1. Development and Validation of a Rural Stress Instrument

Abstract

Rural stress is the physiological reaction brought upon by internal and socioeconomic stressors characteristic of rural residents. This type of stress is exacerbated by the complex issues facing rural populations and serves to inhibit the growth of vibrant, resilient communities. The current study sought to construct and validate an instrument intended to quantify perceptions of rural stress among neighbors and communities. The proposed scale was confirmed to have robust content, internal structure, and consequential validity. An exploratory factor analysis indicated one variable accounted for 57.74% of the total variance, while a one-way between subjects ANOVA found a statistically significant effect of group on scale scores under rural-urban continuum (RUC) conditions. One implication from these findings is that changes in communities may function as predictors of stress. Agricultural and extension educators hold a unique position that allows them to interact with the community on a daily basis. We recommend that they leverage this position to educate residents on the effects of stress, reduce the stigma associated with help-seeking behaviors, and discuss available stress management resources. Future research should consider modifying the instrument to focus on individual levels of stress instead of perceptions of stress within the community.

2. The Development and Validation of an Empirical Scale to Inform Community Diagnostics and Change

Abstract

Purpose: The purpose of this research was to develop and validate a robust scale, based on the community capital framework, to provide an empirical measure of community perceptions. Design/methodology/approach: The proposed measure was completed by a sample of Georgia citizens (n = 3374) representing a diverse cross-section of community characteristics. Findings: Data indicate that the six proposed scales (natural, social, human, cultural, built-financial, and political) subsumed within the instrument measure unique underlying latent variables with evidence of internal validity through multiple analytic techniques. Practical implications: This study represents an initial examination of a quantitative instrument -- the Community Diagnostics + Social Impact (CD + SI) Toolkit™ -- that can be used by researchers, educators, and community development professionals to measure the stocks of the various capitals within a community. Theoretical implications: The theoretical implications of this study are that the CD + SI Toolkit™ holds promise as a valid and reliable instrument for establishing holistic community profiles, identifying targeted areas for in-depth research inquiry or programmatic design, guiding policy design and implementation, and ultimately evaluating the impacts of community development programs and policies. The CD + SI Toolkit™ addresses an omission in the literature and in broader community development. Originality/value: While various frameworks such as the CCF exist to provide a more general blueprint for program development and assessment, the CD + SI Toolkit™ represents an effort to specifically quantify the community capitals and establish a replicative process for measuring community capitals. The results of this initial validation effort demonstrate the potential for such a tool.

Abstract

We study both experimentally and theoretically the self-assembly of binary block copolymers in dilute solution, where self-assembly is triggered by changing the solvent from the common good solvent THF to the selective solvent water, and where the two species on their own in water form vesicles and spherical micelles respectively. We find that in water the inter-micellar exchange of these block copolymers is very slow so that the self-assembled structures are in local but not global equilibrium (i.e., they are non-ergodic). This opens up the possibility of controlling micelle morphology both thermodynamically and kinetically. Specifically, when the two species are first dissolved in THF before mixing and self-assembly (`premixing') by dilution with water, the morphology is found to depend on the mixing ratio of the two species, going gradually from vesicles via `bulbed' rods, rings, Y-junctions and finally to spherical micelles as we increase the proportion of the sphere-formers. On the other hand, if the two species are first partially self-assembled (by partial exchange of the solvent with water) before mixing and further self-assembly (`intermediate mixing'), novel metastable structures, including nanoscopic pouches, emerge. These experimental results are corroborated by self-consistent field theory calculations (SCFT) which reproduce the sequence of morphologies seen in the pre-mixing experiments. SCFT also reveals a clear coupling between polymer composition and aggregate curvature, with regions of positive and negative curvature being stabilized by an enrichment and depletion of sphere formers respectively. Our study demonstrates that both thermodynamic and kinetic blending of block copolymers are effective design parameters to control the resulting structures and allow us to access a much richer range of nano-morphologies than is possible with monomodal block copolymer solutions., Comment: 31 pages including 14 figures (double-spaced)

Abstract

We present a combined experimental and theoretical study on the role of copolymer architecture in the self-assembly of binary PEO-PCL mixtures in water-THF, and show that altering the chain geometry and composition of the copolymers can control the form of the self-assembled structures and lead to the formation of novel aggregates. First, using transmission electron microscopy and turbidity measurements, we study a mixture of sphere-forming and lamella-forming PEO-PCL copolymers, and show that increasing the molecular weight of the lamella-former at a constant ratio of its hydrophilic and hydrophobic components leads to the formation of highly-curved structures even at low sphere-former concentrations. This result is explained using a simple argument based on the effective volumes of the two sections of the diblock and is reproduced in a coarse-grained mean-field model: self-consistent field theory (SCFT). Using further SCFT calculations, we study the distribution of the two copolymer species within the individual aggregates and discuss how this affects the self-assembled structures. We also investigate a binary mixture of lamella-formers of different molecular weights, and find that this system forms vesicles with a wall thickness intermediate to those of the vesicles formed by the two copolymers individually. This result is also reproduced using SCFT. Abstract

RF Ablation of VT in Chronic Cocaine Users Background Cocaine use is a known but rare cause of cardiac arrhythmias. Ventricular arrhythmias related to cocaine may not respond to antiarrhythmic drugs and may need treatment with radiofrequency ablation. Objectives We describe the clinical and electrophysiological characteristics of cocaine-related ventricular tachycardia (VT) from a multicenter registry. Methods Subjects presenting with VT related to cocaine use and being considered for radiofrequency ablation have been included in the study. Patients who were refractory to maximal medical therapy underwent radiofrequency ablation of the VT. Clinical, procedural variables, efficacy, and safety outcomes were assessed. Results A total of 14 subjects met study criteria (age 44 ± 13, range 18- to 68-year-old with 79% male, 71% Caucasian). MRI showed evidence of scar only in 43% of patients (6/14). The mechanism of VT was focal in 50% (n = 7) and scar related reentry in 50% (n = 7) based on 3D mapping. The mean VT cycle length was 429 ± 96 milliseconds. The site of origin was epicardial in 16% (3/18) of VTs. Most clinical VTs were hemodynamically stable (75%). Mean ejection fraction at the time of admission was 44 ± 14%. Duration of procedure was 289 ± 50 minutes. One subject developed pericardial tamponade requiring drainage. At 18 ± 11 months follow-up, freedom from arrhythmia was seen in 86% (1 case lost to follow-up and 2 died). Conclusion Radiofrequency ablation is not only feasible but also safe and effective in patients who have drug refractory VT related to chronic cocaine use. © 2014 Wiley Periodicals, Inc.

We introduce three measures which quantify the degree to which quantum systems possess the robustness exhibited by classical systems when subjected to continuous observation. 7. Approximate Master Equations for Atom Optics

Abstract

In the field of atom optics, the basis of many experiments is a two level atom coupled to a light field. The evolution of this system is governed by a master equation. The irreversible components of this master equation describe the spontaneous emission of photons from the atom. For many applications, it is necessary to minimize the effect of this irreversible evolution. This can be achieved by having a far detuned light field. The drawback of this regime is that making the detuning very large makes the timestep required to solve the master equation very small, much smaller than the timescale of any significant evolution. This makes the problem very numerically intensive. For this reason, approximations are used to simulate the master equation which are more numerically tractable to solve. This paper analyses four approximations: The standard adiabatic approximation; a more sophisticated adiabatic approximation (not used before); a secular approximation; and a fully quantum dressed-state approximation. The advantages and disadvantages of each are investigated with respect to accuracy, complexity and the resources required to simulate. In a parameter regime of particular experimental interest, only the sophisticated adiabatic and dressed-state approximations agree well with the exact evolution., Comment: 11 pages, 5 figures. Abstract

The International Liaison Committee on Resuscitation engages in a continuous review of new, peer-reviewed, published cardiopulmonary resuscitation and first aid science. Draft Consensus on Science With Treatment Recommendations are posted online throughout the year, and this annual summary provides more concise versions of the final Consensus on Science With Treatment Recommendations from all task forces for the year. Topics addressed by systematic reviews this year include resuscitation of cardiac arrest from drowning, extracorporeal cardiopulmonary resuscitation for adults and children, calcium during cardiac arrest, double sequential defibrillation, neuroprognostication after cardiac arrest for adults and children, maintaining normal temperature after preterm birth, heart rate monitoring methods for diagnostics in neonates, detection of exhaled carbon dioxide in neonates, family presence during resuscitation of adults, and a stepwise approach to resuscitation skills training. Members from 6 International Liaison Committee on Resuscitation task forces have assessed, discussed, and debated the quality of the evidence, using Grading of Recommendations Assessment, Development, and Evaluation criteria, and their statements include consensus treatment recommendations. Insights into the deliberations of the task forces are provided in the Justification and Evidence-to-Decision Framework Highlights sections. In addition, the task forces list priority knowledge gaps for further research. Additional topics are addressed with scoping reviews and evidence updates.

11. The CFD assisted design and experimental testing of a wing-sail with high lift devices

Abstract

A wingsail is a solid symmetrical aerofoil section which creates thrust in the same manner as a conventional sail. Wingsails may either be used as a sole power unit, e. g. for a yacht or catamaran, or as an auxiliary power unit on a larger craft, e. g. fishing vessels, cargo ships or passenger liners. To augment the thrust created by the wingsail, high lift devices are employed to increase both the maximum lift and the stall incidence of the aerofoil. A wingsail must be symmetrical and capable of creating an equal lift force with the flow approaching the leading edge from either side of the wing centreline, i. e. the wingsail surface must act as either the upper, or lower pressure surface. Initial experimental work proved that using a symmetrical slat as a leading edge high lift device both delayed the separation of flow over the wingsail upper surface and increased the effective camber of the aerofoil. To increase the thrust created still further, this leading edge high lift device was combined with a trailing edge high lift device, a symmetrical single slotted flap. Due to the large number of possible model configurations, a commercially available CFD package was introduced to assist with the design. A series of validation tests comparing the CFD with published and experimental results showed a qualitative agreement with these results. However, the CFD predictions were not sufficiently accurate to be used quantitatively. The computationally designed triple element model was tested experimentally. Lift, drag, pitching moment and pressure distribution measurements were taken from the model. The results of this testing showed that the triple element wingsail increased the plain wing Coax by 68% and the stall incidence by between 4* and 6'. The final triple element wingsail design also increased the thrust of a plain wingsail over the whole operating region. Thrust was increased by up to 83% at the wind angles where a wingsail is most efficient. The results also proved that a commercially available CFD package can be used as an effective and time saving tool for wingsail design.

Abstract

AIM: We conducted a systematic review and meta-analysis to answer the question: Does the implementation of Paediatric Early Warning Systems (PEWS) in the hospital setting reduce mortality, cardiopulmonary arrests, unplanned codes and critical deterioration events among children, as compared to usual care without PEWS? METHODS: We conducted a comprehensive search using Medline, EMBASE, Cochrane Central Register of Controlled Trials, Cumulative Index to Nursing and Allied Health Literature and Web of Science. We included studies published between January 2006 and April 2022 on children <18 years old performed in inpatient units and emergency departments, and compared patient populations with PEWS to those without PEWS. We excluded studies without a comparator, case control studies, systematic reviews, and studies published in non-English languages. We employed a random effects meta-analysis and synthesised the risk and rate ratios from individual studies. We used the Scottish Intercollegiate Guidelines Network (SIGN) to appraise the risk of bias. RESULTS: Among 911 articles screened, 15 were included for descriptive analysis. Fourteen of the 15 studies were pre- versus post-implementation studies and one was a multi-centre cluster randomised controlled trial (RCT). Among 10 studies (580,604 hospital admissions) analysed for mortality, we found an increased risk (pooled RR 1.18, 95% CI 1.01-1.38, p = 0.036) in the group without PEWS compared to the group with PEWS. The sensitivity analysis performed without the RCT (436,065 hospital admissions) showed a non-significant relationship (pooled RR 1.17, 95% CI 0.98-1.40, p = 0.087). Among four studies (168,544 hospital admissions) analysed for unplanned code events, there was an increased risk in the group without PEWS (pooled RR 1.73, 95%CI 1.01-2.96, p = 0.046) There were no differences in the rate of cardiopulmonary arrests or critical deterioration events between groups. Abstract

This is the sixth annual summary of the International Liaison Committee on Resuscitation International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science With Treatment Recommendations. This summary addresses the most recently published resuscitation evidence reviewed by International Liaison Committee on Resuscitation Task Force science experts. Topics covered by systematic reviews include cardiopulmonary resuscitation during transport; approach to resuscitation after drowning; passive ventilation; minimizing pauses during cardiopulmonary resuscitation; temperature management after cardiac arrest; use of diagnostic point-of-care ultrasound during cardiac arrest; use of vasopressin and corticosteroids during cardiac arrest; coronary angiography after cardiac arrest; public-access defibrillation devices for children; pediatric early warning systems; maintaining normal temperature immediately after birth; suctioning of amniotic fluid at birth; tactile stimulation for resuscitation immediately after birth; use of continuous positive airway pressure for respiratory distress at term birth; respiratory and heart rate monitoring in the delivery room; supraglottic airway use in neonates; prearrest prediction of in-hospital cardiac arrest mortality; basic life support training for likely rescuers of high-risk populations; effect of resuscitation team training; blended learning for life support training; training and recertification for resuscitation instructors; and recovery position for maintenance of breathing and prevention of cardiac arrest. Members from 6 task forces have assessed, discussed, and debated the quality of the evidence using Grading of Recommendations Assessment, Development, and Evaluation criteria and generated consensus treatment recommendations. Abstract

The uniqueness of communities precludes a homogenous approach to program design, delivery, and evaluation. To account for such uniqueness, such approaches should necessitate that program managers have a thorough understanding of the targeted participants or consumers. This understanding can be achieved by determining residents' perceived personal ability, or personal agency, to act and function within the community. This paper represents an effort to operationalize the various capitals that comprise the Community Capitals Framework, focusing on the analysis and validation of an empirical instrument designed to quantify and measure perceived personal agency. Specifically, the objectives of this study were to (1) establish the factor structure of an overall personal agency scale and its capital components, (2) determine if the proposed models adequately fit the collected data, and (3) validate the scales by analyzing internal structure and consistency. The findings indicate that this personal agency scale is a valid and reliable instrument for developing community profiles based on personal agency perceptions. In addition, to account for individuals' personal agency offers a significant contribution to potentially evaluating the social impact of community development efforts longitudinally.
