Erken çocukluk gelişimi genetik, sosyal, biyolojik ve çevresel çok çeşitli faktörlerden etkilenir. Annenin ruhsal iyilik hali çocuklarda büyüme ve gelişmeyi etkileyen en önemli etmenlerden biridir. Son yıllarda yapılan araştırmalar, babaların ruh sağlığının da öneminin altını çizmektedir. Ebeveynlerin yaklaşık %18\"i ruhsal bozukluk yaşayabilmektedir. Perinatal depresyon, annelerde %20 sıklıkla, babalarda ise %10 sıklıkla bildirilmiştir. Anne veya babanın depresyon, anksiyete veya stres yaşaması, çocuklarda gelişimsel gecikmeler, büyüme geriliği, bodurluk, duygusal sorunlar ve davranış sorunlarıyla ilişkilendirilmiştir. Çocuklar üzerindeki etkiler henüz bebeklik döneminde ortaya çıkmakta ve adölesan ve erişkin dönemde de devam edebilmektedir. Annede peripartum depresyonun taranması rutin çocukluk çağı izlemlerinin bir parçası olarak önerilmektedir. Yeni araştırmalarla babaların da ruhsal iyilik halinin çocuklar üzerindeki etkileri ortaya konuldukça, babaların da babalığa geçiş sürecinde desteklenmesinin altı çizilmektedir. Çocuk sağlığı uzmanlarının, anne ve babanın ruh sağlığını değerlendirmesi, aile merkezli bütüncül yaklaşımın bir parçası olarak ele alınmalıdır. Numerous factors such as genetic, social, biologic and environmental can impact early childhood development. Maternal mental well-being is one of the most important factors that can affect child growth and development. Almost 18% of the parents experience mental disorders. The prevalence of perinatal depression is 20% in mothers and 10% in fathers. Parental depression, anxiety and stress are associated with developmental problems, failure to thrive, stunting, emotional and behavioral problems in children. These effects can become apparent early in infancy and extend throughout adolescence and adulthood. Screening maternal perinatal depression is recommended in pediatric primary care. As the effects of paternal mental health on child development have been demonstrated with recent findings, supporting the fathers during transition to fatherhood is emphasized. Evaluation of maternal and paternal mental health by child health specialists, should be integrated in well-child visits as part of family centered care.

Amaç:Evlilik ilişkisi, erken çocukluk dönemi uyku sorunları olan bir çocuğun bakımında özellikle önemli olabilir. Amacımız, annenin evlilikten aldığı doyumun, erken çocukluk dönemi uyku sorunları üzerine etkisini incelemektir.Gereç ve Yöntemler:Çocuk Sağlığı İzlem Polikliniğimizden takipli 3-12 ay arası bebeklerin anneleri, anne ve bebek uykusu ile ilgili soruları içeren Kısa Bebek Uyku Anketi\"nden türetilen sorularla değerlendirildi. Annenin evlilik doyumu, Evlilik Yaşamı Ölçeği (EYÖ) ile; annenin depresif semptomları Edinburgh Doğum Sonrası Depresyon Ölçeği (EDSDÖ) ile; anne-bebek bağlanması ise Doğum Sonrası Bağlanma Ölçeği (DSBÖ) ile değerlendirildi.Bulgular:Yüz dört anne-bebek ikilisinde, bebeklerde uyku sorunu sıklığı %39,4 olarak bildirildi. Uyku sorunu olan ve olmayan bebeklerin annelerinin EYÖ puanları arasında fark saptanmadı. Anne EYÖ puanı ile bebeklerin gece uyanma sayıları negatif korelasyon gösterdi (rs: - 0,27, p=0,005). Babaların %18,3\"ü anne-bebek ikilisinden farklı odada uyumaktaydı. Babanın farklı odada uyuduğu ailelerde, annelerin EYÖ puanı daha düşük saptandı (p=0,008). Background In the context of containment measures against the COVID-19 pandemic, the aims were to examine the impact of lockdown and school closures on childs' and adolescents' health and well-being and social inequalities in health. Methods Literature review by searching five databases until November 2020. We included quantitative peer-reviewed studies reporting health and well-being outcomes in children (0-18 years) related to closure measures' impact due to COVID-19. A pair of authors assessed the risk of bias of included studies. A descriptive and narrative synthesis was carried out. Findings Twenty-two studies, including high-income, middle-income and low-income countries, fulfilled our search criteria and were judged not to have an increased risk of bias. Studies from Australia, Spain and China showed an increase in depressive symptoms and decrease in life satisfaction. A decrease in physical activity and increase in unhealthy food consumption were shown in studies from two countries. There was a decrease in the number of visits to the emergency department in four countries, an increase in child mortality in Cameroon and a decrease by over 50% of immunisations administered in Pakistan. A significant drop of 39% in child protection medical examination referrals during 2020 compared with the previous years was found in the UK, a decrease in allegations of child abuse and neglect by almost one-third due to school closures in Florida, and an increase in the number of children with physical child abuse trauma was found in one centre in the USA. Interpretation From available reports, pandemic school closure and lockdown have adverse effects on child health and well-being in the short and probably long term. We urge governments to take the negative public health consequences into account before adopting restrictive measures in childhood.

Breastfeeding difficulties are common with a reported occurrence of approximately 70%, leading to adverse outcomes such as early cessation of breastfeeding and postpartum depression. Professional breastfeeding support is associated with a higher success of exclusive breastfeeding. The aim of this paper is to discuss the common breastfeeding difficulties, one of the biggest obstacles to exclusive breastfeeding, in light of evidence-based research. Poor latching and nipple pain are the main concerns about breastfeeding. Failure to diagnose these problems result in low milk supply and inadequate weight gain of the baby. Recent ultrasound studies reveal new information about breastfeeding. For a successful breastfeeding early skin to skin contact of the mother-infant dyad, stimulating the baby’s primitive reflexes without missing the postpartum 1-2-hour window period, giving self-confidence to the mother, instead of hands on interventional approaches, preparing the mother physically and mentally for a comfortable breastfeeding, frequent breastfeeding in an optimal position is recommended. Herbal products used to increase breast milk supply are not recommended due to the lack of standardized doses, possible pollutants, allergic potential and possible drug interactions, Emzirme sorunları doğum sonrası ilk bir ayda yaklaşık %70 sıklıkla yaygın görülmekte, emzirmenin erken sonlanması, postpartum depresyon gibi olumsuz sonuçlar doğurmaktadır. Bu yazının amacı sadece anne sütüyle beslenmenin önündeki en büyük engellerden, yaygın görülen emzirme sorunlarını kanıta dayalı olarak güncel araştırmalar eşliğinde tartışmaktır. Bebeğin memeye yerleşme sorunları ve ağrılı meme ucu emzirme ile ilgili kaygıların başında gelmektedir. Bu sorunların erken saptanmaması halinde sütte azalma ve bebekte yetersiz tartı alımı ile sonuçlanmaktadır. Son yıllarda yapılan ultrason çalışmaları emzirme konusunda yeni bilgiler ortaya koymaktadır. Başarılı emzirmede anne bebek ikilisinin erken ten tene teması, doğum sonrası 1-2 saatlik pencere döneminin kaçırılmayarak bebeğin primitif reflekslerinin harekete geçirilmesi, annenin güvenini kıran müdahaleci yaklaşımlar yerine, anneye özgüven kazandırılması, rahat bir emzirme için bedenen ve ruhen hazırlanması, uygun memede yerleşimle, sık emzirme önerilmektedir. Anne sütünü arttırmak için kullanılan bitkisel ürünlerde standardize doz olmaması, olası kirleticiler, alerjik potansiyelleri ve olası ilaç etkileşimleri nedeniyle kullanılması önerilmemektedir.

Introduction: Asthma is one of the most common respiratory disorders in clinical practice, affecting up to 13% of people worldwide. Leukotriene antagonists have long been used in the treatment of asthma. This study was conducted to compare the effectiveness of the montelukast and zafirlukast treatment in children diagnosed with asthma. Methods: Sixty-one patients diagnosed with asthma in the age group of 6-14 years were included in the study. Patients were consecutively divided into 2 groups, and the first group was given 5 mg of montelukast before sleep and the second group received 10 mg/day of zafirlukast in two doses. The patients were followed up monthly for 6 months. Short acting beta agonist and inhaled corticosteroid use, daytime symptoms, night awakenings, admission to the emergency department andFEV1 value were compared before and after the treatment and between two groups. Results: Significant improvement was observed in both groups during post-treatment period when compared to the pre-treatment period in terms of the short acting beta agonist and inhaled corticosteroid use, daytime symptoms, night awakenings, mean FEV I. However, no statistical difference was determined between two groups. Discussion and Conclusion: It was found that monteluksat and zafirlukast were effective in asthma treatment and there was no difference between their effects.
