Abstract

Background: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare hematologic disorder characterized by a deficiency of GPIanchored proteins in blood cells. It is associated with hemolysis, thromboembolic events, and bone marrow failure. Management includes complement inhibitors, anticoagulation, and stem cell transplantation. Spontaneous remission of PNH is rarely reported. Case Presentation: A 28-year-old male initially diagnosed with aplastic anemia developed portal vein thrombosis and was subsequently diagnosed with PNH. Due to the unavailability of complement inhibitors, anticoagulation therapy and supportive care were employed. Splenectomy with splenorenal shunt was performed for chronic thrombosis and associated complications. The patient achieved spontaneous remission with normalized blood counts and diminished PNH clone. Conclusion: Managing PNH in resource-limited settings presents challenges due to the unavailability of complement inhibitors. Spontaneous remission of PNH is rarely reported and needs further research. A multidisciplinary approach, accessibility to diagnostic tests, and advanced treatments will enhance PNH management in resource-limited settings. [ABSTRACT FROM AUTHOR]

30. Socio-Economic Conditions of Silk Handloom Weavers in Azamgarh and Varanasi Districts of Uttar Pradesh

Abstract

The handloom weaving industry in India employs 14% of the poor (second only to agriculture) and provides around 11% of total textile exports annually. The availability of various fabric patterns and the production of garments and outfits, which are the occupations of millions of weavers employed by the handloom sector in India, have contributed to the rise in popularity of India's handloom industry. The Indian state of Uttar Pradesh has around 130778 handloom weavers. This study discusses the socio-economic conditions of silk handloom weavers in the Azamgarh and Varanasi districts of Uttar Pradesh. The study is based on primary data through a structures interview schedule. The total sample respondents for the survey from Azamgarh and Varanasi were 405, comprising 205 samples from the Azamgarh district and 200 samples from the Varanasi district. In Azamgarh district, 35 representatives were from Akbarpur, 40 samples were from Atraulia, 32 samples were from Jahanagaj, 28 were from Jeeyanpur, and 70 were from Mubarakpur. In the same way, 200 samples were selected from Varanasi, in which 60 samples were from Bajardiha, 50 samples from Jaitpura, 30 samples were from Jallalipura, 30 were from Lohta, and 30 were from Madanpura. The data were evaluated using a simple percentage, annual growth, and compound annual growth rate and presented by pie and bar diagrams. This study is based on location, age, gender, religion, social group, educational qualification, type and size of the family, employment status, type and ownership of dwelling units, ration card, source of loan and purpose of the loan, monthly household income from handloom related activities, and possession of looms. The study results indicated that the position of handloom weavers was miserable. The youth participation in this profession is meagre 3.5 per cent. They are not interested in choosing this as a profession. The majority of workers are men, 91 per cent, and the women worker's participation is not in a good ratio. The Muslim religion dominates the silk handloom weaving industry at 65 per cent. The weaving activity is shared mainly by OBCs 63.1 per cent (Other Backward Class) social group. Most households (63 per cent) have a monthly household income range of less than Rs. 5001-7500, and 56.8 per cent of the respondents have a monthly consumption expenditure of Rs. 5001-7500.

Abstract

Purpose The coastal zone of Bangladesh that is in the front line of the battle against climate change faced over 200 natural disasters in the past 40 years, and most of the disasters were cyclones. The inevitable cyclone shelter (CS), the backbone of disaster management (DM), provides short-term safety for the disaster victims in Bangladesh. This study aims to explore the community-based limitations and sustainable development features of CSs including the gender issues. Design/methodology/approach A questionnaire survey was carried out among 230 community people to identify the requirements and sustainable development features of CSs. A field visit was carried out in 23 CSs to capture its existing facilities. Key informant interviews were conducted in the office of Upazila Engineers to strengthen survey data. Findings This research found that the plan of CSs, quality of construction, capacity, facilities, entrance and exit, space allocation, management and policy were not capable enough to fulfill the needs and requirements of the community people. Due to lack of separate facilities, women and girls avoided shelters for fear of sexual and mental harassment in CSs, as they had experiences in the earlier events of cyclones. Insufficient facilities discourage community from using the shelters. Research limitations/implications Women and girls were shy to share their experience in CSs. The historical data were limited in the study area. To the best of the authors’ knowledge, this research presents the actual community-based outcome. During CCRIP training program, the authors met 3,625 community people, and participatory discussions were made to explore the participants’ experiences and perceptions about the sustainable development of CSs. Practical implications South-Asian coastal zones are prone to natural, quasi-natural hazard and disasters, where shelters are required for protecting lives of community people during such disasters such as cyclones, storm surges, and floods. Therefore, this study can help in making sustainable development decisions in terms of constructing shelters in disaster-prone countries like Bangladesh. Social implications The outcomes of this investigation are useful for uplifting psychosocial status to protect lives during disasters such as cyclones, storm surges and floods and increase accessibility to shelters, and users will consider CSs as a social asset. In turn, the acceptability of CSs into community level are expected to be increased for combating against cyclones, storm surges, and floods. Originality/value This study introduces the bottom-up approach that refers to the community-based decision-making to identify the limitations and sustainable improvement of CSs. This research contributes to bridging the gaps between decision-makers and users of CSs. From the authors’ field experience, it can be said that this is the first fieldwork regarding the objectives.

Abstract

Background: Healthy life expectancy (HALE) at birth measures the standard of living anticipated. Objective: The purpose of this study was to identify the main global risk factors for HALE at birth. Methodology: The data of 212 countries have been extracted from the data hub of World Health Organization, Worldometer, World Bank, and United Nations. The HALE at birth was considered as the dependent variable and the death rate due to Coronavirus disease 2019 (COVID 19), recovery rate from COVID 19, tuberculosis (TB) incidence, universal health coverage (UHC) service index, tobacco smoking, alcohol consumption rate, human immunodeficiency virus (HIV) prevalence rate, average household size, gross domestic product (GDP), and current health expenditure were considered as the independent variables. Descriptive statistics, Pearson's correlation analysis, and multiple linear regression model were used as the statistical tools to analyze the data. Results: The study results revealed that the HALE is found lowest in Central African Republic (44.9 years) and highest in Singapore (76.2 years). On the other hand, the death rate due to COVID 19 is found highest in Nicaragua (32.0%). In case of the recovery rate from COVID 19, the lowest value is seen in Tajikistan (0.0). Again, the lowest UHC service index is found in Montserrat (0.0). Alcohol consumption rate is found highest in Moldova Republic (15.20). In case of HIV prevalence rate, the highest value is seen in Eswatini (27.30). Average household size is seen highest in Senegal (8.30). The recovery rate from COVID 19, UHC service index, GDP, current health expenditure TB incidence, tobacco smoking, HIV prevalence rate and average household size were significantly correlated with the HALE at birth. Again, the multiple linear regression model identified the UHC service index, alcohol consumption rate, HIV prevalence rate and average household size as the significant factors which are responsible for the variation in HALE at birth worldwide. Conclusion: The UHC service index, alcohol consumption rate, HIV prevalence rate and average household size are the most associate factors of HALE at birth globally. [ABSTRACT FROM AUTHOR]

Abstract

Since the LoRa is an open standard and supports the sustainable smart cities to develop, which are associated with the circular economy concepts, the researchers are being attracted towards the LoRa (Long Range). Therefore, a development architecture of long range (LoRa) gateway communication technology using artificial intelligence is proposed in this paper. Mysignal development shield is used by IoT devices and sensors for maintenance and remote monitoring. LoRa implements wireless system to transmit physical data to pc or cloud that is gathered by multiple sensors enabled by MySignals. The tasks performed by LoRa are discussed with cloud and IoT server data flow system based on AI. Through LoRa the communication is being implemented with the program of hyper terminal. By getting good results from sensors, the platform of MySignal is developing the proposed architecture. As per outcomes of Simulation clearly shows that many technologies of IoT, complexity reduction, and minimization of delays in the process of decision making at the customer end are supported by LoRa WAN (Wireless Area Network) & hybrid Wi-Fi network. As the deployment cost is less it supports one moveable gateway.

Abstract

Background: There is accumulating evidence that roadside pollution is detrimental to health. Traffic police personnel are exposed to roadside pollution due to the nature of their job and are at risk of adverse health outcomes. Objective: This study aims to compare the risk of adverse respiratory symptoms in different categories of traffic police including traffic constables, sergeants, and inspectors. Methods: The study population consisted of 369 randomly selected traffic police personnel from the city of Chittagong in Bangladesh. Information on their occupation and respiratory health symptoms were collected. The health outcomes were coughing, coughing sputum, coughing up blood, shortness of breathing, wheezing, and chest pain Result: The risk of coughing [adjusted Odds Ratio (AOR) = 4.469, 95% CI=1.265-15.793], coughing sputum [AOR= 3.687, 95% CI= 1.004 -13.540], coughing up blood [AOR=1.040, 95% CI=0.227-6.162], shortness of breathing [AOR=3.937, 95% CI=1.069-14.500], wheezing [AOR= 2.464, 95% CI= 0.613-9.906] and chest pain with deep breathing [AOR=2.163,95% CI= 0.560-8.349] was higher in traffic constables compared to inspectors. In sergeants odds increased for coughing up blood [AOR=1.102, 95% CI= 0.283-4.286] and wheezing [AOR=1.260, 95% CI= 0.304-5.229]. Conclusion: There was a substantial difference in the risk of studied respiratory symptoms between different categories of traffic police jobs.
