechanisms. Background: Researchers are continuing to investigate the impact of constraint-induced aphasia therapy (CIAT) programs on the spoken language capabilities of people with aphasia at the utterance and discourse level. Currently, there is a lack of consensus on how spoken productions should be measured and there are no investigations that explore the spoken language that people with aphasia useduringa CIAT program. Therefore, clinicians and researchers cannot truly know if they are utilizing the most revealing outcome measures to illuminate treatment induced gains in spoken production. Aims: The purpose of this investigation was to examine thewithin treatmentlearning of people with aphasia in a CIAT program, by exploring the sensitivity of measures of content, length, syntax, and efficiency at the utterance level and reveal linguistic gains. Methods & Procedures: A repeated measures design was employed to examine 200 utterance samples from eight participants with various types and severity of chronic aphasia between early and late treatment sessions. The measures of correct information units (CIUs), counted words, T-Units, CIUs per utterance, mazes, and mean length of utterance were compared. Outcomes & Results: Nonparametric analyses revealed significant positive gains for the participants in the number of produced CIUs (p = 0.035), counted words (p = 0.012), T-Units, (p = 0.025), and CIUs per utterance (p = 0.012). A significant decrease in the amount of mazes (p = 0.028) was also found. No significant increase was detected in the participants’ mean length of utterance (p = 0.161). Conclusions: The participants displayed advances in the content, syntax, and efficiency of their spoken utterances during the course of a 10-day CIAT program. Specifically, the measures of CIUs, counted words, mazes, and CIUs per utterance emerged as being the most sensitive to capture the within treatment changes made by the participants. Aphasia is prevalent in people following stroke, which can have a significant impact on the quality of life of the patients with stroke. One of the new methods for treatment of patients with aphasia is constraint-induced aphasia therapy (CIAT). The aim of this study was to investigate the efficacy of CIAT on naming deficits in individuals with chronic aphasia. This study had a prospective, single-subject study with A-B-A design. The CIAT was administered to two patients with chronic aphasia. Participants were a 57-year-old male and a 45-year-old female and had a stroke 60 and 36 months ago, respectively. In this study, the naming test was used as the outcome measure. The naming test was administered in three baseline sessions with 1 week interval between tests (phase A). Patients received CIAT for four consecutive weeks (3 days/week). Four measurements were taken during the treatment phase (phase B). In follow-up phase (phase A) two other measurements were performed. Visual analysis consisting of level, regression line, and variability were used to determine the effects of CIAT on naming. Both participants increased scores on naming test after phase A and B. The mean of the naming score improved from the baseline to the intervention phase in both participants. There was a positive trend in naming scores during the treatment phase compared with the trend in the baseline demonstrated by both participants. The results of this study showed that the CIAT can be effective in improving the naming deficit in patients with chronic post-stroke aphasia.

Background: Much recent progress has been made in developing speech–language therapy in primary progressive aphasia (PPA). Several treatment approaches that have shown significant effects with people with aphasia have been adapted and re-evaluated for PPA. Constraint-induced aphasia therapy (CIAT) is a well-evaluated method that has yielded significant language improvements in people with post-stroke aphasia but has not yet been evaluated with people with PPA. Nevertheless, the combination of CIAT features like massed practice and a motivating communicative setting seem likely to make it a suitable tool for improving the speech and language performance of individuals with PPA as well. Aims: This study investigates the effectiveness of a modified CIAT protocol on word retrieval, grammatical structure and connected speech in two individuals with non-fluent variant PPA (nfvPPA). Methods and procedures: Two participants with nfvPPA took part in a 9-day intensive CIAT-based group therapy with additional computer-based home training. Stimuli were 120 photos of people performing daily life activities, which could be described using a simple (e.g., “The man is mowing the lawn”) or reduced (e.g., “mowing the lawn”) sentence structure. During the treatment phase, the participants were required to request picture cards from other group members using spoken language only. The task difficulty was increased hierarchically (shaped) in accordance to each participant’s performance level. Outcomes and results: Directly after therapy, both participants achieved significant improvements in their noun and verb naming accuracy and their grammatical structure for trained items. Training effects were maintained 2 months after therapy. Moreover, generalisation to different pictures of the same item was found for both participants and one participant also showed improved grammatical structure when describing untrained pictures. No significant generalisation to untrained connected speech samples was observed for either participant. Conclusion: This study illustrates that CIAT can be effective in people with PPA. However, further modifications of CIAT should be considered to facilitate generalisation and in order to determine which aspects of the treatment are most important. [ABSTRACT FROM PUBLISHER]

Background: Communication outcomes following stroke are improved when treatments for aphasia are administered early, within the first 3 months after stroke, and provided for more than 2 hours per week. However, uncertainty remains about the optimal type of aphasia therapy. Aims: We compared constraint-induced aphasia therapy (CIAT) with individual, impairment-based intervention, both administered early and daily after acute stroke. Methods&Procedures: This prospective, single-blinded, randomised, controlled trial recruited participants with mild to severe aphasia within 10 days of an acute stroke from acute/subacute Perth metropolitan hospitals (n = 20). Participants were allocated by computer-generated block randomisation method to either the CIAT (n = 12) or individual, impairment-based intervention group (n = 8) delivered at the same intensity (45–60 min, 5 days a week) for 20 sessions over 5 weeks (15–20 hours total). The primary outcome, measured after completing the intervention, was the Aphasia Quotient (AQ) from the Western Aphasia Battery. Secondary outcomes were the AQ at 12 and 26 weeks post stroke, a Discourse Analysis (DA) score and the Stroke and Aphasia Quality of Life Scale (SAQoL), measured at therapy completion, 12 and 26 weeks post stroke. There was a 10% (n = 2) dropout at the primary end point, both participants were in the CIAT group. The estimates for each treatment group were compared using repeated measures ANOVAs. Data from the 26-week follow-up assessment are presented, however, were not included in the between-group comparisons due to the low number of data points in each group. Outcomes & Results: Within groups analyses comparing performance at baseline, therapy completion, and 12 weeks post stroke revealed a statistically significant treatment effect for the AQ (p < .001), DA (p = .002), and SAQoL (p < .001). Between groups analysis found there was no significant difference between the CIAT and individual therapy groups on any outcome measure. Conclusions: CIAT and individual therapy produced comparable amounts of change in the very early phase of recovery suggesting a standard, intensive daily dose of therapy within this period of recovery is feasible and beneficial. There were no significant differences between the two groups demonstrating that CIAT, which is provided in a group format, may be a viable option in the very early phase of aphasia recovery. The role of the two hemispheres in the neurorehabilitation of language is still under dispute. This study explored the changes in language-evoked brain activation over a two-week treatment interval with intensive constraint induced aphasia therapy (CIAT), which is also called intensive language action therapy (ILAT). Functional magnetic resonance imaging (fMRI) was used to assess brain activation in perilesional left hemispheric and in homotopic right hemispheric areas during passive listening to high and low-ambiguity sentences and non-speech control stimuli in chronic non-fluent aphasia patients. All patients demonstrated significant clinical improvements of language functions after therapy. In an event-related fMRI experiment, a significant increase of BOLD signals was manifest in right inferior frontal and temporal areas. This activation increase was stronger for highly ambiguous sentences than for unambiguous ones. These results suggest that the known language improvements brought about by intensive constraint-induced language action therapy at least in part relies on circuits within the right-hemispheric homologues of left-perisylvian language areas, which are most strongly activated in the processing of semantically complex language.

The authors comment on the contribution of formulaic expressions to seminal approaches in recovery from speech and language disorders after stroke. They discuss music-based rehabilitation programs for these patients, communicative-pragmatic rehabilitation programs for aphasia patients, and the impact of these programs on the patient's motivation, well-being and quality of life.

Changes in ERP (P100 and N400) and root mean square (RMS) were obtained during a silent reading task in 28 patients with chronic post-stroke aphasia in a randomized, double-blind, placebo-controlled trial of both memantine and constraint-induced aphasia therapy (CIAT). Participants received memantine/placebo alone (weeks 0–16), followed by drug treatment combined with CIAT (weeks 16–18), and then memantine/placebo alone (weeks 18–20). ERP/RMS values (week 16) decreased more in the memantine group than in the placebo group. During CIAT application (weeks 16–18), improvements in aphasia severity and ERP/RMS values were amplified by memantine and changes remained stable thereafter (weeks 18–20). Changes in ERP/RMS occurred in left and right hemispheres and correlated with gains in language performance. No changes in ERP/RMS were found in a healthy group in two separated evaluations. Our results show that aphasia recovery induced by both memantine alone and in combination with CIAT is indexed by bilateral cortical potentials. [ABSTRACT FROM AUTHOR]

Purpose: This Phase I study investigated behavioral and functional MRI (fMRI) outcomes of 2 intensive treatment programs to improve naming in 2 participants with chronic moderate-to-severe aphasia with comorbid apraxia of speech (AOS). Constraint-induced aphasia therapy (CIAT; Pulvermüller et al., 2001) has demonstrated positive outcomes in some individuals with chronic aphasia. Whether constraint to the speech modality or treatment intensity is responsible for such gains is still under investigation. Moreover, it remains to be seen whether CIAT is effective in individuals with persistent severe nonfluent speech and/or AOS. Method: A single-subject multiple-baseline approach was used. Both participants were treated simultaneously, first with Promoting Aphasics' Communicative Effectiveness (PACE; Davis & Wilcox, 1985) and then with CIAT. Pre-/posttreatment testing included an overt naming fMRI protocol. Treatment effect sizes were calculated for changes in probe accuracy from baseline to posttreatment phases and maintenance where available. Results: Both participants made more and faster gains in naming following CIAT. Treatment-induced changes in BOLD activation suggested that better naming was correlated with the recruitment of perilesional tissue. Conclusion: Participants produced more target words accurately following CIAT than following PACE. Behavioral and fMRI results support the notion that the intense and repetitive nature of obligatory speech production in CIAT has a positive effect on word retrieval, even in participants with chronic moderate-to-severe aphasia with comorbid AOS. Constraint-Induced Movement Therapy (CIMT) is a powerful and well evaluated therapeutic tool for the treatment of post-stroke paresis. CIMT is based on an intensive training (massed practice) principle and a gradual rebuilding of movement functions (shaping principle). In this article we will review how CIMT principles can be adapted to treat post-stroke aphasia, thereby establishing a Constraint-Induced Aphasia Therapy (CIAT). First results of this new approach suggest success and feasibility for the treatment of chronic aphasia. The observation of reorganizational changes in brain activity following intensive language training add to previous evidence that CIMT-based therapy may lead to macroscopic remodelling of cortical network architecture. Introduction/Background Constraint-induced aphasia therapy (CIAT) has been widely applied as post-stroke aphasia rehabilitation to encourage intensive verbal practice with supported verbal cuing instead of previous habitual compensatory strategies (Pulvermuller et al., 2001). However, this therapy has not yet been reported in Japan; thus, we applied intensive CIAT training to three of our patients with chronic aphasia. Participants Patient 1: aged 72 years, female, three years after onset of Broca aphasia. Patient 2: aged 67 years, male, two years after onset of transcortical aphasia. Patient 3: aged 52 years, female, two years after onset of anomia. Methods The CIAT-II protocol (Johnson et al., 2015) was modified as the Japanese version, which consisted of intensive training using five expressive language exercises and methods of Shaping and Transfer Package for 5 × 3-h weekly sessions for 3 weeks. The Japanese version of the Aphasia Quotient (AQ) in the Western aphasia battery and the Amount Scale (AS) of the Verbal Activity Log (VAL) were also modified to assess aphasia before and after the protocol. Results The AS improved from 2.7 to 4.6, from 1.4 to 2.8, and from 2.2 to 3.8 in patients 1, 2, and 3, respectively. The AQ did not greatly change in any patient (from 70.8 to 72, from 70 to 73.1, and from 85.9 to 93.7, respectively). Conclusion The results indicated that the CIAT could improve aphasia assessed by the AS, which was consistent with previous study of CIAT (Johnson et al., 2015) and constraint-induced movement therapy (Taub et al., 2013).
