Abstract

A burgeoning topic in the current energy transition are the huge costs of redispatch congestion management (CM) in large transmission systems. One of the German transmission system operators (TSOs) raised the critical inquiry of how to allocate the redispatch costs amongst TSOs in an equitable and beneficial way. Previously, a Shapley value-based approach has been introduced on small test grids, using the linear DC approximation of optimal power flow (OPF). However, within the application of CM, its feasibility and accuracy for large-scale power grids and its impact on the computed congestions remain uncertain. Therefore, this study investigates the applicability of the DC OPF compared to the exact AC OPF with regard to the Shapley values, for both small and large-scale grids. Numerical simulation shows significant differences in the congested lines, the overall redispatch costs, and the Shapley values. Abstract

The increase in renewable energy sources (RESs), like wind or solar power, results in growinguncertainty also in transmission grids. This affects grid stability through fluctuating energy supplyand an increased probability of overloaded lines. One key strategy to cope with this uncertainty isthe use of distributed energy storage systems (ESSs). In order to securely operate power systemscontaining renewables and use storage, optimization models are needed that both handle uncertaintyand apply ESSs. This paper introduces a compact dynamic stochastic chance-constrained DC optimalpower flow (CC-OPF) model, that minimizes generation costs and includes distributed ESSs. AssumingGaussian uncertainty, we use affine policies to obtain a tractable, analytically exact reformulation asa second-order cone problem (SOCP). We test the new model on five different IEEE networks withvarying sizes of 5, 39, 57, 118 and 300 nodes and include complexity analysis. The results showthat the model is computationally efficient and robust with respect to constraint violation risk. Thedistributed energy storage system leads to more stable operation with flattened generation profiles.Storage absorbed RES uncertainty, and reduced generation cost.

3. Imaginative Children in the Classroom: Mixed-Methods Examining Teacher Reported Behavior, Play Observations and Child Assessments

Abstract

During imaginative play, children may learn foundational skills important for academic success. Indeed, imaginative children, high in fantasy orientation (FO), may have advantages in skills that support positive classroom and social behavior. Yet findings are mixed regarding the classroom behavior of children high in fantasy orientation. The purpose of the current study was to examine the cognitive and social skills of children who typically engage in imaginative play in comparison to their peers in classroom contexts across multiple informants: child, teacher, and classroom observers. Ninety preschoolers and their teachers completed measures of fantasy orientation (FO), self-regulation, emotion knowledge, vocabulary, and social competence. Further, observers reported on children's classroom behaviors. Research Findings: Cluster analyses revealed children with higher scores on direct assessments of executive functioning, emotion knowledge, and vocabulary had higher FO. Additionally, children whose teachers reported them as socially competent had higher FO. Finally, children who were rated by classroom observers as high in emotive social play, peer conversation, and physical activity had higher FO. Thus, across multiple informants, children high in FO and imaginative play exhibited higher cognitive and social skills in the classroom. Practice or Policy: These findings suggest that children high in FO may have benefits in skills that are related to positive academic skills in comparison to peers. Curricula should continue to examine the utility of imaginative play as a context to support cognitive and socioemotional development.

Abstract

BackgroundIn COVACTA, a randomised, placebo-controlled trial in patients hospitalised with coronavirus disease-19 (COVID-19), tocilizumab did not improve 28-day mortality, but shortened hospital and intensive care unit stay. Longer-term effects of tocilizumab in patients with COVID-19 are unknown. Therefore, the efficacy and safety of tocilizumab in COVID-19 beyond day 28 and its impact on Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) clearance and antibody response in COVACTA were investigated.MethodsAdults in Europe and North America hospitalised with COVID-19 (N = 452) between April 3, 2020 and May 28, 2020 were randomly assigned (2:1) to double-blind intravenous tocilizumab or placebo and assessed for efficacy and safety through day 60. Assessments included mortality, time to hospital discharge, SARS-CoV-2 viral load in nasopharyngeal swab and serum samples, and neutralising anti-SARS-CoV-2 antibodies in serum. ClinicalTrials.gov registration: NCT04320615.FindingsBy day 60, 24·5% (72/294) of patients in the tocilizumab arm and 25·0% (36/144) in the placebo arm died (weighted difference -0·5% [95% CI -9·1 to 8·0]), and 67·0% (197/294) in the tocilizumab arm and 63·9% (92/144) in the placebo arm were discharged from the hospital. Serious infections occurred in 24·1% (71/295) of patients in the tocilizumab arm and 29·4% (42/143) in the placebo arm. Median time to negative reverse transcriptase-quantitative polymerase chain reaction result in nasopharyngeal/oropharyngeal samples was 15·0 days (95% CI 14·0 to 21·0) in the tocilizumab arm and 21·0 days (95% CI 14·0 to 28·0) in the placebo arm. All tested patients had positive test results for neutralising anti-SARS-CoV-2 antibodies at day 60.InterpretationThere was no mortality benefit with tocilizumab through day 60. Tocilizumab did not impair viral clearance or host immune response, and no new safety signals were observed. Future investigations may explore potential biomarkers to optimize patient selection for tocilizumab treatment and combination therapy with other treatments.FundingF. Abstract

Altered myeloid inflammation and lymphopenia are hallmarks of severe infections. We identified the upregulated EN-RAGE gene program in airway and blood myeloid cells from patients with acute lung injury from SARS-CoV-2 or other causes across 7 cohorts. This program was associated with greater clinical severity and predicted future mechanical ventilation and death. EN-RAGEhi myeloid cells express features consistent with suppressor cell functionality, including low HLA-DR and high PD-L1. Sustained EN-RAGE program expression in airway and blood myeloid cells correlated with clinical severity and increasing expression of T cell dysfunction markers. IL-6 upregulated many EN-RAGE program genes in monocytes in vitro. IL-6 signaling blockade by tocilizumab in a placebo-controlled clinical trial led to rapid normalization of EN-RAGE and T cell gene expression. This identifies IL-6 as a key driver of myeloid dysregulation associated with worse clinical outcomes in COVID-19 patients and provides insights into shared pathophysiological mechanisms in non-COVID-19 ARDS.

Abstract

Background: Similar immune responses in the nasal and bronchial mucosa implies that nasal allergen challenge (NAC) is a suitable early phase experimental model for drug development targeting allergic rhinitis (AR) and asthma. We assessed NAC reproducibility and the effects of intranasal corticosteroids (INCS) on symptoms, physiology, and inflammatory mediators. Methods: 20 participants with mild atopic asthma and AR underwent three single blinded nasal challenges each separated by three weeks (NCT03431961). Cohort A (n = 10) underwent a control saline challenge, followed by two allergen challenges. Cohort B (n = 10) underwent a NAC with no treatment intervention, followed by NAC with 14 days pre‐treatment with saline nasal spray (placebo), then NAC with 14 days pre‐treatment with INCS (220 μg triamcinolone acetonide twice daily). Nasosorption, nasal lavage, blood samples, forced expiratory volume 1 (FEV1), total nasal symptom score (TNSS), peak nasal inspiratory flow (PNIF) were collected up to 24 h after NAC. Total and active tryptase were measured as early‐phase allergy biomarkers (≤30 min) and IL‐13 and eosinophil cell counts as late‐phase allergy biomarkers (3–7 h) in serum and nasal samples. Period‐period reproducibility was assessed by intraclass correlation coefficients (ICC), and sample size estimates were performed using effect sizes measured after INCS. Results: NAC significantly induced acute increases in nasosorption tryptase and TNSS and reduced PNIF, and induced late increases in nasosorption IL‐13 with sustained reductions in PNIF. Reproducibility across NACs varied for symptoms and biomarkers, with total tryptase 5 min post NAC having the highest reproducibility (ICC = 0.91). Treatment with INCS inhibited NAC‐induced IL‐13 while blunting changes in TNSS and PNIF. For a similar crossover study, 7 participants per treatment arm are needed to detect treatment effects comparable to INCS for TNSS. Conclusion: NAC‐induced biomarkers and symptoms are reproducible and responsive to INCS. 29. Third Grade Emergent Bilinguals' Voices and Experiences Learning English Literacies in School: A Post-Intentional Phenomenological Exploration

Abstract

This study explored the lived experiences of three third grade emergent bilinguals learning English literacies such as speaking, reading, and writing in school using a post-intentional phenomenological approach (Vagle, 2014). Learning what is unfamiliar, especially academics can be challenging for emergent bilinguals. Young elementary children are influenced by a variety of factors in school and their experiences in elementary classrooms help shape who they are and how they learn daily and in the future. In turn, teachers do not always recognize the knowledge and language skills of linguistically diverse students (Zwiers, 2013). Non-native English speakers face many difficulties in mainstream classrooms such as navigating literacies in a new language academically and socially. This phenomenological research study uses theories from Gonzalez, Moll, & Amanti (2013), Alison Cook-Sather (2002, 2006), and Max van Manen (1982, 2014) who highlight key ideas such as funds of knowledge, student voice, and phenomenology. These theories guided and supported the findings from the study of the phenomenon of learning literacies of reading, speaking, and writing in school with third grade emergent bilinguals. Through the use of students' own voices, this study investigated: 1) How might literacies of reading, writing, and speaking English take shape for third grade emergent bilinguals in school? And 2) How might emergent bilinguals' cultural and linguistic funds of knowledge be used at school? Data were collected through classroom visits, observations, and conversations with participants were guided using the five-component process from Vagle's (2014) post-intentional phenomenological research design. The data were analyzed using the whole-parts-whole phenomenological approach (Vagle, 2014). The findings represent three tentative manifestations: "challenge," "interaction," and "native language." The tentative manifestation "challenge" relates to the idea of a task being difficult, in which people are pushed to use skills to their full potential. The second tentative manifestation "interaction" refers to the sense of being, communicating, and collaborating with another person. The third tentative manifestation "native language" refers to the participants knowing and using their native language throughout engagement with learning literacies in school. Commonalities between participants were revealed through the tentative manifestations. The findings support the need to allow emergent bilinguals' voices, cultures, and linguistic backgrounds to be present in their classrooms for maximum learning success. Background: In COVACTA, a randomised, placebo-controlled trial in patients hospitalised with coronavirus disease-19 (COVID-19), tocilizumab did not improve 28-day mortality, but shortened hospital and intensive care unit stay. Longer-term effects of tocilizumab in patients with COVID-19 are unknown. Therefore, the efficacy and safety of tocilizumab in COVID-19 beyond day 28 and its impact on Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) clearance and antibody response in COVACTA were investigated. Methods: Adults in Europe and North America hospitalised with COVID-19 (N = 452) between April 3, 2020 and May 28, 2020 were randomly assigned (2:1) to double-blind intravenous tocilizumab or placebo and assessed for efficacy and safety through day 60. Assessments included mortality, time to hospital discharge, SARS-CoV-2 viral load in nasopharyngeal swab and serum samples, and neutralising anti-SARS-CoV-2 antibodies in serum. ClinicalTrials.gov registration: NCT04320615. Findings: By day 60, 24·5% (72/294) of patients in the tocilizumab arm and 25·0% (36/144) in the placebo arm died (weighted difference -0·5% [95% CI -9·1 to 8·0]), and 67·0% (197/294) in the tocilizumab arm and 63·9% (92/144) in the placebo arm were discharged from the hospital. Serious infections occurred in 24·1% (71/295) of patients in the tocilizumab arm and 29·4% (42/143) in the placebo arm. Median time to negative reverse transcriptase-quantitative polymerase chain reaction result in nasopharyngeal/oropharyngeal samples was 15·0 days (95% CI 14·0 to 21·0) in the tocilizumab arm and 21·0 days (95% CI 14·0 to 28·0) in the placebo arm. All tested patients had positive test results for neutralising anti-SARS-CoV-2 antibodies at day 60. Interpretation: There was no mortality benefit with tocilizumab through day 60. Abstract

International audience; Objective: High rates of sexually transmitted infections (STIs) have been reported among pre-exposure prophylaxis (PrEP) users. We wished to assess the incidence and risk factors for recurrent STIs.Design: The ANRS IPERGAY trial was a prospective study investigating PrEP among MSM and transgender women in outpatient clinics in France and Canada. In all, 429 participants were enrolled, offered up to 4 years of PrEP and screened for bacterial STIs (syphilis, chlamydia and gonorrhea) at baseline and every 6 months.Methods: STIs incidence was calculated yearly. Cox proportional hazards model regression was used to explore associations between participants characteristics at baseline and recurrent STI during follow-up.Results: Over a median follow-up of 23 months, bacterial STI incidence was 75, 33, 13, 32 and 30 per 100 person-years for all STIs, rectal STIs, syphilis, gonorrhea and chlamydia, respectively. STI incidence significantly increased from the first year to the fourth year of the study (55 vs. 90 per 100 person-years, P < 0.001). During the study period, 167 participants (39%) presented with more than one bacterial STIs which accounted for 86% of all STIs. Baseline risk factors associated with recurrent STIs in a multivariate analysis were an STI at baseline [hazards ratio: 1.48 (95% confidence interval (CI): 1.06-2.07), P = 0.02], more than eight sexual partners in prior 2 months [hazards ratio: 1.72 (95% CI: 1.21-2.43), P = 0.002] and the use of gamma-hydroxybutyrate [hazards ratio: 1.66 (95% CI: 1.16-2.38), P = 0.005].Conclusion: STI incidence was high and increased over time. Most STIs were concentrated in a high-risk group that should be targeted for future interventions.

Abstract

During imaginative play, children may learn foundational skills important for academic success. Indeed, imaginative children, high in fantasy orientation (FO), may have advantages in skills that support positive classroom and social behavior. Yet findings are mixed regarding the classroom behavior of children high in fantasy orientation. The purpose of the current study was to examine the cognitive and social skills of children who typically engage in imaginative play in comparison to their peers in classroom contexts across multiple informants: child, teacher, and classroom observers. Ninety preschoolers and their teachers completed measures of fantasy orientation (FO), self-regulation, emotion knowledge, vocabulary, and social competence. Further, observers reported on children's classroom behaviors. Research Findings: Cluster analyses revealed children with higher scores on direct assessments of executive functioning, emotion knowledge, and vocabulary had higher FO. Additionally, children whose teachers reported them as socially competent had higher FO. Finally, children who were rated by classroom observers as high in emotive social play, peer conversation, and physical activity had higher FO. Thus, across multiple informants, children high in FO and imaginative play exhibited higher cognitive and social skills in the classroom. Practice or Policy: These findings suggest that children high in FO may have benefits in skills that are related to positive academic skills in comparison to peers. Curricula should continue to examine the utility of imaginative play as a context to support cognitive and socioemotional development. [ABSTRACT FROM AUTHOR]
