Abstract

Background: Neuroinflammatory diseases are progressive leading to loss of function and disability. Although palliative care (PC) utilization has increased globally, it has scarcely increased in neurology. Objectives: To explore PC attitudes and knowledge among patients with neuroinflammatory diseases, such as multiple sclerosis, neuromyelitis optica spectrum disorder, and myelin oligodendrocyte glycoprotein antibody-associated disease. Methods: A cross-sectional 1-year study was conducted using the Palliative Care Knowledge Scale (PaCKS) and the PC Health Information National Trends Survey (HINTS). Murray's transition theory guided this study, which integrates palliative services including decision making, communication, and coordinated care. Results: The majority of study patients were female (69%) (N = 86) and White (79%). Forty-two percent indicated that they had never heard about PC, 46% said that they knew a little bit about PC, and 12% said that they knew a lot about PC. Fifty percent of patients knew the goals of PC and had knowledge about PC services. Forty-four percent to 60% agreed that PC goals include helping friends and family to cope with a patient's illness, offering social and emotional support, and managing pain and other symptoms. Patients who self-reported being familiar with PC performed significantly better on the PaCKS than those unfamiliar with PC (p < 0.001), and those who self-reported moderate or severe memory loss performed significantly worse on the PaCKS than those with mild memory loss (p = 0.027). There was an association between higher education and PC knowledge and between patients' PaCKS scores and their self-reported HINTS PC knowledge. Conclusions: Patients have partial PC knowledge. Abstract

After encounter with a central nervous system (CNS)-derived autoantigen, lymphocytes leave the lymph nodes and enter the CNS. This event leads only rarely to subsequent tissue damage. Genes relevant to CNS pathology after cell infiltration are largely undefined. Myelin-oligodendrocyte-glycoprotein (MOG)-induced experimental autoimmune encephalomyelitis (EAE) is an animal model of multiple sclerosis (MS), a chronic autoimmune disease of the CNS that results in disability. To assess genes that are involved in encephalitogenicity and subsequent tissue damage mediated by CNS-infiltrating cells, we performed a DNA microarray analysis from cells derived from lymph nodes and eluted from CNS in LEW.1AV1 (RT1(av1)) rats immunized with MOG 91-108. The data was compared to immunizations with adjuvant alone or naive rats and to immunizations with the immunogenic but not encephalitogenic MOG 73-90 peptide. Here, we show involvement of Cd38, Cxcr4 and Akt and confirm these findings by the use of Cd38-knockout (B6.129P2-Cd38(tm1Lnd)/J) mice, S1P-receptor modulation during EAE and quantitative expression analysis in individuals with MS. The hereby-defined underlying pathways indicate cellular activation and migration pathways mediated by G-protein-coupled receptors as crucial events in CNS tissue damage. These pathways can be further explored for novel therapeutic interventions.

22. Untersuchungen zum Vorkommen von Verotoxin-bildenden Escherichia coli (VTEC) bei Rehwild in Hessen

Abstract

In einer repräsentativen Prävalenzstudie sollte für das Bundesland Hessen das Vorkommen von Verotoxin-bildenden E. coli (VTEC) bei Rehwild ermittelt werden. Dazu wurden Kotproben von 353 erlegten Rehen aus Revieren von 12 hessischen Forstämtern aus insgesamt 12 Landkreisen einbezogen. Nach selektiver kultureller Anreicherung wurden im Enzymimmunoassay (EIA) bzw. in einem immunchromatographischen Verfahren (Duopath®) Verotoxin-positiv getestete Proben weitergehend untersucht. Mittels konventioneller PCR und Anzüchtung der VTEC auf selektiv-elektiven Nährmedien konnten insgesamt 68 Tiere (19,3 %) als Ausscheider solcher Stämme nachgewiesen werden. Unter den positiven Rehen waren die juvenilen Tiere (29,84%) hoch signifikant (p < 0,001) häufiger betroffen als die beiden anderen Altersklassen „subadult“ (14,75 %) und „adult“ (12,80 %). Eine Abhängigkeit hinsichtlich des Geschlechts konnte nicht festgestellt werden. In Abhängigkeit von den Landkreisen gab es signifikante (p < 0,05) Unterschiede in den Prävalenzen (10,5 % - 41,4 %); in zwei Landkreisen konnten keine positiven Tiere nachgewiesen werden. Die molekular-basierte Typisierung zur Erfassung der Verotoxinsubtypen zeigte, dass der Subtyp 2b (38,5 %) dominierte, gefolgt von dem Subtypen VT 2g (30,8 %) sowie den Subtypen VT 1c, VT 1d (jeweils 12,8 %) und VT 1a (5,1 %). Von den insgesamt 68 positiven Rehen konnten 79 verschiedene E. coli-Isolate gewonnen werden. Bei sieben Tieren konnten neben verschiedenen Serotypen auch VTEC-Stämme mit unterschiedlichen Genkombinationen nachgewiesen werden. Lediglich ein VTEC-Isolat erwies sich als positiv für das E. coli attaching and effacing-Gen (eae), das regelmäßig bei hoch virulenten VTEC-Stämmen anzutreffen ist. Das Enterohämolysin-Gen konnte bei 27 Isolaten nachgewiesen werden. Unter den 79 Isolaten waren 27 verschiedene Serotypen zu finden. Am häufigsten wurden der Serotyp O21:H21(16,6 %) und O74:H28 (16,6 %) detektiert, gefolgt von O146:H28 (7,7 %), O154:H31(6,4 %) und O174:H8 (6,4 %). O174:H8 konnte in Zusammenhang mit den Verotoxin-Subtypen VT 1a und VT 1c sowie dem e-hly-Gen schon bei schweren Krankheitsverläufen des Menschen nachgewiesen werden. Insgesamt kann aufgrund der hier erhobenen Befunde davon ausgegangen werden, dass Rehwild in Hessen als natürliches Reservoir für VTEC angesehen werden muss. Prinzipiell handelt sich jedoch bei den isolierten VTEC-Stämmen vorwiegend um Stammformen mit geringer Virulenz. Zusätzlich deutet die Tatsache, dass VTEC aus Rehwild und landwirtschaftlichen Nutztieren Gemeinsamkeiten aufweisen und sich die Tierzahl pro landwirtschaftliche Fläche auf die Prävalenz auswirkt, auf eine wechselseitige Übertragung hin.
