Abstract

There has been continuous progress in diabetes management over the last few decades, not least due to the widespread dissemination of continuous glucose monitoring (CGM) and automated insulin delivery systems. These technological advances have radically changed the daily lives of people living with diabetes, improving the quality of life of both children and their families. Despite this, hypoglycemia remains the primary side-effect of insulin therapy. Based on a systematic review of the available scientific evidence, this paper aims to provide evidence-based recommendations for recognizing, risk stratifying, treating, and managing patients with hypoglycemia. The objective of these recommendations is to unify the behavior of pediatric diabetologists with respect to the timely recognition and prevention of hypoglycemic episodes and the correct treatment of hypoglycemia, especially in patients using CGM or advanced hybrid closed-loop systems. All authors have long experience in the specialty and are members of the Italian Society of Pediatric Endocrinology and Diabetology. The goal of treating hypoglycemia is to raise blood glucose above 70 mg/dL (3.9 mmol/L) and to prevent further decreases. Oral glucose at a dose of 0.3 g/kg (0.1 g/kg for children using “smart pumps” or hybrid closed loop systems in automated mode) is the preferred treatment for the conscious individual with blood glucose

Abstract

Using school choice as a motivating example, we introduce a stylized model of a many-to-one matching market where the clearinghouse aims to implement contingent priorities, i.e., priorities that depend on the current assignment, to prioritize students with siblings and match them together. We provide a series of guidelines and introduce two natural approaches to implement them: (i) absolute, whereby a prioritized student can displace any student without siblings assigned to the school, and (ii) partial, whereby prioritized students can only displace students that have a less favorable lottery than their priority provider. We study several properties of the corresponding mechanisms, including the existence of a stable assignment under contingent priorities, the complexity of deciding whether there exists one, and its incentive properties. Furthermore, we introduce a soft version of these priorities to guarantee existence, and we provide mathematical programming formulations to find such stable matching or certify that one does not exist. Finally, using data from the Chilean school choice system, we show that our framework can significantly increase the number of students assigned to their top preference and the number of siblings assigned together relative to current practice.

3. Five Parameters for Studying Leadership Styles in Orchestra Conductors

Abstract

This study aims to investigate leadership in orchestra conducting and interrogate educational principles to improve conducting pedagogy. Effective leadership implies a set of interpersonal, communicational, and emotional skills combined with a high level of expertise, which may enable the leader to not only achieve excellent outcomes but also create a positive and collaborative working climate and vision for the performance. This work demonstrates that there are strong implications between effective leadership and orchestra conducting. We examine how orchestra conductors are perceived to be effective leaders and deduce from general leadership theories the following five parameters: charisma, stage presence, nonverbal communication, relationships with musicians, and leadership style. Interviews with orchestra conductors and performers support these five parameters for effective orchestra conducting. We perform a detailed analysis of the profiles of two renowned orchestra leaders-- namely, Herbert von Karajan and Gustavo Dudamel-- to test the five parameters and distill educational implications for both scholars and practitioners. The results are presented and discussed, along with implications for the education of orchestra conductors.

7. Design and Implementation of an Heuristic-Enhanced Branch-and-Bound Solver for MILP

Abstract

We present a solver for Mixed Integer Programs (MIP) developed for the MIP competition 2022. Given the 10 minutes bound on the computational time established by the rules of the competition, our method focuses on finding a feasible solution and improves it through a Branch-and-Bound algorithm. Another rule of the competition allows the use of up to 8 threads. Each thread is given a different primal heuristic, which has been tuned by hyper-parameters, to find a feasible solution. In every thread, once a feasible solution is found, we stop and we use a Branch-and-Bound method, embedded with local search heuristics, to ameliorate the incumbent solution. The three variants of the Diving heuristic that we implemented manage to find a feasible solution for 10 instances of the training data set. These heuristics are the best performing among the heuristics that we implemented. Our Branch-and-Bound algorithm is effective on a small portion of the training data set, and it manages to find an incumbent feasible solution for an instance that we could not solve with the Diving heuristics. Overall, our combined methods, when implemented with extensive computational power, can solve 11 of the 19 problems of the training data set within the time limit. 8. Capacity Variation in the Many-to-one Stable Matching

Abstract

The many-to-one stable matching problem provides the fundamental abstraction of several real-world matching markets such as school choice and hospital-resident allocation. The agents on both sides are often referred to as residents and hospitals. The classical setup assumes that the agents rank the opposite side and that the capacities of the hospitals are fixed. It is known that increasing the capacity of a single hospital improves the residents' final allocation. On the other hand, reducing the capacity of a single hospital deteriorates the residents' allocation. In this work, we study the computational complexity of finding the optimal variation of hospitals' capacities that leads to the best outcome for the residents, subject to stability and a capacity variation constraint. First, we show that the decision problem of finding the optimal capacity expansion is NP-complete and the corresponding optimization problem is inapproximable within a certain factor. This result holds under strict and complete preferences, and even if we allocate extra capacities to disjoint sets of hospitals. Second, we obtain analogous computational complexity results for the problem of capacity reduction. Finally, we study the variants of these problems when the goal is to maximize the size of the final matching under incomplete preference lists.

Abstract

This paper reports on the first international competition on AI for the traveling salesman problem (TSP) at the International Joint Conference on Artificial Intelligence 2021 (IJCAI-21). The TSP is one of the classical combinatorial optimization problems, with many variants inspired by real-world applications. This first competition asked the participants to develop algorithms to solve a time-dependent orienteering problem with stochastic weights and time windows (TD-OPSWTW). It focused on two types of learning approaches: surrogate-based optimization and deep reinforcement learning. In this paper, we describe the problem, the setup of the competition, the winning methods, and give an overview of the results. The winning methods described in this work have advanced the state-of-the-art in using AI for stochastic routing problems. Overall, by organizing this competition we have introduced routing problems as an interesting problem setting for AI researchers. Abstract

Motivated by the shortage of seats that the Chilean school choice system is facing, we introduce the problem of jointly increasing school capacities and finding a student-optimal assignment in the expanded market. Due to the theoretical and practical complexity of the problem, we provide a comprehensive set of tools to solve the problem, including different mathematical programming formulations, a cutting plane algorithm, and two heuristics that allow obtaining near-optimal solutions quickly. On the theoretical side, we show the correctness of our formulations, different properties of the objective and feasible region that facilitate computation, and also several properties of the underlying mechanism to find a student-optimal matching under capacity expansions. On the computational side, we use data from the Chilean school choice system to demonstrate the impact of our framework and derive insights that could help alleviate the problem. Our results show that each additional seat can benefit multiple students and that we can effectively target the assignment of previously unassigned students or improve the assignment of several students through improvement chains. Nevertheless, our results show that the marginal effect of each additional seat is decreasing and that simply adding seats is insufficient to ensure every student gets assigned to some school. Finally, we discuss several extensions of our framework, showcasing its flexibility to accommodate different needs.

Abstract

Abstract Background No univocal recommendation exists for microbiological diagnosis of ventilator-associated pneumonia (VAP). Sampling of either proximal or distal respiratory tract likely impacts on the broad range of VAP incidence between cohorts. Immune biomarkers to rule-in/rule-out VAP diagnosis, although promising, have not yet been validated. COVID-19-induced ARDS made VAP recognition even more challenging, often leading to overdiagnosis and overtreatment. We evaluated the impact of different respiratory samples and laboratory techniques on VAP incidence and microbiological findings in COVID-19 patients. Methods Prospective single-centre cohort study conducted among COVID-19 mechanically ventilated patients in Policlinico Hospital (Milan, Italy) from January 2021 to May 2022. Microbiological confirmation of suspected VAP (sVAP) was based on concomitant endotracheal aspirates (ETA) and bronchoalveolar lavage (BAL). Conventional and fast microbiology (FILMARRAY® Pneumonia Panel plus, BALFAPPP) as well as immunological markers (immune cells and inflammatory cytokines) was analysed. Results Seventy-nine patients were included. Exposure to antibiotics and steroid therapy before ICU admission occurred in 51/79 (64.6%) and 60/79 (65.9%) patients, respectively. Median duration of MV at VAP suspicion was 6 (5–9) days. Incidence rate of microbiologically confirmed VAP was 33.1 (95% CI 22.1–44.0) and 20.1 (95% CI 12.5–27.7) according to ETA and BAL, respectively. Concordance between ETA and BAL was observed in 35/49 (71.4%) cases, concordance between BALFAPPP and BAL in 39/49 (79.6%) cases. With BAL as reference standard, ETA showed 88.9% (95% CI 70.8–97.7) sensitivity and 50.0% (95% CI 28.2–71.8) specificity (Cohen’s Kappa 0.40, 95% CI 0.16–0.65). BALFAPPP showed 95.0% (95% CI 75.1–99.9) sensitivity and 69% (95% CI 49.2–84.7) specificity (Cohen’s Kappa 0.60, 95% CI 0.39–0.81). BAL IL-1β differed significantly between VAP (135 (IQR 11–450) pg/ml) and no-VAP (10 (IQR 2.9–105) pg/ml) patients (P = 0.03). Conclusions In COVID-19 ICU patients, differences in microbial sampling at VAP suspicion could lead to high variability in VAP incidence and microbiological findings. Concordance between ETA and BAL was mainly limited by over 20% of ETA positive and BAL negative samples, while BALFAPPP showed high sensitivity but limited specificity when evaluating in-panel targets only. These factors should be considered when comparing results of cohorts with different sampling. BAL IL-1β showed potential in discriminating microbiologically confirmed VAP. This study presents experimental and computational analyses of the phase composition and mechanical behavior of a functionally graded material (FGM) grading from 100 vol% 304L stainless steel (SS304L) to 100 vol% Ni-20Cr (NiCr) in 10 vol% increments. The mechanical behavior of the FGM was evaluated by extracting tensile specimens from SS304L-rich and NiCr-rich regions of the FGM, with the former behaving in a ductile manner and the latter being relatively brittle. The brittle response of the NiCr-rich sample was explained by the presence of the eutectic FCC+BCC phase in this sample, as identified by Scheil-Gulliver simulations along with experimental phase and composition analyses. The spatially varying yield and flow behavior of the FGM was determined through comparison of finite element analysis simulations with experimentally measured stress-strain curves and surface strain contours. The results highlight that the overall ductile behavior of SS304L and brittle behavior of NiCr are preserved in the SS304L/NiCr FGM but the formation of the brittle eutectic phase further reduced the ductility within the regions where it formed (90 vol% NiCr/10 vol% SS304L)., Comment: 39 pages, 13 figures, 3 tables

Abstract Aims Due to the shortage of heart donors, increasing numbers of heart transplantation (HTx) candidates are receiving long‐term mechanical circulatory support (MCS) as bridge‐to‐transplantation. Treatment with MCS is associated with increased formation of anti‐human leukocyte antigen antibodies (allosensitization), but whether this affects post‐HTx outcomes is unclear. Methods and results We included all adult patients who received long‐term MCS as bridge‐to‐transplantation and underwent subsequent HTx at our centre between 2008 and 2018. We also enrolled medically treated HTx recipients without prior MCS as controls. These controls were matched by age, sex, diagnosis, and transplantation era. Outcome parameters were compared between the two study groups. A total of 126 patients (48 ± 15 years, 84% male) were included of whom 64 were bridged with MCS and 62 were matched controls. Pre‐HTx allosensitization occurred more frequently in the MCS group than in the control group (27% vs. 11%, P = 0.03). At post‐HTx year 10, the overall survival probability was 84% among patients treated with MCS and 90% among those medically managed (P = 0.32). At post‐HTx year 1, freedom from treated rejections (≥ISHLT 2R) was 69% in the MCS group and 70% in the control group (P = 0.94); and freedom from any rejection was 8% and 5%, respectively (P = 0.98). There were no differences in renal function or cardiac allograft vasculopathy (grade ≥ 1) between groups at 1, 3, and 5 years post‐HTx. Conclusions Although patients treated with MCS had a higher frequency of pre‐HTx allosensitization, there were no significant differences in post‐HTx graft survival, biopsy‐proven rejections, or renal function as compared with patients not bridged with MCS.

Abstract

Additive manufacturing (AM), through directed energy deposition, supports planned composition changes between locations within a single component, allowing for functionally graded materials (FGMs) to be developed and fabricated. The formation of deleterious phases along a particular composition path can cause significant cracking during the AM build process that makes the composition path unviable to produce these FGMs, but it is challenging to predict which phases will be present in as-built additively manufactured parts by analyzing only equilibrium phase relations. Solute segregation during solidification can lead to the formation of non-equilibrium phases that are stable at compositions far from the nominal composition of the melt, leading to crack formation. In this work, we developed a Scheil-Gulliver simulation tool based on pycalphad. We used this tool to compare the non-equilibrium phases predicted to form during solidification using the Scheil-Gulliver model with experimentally measured phases at several locations with different composition in a Ti-6Al-4V to Invar-36 FGM and a commercially pure Ti to Invar-36 FGM. We showed that the phases predicted to form by the Scheil-Gulliver model outperform the predictions made by assuming equilibrium solidification. Specific immune suppression types have been associated with a greater risk of severe COVID-19 disease and death. We analyzed data from patients >17 years that were hospitalized for COVID-19 at the “Fondazione IRCCS Ca′ Granda Ospedale Maggiore Policlinico” in Milan (Lombardy, Northern Italy). The study included 1727 SARS-CoV-2-positive patients (1,131 males, median age of 65 years) hospitalized between February 2020 and November 2022. Abstract

Introducción: La violencia de pareja (VP) perpetrada y sufrida es un problema de salud pública mundial. Una de las variables asociadas más estudiadas ha sido la victimización infantil-juvenil (VI). Objetivo: Analizar el rol de la VI en la VP en población general de Argentina, considerando a las modalidades específicas de violencias tanto sufridas, como ejercidas, y a posibles diferencias entre hombres y mujeres. Método: Se encuestó a 243 adultos de Argentina. Se evaluaron sus datos sociodemográficos, la VI y la VP perpetrada y sufrida en la adultez, mediante el Juvenile Victimization Questionnaire (JVQ) y la Conflict Tactics Scale (CTS2). Se llevaron a cabo análisis bivariados y multivariados para calcular asociaciones entre las variables. Resultados: Un 99% de la muestra total indicó haber sufrido al menos un acto de VI, sin diferencias entre los sexos. En cuanto a la VP perpetrada y sufrida, ambos sexos presentaron prevalencias similares, con la única excepción de la violencia sexual perpetrada, significativamente mayor en varones (41.70%) que en mujeres (19.10%). Las asociaciones registradas entre VI y VP fueron específicas. Conclusiones: Las asociaciones sugieren que determinados tipos de VI constituyen un factor de riesgo para ciertos tipos de VP en la adultez. Se discuten las implicancias de los hallazgos para la prevención y la investigación.

Abstract

Background: Despite the well-known efficacy of anti-COVID-19 vaccines in preventing morbidity and mortality, several vaccinated individuals are diagnosed with SARS-CoV-2 breakthrough infection, which might require hospitalisation. This multicentre, observational, and retrospective study aimed to investigate the clinical characteristics and outcomes of vaccinated vs. non-vaccinated patients, both hospitalised with SARS-CoV-2 infection in 3 major hospitals in Northern Italy. Methods: Data collection was retrospective, and paper and electronic medical records of adult patients with a diagnosed SARS-CoV-2 infection were pseudo-anonymised and analysed. Vaccinated and non-vaccinated individuals were manually paired, using a predetermined matching criterion (similar age, gender, and date of hospitalisation). Demographic, clinical, treatment, and outcome data were compared between groups differing by vaccination status using Pearson's Chi-square and Mann-Whitney tests. Moreover, multiple logistic regression analyses were performed to assess the impact of vaccination status on ICU admission or intra-hospital mortality. Results: Data from 360 patients were collected. Vaccinated patients presented with a higher prevalence of relevant comorbidities, like kidney replacement therapy or haematological malignancy, despite a milder clinical presentation at the first evaluation. Non-vaccinated patients required intensive care more often than their vaccinated counterparts (8.8% vs. 1.7%, p = 0.002). Contrariwise, no difference in intra-hospital mortality was observed between the two groups (19% vs. 20%, p = 0.853). These results were confirmed by multivariable logistic regressions, which showed that vaccination was significantly associated with decreased risk of ICU admission (aOR=0.172, 95%CI: 0.039–0.542, p = 0.007), but not of intra-hospital mortality (aOR=0.996, 95%CI: 0.582–1.703, p = 0.987). Abstract

Background Cardiac involvement can be an initial manifestation in sarcoidosis. However, little is known about the association between various clinical phenotypes of cardiac sarcoidosis (CS) and outcomes. We aimed to analyze the relation of different clinical manifestations with outcomes of CS and to investigate the relative importance of clinical features influencing overall survival. Methods and Results A retrospective cohort of 141 patients with CS enrolled at 2 Swedish university hospitals was studied. Presentation, imaging studies, and outcomes of de novo CS and previously known extracardiac sarcoidosis were compared. Survival free of primary composite outcome (ventricular arrhythmias, heart transplantation, or death) was assessed. Machine learning algorithm was used to study the relative importance of clinical features in predicting outcome. Abstract

Introduction Treatment strategies for primary aldosteronism (PA) include unilateral adrenalectomy and medical treatment with mineralocorticoid receptor (MR) antagonists. Whether these two different treatment strategies are comparable in mitigating the detrimental effect of PA on outcomes is still debated.Objectives The primary aim of this systematic review is to identify, appraise and synthesise existing literature comparing clinical outcomes after treatment in patients with PA.Methods and analysis A systematic and comprehensive search will be performed using PubMed, Web of Science and EMBASE, for studies published until December 2022. Observational and interventional studies will be eligible for inclusion. The quality of observational studies will be assessed using the Newcastle–Ottawa Scale, while interventional studies will be assessed using the Cochrane Effective Practice Organization of Care tool. The collected evidence will be narratively synthesised. We will perform meta-analysis to pool estimates from studies considered to be homogeneous. Reporting of the systematic review and meta-analysis will be in accordance with the Meta-analysis of Observational Studies in Epidemiology Preferred Reporting Items for Systematic reviews and Meta-Analysis guidelines.Ethics and dissemination As this study is based solely on the published literature, no ethics approval is required. This review will aim to provide some estimates on outcomes, including survival, rates of clinical and biochemical control, cardiovascular and cerebrovascular events, as well as data on quality of life and renal function, in patients with PA treated surgically or with MR antagonists. The study findings will be presented at scientific meetings and will be published in an international peer-reviewed scientific journal.PROSPERO registration number CRD42022362506.
