11 juli 2023, Contains fulltext : 295281.pdf (Publisher’s version ) (Open Access), An Elimination Diet (ED) may be effective in reducing symptoms of Attention-Deficit/Hyperactivity Disorder (ADHD), but has never been compared to an active control condition [i.e., Healthy Diet (HD)]. In a two-armed RCT, a total of N = 165 children (5-12 years) with ADHD were randomized by means of minimization (1:1) to either an ED (N = 84) or HD (N = 81) within two Dutch child and adolescent psychiatry centers. The design included a non-randomized comparator arm including N = 58 children being treated with Care as Usual (CAU). Treatment allocation was unblinded. The primary outcome was a 5-point ordinal measure of respondership based on a combination of parent and teacher ratings on ADHD and emotion regulation, determined after 5 weeks of treatment. Ordinal regression analyses were done on an intention-to-treat basis. Fewer ED (35%) than HD (51%) participants showed a partial to full response, despite overall good-to-excellent treatment adherence (> 88%) and comparable high parental prior believes. A younger age and higher problem severity predicted a better respondership. CAU-preferring participants responded more often favorably (56%) compared to ED-but not HD-participants. Small-to-medium improvements in physical health (blood pressure, heart rate, and somatic complaints) were found in response to ED/HD versus decrements in response to CAU (74% received psychostimulants). The lack of superiority of the ED versus HD suggests that for the majority of children, dietary treatment response is not rooted in food-allergies/-sensitivities. The comparable results for treatment with HD and CAU are remarkable given that CAU participants were probably 'easier to treat' than HD (and ED) participants with proportionally fewer with a (suboptimal/non-response to) prior treatment with medication (4% versus 20%). Item does not contain fulltext, Forderungen nach unbürokratischem Handeln der deutschen Verwaltung sind bei weitem kein Novum, doch häuften sich diese in besonderer Weise während der Corona-Pandemie. Das Beispiel der Corona-Soforthilfen in Nordrhein-Westfalen verdeutlicht, dass die Forderung nach einer unbürokratischen Bürokratie die falsche Lösung für die durch die ökonomische Bürokratietheorie belegte Ineffizienz der Verwaltung ist. Aus dieser Erkenntnis kann nicht hervorgehen, dass eine effiziente Bürokratie durch geschwächte juristische Prüfung oder übereilte Verfahren ausgezeichnet sein sollte. Zur Reduzierung der echten oder auch nur vermeintlichen Ineffizienzen werden andere Lösungen benötigt. In diesem Kapitel wird aus juristischer und mikroökonomischer Sicht untersucht, welche Relevanz bürokratische Verfahren im Umgang mit öffentlichen Mitteln in Krisensituation, wie einer Pandemie, haben. Der Schwerpunkt dieser Analyse liegt dabei auf der theoretischen Prüfung der Frage, ob die Forderung nach unbürokratischen Verwaltungsabläufen umsetzbar ist. 5. Financial problems and debt as predictive factors of recidivism

- Abstract
This study aimed to examine the unique predictive validity of debt regarding recidivism and what elements are specifically predictive. This was done based on a Dutch sample of 250 people on probation using an explorative research tool, the Finances, Debt, and Offending Scale (FDOS). Cox survival regression and receiver operating characteristics analyses were conducted on the total FDOS and its individual items as predictors and diverse recidivism criteria. The average follow-up duration was 5.41 years. The results show that debt moderately predicts recidivism (adjusted for other predictors). The FDOS significantly predicted recidivism on all three levels of severity, and regarding the type of crime, it predicted recidivism in property and drug-related crime. Earlier debt and probation officers’ indications of whether finances are criminogenic were especially predictive. These insights may help frontline service providers better understand the role of finances in recidivism, measure financial problems in risk assessment, and select interventions.

8. How serious is debt among probation clients: A study into the prevalence and scope of debt in a Dutch probation sample

- Abstract
Although studies point to a relationship between debt and crime, there is a limited understanding of their reciprocal relationship and possible mediating risk factors. Moreover, knowledge about the prevalence and scope of debt among offenders is lacking. Therefore, the present study analyzed 250 client files including risk assessment data from the Dutch probation service on the prevalence of debt and possibly related risk factors. The results show that debt is highly prevalent and complex, which underlines the importance of acquiring more knowledge about debt as a potential risk factor for relapse during supervision. It was found that problems with regard to childhood and living situation, education and work/daytime activities, and mental and physical health may be possible underlying risk factors in the relationship between debt and crime. These insights can help professionals adequately support clients with regard to debt in order to prevent recidivism.

10. How to assist probationers with debt problems during supervision?: A qualitative study into the experiences of both probation officers and clients

Little research exists on what works in the supervision of offenders with debt problems. This qualitative study aims to provide insight into the barriers probation officers and clients experience during supervision regarding debt and the support that clients need. Interviews were conducted with 33 Dutch probation officers and 16 clients. The results show that debt often negatively influences clients’ lives and hinders their resocialization. Probation officers lack effective methods to support clients with debt problems. To adequately help clients with debt problems, probation officers should obtain more knowledge about effective interventions and collaborate more closely with debt specialists from the probation supervision outset.

- Abstract
Wiedemann-Steiner syndrome (WSS) is an autosomal dominant disorder caused by monoallelic variants in KMT2A and characterized by intellectual disability and hypertrichosis. We performed a retrospective, multicenter, observational study of 104 individuals with WSS from five continents to characterize the clinical and molecular spectrum of WSS in diverse populations, to identify physical features that may be more prevalent in White versus Black Indigenous People of Color individuals, to delineate genotype-phenotype correlations, to define developmental milestones, to describe the syndrome through adulthood, and to examine clinicians' differential diagnoses. Sixty-nine of the 82 variants (84%) observed in the study were not previously reported in the literature. Common clinical features identified in the cohort included: developmental delay or intellectual disability (97%), constipation (63.8%), failure to thrive (67.7%), feeding difficulties (66.3%), hypertrichosis cubiti (57%), short stature (57.8%), and vertebral anomalies (46.9%). The median ages at walking and first words were 20 months and 18 months, respectively. Hypotonia was associated with loss of function (LoF) variants, and seizures were associated with non-LoF variants. This study identifies genotype-phenotype correlations as well as race-facial feature associations in an ethnically diverse cohort, and accurately defines developmental trajectories, medical comorbidities, and long-term outcomes in individuals with WSS.

Contains fulltext : 202677.pdf (publisher's version ) (Closed access), BACKGROUND: Missense variants in SMAD2, encoding a key transcriptional regulator of transforming growth factor beta signalling, were recently reported to cause arterial aneurysmal disease. OBJECTIVES: The aims of the study were to identify the genetic disease cause in families with aortic/arterial aneurysmal disease and to further define SMAD2 genotype-phenotype correlations. METHODS AND RESULTS: Using gene panel sequencing, we identified a SMAD2 nonsense variant and four SMAD2 missense variants, all affecting highly conserved amino acids in the MH2 domain. The premature stop codon (c.612dup; p.(Asn205*)) was identified in a marfanoid patient with aortic root dilatation and in his affected father. A p.(Asn318Lys) missense variant was found in a Marfan syndrome (MFS)-like case who presented with aortic root aneurysm and in her affected daughter with marfanoid features and mild aortic dilatation. In a man clinically diagnosed with Loeys-Dietz syndrome (LDS) that presents with aortic root dilatation and marked tortuosity of the neck vessels, another missense variant, p.(Ser397Tyr), was identified. This variant was also found in his affected daughter with hypertelorism and arterial tortuosity, as well as his affected mother. The third missense variant, p.(Asn361Thr), was discovered in a man presenting with coronary artery dissection. Variant genotyping in three unaffected family members confirmed its absence. The last missense variant, p.(Ser467Leu), was identified in a man with significant cardiovascular and connective tissue involvement. CONCLUSION: Taken together, our data suggest that heterozygous loss-of-function SMAD2 variants can cause a wide spectrum of autosomal dominant aortic and arterial aneurysmal disease, combined with connective tissue findings reminiscent of MFS and LDS.

Contains fulltext : 176973.pdf (publisher's version ) (Open Access), Bicuspid aortic valve (BAV) is the most common congenital heart defect. Although many BAV patients remain asymptomatic, at least 20% develop thoracic aortic aneurysm (TAA). Historically, BAV-related TAA was considered as a hemodynamic consequence of the valve defect. Multiple lines of evidence currently suggest that genetic determinants contribute to the pathogenesis of both BAV and TAA in affected individuals. Despite high heritability, only very few genes have been linked to BAV or BAV/TAA, such as NOTCH1, SMAD6, and MAT2A. Moreover, they only explain a minority of patients. Other candidate genes have been suggested based on the presence of BAV in knockout mouse models (e.g., GATA5, NOS3) or in syndromic (e.g., TGFBR1/2, TGFB2/3) or non-syndromic (e.g., ACTA2) TAA forms. We hypothesized that rare genetic variants in these genes may be enriched in patients presenting with both BAV and TAA. We performed targeted resequencing of 22 candidate genes using Haloplex target enrichment in a strictly defined BAV/TAA cohort (n = 441; BAV in addition to an aortic root or ascendens diameter >/= 4.0 cm in adults, or a Z-score >/= 3 in children) and in a collection of healthy controls with normal echocardiographic evaluation (n = 183). After additional burden analysis against the Exome Aggregation Consortium database, the strongest candidate susceptibility gene was SMAD6 (p = 0.002), with 2.5% (n = 11) of BAV/TAA patients harboring causal variants, including two nonsense, one in-frame deletion and two frameshift mutations. All six missense mutations were located in the functionally important MH1 and MH2 domains. In conclusion, we report a significant contribution of SMAD6 mutations to the etiology of the BAV/TAA phenotype.

- Abstract
Bicuspid aortic valve (BAV) is the most common congenital heart defect. Although many BAV patients remain asymptomatic, at least 20% develop thoracic aortic aneurysm (TAA). Historically, BAV-related TAA was considered as a hemodynamic consequence of the valve defect. Multiple lines of evidence currently suggest that genetic determinants contribute to the pathogenesis of both BAV and TAA in affected individuals. Despite high heritability, only very few genes have been linked to BAV or BAV/TAA, such as NOTCH1, SMAD6, and MAT2A. Moreover, they only explain a minority of patients. Other candidate genes have been suggested based on the presence of BAV in knockout mouse models (e.g., GATA5, NOS3) or in syndromic (e.g., TGFBR1/2, TGFB2/3) or non-syndromic (e.g., ACTA2) TAA forms. We hypothesized that rare genetic variants in these genes may be enriched in patients presenting with both BAV and TAA. We performed targeted resequencing of 22 candidate genes using Haloplex target enrichment in a strictly defined BAV/TAA cohort (n = 441; BAV in addition to an aortic root or ascendens diameter = 4.0 cm in adults, or a Z-score = 3 in children) and in a collection of healthy controls with normal echocardiographic evaluation (n = 183). After additional burden analysis against the Exome Aggregation Consortium database, the strongest candidate susceptibility gene was SMAD6 (p = 0.002), with 2.5% (n = 11) of BAV/TAA patients harboring causal variants, including two nonsense, one in-frame deletion and two frameshift mutations. All six missense mutations were located in the functionally important MH1 and MH2 domains. In conclusion, we report a significant contribution of SMAD6 mutations to the etiology of the BAV/TAA phenotype.

In the original article, we noted two mutation annotation errors. The correction of these two mistakes does not change the scientific conclusions in any way. The authors apologize for these nomenclature errors. Please find below the corrected annotations of those two mutations: (1) The correct RNA and protein annotations of the SMAD6 variant in P99 are c.455_461del and p.Pro152Profs*27, and not c.454_461del and p.Gly166Valfs*23. (2) The correct RNA and protein annotations of the SMAD6 variant in P128 are c.74_79del and p.Ser27_Gly28del, and not c.73_79del and p.Gly26_Ser27del. As a consequence, a correction has been made to RESULTS, Paragraphs 5 and 6: The SMAD6 c.726del variant leads to a frameshift (p.Lys242Asnfs*300) and a predicted protein with a C-terminal extension due to loss of the intended stop codon. The c.455_461del frameshift variant (p.Pro152Profs*27) causes the introduction of a premature stop codon, most likely resulting in haploinsufficiency due to nonsense-mediated mRNA decay (NMD). Also the two nonsense variants (p.Tyr279* and p.Tyr288*) are predicted to lead to NMD. All of the missense variants cluster in the functionally important MH1 and MH2 domains (Makkar et al., 2009) (amino acids 148-275 and 331-496, respectively), which is not the case for the sole missense variant (p.Ser130Leu) found in a control individual (Figure 2). All but one (p.Arg443His) of the identified variants were absent in the ExAC control cohort (v0.3.1; Supplementary Table 2). Moreover, the missense variants in the patient cohort (7/7) are enriched in the MH1 and MH2 domains when compared to ExAC controls (n = 228/430; p = 0.02).

With new technologies such as machine-to-machine communications there is a growing need to test large-formfactor devices in reverberation chambers. The performance of the reverberation chamber can be affected by the physical characteristics of these devices. In this paper it is discussed how the spatial uniformity is affected by a large object. A distinction is made between reflecting and absorbing objects. A link is shown between the spatial uniformity and the K-factor. It is shown that large metallic objects cause minimal degradation in the spatial uniformity. Absorbing objects can cause substantial degradation in the spatial uniformity, motivating the need for testing guidelines for these objects.

Gevoeligheidsanalyse beschrijft de invloed van parameter-variaties op de uitkomsten van een wiskundige model. Het begrip "gevoeligheid van een grootheid ten opzichte van een parameter" geeft aan in welke mate toestandsgrootheden afhankelijk zijn van parameters. Als het model beschreven wordt door een stelsel gewone differentiaalvergelijkingen kan daaruit een stelsel gevoeligheidsvergelijkingen worden afgeleid. Het FORTRAN-programma ODE3 lost de model- en gevoeligheidsvergelijkingen op. Bovendien geeft het desgewenst files met plot-instructies, zodat de resultaten geplot kunnen worden. Het programma is getest door het toe te passen op een viertal praktijkmodellen, te weten: planktongroei in de Loosdrechtse Plassen, giftoediening aan ratten, zeespiegelstijging en prooi-roofdier-modellen van Volterra-Lotka.

Risico’s en faalkosten in zorgbouwprojecten zijn zo oud als er zorgbouwprojecten bestaan. Tal van factoren zijn van invloed op de haalbaarheid en het succes van vastgoedinvesteringen in de zorgsector: de snelle nationale en internationale ontwikkelingen op het gebied van zorg- en gebouwconcepten, veranderende eisen en wensen van cliënten, bezuinigingen, arbeidsproductiviteit, (budget- en tijd)planningen, organisatie, samenwerkende partners, sociale media en nieuwe technologie. Niet alle risico’s kunnen worden voorkomen en niet alle veranderingen in de interne en externe omgeving kunnen worden beheerst. Maar kent u de risico’s die uw project bedreigen en kunt u die wel tijdig identificeren en beheersen? Beschikt u over praktische, systematische methoden om potentiele risico’s in kaart te brengen, die de baas te zijn én te blijven? In de zorgbouw worden bewust of onbewust verschillende methoden toegepast voor het identificeren en beheersen van risico’s. In de loop van 2011 heeft TNO onderzocht welke methode zich het beste leent voor de implementatie van risicomanagement in de zorgbouw. Deze methode moet in de dynamische zorgsector met uiteenlopende nieuwe vraagstukken systematisch en praktisch toegepast kunnen worden in de initiatieffase van een bouwproject. De bestaande Risico Diagnose Methode (hierna: RDM)1, ontworpen door professor J.I.M. Halman, leende zich goed voor ombouw en toepassing in de zorgbouw. TNO heeft samen met professor Halman deze methode geschikt gemaakt voor de zorgbouw, waarna deze is gevalideerd in de initiatieffase van een bouwproject van de zorgorganisatie ASVZ. Tijdens de validatie van de RDM is een diagnose gesteld van de risico’s die het zorgbouwproject bedreigen en de kloof tussen kennis, kunde en mogelijkheden is in kaart gebracht. Medewerkers en bestuur van ASVZ, die actief hebben geparticipeerd in dit project, zijn van mening dat de noodzaak en het nut van deze methode zijn bewezen. In dit rapport wordt na de inleiding ingegaan op de algemene theorie en achtergrond van risicomanagement, waarna de essentie en toepassing van de RDM wordt beschreven. De jarenlange ervaring en kennis van TNO / DuCHA (voormalige Bouwcollege) medewerkers op het gebied van allerlei vormen van zorgbouw vormden de input voor ombouw van de RDM voor de zorgbouw. Kennis van de trends en risico’s in de zorgbouw (hoofdstuk 4 en 5) is noodzakelijk voor het succesvol implementeren van de RDM. In de tekst wordt ingegaan op de politieke ontwikkelingen, haalbaarheid, markt en concurrentie, organisatie, zorg- en woonconcepten, financiën, draagvlak, locatie, grond en samenwerkingsverbanden. Background Aneurysms affecting the aorta are a common condition associated with high mortality as a result of aortic dissection or rupture. Investigations of the pathogenic mechanisms involved in syndromic types of thoracic aortic aneurysms, such as Marfan and Loeys-Dietz syndromes, have revealed an important contribution of disturbed transforming growth factor (TGF)-β signaling. Objectives This study sought to discover a novel gene causing syndromic aortic aneurysms in order to unravel the underlying pathogenesis. Methods We combined genome-wide linkage analysis, exome sequencing, and candidate gene Sanger sequencing in a total of 470 index cases with thoracic aortic aneurysms. Extensive cardiological examination, including physical examination, electrocardiography, and transthoracic echocardiography was performed. In adults, imaging of the entire aorta using computed tomography or magnetic resonance imaging was done. Results Here, we report on 43 patients from 11 families with syndromic presentations of aortic aneurysms caused by TGFB3 mutations. We demonstrate that TGFB3 mutations are associated with significant cardiovascular involvement, including thoracic/abdominal aortic aneurysm and dissection, and mitral valve disease. Other systemic features overlap clinically with Loeys-Dietz, Shprintzen-Goldberg, and Marfan syndromes, including cleft palate, bifid uvula, skeletal overgrowth, cervical spine instability and clubfoot deformity. In line with previous observations in aortic wall tissues of patients with mutations in effectors of TGF-β signaling (TGFBR1/2, SMAD3, and TGFB2), we confirm a paradoxical up-regulation of both canonical and noncanonical TGF-β signaling in association with up-regulation of the expression of TGF-β ligands. Conclusions Our findings emphasize the broad clinical variability associated with TGFB3 mutations and highlight the importance of early recognition of the disease because of high cardiovascular risk.

1 december 2015, Item does not contain fulltext, This review focusses on impact of a better knowledge of pathogenic mechanisms of Marfan and related disorders on their treatment strategies. It was long believed that a structural impairment formed the basis of Marfan syndrome as deficiency in the structural extracellular matrix component, fibrillin-1 is the cause of Marfan syndrome. However, the study of Marfan mouse models has revealed the strong involvement of the transforming growth factor-beta signalling pathway in the pathogenesis of Marfan. Similarly, this pathway was demonstrated to be key in the pathogenesis of Loeys-Dietz and Shprintzen-Goldberg syndrome. The elucidation of the underlying pathogenic mechanisms has led to new treatment strategies, targeting the overactive TGF-beta pathway. Various clinical trials are currently investigating the potential new treatment options. A meta-analysis will contribute to a better understanding of the various trial results.

Die Flächennutzung durch erneuerbare Energien (EE), insbesondere die räumliche Allokation von Erzeugungsanlagen, wird maßgeblich getrieben durch das Zusammenspiel des zentralen Fördermechanismus des Erneuerbare-Energien-Gesetzes (EEG) mit den dezentralen Entscheidungen der regionalen Gebietskörperschaften über die Flächenausweisung für die Errichtung von Erzeugungsanlagen. Die nationalen Förderanreize für regenerative Stromerzeugung zielen auf die Realisierung konkreter EE-Projekte auf regional-lokaler Ebene, sodass den jeweiligen Planungsträgern über die Formulierung von Anforderungen an die Standortwahl und die Anlagenbedingungen (z. B. maximale Höhe für Windkraft, Nutzungssatzungen für Photovoltaik im Stadtgebiet usw.) indirekt eine strategische Rolle für der Ausgestaltung des regionalen Energiesystems zukommt, die über Umfang und Technologiemix des EE-Ausbaus mitbestimmt.

- Abstract
DELPHI is a 4pi detector with emphasis on particle identification, three-dimensional information, high granularity and precise vertex determination. The design criteria, the construction of the detector and the performance during the first year of operation at the large electron positron collider (LEP) at CERN are described.
