This paper summarized integrated traditional Chinese and western medicine treatment and nursing of patient with postsurgical gastroparesis syndrome. Changes in the patient’s condition were assessed and analyzed, and a comprehensive treatment plan including acupoint application combined with moxibustion, dietary guidance, gastrointestinal decompression, psychological care and other interventions were carried out to relieve the symptom of stomach fullness and improve the early rehabilitation of gastrointestinal function. (本文总结1例胃癌术后胃瘫综合征患者的中西医结合治疗及护理体会。患者入院后完善护理评估, 根据患者的病情变化, 及时调整干预方案, 采用穴位贴敷联合艾灸疗法干预, 并配合饮食指导、胃肠减压护理、心理疏导等常规护理措施, 有效缓解患者胃脘痞满症状, 促进患者早期恢复正常饮食。)

This paper summarized the nursing management of a patient with severe gastroparesis syndrome after gastric cancer surgery treated with scraping moxibustion. Nursing assessment on gastroparesis syndrome and mental status of the patient was carried out before treatment. Based on the conventional treatment and nursing, the modified scraping-moxibustion therapy was carried out to relieve the gastroparesis syndrome and patient’s negative emotion, and improve the postoperative quality of life. This paper summarized the nursing management for a patient with gastroparesis syndrome after gastrectomy for gastric cancer treated with Traditional Chinese Medicine (TCM) ointment massage and acupoint application. TCM herbs for making ointment and related acupoints were selected based on the syndrome differentiation. The TCM ointment massage and acupoint application was carried out to relieve the symptoms of gastroparesis after gastrectomy and improve the recovery of gastrointestinal function. Objective To investigate the risk factors for postsurgical gastroparesis syndrome (PG S) after surgery for stomach cancer. Methods A total of 684 patients with gastric cancer who underwent surgery for stomach cancer from Jan. 1 ，2010 to Dec. 3 1 ，2014 in Tai'an Tumor Prevention and Treatment Hospital，including 475 males and 209 fem ales， with an average age of 59. 9 years were identified and included in this study. There were 206 cases of gastric cardia and gastric fundus cancers and 478 cases of gastric antrum cancer. 206 cases underwent proximal radical subtotal gastrectomy and D2 lymph node dissection，478 distal radical subtotal gastrectomy，206 residual esophagogastric anastomosis，311 Billroth- I anastomosis，99 Billroth-n anastomosis，and 68 Billroth-n plus Roux-en-y anastomosis. The incidence and risk factors of PGS were analyzed. Results All of the 684 patients were successfully operated. Among them ，48 ( 7. 0% ) encountered PGS. The univariate analysis showed that age， smoking index， alcohol consumption index， HP infection， scores of anxiety， preoperative albumin level， preoperative pyloric obstruction， site of resection， mode of anastomosis， whether to preserve the vagus nerve trunk， perioperative blood glucose level， abdominal cavity infection， and usage of postoperative analgesia pump were related to the occurrence of PGS ( P<0.05 for a ll)，while sex，hypertension，diabetes，perioperative hemoglobin level， perioperative electrolyte im balance， operation duration， intraoperative blood loss， size of gastric remnant and number of dissected lymph nodes were not significantly related to the occurrence of PG S( P >0.05 for a ll). The multivariate binary logistic regression analysis showed that age， HP infection， scores of anxiety，perioperative albumin level, preoperative pyloric obstruction, site of resection, mode of anastomosis, whether to preserve the vagus nerve trunk, perioperative blood glucose level and abdominal cavity infection were risk factors for PGS ( P <0.05 for a ll) ； while the age ( <67 years old) , perioperative albumin level (> 3 5 g/L ) and preservation of the vagus nerve trunk were protective factors of PGS ( P < 0 .05 for a ll). Conclusions The occurrence of PGS is affected by many factors. Detailed evaluation of patients' symptoms and physical signs before operation and rectifying and eliminating risk factors are important to prevent and reduce the occurrence of PGS in patients with gastric cancer.
