42. Does a high Mandard score really define a poor response to chemotherapy in oesophageal adenocarcinoma?

Abstract

Background: A high Mandard score implies a non-response to chemotherapy in oesophageal adenocarcinoma. However, some patients exhibit tumour volume reduction and a nodal response despite a high score. This study examines survival and recurrence patterns in these patients.Methods: Clinicopathological factors were analysed using multivariable Cox regression assessing time to death and recurrence. Computed tomography-estimated tumour volume change was examined in a subgroup of consecutive patients.Results: Five hundred and fifty-five patients were included. Median survival was 55 months (Mandard 1-3) and 21 months (Mandard 4 and 5). In the Mandard 4 and 5 group (332 patients), comparison between complete nodal responders and persistent nodal disease showed improved survival (90 vs 18 months), recurrence rates (locoregional 14.75 vs 28.74%, systemic 24.59 vs 48.42%) and circumferential resection margin positivity (22.95 vs 68.11%). Complete nodal response independently predicted improved survival (hazard ratio 0.34 (0.16-0.74). Post-chemotherapy tumour volume reduction was greater in patients with a complete nodal response (-16.3 vs -7.7 cm3, p = 0.033) with no significant difference between Mandard groups.Conclusion: Patients with a complete nodal response to chemotherapy have significantly improved outcomes despite a poor Mandard score. High Mandard score does not correspond with a non-response to chemotherapy in all cases and patients with nodal downstaging may still benefit from adjuvant chemotherapy. [ABSTRACT FROM AUTHOR]

Abstract

In an intensifying effort to track ocean change and distinguish between natural and anthropogenic drivers, sustained ocean time series measurements are becoming increasingly important. Advancements in the ocean carbon observation network over the last decade, such as the development and deployment of Moored Autonomous pCO2 (MAPCO2) systems, have dramatically improved our ability to characterize ocean climate, sea–air gas exchange, and biogeochemical processes. The MAPCO2 system provides high-resolution data that can measure interannual, seasonal, and sub-seasonal dynamics and constrain the impact of short-term biogeochemical variability on carbon dioxide (CO2) flux. Overall uncertainty of the MAPCO2 using in situ calibrations with certified gas standards and post-deployment standard operating procedures is < 2 μatm for seawater partial pressure of CO2 (pCO2) and < 1 μatm for air pCO2. The MAPCO2 maintains this level of uncertainty for over 400 days of autonomous operation. MAPCO2 measurements are consistent with shipboard seawater pCO2 measurements and GLOBALVIEW-CO2 boundary layer atmospheric values. Here we provide an open-ocean MAPCO2 data set including over 100 000 individual atmospheric and seawater pCO2 measurements on 14 surface buoys from 2004 through 2011 and a description of the methods and data quality control involved. The climate-quality data provided by the MAPCO2 have allowed for the establishment of open-ocean observatories to track surface ocean pCO2 changes around the globe. Abstract

Background: The role of adjuvant therapy in patients with oesophagogastric adenocarcinoma treated by neoadjuvant chemotherapy is contentious. In UK practice, surgical resection margin status is often used to classify patients for receiving adjuvant treatment. The aim of this study was to assess the survival benefit of adjuvant therapy in patients with positive (R1) resection margins. Methods: Two prospectively collected UK institutional databases were combined to identify eligible patients. Adjusted Cox regression analyses were used to compare overall and recurrence‐free survival according to adjuvant treatment. Recurrence patterns were assessed as a secondary outcome. Propensity score‐matched analysis was also performed. Results: Of 616 patients included in the combined database, 242 patients who had an R1 resection were included in the study. Of these, 112 patients (46·3 per cent) received adjuvant chemoradiotherapy, 46 (19·0 per cent) were treated with adjuvant chemotherapy and 84 (34·7 per cent) had no adjuvant treatment. In adjusted analysis, adjuvant chemoradiotherapy improved recurrence‐free survival (hazard ratio (HR) 0·59, 95 per cent c.i. 0·38 to 0·94; P = 0·026), with a benefit in terms of both local (HR 0·48, 0·24 to 0·99; P = 0·047) and systemic (HR 0·56, 0·33 to 0·94; P = 0·027) recurrence. In analyses stratified by tumour response to neoadjuvant chemotherapy, non‐responders (Mandard tumour regression grade 4–5) treated with adjuvant chemoradiotherapy had an overall survival benefit (HR 0·61, 0·38 to 0·97; P = 0·037). In propensity score‐matched analysis, an overall survival benefit (HR 0·62, 0·39 to 0·98; P = 0·042) and recurrence‐free survival benefit (HR 0·51, 0·30 to 0·87; P = 0·004) were observed for adjuvant chemoradiotherapy versus no adjuvant treatment. Conclusion: Adjuvant therapy may improve overall survival and recurrence‐free survival after margin‐positive resection. This pattern seems most pronounced with adjuvant chemoradiotherapy in non‐responders to neoadjuvant chemotherapy. [ABSTRACT FROM AUTHOR]

Abstract

Footprints: The Evolution of Land Conservation and Reclamation in Alberta is the story of how Alberta’s land conservation and reclamation program came into being and how it has progressed over the past half century. Our goal is to provide an objective description for current and future generations. We believe it may be of interest and value to practitioners actively engaged in the numerous conservation and reclamation components, to landowners whose land is being disturbed, to industrial users responsible for the disturbance, to elected and appointed officials having a moral duty to see to the land’s conservation and reclamation, and to college or university students considering a career in this or a related field. The book was initiated by several dozen retired or still-active land reclamation practitioners whose careers, in some instances, reach as far back as the 1960s. Some are still employed in public or private life, conserving and reclaiming our rich natural heritage. This is being written to help assess how effectively we have, or are, conserving our land base and providing the stewardship required to pass our legacy on to our progeny. Those participating in the book’s creation include professional writers, former and current government regulators, researchers, academics, and former to current industry reclamation managers or practitioners. Some have contributed text, memories of their actions and observations, photographs and documents, or simply fragmented recollections to help piece together this history.

Abstract

HiBar (hyperbaric) continuous pressure filtration can be used to achieve low moisture contents, high specific solids performance and efficient filter cake wash in the processing of fine-grained, difficult to filter material. The lowest cake moisture contents are obtained using HiBar steam pressure filtration in which the filter cake is treated with steam immediately after emerging from the slurry. Using a specially designed steam hood, the filter cake is only partially exposed to steam, which accelerates and intensifies the dewatering process. Benefits include the ability to process mixtures of coarse and fine fractions, reduced transport costs, bulk flow behaviour when discharging to railway wagons, improved transport in cold regions with long periods of frost and lower or zero energy costs for thermal drying. Experience gained during the operation of a pilot plant treating coal ultrafines at the Auguste Victoria coal washery in Germany is described.
