1. DKIST unveils the serpentine topology of quiet Sun magnetism in the photosphere

Abstract

We present the first quiet Sun spectropolarimetric observations obtained with the Visible SpectroPolarimeter (ViSP) at the $4-$m Daniel K. Inouye Solar Telescope (DKIST). We recorded observations in a wavelength range that includes the magnetically sensitive Fe I $6301.5/6302.5$ $\AA$ doublet. With an estimated spatial resolution of 0.08'', this represents the highest spatial resolution full-vector spectropolarimetric observations ever obtained of the quiet Sun. We identified $53$ small-scale magnetic elements, including $47$ magnetic loops and $4$ unipolar magnetic patches, with linear and circular polarisation detected in all of them. Of particular interest is a magnetic element in which the polarity of the magnetic vector appears to change three times in only $400$ km and which has linear polarisation signals throughout. We find complex Stokes $V$ profiles at the polarity inversion lines of magnetic loops and discover degenerate solutions, as we are unable to conclusively determine whether these arise due to gradients in the atmospheric parameters or smearing of opposite polarity signals. We analyse a granule which notably has linear and circular polarisation signals throughout, providing an opportunity to explore its magnetic properties. On this small scale we see the magnetic field strength range from $25$ G at the granular boundary to $2$ kG in the intergranular lane (IGL), and sanity check the values with the weak and strong field approximations. 3. Temporal evolution of small-scale internetwork magnetic fields in the solar photosphere

Abstract

While the longitudinal field that dominates photospheric network regions has been studied extensively, small scale transverse fields have recently been found to be ubiquitous in the quiet internetwork photosphere. Few observations have captured how this field evolves. We aim to statistically characterise the magnetic properties and observe the temporal evolution of small scale magnetic features. We present two high spatial/temporal resolution observations that reveal the dynamics of two disk centre internetwork regions taken by the new GRIS/IFU (GREGOR Infrared Spectrograph Integral Field Unit) with the highly magnetically sensitive Fe I line pair at 15648.52 {\AA} and 15652.87 {\AA}. With the SIR code, we consider two inversion schemes: scheme 1 (S1), where a magnetic atmosphere is embedded in a field free medium, and scheme 2 (S2), with two magnetic models and a fixed stray light component. S1 inversions returned a median magnetic field strength of 200 and 240 G for the two datasets, respectively. We consider the median transverse (horizontal) component, among pixels with Stokes Q or U, and the median unsigned longitudinal (vertical) component, among pixels with Stokes V, above a noise threshold. We determined the former to be 263 G and 267 G, and the latter to be 131 G and 145 G, for the two datasets, respectively. We present three regions of interest (ROIs), tracking the dynamics of small scale magnetic features. We apply S1 and S2 inversions to specific profiles, and find S2 produces better approximations when there is evidence of mixed polarities. We find patches of linear polarization with magnetic flux density between 130 and 150 G, appearing preferentially at granule/intergranular lane (IGL) boundaries. Abstract

Purpose Methods Results Conclusions Level of Evidence Anterior cruciate ligament (ACL) reconstruction is an evolving field, though there remains sparse epidemiological data on the treatment of ACL ruptures. The objective of this study is to analyse the trends in the rate of ACL reconstruction (ACLR) in adult patients over the past two decades in the Australian population.The incidence of ACLR between 2001 and 2020 in patients 15 years and over was analysed using the Australian Medicare Benefits Schedule (MBS) database. The data reflect patients with private health coverage (45% of the Australian population). An offset term was introduced using national population data to account for population changes over the study period.A total of 160,353 ACLRs were performed in Australia under the MBS in the 20‐year period from 2001 to 2020. An annual increase in the total volume and per capita rate of ACLRs was found (p < 0.05). The annual volume of cases increased by 82%, from 5512 in 2001 to a peak of 10,011 in 2017. This increase was seen across all age groups (p < 0.05) and both sexes (p < 0.05), with a greater proportion of reconstructions performed on males (n = 102,357, 64%) than females (n = 57,996, 36%). In 2020, the rate of adult ACLRs decreased to a level last seen in 2004, likely due to the effects of COVID‐19.The incidence of ACLR in adult patients has increased in Australia over the 20‐year study period. Abstract

Background: The optimal management of distal radius fractures remains a challenge for orthopaedic surgeons. The emergence of Artificial Intelligence (AI) and Large Language Models (LLMs), especially ChatGPT, affords significant potential in improving healthcare and research. This study aims to assess the accuracy and consistency of ChatGPT's knowledge in managing distal radius fractures, with a focus on its capability to provide information for patients and assist in the decision‐making processes of orthopaedic clinicians. Methods: We presented ChatGPT with seven questions on distal radius fracture management over two sessions, resulting in 14 responses. These questions covered a range of topics, including patient inquiries and orthopaedic clinical decision‐making. We requested references for each response and involved two orthopaedic registrars and two senior orthopaedic surgeons to evaluate response accuracy and consistency. Results: All 14 responses contained a mix of both correct and incorrect information. Among the 47 cited references, 13% were accurate, 28% appeared to be fabricated, 57% were incorrect, and 2% were correct but deemed inappropriate. Consistency was observed in 71% of the responses. Conclusion: ChatGPT demonstrates significant limitations in accuracy and consistency when providing information on distal radius fractures. 12. Exploring Magnetic Loops and Serpentine Fields in the Quiet Sun with the GRIS-IFU

Abstract

Synthetic observations produced from radiative magnetohydrodynamic simulations have predicted that higher polarization fractions in the quiet solar photosphere would be revealed by increasing the total integration time of observations at GREGOR resolutions. We present recently acquired disk center observations of the Fe i 15648.5 Å line obtained with the GREGOR telescope equipped with the GRIS-IFU during excellent seeing conditions, showing exceptionally high polarization fractions. Our observations reveal an internetwork region with a majority (>60%) of magnetized pixels displaying a clear transverse component of the magnetic field. This result is in stark contrast to previous disk center GRIS-IFU observations in this spectral line, which had predominantly vertical magnetic fields in the deep photosphere. At the same time, the median magnetic field strength is weaker than previous GRIS-IFU observations, indicating that the larger fraction of polarization signals cannot be explained by a more active target. We use the Stokes Inversion based on Response functions (SIR) code to analyze the data, performing over 45 million inversions, and interrogate the impact of two conflicting approaches to the treatment of noise on the retrieval of the magnetic inclination and azimuth. We present several case studies of the zoo of magnetic features present in these data, including small-scale magnetic loops that seem to be embedded in a sea of magnetism, and serpentine fields, focusing on regions where full-vector spectropolarimetry has been achieved. We also present a new open-source Python 3 analysis tool, SIR Explorer, which we use to examine the dynamics of these small-scale magnetic features. © 2023. The Author(s). 24. Lumbar intervertebral disc replacement in Australia: An epidemiological study.

Abstract

Introduction: Favorable short- and long-term outcomes have been reported for lumbar intervertebral total disc replacement (L-TDR). However, there is little evidence regarding the uptake of L-TDR in practice. The objective of this study was to analyze Australian-based population trends in L-TDR over the past 5 years., Methods: The 5-year incidence of L-TDR from 2019 to 2023 in adult patients was analyzed using the Medicare Benefits Schedule (MBS) database. Data were stratified by sex and year, with an offset term introduced using population data from the Australian Bureau of Statistics to account for population changes over the study period., Results: A total of 1558 L-TDRs were completed in Australia under the MBS in the 5 years of interest. The 5-year annual mean case volume was 311.6 cases per annum. A downtrend and plateau in the rate of L-TDR has been seen from 2021 onward. The distribution of L-TDR across ages showed a significantly higher concentration in the 35-44 and 45-54 age groups (P < 0.05). More operations were performed in males (n = 876, 56.2%) than females (n = 682, 43.8%)., Conclusions: The uptake of L-TDR has declined throughout the 5-year study period in Australia.
