34. Estimates of Quality-Adjusted Life-Year Loss for Injuries in the United States.

Abstract

Purpose. The goal of this study is to develop an approach for estimating nationally representative quality-adjusted life-year (QALY) loss from injury and poisoning conditions using data collected in the Medical Expenditure Panel Survey (MEPS) and the National Health Interview Survey (NHIS). Methods: This study uses data from the 2002– 2015 NHIS and MEPS surveys. Injuries were identified in the MEPS medical events file and through self-reporting of medical conditions. We restricted our model to 163,731 adults, for which we predict a total of 294,977 EQ-5D scores using responses to the self-administered questionnaire. EQ-5D scores were modeled using age, sex, comorbidities, and binary indicators of the presence and duration of injury at the time of the health status questionnaire. These models consider nonlinearity over time during the first 3 y following the injury event. Results. Injuries are identified in MEPS using medical events that provide a reasonable proxy for the date of injury occurrence. Healthrelated quality of life (HRQL) decrements can be estimated using binary indicators of injury during different time periods. When grouped into 29 injury categories, most categories were statistically significant predictors of HRQL scores in the first year after injury. For these groups of injuries, mean first-year QALY loss estimates range from 0.005 (sprains and strains of joints and adjacent muscles, n = 7067) to 0.109 (injury to nerves and spinal cord, n = 71). Fewer estimates are significant in the second and third years after injury, which may reflect a return to baseline HRQL. Conclusion. This research presents both a framework for estimating QALY loss for short-lived medical conditions and nationally representative, community-based HRQL scores associated with a wide variety of injury and poisoning conditions. 36. Flexible Work Hours and Other Job Factors in Parental Time with Children

Abstract

Flexible working hours are typically seen to be advantageous to working parents, as the flexible hours more easily allow responsibilities of care and employment be balanced. But do flexible work hours actually mean that parents can spend more time with their children? This article explores this for parents of young children in Australia. The analyses use the time use diaries of children in the two cohorts of the Longitudinal Study of Australian Children (LSAC), from the first wave of the study in 2004. The study children in each cohort were aged about one year old and 4-5 years old. For each child, a weekday and weekend diary were completed, giving 5,579 weekday diaries and 4,478 weekend diaries. The diaries captured details of the children's activities and of who they were with in each 15 min period of a day, and so allowed calculation of the total amount of time the child was with their mother and with their father. Multivariate analyses were used to determine whether amounts of mother-child and father-child time varied according to flexibility of work hours, taking account also of other job characteristics, family and child characteristics. The analyses showed that flexible work hours had only weak independent relationships with mothers' and fathers' time with children. Inasmuch as flexible hours are beneficial for parents, it appears that this is related to their ability to distribute their time between work and family time, rather than giving them more time with children.

48. Long Hours and Longings: Australian Children's Views of Fathers' Work and Family Time

Abstract

Using two waves of paired data from a population sample of 10- to 13-year-old Australian children (5,711 father–child observations), the authors consider how the hours, schedules, intensity, and flexibility of fathers' jobs are associated with children's views about fathers' work and family time. A third of the children studied considered that their father works too much, one eighth wished that he did not work at all, and one third wanted more time with him or did not enjoy time together. Logistic regression modeling revealed that working on weekends, being time pressured, being unable to vary start and stop times, and working long hours generated negative views in children about fathers' jobs and time together. The time dilemmas generated by fathers' work devotions and demands are salient to and subjectively shared by their children.
